Introduction: The main objective of this work was to evaluate the usefulness of a post-emergency urology clinic (PEUC). Patients and methods: This was a monocentric, observational, descriptive and retrospective study over three consecutive months (January to March) of three distinct years: 2014, 2015 and 2016. Only patients over 18 years of age who consulted the emergency department for a urological disease were included in the PEUC. Results: A total number of 465 patients were scheduled for a PEUC and out of which only 265 patients (57%) attended the consultation. 222 patients (48%) finally met the inclusion criteria. The most frequent urological emergencies seen in PEUC were renal colic (RC) (45%), acute urine retention (14%) and urinary tract infections (16%) without any difference over the years. There was a significant decrease in the PEUC timeframe over the years: less than 2 weeks for 60% of patients in 2016 compared with 30% and 38% in 2014 and 2015, respectively (P = 0.003). The percentage of diagnosis modification by the urologists remained low (6% over three years, P = 0.94). Treatment modification was mainly surgical (22 %; P = 0.75). The PEUC has enabled follow-up for more than 50% of patients who had been enrolled over time (60%; P = 0.31). Conclusion: The PEUC seems to demonstrate usefulness both in terms of diagnosis and appropriate treatment with mid-term follow-up.
La maladie de Cacchi-Ricci se traduit par des dilatations kystiques des canaux collecteurs contenant dans environ 20 % des calculs. En grossissant ces calculs, peuvent éroder la paroi kystique, et passer dans la voie excrétrice. Le traitement est classiquement effectué en période de crise de coliques néphrétiques. Ce travail propose un traitement préventif des lithiases retro-papillaires : urétéroscopie souple laser avec une incision des ectasies retro-papillaires. Deux patients âgés de 64 et 66 ans chez qui, suite à des lombalgies et des épisodes des coliques néphrétiques, un diagnostic de MCR avait été établi à l’aide d’un uro-scanner. Les patients ont bénéficié d’une urétérorénoscopie souple et une incision au laser holmium des ectasies rétro-papillaires, avec libération, fragmentation et vaporisation des calculs. La suite de la prise en charge a été déterminée par le contrôle radiologique (TDM). Le bilan métabolique et le suivi en néphrologie à partir de 8 semaines du retrait de la sonde double J. Un scanner d’évaluation à 12 mois après l’opération. Chaque patient a eu deux procédures de l’urétéroscopoie souple laser par côté atteinte. La durée médiane de l’intervention était de 91 min [75–107]. L’incision au laser des ectasies rétro-papillaires, est sans danger d’hémorragie car il s’agit d’une zone peu vascularisée. La libération des calculs intra-kystiques permet leur fragmentation et leur vaporisation en prévention des crises de coliques néphrétiques. Le scanner de contrôle à 4 semaines postopératoire définit la suite de la prise en charge : urétéroscopie second look ou ablation de la sonde double J. Nos patients ont été rendus « stone free ». Aucune complication per- ou postopératoire n’a été constatée. Lors de surveillance les patients ont rapporté une diminution des lombalgies sans épisodes de colique néphrétique. Les incisions préventives endoscopiques des ectasies kystiques rétro-papillaires avec une fragmentation et une vaporisation des calculs est une technique facile, sans risque majeur d’hémorragie, permettrait de modifier l’évolution naturelle de la MCR. Cette technique ouvre la porte à un plus vaste projet de recherche afin d’améliorer la prise en charge des patients présentant la maladie de « Rein en éponge ».

Seuls les patients de plus de 18 ans ayant consulté aux urgences pour un motif urologique et relevant de la CPUU ont été inclus. Résultats : Au total, 465 patients ont été programmés en CPUU, dont 265 (57 %) se sont présentés à la consultation. Deux cent vingt-deux patients (48 %) ont finalement répondu aux critères d'inclusion. Les principaux diagnostics relevant de la CPUU étaient principalement les coliques néphrétiques (45 %), la rétention aiguë d'urine (14 %) et les infections de l'appareil urinaire (16 %). On note une diminution significative du délai de CPUU passant à moins de deux semaines pour 60 % des patients en 2016 contre 30 et 38 % respectivement en 2014 et 2015 (p = 0,003). Le nombre de modifications diagnostiques à la sortie de la CPUU reste faible (6 % sur les trois ans ; p = 0,94). La modification thérapeutique par les urologues était principalement chirurgicale (22 % ; p = 0,75). La CPUU a permis d'instaurer un suivi au long cours pour plus de la moitié des patients (60 % ; p = 0,31). Conclusion : La CPUU semble démontrer un intérêt tant sur le plan diagnostique que sur le plan du suivi spécialisé dans notre centre.
Renal stone incidence has progressively increased in industrialized countries, but the implication of Randall plaque in this epidemic remains unknown. Our objectives were to determine whether the prevalence of Randall plaque-related stones increased during the past decades after having analyzed 30,149 intact stones containing mainly calcium oxalate since 1989 (cross-sectional study), and to identify determinants associated with Randall plaque-related stones in patients (case-control study).The proportion of Randall plaque-related stones was assessed over 3 time periods: 1989-1991, 1999-2001, and 2009-2011. Moreover, we analyzed clinical and biochemical parameters of 105 patients affected by calcium oxalate stones, with or without plaque.Of 30,149 calcium oxalate stones, 10,282 harbored Randall plaque residues (34.1%). The prevalence of Randall plaque-related stones increased dramatically during the past years. In young women, 17% of calcium oxalate stones were associated with Randall plaque during the 1989-1991 period, but the proportion rose to 59% 20 years later (P< 0.001). Patients with plaques experienced their first stone-related event earlier in life as compared with those without plaque (median age 26 vs 34 years, P = 0.02), had increased ionized serum calcium levels (P = 0.04), and increased serum osteocalcin (P = 0.001) but similar 25-hydroxyvitamin D levels. The logistic regression analysis showed that age (odds ratio [OR] 0.96, confidence interval [CI] 0.926-0.994, P = 0.02), weight (OR 0.97, CI 0.934-0.997, P = 0.03), and osteocalcin serum levels (OR 1.12, CI 1.020-1.234, P = 0.02) were independently associated with Randall plaque. The prevalence of the FokI f vitamin D receptor polymorphism was higher in patients with plaque (P = 0.047).In conclusion, these findings point to an epidemic of Randall plaque-associated renal stones in young patients, and suggest a possible implication of altered vitamin D response.
Candiduria may be a marker of serious fungal infections such as pyelonephritis. With the exception of fluconazole and flucytosine, antifungals drugs are not excreted into the urine as active drugs, making the management of infection due to fluconazole-resistant Candida difficult. We report a case of recurrent Candida parapsilosis candiduria in a kidney transplant recipient suffering from chronic ureteral obstruction requiring permanent ureteral catheterization (double-J stent). Attempts to remove the stent led to pyelonephritis episodes during which only Candida was isolated from the urine. Following several courses of azole-based therapy, the causative agent became resistant to fluconazole. Clinical and mycological cure were obtained combining irrigations of caspofungin through a percutaneous calicostomy catheter and oral flucytosine. This strategy may represent an interesting therapeutic alternative in case of fluconazole-resistant symptomatic candiduria.
La ritenzione acuta di urina nell’adulto è una patologia frequente nell’uomo e rara nella donna, che interessa soprattutto i soggetti anziani. La diagnosi si basa sull’esame clinico, di fronte a un quadro che associa, nella maggior parte dei casi, un’incapacità totale di urinare a una massa pelvica mediana dolorosa, il «globo vescicale». In caso di dubbio, la conferma viene mediante ecografia in Pronto Soccorso. Il trattamento in urgenza è il drenaggio vescicale, realizzato mediante cateterizzazione uretrale o catetere sovrapubico. Queste due tecniche hanno ognuna le loro indicazioni e controindicazioni. Un esame clinico iniziale rapido e mirato deve poter far scegliere tra questi due gesti, praticati in un contesto di asepsi rigorosa, secondo una tecnica stereotipata. In un secondo tempo, un esame clinico approfondito ed eventuali esami complementari laboratoristici, radiologici e/o endoscopici permettono di identificare la causa della ritenzione acuta di urina. Il monitoraggio del drenaggio vescicale deve essere rigoroso per evidenziare il prima possibile le potenziali complicanze (sindrome da rimozione di ostacolo, emorragia ex vacuo). Le eziologie sono dominate dagli ostacoli vescicoprostatici nell’uomo anziano (iperplasia benigna della prostata, prostatite acuta, cancro della prostata, coagulo vescicale, ecc.), dai traumi perineali e dalle patologie uretrali nell’uomo giovane (stenosi postinfettive in particolare) e dalle patologie neurologiche e ginecologiche (prolasso, tumori, patologie del postpartum) nella donna. Una causa iatrogena (assunzione di farmaci) deve sempre essere ricercata, in quanto è un fattore scatenante potenziale di qualsiasi patologia sottostante. L’evoluzione e la prognosi successive si concepiscono solamente alla luce del trattamento eziologico.
L’hypercalciurie est retrouvée chez 50 % des patients ayant une maladie lithiasique. Cette hypercalciurie est en rapport avec une hyperabsorption digestive du calcium primitive ou secondaire (à une déminéralisation osseuse). L’hypercalciurie est parfois en rapport avec une hypercalcitriolémie dont l’étiologie est le plus souvent indéterminée. L’objectif de cette étude est d’étudier la prévalence de l’hypercalcitriolémie et ses liens avec l’hyperabsorption digestive du calcium et la déminéralisation osseuse dans une population de patients lithiasiques hypercalciuriques. Un total de 442 hommes lithiasiques hospitalisés pour test de charge en calcium entre 2008 et 2013 ont été analysés rétrospectivement, 315 patients ont été inclus après exclusion des patients ayant une hyperparathyroïdie primaire, une normocalciurie ou une insuffisance rénale. La prévalence de l’hypercalcitriolémie est de 50 % dans notre population. La calcitriolémie est associée positivement au taux de 25-hydroxyvitamine D3 (p < 0,0001) et à l’absorption digestive du calcium (p < 0,001) évaluée par Ca/créatinine urinaire avant et après charge calcique (delta Ca/créat). Il n’est pas détecté de différence en ce qui concerne les facteurs de risque lithiasique ou la minéralisation osseuse. Après ajustement sur l’âge, les déterminants de l’absorption digestive du calcium sont la calcitriolémie (r = 0,17, p = 0,03) et le T-score du radius (r = –0,34, p = 0,02) dans le groupe calcitriol élevé contrairement au groupe calcitriol normal où seul eDFG est significatif (r = 0,26, p = 0,001). La présence d’une hypercalcitriolémie est associée à une hyperabsorption digestive du calcium mais n’est pas associée à une déminéralisation osseuse. Dans le groupe calcitriol élevé, Il existe cependant une association entre absorption digestive et minéralisation osseuse. Cette apparente contradiction est probablement expliquée par une implication du récepteur du calcitriol (VDR). En effet, le phénotype digestif et osseux serait déterminé par l’action combinée du calcitriol et du niveau d’expression tissulaire du VDR.
Objective. - We aim to evaluate the outcome of flexible ureterorenoscopy (F-URS) with holmium laser in treating renal diverticular stone as a minimal invasive option.Patients and methods. - We reviewed retrospectively the records of 45 patients who underwent F-URS using holmium laser from 2004 to 2011 for symptomatic renal diverticular stone. The presenting symptoms were renal colic, urinary tract infection or hematuria. The F-URS used after the Shock Wave Lithotripsy (SWL) failure in 30 (66.7%) patients. The repeated treatment by F-URS was carried out twice for four patients; in three patients, it was correlated to the size of the stone and it was because of failure to identify the diverticulum in one patient. The follow up visit range were between 4 to 6 weeks with plain radiograph (KUB) and renal ultrasound or non-contrast CT scan (NCCT).Results. - Of our data, there were 45 patients, 22 women and 23 men (mean age of 46.3years, range 18-72). We found 28 patients (62.2%) who had stone free (SF), 10 patients (22.2%) with clinically insignificant residual fragments (less than 3 mm in size) and residual fragments (RF) in seven patients (15.6%). Success rate was considered as stone free or insignificant residual fragment, which was obtained in 38 patients (84.4%). The symptom-free was in 42 patients (93.3%).Conclusion. - In our experience, F-URS using holmium laser was very effective minimally-invasive technique that could be the best option for the treatment of renal diverticular stone especially for those who resist the treatment with the SWL. Microsporum gypseum is a common inhabitant of the soil, occasionally responsible for human and animal ringworm. Few reports describe the treatment of dermatologic diseases due to M. gypseum. The objective of this study was to evaluate retrospectively cases of M. gypseum infection in dogs and cats.The occurrence of infection by this dermatophyte was retrospectively evaluated in dermatological specimens from 15,684 dogs and cats dermatologically diseased from Italy. Clinical outcome after treatment with griseofulvin combined with topical enilconazole was evaluated in 41 dogs and, out of label, 10 cats. Furthermore, in vitro susceptibility to griseofulvin and enilconazole was evaluated on 31 clinical isolates of M. gypseum.One hundred and eighty-five specimens out of 15,684 (1.1%) scored positive for M. gypseum. The treatment failed to achieve both mycological and clinical cure in 16 dogs (39%) and four cats (40%), as well as fungal isolates demonstrated a very poor in vitro sensitivity when tested versus griseofulvin: the MIC value was 150 μg/mL. The ED50 value was calculated at 66 μg/mL.Blind treatments with griseofulvin in ringworm due to M. gypseum should be avoided.Microsporum gypseum est un dermatophyte géophile banal, occasionnellement responsable de teigne humaine et animale. Il y a peu d’information bibliographique sur le traitement des dermatophytoses à M. gypseum. Le but de cette étude était d’analyser rétrospectivement des cas de teigne à M. gypseum chez le chien et le chat.L’infection par ce dermatophyte a été évaluée rétrospectivement grâce à 15 684 échantillons provenant de chiens et de chats atteints de dermatose en Italie. Le résultat du traitement par la griséofulvine combinée à l’énilconazole topique a été évalué chez 41 chiens, et, hors AMM, chez dix chats. De plus, la sensibilité in vitro de 31 souches de M. gypseum d’origine clinique a été évaluée vis-à-vis de la griséofulvine et de l’énilconazole.M. gypseum a été identifié dans 185 des 15 684 échantillons (1,1 %). Le traitement n’a pas permis d’obtenir de guérison à la fois clinique et mycologique chez 16 chiens (39 %) et quatre chats (40 %). Les souches isolées ont montré une très faible sensibilité à la griséofulvine, avec une CMI de 150 μg/mL. La valeur de la DE50 était de 66 μg/mL.Il est préférable d’éviter l’utilisation empirique de la griséofulvine dans le traitement de la teigne à M. gypseum.
Urinary schistosomiasis is very frequent, but there are few data upon its epidemiology in western countries. We wanted to describe the cohort from Tenon hospital, in Paris, France, near a big subsaharian community.Methods. - We searched in our clinical files database for "urinary schistosomiasis" encoding.Results. - The cohort comprised 207 men and 34 women seen for the first time at the mean age of 34, mainly for haematuria or LUTS. Patients were mainly native from subsaharian Africa. The lost to follow up rate was 54%. Diagnosis was made on sole endoscopic finding in half of the cases. For non-tumor pathology, were made seven cystoplasties et 12 ureteral dilations. Tumoral pathology was frequent and severe (15/81 from the same age range), mainly represented by urothelial histology (8/14). Imported cases were rare (five cases).Conclusion. - Despite its limitations, different characteristics from this cohort seemed noticeable: frequency of sole lower urinary tract symptoms, frequency and severity of tumoral diseases, mainly with urothelial carcinoma as histology.
