During the last few years, new studies on biological effects of strong static magnetic fields and on thermal effects of high-frequency electromagnetic fields used in magnetic resonance imaging (MRI) were published. Many of these studies have not yet been included in the current safety recommendations. Scientific publications since 2010 on biological effects of static and electromagnetic fields in MRI were researched and evaluated. New studies confirm older publications that have already described effects of static magnetic fields on sensory organs and the central nervous system, accompanied by sensory perceptions. A new result is the direct effect of Lorentz forces on ionic currents in the semicircular canals of the vestibular system. Recent studies of thermal effects of high-frequency electromagnetic fields were focused on the development of anatomically realistic body models and a more precise simulation of exposure scenarios. Strong static magnetic fields can cause unpleasant sensations, in particular, vertigo. In addition, they can influence the performance of the medical staff and thus potentially endanger the patient's safety. As a precaution, medical personnel should move slowly within the field gradient. High-frequency electromagnetic fields lead to an increase in the temperature of patients' tissues and organs. This should be considered especially in patients with restricted thermoregulation and in pregnant women and neonates; in these cases exposure should be kept as low as possible.
In Germany, persons who are to be exposed to radiation for medical research purposes are protected by a licensing requirement. However, there are considerable uncertainties on the part of the applicants as to whether licensing by the competent Federal Office for Radiation Protection is necessary, and regarding the choice of application procedure.The article provides explanatory notes and practical assistance for applicants and an outlook on the forthcoming new regulations concerning the law on radiation protection of persons in the field of medical research.Questions and typical mistakes in the application process were identified and evaluated.The qualified physicians involved in a study are responsible for deciding whether a license is required for the intended application of radiation. The decision can be guided by answering the key question whether the study participants would undergo the same exposures regarding type and extent if they had not taken part in the study. When physicians are still unsure about their decision, they can seek the advisory service provided by the professional medical societies. Certain groups of people are particularly protected through the prohibition or restriction of radiation exposure. A simplified licensing procedure is used for a proportion of diagnostic procedures involving radiation when all related requirements are met; otherwise, the regular licensing procedure should be used. The new radiation protection law, which will enter into force on the 31st of december 2018, provides a notification procedure in addition to deadlines for both the notification and the licensing procedures. In the article, the authors consider how eligible studies involving applications of radiation that are legally not admissible at present may be feasible in the future, while still ensuring a high protection level for study participants.
Klinische Studien belegen, dass durch kombinierte PET/CT-Untersuchungen die diagnostische Aussagekraft verglichen mit separat durchgeführten PET- und CT-Untersuchungen verbessert werden kann. Allerdings sind PET/CT-Untersuchungen für Patienten mit einer im Vergleich zu anderen medizinischen Bildgebungsverfahren relativ hohen Strahlenexposition verbunden, so dass sie einer sorgfältigen Rechtfertigung und Optimierung bedürfen, um Wiederholungsuntersuchungen bzw. unnötig hohe Strahlenexpositionen zu vermeiden. Die derzeitige Einführung der hybriden PET/MRT-Technologie in die klinische Routine kann nicht nur zu einer weiteren Verbesserung der bildgebenden Diagnostik führen, sondern insbesondere auch zu einer Reduktion der Patientenexposition. Aber auch beim Einsatz der MRT sind Gesundheitsrisiken für die Patienten sorgfältig zu berücksichtigen. Dieser Übersichtsartikel soll Radiologen und Nuklearmedizinern die notwendigen biophysikalischen und biologischen Grundlagen für die Risikobewertung von PET/CT- und PET/MRT-Untersuchungen vermitteln. Darüber hinaus werden Überlegungen und Strategien zur Rechtfertigung bzw. Optimierung dieser Bildgebungstechniken vorgestellt, die dem Ziel dienen, die diagnostischen Anforderungen mit denjenigen des Patientenschutzes in Einklang zu bringen.
Ziele: Die Gewebeperfusion kann mit der Methode des steilsten Gradienten (MSG) aus dynamischen kontrastverstärkten CT-Daten berechnet werden. Dieser Ansatz beruht auf der Annahme, dass bis zu dem Zeitpunkt, zu dem die Steigung (der Gradient) der Gewebekurve ihr Maximum erreicht, der Abstrom des applizierten Kontrastmittels (KM) aus der terminalen Strombahn vernachlässigt werden kann. Ist diese Bedingung nicht erfüllt, so wird die Perfusion unterschätzt. Ziel dieser Arbeit war es, die Genauigkeit der MSG anhand von simulierten Datensätzen systematisch zu untersuchen. Methode: Hierzu wurden unter Verwendung einer gemessenen arteriellen Inputfunktion (AIF) 9600 Gewebekurven für 4 Rauschniveaus, 6 Abtaststrategien und 40 Kombinationen der relevanten physiologischen Gewebeparameter (Perfusion, kapillare Permeabilität und relative Verteilungsvolumina des intravasalen und interstitiellen Raums) mittels eines etablierten Simulationsprogramm (MMID4) berechnet. Für jeden Datensatz wurde die Perfusion ermittelt als maximale Steigung der Gewebekurve dividiert durch den Maximalwert der AIF. Ergebnis: Wie erwartet lieferte die MSG systematisch zu niedrige Perfusionswerte, wobei der Fehler mit zunehmender Perfusion und abnehmendem Intravasalvolumen anstieg. Gefäßpermeabilität und interstitielles Volumen hatten dagegen keinen Einfluss auf die Ergebnisse. Bei einer zeitlichen Auflösung von 0,72s war der Fehler kleiner als 23%, wenn die mittlere Transitzeit der KM-Moleküle, d.h. der Quotient aus relativem Plasmavolumen und Perfusion, größer war als 6s. Eine Vergrößerung des Abtastintervalls und des Rauschniveaus resultierte in einer deutlichen Abnahme der Genauigkeit (systematischer Fehler) bzw. der Präzision (Streuung) der Schätzwerte. Schlussfolgerung: Die MSG ermöglicht eine einfache und robuste Schätzung der Gewebeperfusion. Die erreichbare Genauigkeit und Präzision ist für viele klinische Fragestellungen ausreichend, da die mittlere Transitzeit in den meisten Geweben, insbesondere in Tumoren, hinreichend lang ist.
Ziele: Die pharmakokinetische Analyse dynamischer kontrastverstärkter Bilddaten ermöglicht die Schätzung physiologischer Gewebeparameter wie der Perfusion, der kapillaren Permeabilität und der Verteilungsvolumina. Ein Hauptproblem stellt dabei der „Bias-versus-variance trade-off“ dar: Ist die Anzahl der Modellparameter zu groß, so ist die Parameterschätzung instabil (Variance), ist sie dagegen zu gering, so treten systematische Fehler auf (Bias). Zur Lösung dieses Dilemmas wurde von Akaike ein informationstheoretischer Ansatz entwickelt, mit dessen Hilfe nicht nur das „beste“ unter verschiedenen konkurrierenden Modellen ausgewählt werden kann, sondern die Ergebnisse verschiedener Modelle für die Parameterschätzung genutzt werden können. Ziel unserer Arbeit war die Erprobung dieses Ansatzes im Kontext der dynamischen Bildgebung. Methode: Mittels eines etablierten Simulationsprogramms (MMID4) wurden verschiedene Gewebekurven simuliert und mit drei Modellen analysiert: einem Zwei-Kompartimente-Modell mit vier Modellparametern und zwei reduzierten Modellen mit drei bzw. zwei Parametern. Für die drei Modelle wurden jeweils die Parameter und die Akaike-Gewichte berechnet, die die statistische Evidenz für das jeweilige Modell angeben. Aus diesen Daten wurden dann Modell-gemittelte Gewebeparameter geschätzt. Ergebnis: Im Vergleich zum Zwei-Kompartimente-Modell ergaben die reduzierten Modelle vergleichbare Ergebnisse in Situationen, in denen der Einfluss eines Gewebeparameters auf die Gewebekurven gering war. Der Mehrmodellansatz resultierte in einer deutlichen Verbesserung der Parameterschätzungen. Schlussfolgerung: Das vorgestellte Konzept zur Analyse dynamischer MRT- oder CT-Bilddaten mittels konkurrierender physiologischer Modelle auf der Basis eines mathematisch stringenten informationstheoretischen Paradigmas vermeidet die A-priori-Festlegung auf ein bestimmtes Modell und verbessert sowohl die Genauigkeit als auch die numerische Stabilität der Parameterschätzungen.
Ziele: DCE-MRT Daten werden häufig mit den Zweikompartimentemodellen von Brix und Tofts analysiert. Der Zusammenhang zwischen den Fitparameter und den physiologischen Größen ist jedoch nicht vollständig verstanden und schwer anhand von Patientendaten zu klären. Daher wurde in dieser Arbeit eine Simulationsplattform geschaffen, die den Vergleich beider Ansätze miteinander und mit den physiologischen Größen erlaubt. Methode: Die für die Simulationen benötigten AIFs wurden in den Carotiden zweier Patienten an einem 1,5T MR-Tomographen gemessen und in Konzentrations-Zeit-Verläufe konvertiert. Gd-DTPA wurde einmal als Bolus (Tofts) und einmal als Kurzinfusion (Brix) appliziert. Unter Verwendung der AIFs wurden Gewebekonzentrationskurven mittels des MMID4-Modells für 40 Kombinationen der Gewebeparameter Perfusion (Fp), Permeabilität (PS), Plasmavolumen (Vp) und interstitielles Volumen (Vi) simuliert und in Signal-Zeit-Kurven umgerechnet. Zur Analyse der numerischen Stabilität wurden für jede Parameterkombination zehn Gewebekurven mit unterschiedlichem Rauschsignal erzeugt. Die Gewebekurven wurden dann nach Tofts und Brix analysiert. Ergebnis: Die Transferkonstanten kep (Brix) und Ktrans (Tofts) waren mit Fp und PS positiv korreliert, wobei kep stärker durch PS und Ktrans durch Fp beeinflusst war. kep korrelierte zudem negativ mit Vi, jedoch positiv mit Vp. Ktrans korrelierte nicht mit Vp und Vi. Die Verteilungsvolumenparameter A (Brix) und vi (Tofts) waren durch Fp und PS gering beeinflusst, korrelierten jedoch stark positiv mit Vp und Vi. A zeigte eine starke Abhängigkeit von Vp, wohingegen vi von Vp und Vi in gleichem Maße abhing. Die Analyse der numerischen Stabilität zeigte eine höhere Rauschanfälligkeit für den Ansatz von Tofts. Schlussfolgerung: Die Parameter beider Modelle stimmen mit ihren Definitionen gut überein. Der Ansatz von Brix zeigte jedoch eine höhere numerische Robustheit und einen lineareren Zusammenhang zwischen Fitparametern und physiologischen Gewebeparametern als der Ansatz von Tofts.
Anti-angiogenic therapy by blocking VEGF signalling combined with standard chemotherapy is a novel strategy for clinical cancer treatment. The mechanisms for enhanced antitumoral effects are still a matter of controversial debate. Tumor vessel “normalization” upon anti-angiogenic therapy leading to improved drug delivery has been proposed as possible mechanism. Therefore, aim of the study was to investigate tumor microvascular function upon anti-VEGFR treatment in highly vascularized melanomas. A detailed intravital-microscopic analysis of tumor microcirculation including the distribution pattern of vessel diameters and blood flow velocities was performed in melanomas grown in dorsal skinfold chambers of hamsters. Animals with highly vascularized established tumors were treated by a VEGFR tyrosin kinase inhibitor (SU5416) on 3 repetitive days. Tumor tissue oxygenation was measured by phosphorescence quenching technique. Overall tumor microcirculation of subcutaneous tumors was investigated by contrast enhanced MRI (CE-MRI). Vessel density was significantly decreased in treated animals. A significant shift in the distribution patterns towards increased vessel diameters and faster red blood cell velocities in remaining tumor vessels was observed upon anti-VEGF treatment, compensating reduced vascular density. Moreover, a trend towards elevated pO2 levels in treated tumors was observed. Compared to controls, inflow kinetics of tumors quantified by CE-MRI as well as overall uptake of contrast agent in tumor tissue were significantly increased following short-term SU5416 treatment. In conclusion the results confirm temporarily improved tumor microvascular function in highly vascularized melanomas upon short term anti-VEGFR treatment leading to enhanced tumor supply and oxygenation potentially improving the efficacy of simultaneous chemo- or radiotherapy.
Neo-vascular targeting by cationic colloidal carriers enables to realize an innovative approach for tumor therapy. EndoTag-2 is a novel vascular targeting agent, comprising the mammalian topoisomerase I inhibitor camptothecin in its carboxylate form complexed to cationic lipid (cationic lipid complexed camptothecin). Here we studied tumor vascular targeting properties, antitumoral effects and mode of action of EndoTag-2. Tumor vascular targeting properties of fluorescently labelled EndoTag-2 were investigated by in vivo microscopy using A-MEL-3 tumors grown in the dorsal skinfold chamber preparation and by fluorescence histology of s.c. LLC-1 carcinomas. Therapeutic effects have been investigated in the s.c. LLC-1 carcinoma model and the L3.6pl human pancreatic cancer model implanted orthotopically in athymic nude mice. Antivascular effects have been studied by histological investigation of tumor microvessel density and non invasive investigation of tumor blood flow by dynamic contrast enhanced MRI imaging (DCE-MRI). EndoTag-2 selectively targeted tumor microvessels as confirmed by quantitative fluorescence microscopy. Compared to controls EndoTag-2 revealed remarkable antitumoral efficiency in s.c. LLC-1 carcinomas implanted in C57/Bl6 mice. Growth and metastasis of orthotopic L3.6pl human pancreatic tumors was significantly inhibited by EndoTag-2 treatment. Quantitative analysis of tumor microvessel density revealed significant reduction of microvessel density in lewis lung carcinomas up to 50%. DCE-MRI confirmed significant reduction of intratumoral vascular volume as well as tumor perfusion upon EndoTag-2 treatment. In conclusion this study shows that cationic lipid complexed camptothecin (EndoTag-2) is a markedly active antitumor agent based on an innovative vascular targeting approach.
Ziele: Bei der tracerkinetischen Analyse kontrastverstärkter Bildserien müssen bei der MRT – im Gegensatz zur CT – verschiedene biophysikalische Aspekte berücksichtigt werden, wie z.B. der Typ und die Messparameter der verwendete Bildgebungssequenz, die Relaxivitäten in den verschiedenen Gewebekompartimenten sowie der Wasseraustausch zwischen ihnen. Da es aufgrund fehlender Daten z.Z. nicht möglich ist, den Einfluss der genannten Determinanten auf die Schätzung physiologischer Gewebeparameter zu quantifizieren, war es das Ziel unserer Studie, für Muskelgewebe die aus dynamischen MRT- und CT-Bildserien berechneten Parameter miteinander zu vergleichen. Methode: Zu diesem Zweck wurden Signal-Zeit-Verläufe im peripheren Muskelgewebe bei Patienten analysiert, die im Rahmen vorangegangener MRT- (n=8) bzw. CT-Studien (n=6) untersucht wurden. Die MRT-Messungen erfolgten mit einer stark T1-gew. SRTF-Sequenz an einem 1,5-T-System nach Gabe von 0,1 mmol/kg-KG Gd-DTPA; die CT-Messungen an einem Spiral-CT-Scanner nach Injektion von 50ml Iopromid. Pro Messung wurden insgesamt 128 MR- bzw. 36 CT-Bilder über einen Zeitraum von jeweils 6min akquiriert. Vor Kontrastmittelgabe wurden 6 SRTF-Datensätze mit unterschiedlichen Recovery-Zeiten akquiriert, um die für die Konvertierung der gemessenen Signal-Zeit- in Konzentrationszeit-Zeit-Verläufe benötigten T1-Zeiten zu ermitteln. Anschließend wurden mit einem 2-Kompartimente-Modell neben den relativen Verteilungsvolumina des intravasalen und interstitiellen Raums (fp bzw. fi) der Blutfluss und das Permeabilitäts-Oberflächen-Produkt pro Plasmavolumen (F/Vp bzw. PS/Vp) berechnet. Ergebnis: Die Analyse der MRT-Daten ergab für Muskelgewebe folgende Werte: fp=0,04 +/- 0,01, fi=0,09 +/- 0,01, F/Vp=2,4 +/- 1,3 1/min und PS/Vp=1,2 +/- 0,5 1/min; die Auswertung der CT-Daten: fp=0,04 +/- 0,01, fi=0,12 +/- 0,01, F/Vp=2,5 +/- 0,9 1/min und PS/Vp=1,6 +/- 0,6 1/min. Schlussfolgerung: Für Muskelgewebe stimmen die vom verwendeten Kontrastmittel unabhängigen Gewebeparameter fp, fi und F/Vp gut miteinander überein. Dies ist ein Indiz dafür, dass die bei der MR-Analyse gemachten Annahmen (u.a. gleiche Relaxivitäten in und schneller Wasseraustausch zwischen Gewebekompartimenten) zumindest für diesen Gewebetyp in guter Näherung erfüllt sind. Deutliche Unterschiede ergaben sich erwartungsgemäß bezüglich der vaskulären Permeabilität, die von der Größe und den physiko-chemischen Eigenschaften des verwendeten Kontrastmittels abhängt.
Ziele: Bei hoher Morbidität und geringer Mortalität des Prostatakarzinoms ist die Diagnostik derjenigen Patienten, die eine Behandlung benötigen schwierig. Zudem zeigen Metastasen verglichen mit dem Primärtumor deutlich andere Charakteristika. Daher ist das Ziel der Studie Surrogatmarker der Tumormalignität zu ermitteln und den Einfluss heterotopen Tumorwachstums im Vergleich zum orthotopen Wachstum zu ermitteln. Methode: Es wurden hormon-sensitive (H), hormon-insensitive (HI) und anaplastische (AT1) Dunning-Tumore subcutan am Hinterbein oder orthotop in der Prostata von Ratten induziert.
Ziele: Ziel der methodenorientierten klinischen Pilotstudie war die Entwicklung und Evaluierung einer dynamischen MRT-Technik zur quantitativen Charakterisierung der Mikrozirkulation und Angiogenese in Brusttumoren. Methode: Bei 18 Patientinnen mit 10 histologisch gesicherten Karzinomen und 8 gutartigen Läsionen wurden 128 MR-Bilder aus je zwei Schichten mit einer stark T1-gew. SRTF-Sequenz mit einer Zeitauflösung von 3,25s akquiriert. Während der Messung wurde Gd-DTPA kontinuierlich über 30s i.v. appliziert. Ausgewertet wurden die Signalveränderungen im Tumor, in der Brustmuskulatur und in der Aorta (arterielle Inputfunktion). Unter Verwendung der ebenfalls ermittelten T1-Zeiten wurden die gemessenen Signalveränderungen in KM-Konzentrationen umgerechnet und mittels eines offenen 2-Kompartimente-Modells analysiert. Neben den relativen Verteilungsvolumina des intravasalen und interstitiellen Raums (fp und fi) konnte der Blutfluss F und das Permeabilitäts-Oberflächen-Produkt pro Plasmavolumen (F/Vp bzw. PS/Vp) separat quantifiziert werden. Ergebnis: Für die 10 Karzinome wurden die folgenden funktionellen Gewebeparameter ermittelt: fp=0,20 (0,07), fi=0,34 (0,16), F/Vp=2,4 (0,7) 1/min und PS/Vp=0,86 (0,62) 1/min. Die beste Trennung zwischen Karzinomen und gutartigen Läsionen ergab das Verhältnis PS/F (0,35 vs. 1,23). Bei der Wahl eines Schwellwertes von PS/F=1 wurden alle Karzinome sowie 6 der 8 gutartigen Läsionen richtig klassifiziert. Für die Brustmuskulatur wurden folgende Parameter ermittelt: fp=0,04 (0,01), fi=0,09 (0,01), F/Vp=2,4 (1,3) 1/min und PS/Vp=1,2 (0,5) 1/min. Schlussfolgerung: Der vorgestellt nichtinvasive Ansatz bietet die Möglichkeit, die Mikrozirkulation (F/Vp) und Angiogenese (fp und PS/Vp) in Tumoren umfassend auf der Basis wohldefinierter physiologischer Parameter zu charakterisieren. Der Vergleich der ermittelten Gewebeparameter mit den Ergebnissen anderer Untersuchungsverfahren ergab sowohl für die Karzinome als auch für das Muskelgewebe eine gute Übereinstimmung.
Planning of radiotherapy is often difficult due to restrictions on morphological images. New imaging techniques enable the integration of biological information into treatment planning and help to improve the detection of vital and aggressive tumour areas. This might improve clinical outcome. However, nowadays morphological data sets are still the gold standard in the planning of radiotherapy. In this paper, we introduce an in-house software platform enabling us to combine images from different imaging modalities yielding biological and morphological information in a workflow driven approach. This is demonstrated for the combination of morphological CT, MRI, functional DCE-MRI and PET data. Data of patients with a tumour of the prostate and with a meningioma were examined with DCE-MRI by applying pharmacokinetic two-compartment models for post-processing. The results were compared with the clinical plans for radiation therapy. Generated parameter maps give additional information about tumour spread, which can be incorporated in the definition of safety margins.
Summary:Clinical studies demonstrate a gain in diagnostic accuracy by employing combined PET/CT instead of separate CT and PET imaging. However, whole-body PET/CT examinations result in a comparatively high radiation burden to patients and thus require a proper justification and optimization to avoid repeated exposure or over-exposure of patients. This review article summarizes relevant data concerning radiation exposure of patients resulting from the different components of a combined PET/CT examination and presents different imaging strategies that can help to balance the diagnostic needs and the radiation protection requirements. In addition various dose reduction measures are discussed, some of which can be adopted from CT practice, while others mandate modifications to the existing hardand software of PET/CT systems.
Advanced bronchial carcinomas by means of perfusion and peak enhancement using dynamic contrast-enhanced multislice CT are characterized. Twenty-four patients with advanced bronchial carcinoma were examined. During breathhold, after injection of a contrast-medium (CM), 25 scans were performed (1 scan/s) at a fixed table position. Density-time curves were evaluated from regions of interest of the whole tumor and high- and low-enhancing tumor areas. Perfusion and peak enhancement were calculated using the maximum-slope method of Miles and compared with size, localization (central or peripheral) and histology. Perfusion of large tumors (> 50 cm3) averaged over both the whole tumor (P = 0.001) and the highest enhancing area (P = 0.003) was significantly lower than that of smaller ones. Independent of size, central carcinomas had a significantly (P = 0.04) lower perfusion (mean 27.9 ml/min/100 g) than peripheral ones (mean 66.5 ml/min/100 g). In contrast, peak enhancement of central and peripheral carcinomas was not significantly different. Between non-small-cell lung cancers and small-cell lung cancers, no significant differences were observed in both parameters. In seven tumors, density increase after CM administration started earlier than in the aorta, indicating considerable blood supply from pulmonary vessels. Tumor perfusion was dependent on tumor size and localization, but not on histology. Furthermore, perfusion CT disclosed blood supply from both pulmonary and/or bronchial vessels in some tumors.
Summary Aim: Improvement of the spatial resolution in positron emission tomography (PET) by incorporation of the image-forming characteristics of the scanner into the process of iterative image reconstruction. Methods: All measurements were performed at the whole-body PET system ECAT EXACT HR+ in 3D mode. The acquired 3D sinograms were sorted into 2D sinograms by means of the Fourier rebinning (FORE) algorithm, which allows the usage of 2D algorithms for image reconstruction. The scanner characteristics were described by a spatially variant line-spread function (LSF), which was determined from activated copper-64 line sources. This information was used to model the physical degradation processes in PET measurements during the course of 2D image reconstruction with the iterative OSEM algorithm. To assess the performance of the high-resolution OSEM algorithm, phantom measurements performed at a cylinder phantom, the hotspot Jaszczack phantom, and the 3D Hoffmann brain phantom as well as different patient examinations were analyzed. Results: Scanner characteristics could be described by a Gaussian-shaped LSF with a full-width at half-maximum increasing from 4.8 mm at the center to 5.5 mm at a radial distance of 10.5 cm. Incorporation of the LSF into the iteration formula resulted in a markedly improved resolution of 3.0 and 3.5 mm, respectively. The evaluation of phantom and patient studies showed that the high-resolution OSEM algorithm not only lead to a better contrast resolution in the reconstructed activity distributions but also to an improved accuracy in the quantification of activity concentrations in small structures without leading to an amplification of image noise or even the occurrence of image artifacts. Conclusion: The spatial and contrast resolution of PET scans can markedly be improved by the presented image restauration algorithm, which is of special interest for the examination of both patients with brain disorders and small animals.
In view of an increasing use of breast MRI supplementing X-ray mammography, the purpose of this study was the development of a method for fast and efficient analysis of dynamic MR image series of the female breast. The image data sets were acquired with a saturation-recovery-turbo-FLASH sequence facilitating the detection of the kinetics of the contrast agent concentration in the whole breast with. In addition, a morphological 3D-FLASH data set was acquired. The dynamic image data sets were analyzed by tracer kinetic modeling in order to describe the physiological processes underlying the contrast enhancement in mathematical terms and thus to enable the estimation of functional tissue specific parameters, reflecting the status of microcirculation. To display morphological and functional tissue information simultaneously, a multidimensional real-time visualization system (using 3D-texture mapping) was developed, which enables a practical and intuitive human-computer interface in virtual reality. The spatially differentiated representation of the computed functional tissue parameters superimposed on the anatomical information offers several possibilities: improved discernibility of contrast enhancement; inspection of the data volume in 3D-space using the features of rotation and transparency variation; localization of lesions in space and thus fast and more natural recognition of topological coherencies. In a feasibility study, it could be demonstrated that multidimensional visualization of contrast enhancement in virtual reality is a practicable idea. Especially, detection and localization of multiple breast lesions may be an important application.
Fragestellung. Dynamische MR-Untersuchungen (dMRT) unter Einsatz von Gd-Kontrastmitteln werden zunehmend erfolgreich zu diagnostischen Zwecken eingesetzt. Ziel dieser Studie war es, unterschiedliche Verfahren zur Quantifizierung der Kontrastmittelanreicherung aufzuzeigen und vergleichend zu bewerten.
Summary Aim: For kinetic modelling of dynamic PET data, the arterial input function can be determined directly from the PET scans if a large artery is visualized on the images. It was the purpose of this study to experimentally and theoretically determine recovery coefficients for cylinders as a function of the diameter and level of background activity. Methods: The measurements were performed using a phantom with seven cylinder inserts (Ø = 5-46 mm). The cylinders were filled with an aqueous 68Ga solution while the main chamber was filled with a 18F solution in order to obtain a varying concentration ratio between the cylinders and the background due to the different isotope half lives. After iterative image reconstruction, the activity concentrations were measured in the center of the cylinders and the recovery coefficients were calculated as a function of the diameter and the background activity. Based on the imaging properties of the PET system, we also developed a model for the quantitative assessment of recovery coefficients. Results: The functional dependence of the measured recovery data from the cylinder diameter and the concentration ratio is well described by our model. For dynamic PET measurements, the recovery correction must take into account the decreasing concentration ratio between the blood vessel and the surrounding tissue. Under the realized measurement and data analysis conditions, a recovery correction is required for vessels with a diameter of up to 25 mm. Conclusions: Based on the experimentally verified model, the activity concentration in large arteries can be calculated from the measured activity concentration in the blood vessel and the background activity. The presented approach offers the possibility to determine the arterial input function for pharmacokinetic PET studies non-invasively from large arteries (especially the aorta).
AIM:Aim of this study was to demonstrate and compare different quantification techniques to assess contrast enhancement in dynamic MRI studies. The diagnostic potential of dynamic MRI studies is increasingly appreciated and already used in different organ systems.METHOD:A patient population of 314 histologically verified breast lesions (138 malignant, 176 benign) were evaluated using a high temporal resolved dynamic sequence. Different quantification techniques such as the use of a cutoff line, time dependent and pharmacokinetic assessment were comparatively evaluated.RESULTS:Time dependent quantification methods revealed higher diagnostic potential which was further improved by in vivo normalization of the contrast availability in the vascular system. Significant differences in the enhancement characteristics were determined between malignant and benign as well within the different histological entities.CONCLUSION:Time dependent quantification methods enable an angiogenic characterization of lesions to improve diagnostic interpretation as well as monitoring during therapy. They are also the basis for automated, color-coded visualization of dynamic studies.
