ZusammenfassungPatienten mit Spinalkanalstenose und osteoligamentärer Verletzung stellen ein sehr heterogenes Kollektiv dar. Neurologisches Defizit, Ausmaß und Lokalisation der Stenose wie auch des Myelonschadens variieren ebenso wie das Ausmaß und die Lokalisation der osteoligamentären Verletzung. Einheitliche Therapierichtlinien, insbesondere zur Operationsindikation, liegen daher nicht vor. Einigkeit besteht darin, dass instabile Wirbelsäulenverletzungen in stabile operativ überführt werden. Bei Vorliegen einer stabilen Verletzung sind patientenspezifische Faktoren für die Wahl der Therapie entscheidend, da die Studienlage nach wie vor kein ausreichendes Evidenzniveau ergibt.
Ein Wirbelsäulentrauma kann zu einer vorübergehenden oder dauerhaften Einschränkung der Lebensqualität aufgrund neurologischer Beeinträchtigungen führen. Neben der mechanischen Stabilisierung ist die Verbesserung der neurologischen Defizite ein wichtiges Ziel der chirurgischen Intervention. Dieser Artikel befasst sich mit verschiedenen modernen Dekompressionstechniken und deren Wirksamkeit und gibt eine kurze Übersicht nicht-konventioneller Maßnahmen, die auf eine neurologische Erholung abzielen.
Civilian penetrating head injury caused by foreign objects is rare in Germany (Europe), but can result in complex neurovascular damage. We report on a patient who in suicidal intent inflicted on himself a penetrating brain injury near the vertex with a captive bolt gun. A laceration at the junction of the middle to the posterior third of the superior sinus occurred by bolt and bone fragments leading to critical stenosis and subsequent thrombosis. Upon surgery, the proximal and distal sinus openings were completely thrombosed. The sinus laceration was closed by suture and the intraparenchymal bone fragments were retrieved. Postoperative angiography disclosed persistent occlusion of the superior sagittal sinus. The patient did not develop any symptoms due to venous congestion (edema, hemorrhage), suggesting sufficient collateral venous outflow. The patient completely recovered despite the complexity of the lesion.
Die Inzidenz der Schädel-Hirn-Verletzungen in Deutschland beträgt ca. 300 000/Jahr, wovon etwa 10 000 als schwere SHT zu klassifizieren sind. Von diesen versterben trotz guten Traumamanagements und hochentwickelter apparativ-technischer Infrastruktur ca. 2750 Patienten.
Zusammenfassung Hintergrund Etwa 30 % aller Verletzungen der Halswirbelsäule betreffen Atlas und Axis. Frakturen kommen vorwiegend bei älteren Patienten vor, während ligamentäre Verletzungen öfter in jüngeren Lebensjahren diagnostiziert werden. Die Anatomie und damit auch die Pathophysiologie unterscheiden sich von der der mittleren und unteren Halswirbelsäule. Daher gibt es eigenständige Verletzungsklassifikationen und sind besondere Behandlungsverfahren anzuwenden. Therapie Instabile Frakturen des Atlas und des Dens axis werden operativ versorgt. Dabei kommen direkte monovertebrale Osteosynthesen und fusionierende Eingriffe zwischen den Segmenten C1 und C2 zur Anwendung. Die Auswahl des richtigen Operationsverfahrens ist für die Frakturheilung von entscheidender Bedeutung.
Brain metastases from leiomyosarcoma are very rare. We present 2 cases of intracranial metastatic leiomyosarcoma, discussing their clinical details, radiological and histological features and treatment outcome. Metastatic leiomyosarcoma to the brain, however, represents a therapeutic challenge, as these tumors are generally resistant to radiation therapy and systemic chemotherapy. Surgery was the treatment of choice in both cases.

Ipsilateraler minimal-invasiver Zugang zur Lendenwirbelsäule (LWS) mithilfe eines selbsthaltenden tubulären Retraktorsystems, partielle oder vollständige Facettektomie, Implantation von Pedikelschrauben und des TLIF(„transforaminal lumbar interbody fusion“)-Cages unter Schonung der Nervenwurzeln, Fusion, kontralaterale Pedikelschraubeninsertion in minimal-invasiver oder perkutaner Technik. Postoperative Mobilisation nach physiotherapeutischer Anleitung, Röntgenkontrolle nach Mobilisation (LWS im Stehen, 2 Ebenen), Follow-up-Untersuchung (klinisch und radiologisch) nach 6 bis 12 Wochen sowie 1 Jahr postoperativ. Fusionsrate >90 %, vergleichbar mit der offenen TLIF. Komplikationsraten niedriger als bei offener TLIF. Kürzere Bestrahlungszeiten und -dosen, niedrigerer intraoperativer Blutverlust. Geringes chirurgisches Trauma führt zu kürzerer Hospitalisationszeit und Arbeitsunfähigkeit. Signifikante Reduktion des ODI (Oswestry Disability Index) und VAS(visuelle Analogskala)-Scores.
Cervical disc arthroplasty (CDA) is an approved surgical treatment option in selected patients with cervical spinal disc degeneration. Even though CDA is a standard procedure since 20 years, there is a lack of information about long term performance. The published reoperation rates after CDA are low and comparable to anterior cervical fusion. The authors describe a severe failure and dysfunction with a partial core dislocation of a cervical prosthesis into the spinal canal (M6-C, Spinal Kinetics, Sunnyvale, CA, USA). Six years after implantation of a cTDR (cervical Total Disk Replacement) of the M6 type at C4/5 level, a 52 year-old women presented herself with new clinical signs of cervical myelopathy and radicular pain. Complete posterior dislocation of the central core of the implant into the spinal canal was identified as a cause. The failed device was removed completely and an ACCF (anterior cervical corpectomy and fusion) was performed. Intraoperatively, rupture of the posterior portion of the mesh tissue with posterior dislocation of the whole prosthesis core was detected. This is the second described case of a severe implant failure with core dislocation in this type of cTDR device. Even though there are thousands of successful implantations and middle term outcomes, it seems to be necessary to continue with long term radiological follow up to exclude similar failure in this type of prosthesis.
Intracranial arachnoid cysts (AC) can lead to a bony thinning of the surrounding skull. We present a young man, in whom a frontal AC has caused a thinning of the frontal calvaria and of the posterior wall of the frontal sinus. This bony vulnerability allowed a frontal sinusitis to spread intracranially and finally led to a subdural empyema with neurological deterioration. The treatment consisted of an emergency craniotomy with evacuation of the empyema and appropriate antibiotic treatment of the causing pathogen (Streptococcus intermedius).
Frakturen des 1. Halswirbelkörpers (Atlas) sind häufig und treten regelmäßig im Rahmen von Hochrasanztraumata und axialen Stauchungen auf. Neben der Computertomographie zur Beurteilung des knöchernen Frakturverlaufes kann durch die Kernspintomographie eine mögliche ligamentäre Begleitverletzung beurteilt werden. Der vorliegende Fall einer 28-jährigen Patientin zeigt die traumatische Fraktur des vorderen und hinteren Atlasbogens bei gleichzeitiger knöcherner Avulsionsverletzung des Ligamentum transversum atlantis (TAL). Die operative Versorgung dieser instabilen Fraktur erfolgte über eine bewegungserhaltende Osteosynthese (ORIF) des Atlas und es konnte eine atlantoaxiale Fusion vermieden werden. Der postoperative Verlauf war komplikationsfrei und die radiologischen Verlaufsuntersuchungen dokumentierten eine gute atlantoaxiale Stabilität.
Although total disc arthroplasty (TDA) is a common procedure for selected cases of degenerative disc disease, until today there are only two cases of TDA infections reported in the literature. We report three cases of postoperative TDA infections, two developed cutaneous fistulas. To eradicate the infectious site, a staged removal of the device, resolute debridement, and stabilization plus fusion is proposed. Surgeons are challenged by (1) major retroperitoneal vessels adherent to the device, (2) surrounding scar tissue, (3) accompanying retroperitoneal abscess, and (4) technical issues when removing and replacing the implant. A staged multidisciplinary team approach involving vascular and plastic surgery as well as spine specialists is mandatory to achieve good results.
Even though in recent years the number of transoral spinal interventions has decreased in Europe and North America - mainly because of the progress in the drug therapy of rheumatism, there are still pathologies that can only be addressed by a transoral approach. The classical transoral approach can be expanded in collaboration with oral-maxillofacial surgery and ear, nose and throat surgery, but this is rarely necessary. The transoral approach is now mainly used for resection of pathological tissue. Additional stabilisation is often necessary and is performed in Europe and North America via a posterior approach, due to the lack of availability of anterior plate systems. Anterior plate systems are still used in India and China. In these countries, the numbers of transoral operations are generally still increasing. Today the indications for transoral spinal surgery consist mainly of infections and tumours, and more rarely of trauma and congenital malformations of the craniocervical junction. The numbers of surgical interventions for these indications has remained constant in recent years. The most recent advancement is the use of endoscopes and transnasal access. With these techniques, additional alternatives and supplements are available for further reducing the morbidity of transoral surgery. Despite the low number of cases, surgical therapy of the corresponding pathologies can be offered to patients with a calculable risk.
Die zervikale Bandscheibenendoprothetik kann sehr gute Ergebnisse erzielen, wenn präoperativ eine sorgfältige Abklärung erfolgt ist und die richtige Patientenselektion getroffen wird. Dieser Artikel soll als Hilfestellung dienen, die Indikation zur Versorgung mit einer zervikalen Bandscheibenprothese zu stellen, die Implantationstechnik als auch Komplikationsmöglichkeiten erläutern und einen Literaturüberblick über das Outcome nach Versorgung mit einer zervikalen Bandscheibenprothese geben.

Today, decompression and instrumented fusion is the standard of care. However, the complete armamentarium of possible treatment options should be carefully considered, and the adequate procedures and instruments used should be chosen individually, when a decision to operate is made.
