La chirurgie des calculs est fréquente avec des stratégies interventionnelles diverses. L'observance réelle des recommandations existantes est peu étudiée. Le volume global par actes est facilement identifiable mais pas la multiplicité des actes par patient (malgré l'impact sur la qualité de vie notamment). Chaîner des séjours permet d'appréhender la prise en charge « en vie réelle ». Identification exhaustive au sein du PMSI national d'un groupe de cliniques des séjours pour calculs urinaires sur une période de 2019 à 2021. – quantification du volume d'actes et distribution régionale, – caractéristiques démographiques, – distribution et moyenne des types de séjours, – proportion de patients avec lithotrities multiples, – maximum et moyenne des endoscopies ou lithotrities pratiquées après endoscopie rétrograde ou chirurgie percutanée première, – réadmission en urgence après urétéroscopie ou chirurgie percutanée – nombre de poses de JJ isolée sur tout le parcours, – taux de séjours ambulatoires. Analyse de 50 295 séjours chez 31 209 patients. (0,52 F/H, âge médian : 54 ans) : – 1,6 séjours en moyenne [1–20], – Séjours sans acte : 13 % des séjours, 19,6 % des patients [par région de 6,4 à 37,2 %], – 24,7 % des patients ont eu au moins une LEC (M : 1,4 [ET0,6 ; 1-12]) dont plus de 3 chez 1,7 % (6,8 % du sous-groupe) (Tableau 1), – 63,3 % des patients ont eu au moins une URS (M : 1,3 [ET0, 1-19]), plus de 3 chez 3 % (4,7 % du sous-groupe) (Tableau 2). Après 12,820 URS « première » : – 63,3 % des patients ont eu au moins une URS (M : 1,3 [ET0, 1-19]), plus de 3 chez 3 % (4,7 % du sous-groupe) (Tableau 2). – 3,2 % des patients ont eu 1,3 LEC en moyenne [ET0,6], – 24,2 % ont eu 1,3 URS en moyenne [ET0,7]. Seulement 26 séjours de NLPC « première », suivies d'autres actes chez 10 (Fig. 1). L'échantillon est bien représentatif en termes démographiques et en volume. L'analyse exclusivement quantitative est une des limites. – la multiplication de séjours est un évènement plus rare qu'attendu, – 16 % de JJ « isolées », – quelques disparités locales : notamment séjours sans acte, – la NLPC de première intention est rare : sous utilisée ?
Faire le point sur la prise en charge chirurgicale des calculs rénaux et/ou urétéraux bilatéraux. Rapporter les critères d’analyse intervenant dans le choix entre un traitement simultané en un temps et un traitement différé en deux temps. Discuter, préciser les arguments pour défendre l’une ou l’autre des deux stratégies. Analyse de la littérature (Pubmed, Medline, langue anglaise et française) portant sur les traitements chirurgicaux des calculs bilatéraux chez l’adulte. Les critères décisionnels reposent sur le nombre et la taille des calculs, la présence ou non d’anomalies sur la voie excrétrice urinaire supérieure, le terrain et les facteurs de comorbidité, l’environnement et l’expérience du chirurgien. Plusieurs séries (rétrospectives) rapportent les résultats et les complications d’un traitement simultané en un temps des calculs bilatéraux. Il existe peu d’études prospectives comparatives. La LEC bilatérale en un temps n’a plus d’indication, l’URS(S) et la NLPC en un temps donnent des bons résultats (SF), en respectant certaines conditions, avec un taux de complications qui reste significatif (fièvre, douleur, hémorragie) sans être significativement supérieur à un traitement différé en deux temps. Les deux techniques (URSS et NLPC) sont parfois combinées (centre expérimenté à deux équipes). Le traitement simultané en un temps apparaît, en termes de résultats et de morbidité, équivalent à un traitement différé en deux temps sous réserve d’une sélection rigoureuse des patients et de l’exécution de l’acte par des équipes expérimentées. Le traitement différé en deux temps des calculs rénaux et/ou urétéraux bilatéraux reste une approche prudente et sûre, encore d’actualité, à privilégier dans les situations de calculs de gros volumes, d’anatomie difficile de la VEUS, de comorbidité importante et en début d’expérience. Le traitement simultané en un temps apparaît équivalent pour certaines équipes, en termes de résultat et de morbidité, à un traitement différé en deux temps sous réserve d’une sélection des patients (calculs de faible à moyen volume, anatomie simple de la VEUS, sans comorbidité importante) et de sa réalisation dans un centre expérimenté. Au cours d’un traitement simultané en un temps, l’équivalent d’une « check-list » intégrant des critères cliniques et biologiques pourrait être recommandée avant de débuter le traitement de la deuxième unité rénale. Review the surgical management of bilateral renal and/or ureteral stones. Report the analysis criteria involved in the choice between a simultaneous or a sequential delayed treatment. Discuss and specify the arguments for defending the first or the second strategies. Analysis of the literature (Pubmed, Medline, English and French language) of surgical management of bilateral urinary stones in adults. The decision criteria are based on the number and size of the stones, the presence or absence of abnormalities of the upper urinary excretory tract, the comorbidity factors, the environment and the surgeon's experience. Several series (retrospective) report the results and complications of simultaneous treatment of bilateral stones during the same procedure. There are few comparative prospective studies. Simultaneous bilateral LEC has no indication. Simultaneous bilateral URS (S) or PCNL give good results (SF) under certain conditions with a significant rate of complications (fever, pain, bleeding) but not significantly superior to sequential delayed treatment. Both techniques (USSR and PCNL) are sometimes combined (experienced center with two teams). The outcome and the morbidity of the simultaneous treatments appears equivalent to the delayed treatment in two stages when performed by experienced surgeons on selected patients. The sequential delayed treatment of bilateral renal and/or ureteral stones remains a safe approach, which has to be favored in case of large volume stones, difficult anatomy of the upper urinary tract, significant comorbidity and at the beginning of a learning curve. In experienced centers on high selected patients (low to medium volume stones, simple anatomy, without significant comorbidity), the outcome and morbidity of the simultaneous treatment appears equivalent to a sequential delayed procedure. In this case, a checklist incorporating clinical and laboratory criteria may be recommended before starting treatment of the second renal unit.
Abstract Urolithiasis has a great importance in the urologist’s practice: it is a common disease which represent a great part of the urologist’s activity, and stone disease is linked to the beginning of urology as an individual medical specialty and to many technical improvements, from the beginning of endoscopy up to extracorporeal shockwave lithotripsy and modern endourology. Imaging and diagnostic techniques and pain relief are described. Usually stones <5mm will pass spontaneously and are managed conservatively. At 5-10mm medical expulsive therapy with an alpha-adrenergic blocker will shorten symptoms. Stones larger than this are likely to need tailored treatment depending on stone type and location and history, choosing from endoscopy, shockwave lithotripsy, or percutaneous lithotripsy.
Objective. - To evaluate predictors of flexible ureterorenoscopes breakage and damage of their optical beam.Materials and methods. - Retrospective survey, single center on 393 interventions with 4 flexible ureterorenoscopes between January 2009 and March 2013. We analyzed factors linked to patient, pathology and surgical technique.Results.- We identified 21 major accidents, a breakage rate of 5.34% and 76 pixels losses in the maintenance of endoscopes and 10 during the procedure. The only statistically significant predictor of loss was the cumulative duration of operating time since the last repair (P=0.04, OR = 1.001 [1-1001]). For lesions of the optical beam between the procedures, parameters appearing as significant were the ureterorenoscope model (P = 0.01, OR = 2.558, 95% CI [1229-5326]), the use of instruments by the working channel: the laser (P = 0.02, OR = 2.06, 95%Cl [1109-3827]), or the use of endoluminal graspers (P = 0.007, OR = 0.467, 95% CI [0269-0809]). Intraoperatively, the number of open or laparoscopic surgery (P = 0.007, OR = 3.105, 95%Cl [1364-7068]), duration of intervention (P = 0.01, OR= 1.023, 95% CI [1.006-10411) and the cumulative duration of intervention (P = 0.003, OR = 1.001, 95% CI [1-1002]) appeared to be statistically significant.Conclusion. - The only predictor of loss of equipment under repair was the cumulative duration of operation time. Objectifs Les complications les plus frequentes apres ureteroscopie souple (URSS) sont d’origine infectieuse. La pyelonephrite aigue (PA) est l’une des plus graves et necessite souvent une re-hospitalisation non programmee. Les objectifs de ce travail etaient de determiner les facteurs pronostics de PA apres URSS, d’evaluer la frequence des re-hospitalisations pour PA et valider l’interet de l’ECBU preleve la veille de l’intervention pour traiter precocement ces patients. Methodes Les complications infectieuses et les re-hospitalisations non programmees dans le mois suivant l’URSS ont ete colligees et evaluees retrospectivement. Les parametres colliges etaient : âge, sexe, IMC, motif de l’URSS (calcul ou TVES), nombre d’ureteroscopies anterieures, nombre d’interventions anterieures pour calculs, nombre, taille et localisation des calculs, bilateralite, anomalies anatomiques associees, duree d’intervention, JJ preoperatoire, drainage postoperatoire, duree operatoire, duree d’hospitalisation, ECBU a j–1 et prescription d’une antibiotherapie dans la semaine precedant l’URSS. La correlation entre ces variables et l’apparition de PA dans le mois suivant l’URSS etait realisee (StatView 4.5 ; SAS Institute, NC) ( p Resultats Entre 2010 et 2013, 266 patients ont beneficie d’au moins une URSS ( n = 325). Parmi les 325 URSS realisees, nous avons observe 24 PA (7,3 %), 17 survenant avant la sortie d’hospitalisation et 7 PA (2,2 %) necessitant une re-hospitalisation non programmee. En analyse univariee, les facteurs pronostiques de PNA post-operatoire etaient : la taille des calculs (plus de 14 mm) ( p = 0,03) ; la duree de l’intervention (plus de 70 mn) ( p p p Conclusion La pyelonephrite aigue apres URSS, avec ou sans re-hospitalisation, est une complication rare. La prescription d’une antibiotherapie dans la semaine precedant l’URSS etait un facteur pronostique independant de PA postoperatoire. La pratique systematique d’un ECBU j–1 pourrait permettre une antibiotherapie precoce pour pres de 63 % des patients et peut-etre limiter les re-hospitalisations pour PA apres URSS.
Évaluer les facteurs prédictifs de casse des urétérorénoscopes souples et de lésions du faisceau optique (par l’analyse de la perte de pixels). Étude rétrospective, monocentrique sur 393 interventions successives réalisées avec 4 urétérorénoscopes souples chez 302 patients dans un service hospitalier universitaire entre janvier 2009 et mars 2013. Nous avons analysé les facteurs liés au patient, à la pathologie, à la technique opératoire et au matériel. Nous avons réalisé une régression logistique univariée, puis une analyse multivariée selon la méthode de Backword sur les facteurs statistiquement significatifs en univariée. Nous avons relevé 21 accidents majeurs (taux de casse de 5,34 %). Le seul facteur prédictif de sinistre statistiquement significatif fut la durée cumulée de temps opératoire depuis la dernière réparation (p = 0,04, OR = 1,001 [1–1,001]). Pour les lésions du faisceau optique entre les procédures (76 évènements), les paramètres apparaissant comme prédictifs étaient le modèle de l’urétérorénoscope (p = 0,01, OR 2,558 IC 95 % [1,229–5,326]), l’utilisation d’instruments par la gaine de travail : laser ou pinces endoluminales. En per-opératoire (10 évènements), le nombre de chirurgie ouverte ou cœlioscopique (p = 0,007, OR = 3,105 ; IC 95 % [1,364–7,068]), la durée d’intervention (p = 0,01, OR = 1,023 ; IC 95 % [1,006–1,041] et la durée cumulée d’intervention (p = 0,003, OR = 1,001 ; IC 95 % [1–1,002]) apparaissaient statistiquement significatifs. Le seul facteur prédictif de sinistres du matériel était la durée cumulée de temps opératoire. Il n’a pas été mis en évidence de différence entre les différents urétérorénoscopes. C’est lors de la désinfection des endoscopes qu’ont lieu la majorité des lésions du faisceau optique.

We evaluated the value of combining noncontrast helical computerized tomography (NCHCT) and color Doppler ultrasound in the assessment of the composition of urinary stones. In vitro, we studied 120 stones of known composition, that separate into the five main types: 18 calcium oxalate monohydrate (COM) stones, 41 calcium oxalate dihydrate (COD) stones, 24 uric acid stones, 25 calcium phosphate stones and 12 cystine calculi. Stones were characterized in terms of their Hounsfield density (HU) in NCHCT and the presence of a twinkling artifact (TA) in color Doppler ultrasound. There were statistically significant HU differences between calcium and non-calcium stones (p < 0.001), calcium oxalate stones and calcium phosphate stones (p < 0.001) and uric acid stones and cystine calculi (p < 0.001) but not between COM and COD stones (p = 0.786). Hence, the HU was a predictive factor of the composition of all types of stones, other than for COM and COD stones within the calcium oxalate class (p > 0.05). We found that the TA does not enable differentiation between calcium and non-calcium stones (p > 0.999), calcium oxalate stones and calcium phosphate stones (p = 0.15), or uric acid stones and cystine calculi (p = 0.079). However, it did reveal a significant difference between COM and COD stones (p = 0.002). The absence of a TA is a predictive factor for the presence of COM stones (p = 0.008). Hence, the association of NCHCT and Doppler enables the accurate classification of the five types of stones in vitro.
Urinary schistosomiasis is very frequent, but there are few data upon its epidemiology in western countries. We wanted to describe the cohort from Tenon hospital, in Paris, France, near a big subsaharian community.Methods. - We searched in our clinical files database for "urinary schistosomiasis" encoding.Results. - The cohort comprised 207 men and 34 women seen for the first time at the mean age of 34, mainly for haematuria or LUTS. Patients were mainly native from subsaharian Africa. The lost to follow up rate was 54%. Diagnosis was made on sole endoscopic finding in half of the cases. For non-tumor pathology, were made seven cystoplasties et 12 ureteral dilations. Tumoral pathology was frequent and severe (15/81 from the same age range), mainly represented by urothelial histology (8/14). Imported cases were rare (five cases).Conclusion. - Despite its limitations, different characteristics from this cohort seemed noticeable: frequency of sole lower urinary tract symptoms, frequency and severity of tumoral diseases, mainly with urothelial carcinoma as histology. INTRODUCTION:Urolithiasis is of health economics concern since it is very frequent. However, there is few data upon its issue in France. METHODS:We have analyzed the data issued from the national coding system for in-hospital stays and interventions, using urolithiasis codes and compared between public and private sectors. We have observed evolution of procedures and stays until 2009. RESULTS:Public and private sectors were quite similar in terms of stays numbers (144,324 in 2009, and an evaluated total cost of more than 168 millions of euros). Since 2000, there has been an increase of more than 20% in the number of stays in the public sector and a stagnation in the private one. Public and private sectors appeared different in terms of: (1) stays without intervention (53 vs 26%; p<0.0001); (2) stays without associated diagnosis (5.78 vs 8.41%; p<0.0001). Since 2006, there has been a stagnation for percutaneous and surgical interventions (less than 5% of the number of interventions) whereas there has been a clear increase in endoscopic (+29% in private sector and +16% in public one) and lithotriptic (+19 and +5%) interventions. CONCLUSION:There were strong disparities between public and private sectors. Endoscopic interventions and lithotrity sessions have shown a sustained increase. Surgical and percutaneous interventions have shown a stagnation.
Clinical results in urinary stones management are often reported using the stone-free (SF) rate, which is simple, reproducible and useful to compare techniques or centers. But this index does not take into account costs or patients' quality of life. In a way, SF "pursuit", which cannot be considered as a universal therapeutic goal could increase costs and decrease patients' comfort. We retrospectively reviewed files of stone management to describe costs according to several items and we emphasize the need for a true efficiency index.
We report the case of a 67-year-old patient, renal transplanted for two years, taken care for carcinoma in situ in bladder and urethra, and treated by intravesical instillations with bacillus Calmette-Guerin (BCG). After failure of a first treatment by Amétycine, a treatment by BCG is instituted during nine weeks. Two months after the end of the treatment, the patient died after bone and liver metastatic invasion of urothelial carcinoma. Management of urothelial high-risk tumour among renal transplanted patient is not clear. Despite successful treatments of few patients reported in literature, this failure called the question of the effectiveness of the BCG therapy in renal transplant patient and suggested an earlier attempt at diagnosis with systematic detection and aggressive therapeutic among these immunodepressed patients.
There are six described complications of sickle cell trait or disease : gross hematuria, renal infarction, papillary necrosis, nephrotic syndrome and urine concentration defects.Because of a high frequency of sickle cell trait patients among patients suffering from medullary kidney carcinoma, it has been called the seventh complication. We briefly review physiopatholgy and diagnosis of these complications.
