Toxic megacolon is an infrequent, but potentially fatal complication of a fulminant colitis. Toxic colonic dilatation, also caused by ischaemic or infectious inflammation like pseudomembranous colitis, mostly occur in patients with inflammatory bowel disease. Toxic mega-colon is defined as segmental or total colonic distension of >6 cm with the presence of clinical signs of acute colitis and systemic toxicity. Because of the associated high morbidity and mortality the early diagnosis and the management play an important role. The free perforation means a fourfold increase in the mortalitiy of the acute colitis. Recognition of toxic megacolon is underlaying by x-ray of the abdomen with colonic distension and a lack of haustral pattern. Accompanying distension of the small bowel can predict the development of the disease. CT scanning shows a diffuse wall thickening, pericolic inflammation and abnormal haustral pattern and can also detect subclinical perforation or abscesses. The management of toxic megacolon should be with intravenous parenteral nutrition, adaequate supplementation of intravenous fluids and correction of electrolytes abnormalities and the therapy of colitis with corticosteroids. Antibiotics are indicated in infectious disease or bacteriemia and also in colonic perforation. Surgical intervention is indicated by the onset of signs of progression of the disease and complications as perforation, uncontrollable bleeding or distension. The surgical procedure of choice is colectomy and ileostomy. The mortality and morbidity was decreased by avoiding rectal excision. The rectum is closed as a Hartmann's procedure or a mucous fistula is created. A secondary ileoanal pouch can be created at a later date. The interdisciplinary approach with optimal timing of surgical intervention can decrease the morbidity and mortality of toxic megacolon.
The etiology of chronic inflammatoric bowel disease is unknown, conservative treatment will be reduced to the management of immunologic and inflammatoric reaction. The large majority of patients affected with Crohn's disease and also nearly 50% of patients with ulcerous colitis require surgery during their clinical history. The indication for surgery is a balance between the severity and the complications of the disease and the potential disadvantages of surgery. For surgical therapy the differentiation of Crohn's disease and ulcerous colitis is essential because of the differentiated character of both diseases. In Crohn's disease surgical therapy will be performed by the management of Crohn's specific complications like stenosis, fistula, and abscess by bowel preserveration. Limited resection and/or strictureplasty do not influence morbidity and rate and also time of recurrence. Ulcerous colitis can be cured when the colorectal mucosa will be removed totally by radical surgery, i.e. restorative proctocolectomy. The satisfactory outcome in 70 to 90% of patients with a cumulative failure rate of 10 to 15% over a 10-year-period improves the quality of life.
The (dis-)advantages of preoperative chemoradiation in patients with esophageal cancer (EC) are still controversial as data are lacking showing a clear cut benefit. Therefore, data of neoadjuvant therapy of our hospital have been analyzed. Since 1994 102 patients with an EC (33 % adenocarcinoma, 67 % squamous cell cancer, scc) were operated after receiving preoperative chemoradiation (36 Gy radiation, 1.8 Gy/day for 4 weeks, 500 mg/m (2) 5-FU for 4 weeks and 20 mg/m (2) Cisplatin, day 1-5, week 1 and 4). Operation was performed usually 8-10 weeks after treatment start. In 11.7 % of patients with an adenocarcinoma a complete pathological response (CR, pT0N0M0) was observed and a pT0 stage in 20.6 %. 38.2 % of these patients were staged as pN0. Postoperative morbidity was observed in 66 % (anastomotic leakage in 20 %, recurrent nerve palsy in 23 %). In-hospital mortality was 5.9 %. 5-year survival was calculated as 30.5 %, in patients wit a CR 66 %.26.5 % of patients with a scc revealed a CR. However no effect at all was observed in 32 % of these patients. 56 % were staged as pN0. Postoperative morbidity was observed in 87 % (anastomotic leakage in 16 %, recurrent nerve palsy in 32 %). In-hospital mortality was 11.8 %. 5-year survival was calculated as 19.2 %, in patients with a CR 45 %. The impact of pN stage was significant (p = 0.0052). These results underline the benefit of neoadjuvant therapy in patients with a CR. Further on, a pN0 stage is an important prognostic indicator. However, it remains open, whether neoadjuvant therapy leads to a downstaging of lymph node involvement, as histological confirmation in clinically positive lymph node is seldom performed prospectively.
In a 36-year-old male with ileocolic Crohn's disease (CD) no long-lasting remission was obtained by treatment with corticosteroids, mesalazine, azathioprine and antibiotics. Surgical interventions due to relapsing fistulae and abscesses resulted in the removal of >1.5 m of small bowel and left only 40 cm of large bowel. In July 2000, a new fistula and abscess developed. The combination of corticosteroids, mesalazine, ciprofloxacin, metronidazol, azathioprine, formula diet and anti-TNF-alpha antibody largely reduced clinical activity, and resection of fistula and abscess were successful. Despite clinical remission, histology showed activity in the small bowel and the colon. In March 2001, stem cell mobilization chemotherapy with cyclophosphamide was performed. It induced an endoscopic remission for 9 months, which was maintained on azathioprine and corticosteroids. After relapse, in March 2002, high-dose chemotherapy with cyclophosphamide and reinfusion of T-cell-depleted autologous peripheral CD34+ blood stem cells were performed. This led to a complete clinical, endoscopical and histological remission for 9 months without any treatment. Thereafter, endoscopy showed initial aphthous lesions with minimal histological signs of inflammation. The patient is asymptomatic, but low-dose prednisolone and methotrexate are prophylactically given. Immunoablative chemotherapy followed by autologous peripheral blood stem cell transplantation may be a beneficial therapeutic option in complicated refractory CD.
Zielsetzung: Da die Ergebnisse in der Behandlung von ösophaguskarzinomen unbefriedigend sind, wurden multimodale Therapieverfahren eingeführt. Die präoperative Radio-Chemotherapie (RCX) führt bei Patienten mit einem kompletten Tumorresponse (CR) nach Resektion zu einer deutlichen Verbesserung der 5-Jahres-überlebensrate. Umstritten ist, ob die präoperative RCX zu einer erhöhten perioperativen Morbidität/Mortalität führt. Wir haben daher unsere eigenen Ergebnisse der präoperativen RCX überprüft. Material und Methoden: Zwischen 1994 und 1999 erhielten alle Patienten (n = 56) mit einem ösopha guskarzinom T > 1, MO eine präoperative RCX bei gegebener allgemeiner Operabilität. Die postoperative Morbidität wurde unterschieden in operative und allgemeine Komplikationen. Verglichen wurde diese Gruppe mit einer historischen Kontrolle, die zwischen 1991 und 1994 konsekutiv operiert wurden (n = 32). Zur statistischen Analyse wurden uni- und multivariate Verfahren angewendet. Ergebnisse: Bei Vergleichbarkeit der Anzahl transmediastinaler (n = 17) und thorako-abdominaler (n = 71) Zugänge und präoperativer Komorbidität zeigte die multivariate Analyse eine signifikante Zunahme an Rekurrensschäden und Pneumonien nach präoperativer RCX (p = 0,009). Einen Unterschied der Ergebnisse zwischen Adeno- und Platteneepithelkarzinomen (n = 25/63) wurde nicht beobachtet. Die Häufigkeit an Anastomoseninsuffizienzen (34% vs. 22%) war ebenso wenig wie die Rate der perioperativen Krankenhausletalität signifikant erhöht (14% vs. 15%). Unsere Langzeitergebnisse bestätigen ein überlebensvorteil für die Patienten mit einer CR, die in 26% der Patienten erreicht wurde, gegenüber der historischen Kontrolle bzw. den Non-Respondern (p = 0,0051) (median: 27 vs. 14 Monate). Zusammenfassungg: Die präoperative RCX führt zu einer Steigerung der perioperativen Morbidität, die nicht nur Patienten mit einer CR betreffen. Ob die erhöhte Rate an Rekurrensschäden durch die Lymphadenektomie in dem bestrahlten Gebiet hervorgerufen wurde, kann in unserer Analyse nicht bewiesen werden.
Zielsetzung: Die Radio-Chemo-Therapie gilt als Therapie der Wahl zur Behandlung des Analkarzinoms. Als Qualitätsmerkmale dieser Therapie gelten eine möglichst hohe Heilungsrate und Erhalt bestmöglicher Lebensqualität, d.h. die Vermeidung einer Kolostomie bzw. Verlängerung des kolostomiefreien Intervalls. Material und Methoden: In einer retrospektiven Studie wurde bei 52 Patienten ein differenziertes Therapiekonzept wie die RCT nach NIGRO, die Kombination von RCT mit Operation und die Intensivierung der Therapiedauer und Dosierung der RCT verglichen (Cisplatin, 5-FU, 20 Gy). Ergebnisse: Bei 44 Patienten waren die Tumoren im Analkanal und 7mal am Analrand lokalisiert. Die Klassifizierung ergab 7mal ein T1, 28mal ein T2 und 10mal ein T3 und 7mal ein T4 Stadium. 34 Patienten waren nodal negativ, bei 7 fand sich ein N1, bei 6 ein N2 und bei 5 eine N3 Stadium. Dreimal lagen zum Diagnosezeitpunkt Fernmetastasen vor. Bei 31 Patienten ergab die Histologie ein Plattenepithelkarzinom, 21 mal ein Übergangsepithelkarzinom. Bei 19 Patienten wurde nur die RCT. Bei weiteren 19 Kombination mit einer Operation durchgeführt. Operativ wurden 6 Patienten, bestrahlt 4, und weitere 4 konnten nur palliativ therapiert werden. Für das gesamte Kollektiv ergab sich eine 5-Jahres-überlebensrate von 76%, für das tumorfreie Überleben von 65% und mit erhaltener Kontinenz 60% Die 5-Jahres-Überlebensrate für die RCT-Gruppe lag bei 84%, für RCT und OP bei 86% und für intensivierte RCT bei 87%. Die 5-Jahres rezidivfreie Zeit (NED) bzw. die 5-Jahres-Kolostomiefreiheit lag für die alleinige RCT bei 70 bzw. 75% für RCT + OP bei 67 bzw. 56% und nach intensivierter RCT bei 75 bzw. 80%. Zusammenfassungg: Nur die Intensivierung der RCT mit 5-FU, Cisplatin und Aufsättigung auf 50 Gy führt zu einer Verbesserung der NED- und kolostomiefreien Zeit, während die Kombination von RCT mit OP die Ergebnisse nicht verbessert. Eine Verbesserung der 5-Jahres-Überlebensrate war nicht signifikant.
Klinisches Merkmal gastrointestinaler Stromatumoren (GIST) ist das variable biologische Verhalten, welches von benignen Zufallsbefunden bis zu metastasierenden Tumoren reicht. In dieser Arbeit wurden klinischer Verlauf und pathohistologische Parameter von 26 Patienten retrospektiv untersucht mit dem Ziel, Prognosefaktoren zu definieren. Das Risiko eines Tumorrezidives war bei Tumoren > 4 cm signifikant (p = 0,024) erhöht. Ein erhöhter Proliferationsindex gemessen mit dem Anti-Ki-67-Antikörper MIB-1 war bei MIB-1 > 3 auch mit einer signifikant (p = 0,0031) gesteigerten Rezidivrate assoziiert. Der MIB-1 Index erscheint daher zur Abschätzung der Prognose geeignet.
In einer retrospektiven Studie wurden 36 Patienten mit zystischer Echinokokkose hinsichtlich Einzugsgebiet, Risikofaktor und chirurgischer Morbidität und Letalität analysiert.70% kamen aus Mittelmeerländern,30% aus Deutschland. Hundehaltung oder Wildtierexposition waren unabhängige Risikofaktoren. 97% der Patienten zeigten einen Leberbefall, ein Patient einen Milzbefall. Bei 77,8% war eine einfache Perizystektomie möglich. Eine Hemihepatektomie rechts war bei 8,3% erforderlich, links bei 5,6%. Lebersegmentresektionen wurden bei 5,6% durchgeführt. Die Lokalrezidivrate lag bei 0%. Die häufigsten Komplikationen waren Abszess (8,3%), Wundinfekt (8,3%), und Biliom (5,5%).Die Mortalität lag bei 2,7% (Sepsis). Schlußfolgernd läßt sich feststellen, daß Morbidität und Letalität die chirurgische Therapie rechtfertigen.
Veränderungen der chirurgischen Technik und ein neues Verständnis der Tumorbiologie erlauben, auch Karzinome des unteren Rektumdrittels kontinenz-erhaltend zu operieren. Voraussetzung hierzu ist ein exaktes präoperatives Staging. Tumorlokalisation, Tumorstadium, Malignitätsgrad und Sphinkterfunktion bestimmen das operative Verfahren. Die komplette Entfernung des Mesorektums führt zur Erhaltung des lateralen Sicherheitsabstandes. Der distale Sicherheitsabstand, dessen Bedeutung in den letzten Jahren relativiert wurde, wird durch intraoperative Schnellschnittuntersuchung gesichert. Als Kontraindikation zur restorativen Rektumentfernung gilt die Infiltration des Sphinkterapparates. Bei präoperativ eingeschränkter Kontinenz führt die restorative Rektumentfernung zu einer Verschlechterung der Kontinenzsituation. Das wesentliche Problem stellt die Verhinderung des Lokalrezidivs dar. Während die komplette Resektion durch die Exstirpation des Mesorektums größtmögliche Sicherheit erreicht, stellen distale Tumorsatellitenknoten im Mesorektum bei partiellen Resektionen ein Risiko dar. Die Lokalexzision von Rektumkarzinomen kann bei T1-Tumoren der Differenzierungsgrade G1 und G2 und korrekter Technik vergleichbare Ergebnisse liefern.
Die chirurgische Therapie des M. Crohn (MC) ist erschwert durch das häufige Auftreten eines Lokalrezidivs. Die Strikturenplastik ist nach wie vor umstritten, da das entzündete Gewebe vollständig in situ verbleibt. Methode: Wir verglichen retrospektiv (1988–1997) die Ergebnisse nach sparsamer Resektion oder Strikturoplastik bei 105 Dünn- und 42 Dickdarmeingriffen bei 93 MC-Patienten. Die statistische Auswertung erfolgte mittels χ2-Analyse und dem nonparametrischen Mann-Whitney-Test. Ergebnisse: Es fand sich kein Unterschied in der Häufigkeit lokaler Rezidive im Dünnund Dickdarm (p = 0.12 und p = 0.48), dem Verbleib von entzündetem Gewebe an den Resektionsrändern (p = 0.46 und p = 0.9) oder dem Intervall bis zum Rezidiv im Dünndarm (p = 0.46). Schlußfolgerung: Die Strikturoplastik ist gleichwertig der sparsamen Resektion und somit ein sinnvolles, darmerhaltendes chirurgisches Verfahren bei MC-Patienten.
Rectal continence preservation became feasible in treatment of cancer of the lower third of the rectum based upon technical evolutions and better understanding of tumor biology. Absolutely necessary precondition is correct preoperative staging. Operative strategy is determined by tumor stage, localization, grade and continence function. In most cases the total mesorectal excision leads to tumor-free circumferential resection margins. Distal safety margins are assured by intraoperative frozen sections. Restorative approaches are contraindicated in case of sphincter infiltration. If continence function is impaired preoperatively, restorative procedures lead to a worse functional result. The local recurrence remains the most important problem in rectal cancer surgery. Safety in resection is achieved by total mesorectal excision. The presence of distal satellite metastasis or lymph node metastasis bear a certain risk for local recurrence after partial resections of middle or upper third rectal cancer. Local resection in T1-Tumors with G1- or G2-Grading may produce comparable results.
Zum Thema Auch wenn konservative Maßnahmen in der Behandlung chronisch entzündlicher Darmerkrankungen den Vorrang haben, werden bei vielen Patienten chirurgische Eingriffe notwendig. Wegen des unterschiedlichen chirurgischen Vorgehens ist präoperativ eine möglichst sorgfältige differenzierende Diagnostik hinsichtlich des Vorliegens eines M. Crohn bzw. einer Colitis ulcerosa erwünscht, was allerdings in 10-15% der Fälle nicht gelingt. Über die chirurgischen Interventionen und ihre Indikationen sowie Komplikationen berichtet die folgende Arbeit. Dabei wird auch über die neuerdings häufiger ausgeführten minimalinvasiven Techniken der laparaskopischen Chirurgie referiert, die dann angewendet werden können, wenn akute Obstruktion, Perforation oder toxische Dilatation zuvor ausgeschlossen worden sind. Für operative Maßnahmen chronisch entzündlicher Darmerkrankungen stellt das Alter keinen limitierenden Faktor dar. Wundheilungsstörungen zählen zu den häufigsten Frühkomplikationen, als Spätkomplikationen nach restaurativer Proktokolektomie werden Stromakomplikationen und die Pouchitis beobachtet, die sogar bei ungestörter Pouchfunktion auftreten kann und deren Genese ungeklärt ist. Die Rezidivraten nach chirurgischer Intervention bei Morbus Crohn und Colitis ulcerosa sind je nach Schweregrad der Erkrankung und elektiver Operation teilweise hoch; auch dies wird differenziert erläutert.
In this retrospective trial, we examined 215 patients with bowel lesions following abdominal injuries. We analyzed the diagnostic procedures, the time to diagnosis, the subsequent surgical therapy, and complications. The diagnosis of bowel lesions remains a diagnostic challenge. All apparative diagnostic procedures (sonography, CT-scan, lavage, laparoscopy, X-ray) fail to diagnose bowel lesions. In our trial, most patients showed clinical signs of Peritonitis leading to diagnosis. Ultrasonographically guided puncture was important, if clinical signs remained unclear. This study underlines the importance of repeated clinical examination for early diagnosis and treatment of bowel injury.
Survival of patients with esophageal carcinoma (EC) can be improved by preoperative neoadjuvant chemoradiation followed by esophageal resection. We introduced this treatment modality in 1994. Treatment results of 25 patients presenting with stage IIa/b or III EC (UICC 1957) were analyzed. After assessment of operability and clinical staging, all patients received radiation therapy (36 Gy, 1.8 Gy/day, days 1-5, weeks 1-4) and chemotherapy (cisplatin 20 mg/m(2), days 1-5, weeks 1 and 4, and 5-fluorouracil 500 mg/m(2), days 1-5, weeks 1-4). Patients were operated upon 8-10 weeks after treatment initiation and clinical restaging. In 3 patients thoracotomy demonstrated inoperability due to local tumor progression into the tracheobronchial tree. Major postoperative morbidity was observed in 72%, 2 patients died after resection while in hospital (8%). The comparison of pretreatment clinical staging with restaging after neoadjuvant therapy revealed tumor down-staging (UICC) in 12 patients, no change in I 1 patients and up-staging in 2. Comparison of restaging after neoadjuvant therapy and pathohistological staging after resection demonstrated down-staging in 2 patients, no change in 8 patients and up-staging in 15 patients. Complete pathohistological remission was observed in 3 patients. Pathohistological findings revealed a different tumor stage (UICC) in 21 patients (11 x up-staging, 10 x down-staging) when compared with pretreatment clinical staging. Mean 2-year survival was 87.5% in patients with remission and 16.5% in patients without remission or progress of disease. The data demonstrate the feasibility of neoadjuvant preoperative chemoradiation in EC. On the basis of staging results some patients benefit from neoadjuvant therapy while others show progress of disease. The impact of neoadjuvant therapy needs to be carefully evaluated to exclude pitfalls due to clinical misstaging.
