Introduction Colorectal cancer represents 1,271 new cases per year in Morocco, However, over the last 30 years we have witnessed a significant increase in the overall survival of metastatic patients due to the arrival of new molecules. Case Presentation We report the case of a 51 years old african female patient diagnosed for a moderately differentiated adenocarcinoma of the rectosigmoid junction MSS, wild RAS/BRAF, treated with colorectal protection and lymph node dissection, followed by 6 cycles of fulfol- cisplatin as an adjuvant. the follow up revealed the appearance of hepatic masses. The liver biopsy was performed showing a secondary localization of an adenocarcinoma of digestive origin. The patient received four lines of the chemotherapy, anti-EGFR antibodies and immunotherapy following the guidelines. However, since no KRAS NRAS BRAF mutation was detected at the DNA level circulating tumor, no rechallenging of the anti- EGFR was possible. The hepatic progression continued with appearance of pulmonary lesions, the decision was made for a palliative care. Conclusion Our case highlights that due to the emergence of new effective molecules currently available, associated with a dedicated onco-surgical approach, more patients will be able to benefit from prolonged survival that did not seem achievable at the time of diagnosis.
HIV infection greatly increases the risk of developing lymphoma. NHL is present in approximately 3% of the HIV-positive population at the time of their HIV diagnosis. Twenty percent of HIV-positive patients develop NHL. Risk factors for an association of the NHL with HIV include: low CD4 T cell count, high viral load, age, and male. We report the case of a 35-year-old mother of three children having a history of the death of the husband following the neurological complications of AIDS, the patient was subsequently diagnosed with HIV + and then placed on retroviral treatment. After 6 months the patient reported a breast swelling with pelvic pain, a breast biopsy performed showing a large B-cell NHL. The extension assessment reveals localizations: mammary, bilateral ovarian and adrenal right of a lymphomatous process, the BOM is normal and the CD4 rate is 517 / mm3. CHOP-type systemic chemotherapy with intrathecal chemotherapy has been initiated. The post-treatment evaluation was in favor of a complete clinical and radiological response. The prognosis of patients with AIDS-related lymphoma is associated with the stage of the disease, extraganglionic involvement including bone marrow, CD4 cell count, and performance status. Median survival varies from 8 to 20 months, which remains much lower than that of non-HIV-associated lymphomas.

Effectuer une mise au point de la prise en charge du lymphome de MALT gastrique en mettant l’accent sur la place de l’éradication d’Helicobacter pylori (HP).
Ovarian granulosa cell tumors (TGO) are rare neoplasms. They arise from sex cord stromal cells of the ovaries. They are characterized by their slow natural history, and their tendency to relapse long time after the initial diagnosis. Complete staging surgery of the disease is the cornerstone of treatment. Chemotherapy is indicated for localized tumors with a high risk of recurrence, and for recurrent or advanced tumors. Prolonged follow-up is recommended.
Introduction : C’est une tumeur maligne rare qui represente moins de 1% des tumeurs malignes de la vessie. Il survient aussi bien chez l’enfant que chez l’adulte avec une incidence maximale au-dela de 60 ans. Il semble atteindre preferentiellement le sexe masculin avec un sexe ratio 3/1. Son traitement repose essentiellement sur une exerese chirurgicale en monobloc parfois precedee d’une chimiotherapie lorsqu’une chirurgie complete est impossible. Observation : Patiente de 80 ans ayant pour antecedents une taille vesicale, qui presente une hematurie totale avec presence de caillots evoluant depuis 3 ans dans un contexte d’alteration d’etat general. Le bilan etiologique revele une masseau niveau de la vessie et dont la resection complete transuretrale etait en faveur d’un pseudo-sarcome. Un an et demi plus tard, la patiente se represente avec une masse pelvienne palpable et de l’hematurie. La biopsie etait cette fois-ci en faveur d’un leiomyosarcome. Le scanner montrait un processus tres localement avance de la vessie pour lequel la patiente fut operee par pelvectomie anterieure et ureterostomie cutanee.L’examen anatomopathologique de la piece operatoire confirmait le diagnostic du leiomyosarcome vesical de grade1. Discussion : C’est une tumeur chimiosensible. Il est recommande de faire une chimiotherapie neoadjuvante pour reduire la tumeur si elle est localement avancee et ne peut etre facilement extirpable en totalite d’emblee ou pour eviter une chirurgie mutilante. La qualite d’exerese est le facteur pronostique essentiel de recidive locale. Le grade histologique est correle au potentiel metastatique. De nouvelles drogues de chimiotherapie ont recemment demontre leur efficacite notamment la gemcitabine dans le sous-type leiomyosarcome. Conclusion : C’est une entite rare dont le traitement doit obligatoirement s’integrer dans une approche multidisciplinaire dans un centre d’expert. Nous avons pour but, a travers cette observation, de faire une mise au point visant a analyser les caracteristiques cliniques, moleculaires et therapeutiques.
The growing teratoma syndrome (GTS) is defined as a detection of an enlarged mass during or after chemotherapy treatment for germ cell tumor. We report a case of an 18-year-old girl treated for growing teratoma syndrome after chemotherapy for malignant germ cell tumor of the ovary associated with peritoneal gliomatosis. Chemotherapy induced normalisation of alpha-fetoprotein rate whereas there was an enlargement of the mass. Subsequent complete resection was performed, and the patient remained in good control for 60 months. This clinical picture suggested the diagnosis of “GTS”. This syndrome can lead to confusion with progression or relapse of a germ cell tumour because of increase in tumour volume during chemotherapy, so it is important to recognize it.
On estime qu’environ 7 200 nouveaux cancers du pancréas exocrine ont été diagnostiqués en France en 2005. Au moment du diagnostic, 20 % des patients sont atteints d’une tumeur jugée opérable, 30 % d’une tumeur localement évoluée inopérable, et 50 % d’une maladie métastatique. Après exérèse chirurgicale, la durée médiane de survie des patients opérés n’est que de 12 à 20 mois en raison de la fréquence des récidives. La place de la radiothérapie chez les patients atteints de cancer du pancréas opérable ou localement évolué est actuellement controversée. En situation adjuvante, le traitement standard est une chimiothérapie par acide folinique et 5-fluoro-uracile ou gemcitabine pendant six mois. En association avec une chimiothérapie concomitante, la radiothérapie postopératoire permettrait d’améliorer la probabilité de survie des patients en situation de résection tumorale incomplète. Ceci reste à démontrer dans un essai prospectif. La chimioradiothérapie néo-adjuvante est une approche prometteuse mais non validée à ce jour. Pour les tumeurs localement évoluées, il n’existe pas de standard thérapeutique. Une chimiothérapie première par gemcitabine suivie chez les patients en situation de non-progression d’une chimioradiothérapie représente une stratégie thérapeutique séduisante qui est en cours de validation. Alors que dans les premiers essais de radiothérapie pancréatique étaient utilisés de grands faisceaux d’irradiation, la tendance actuelle est à la réduction des volumes traités afin d’améliorer la tolérance. La définition des volumes cibles a été améliorée par l’utilisation de la scanographie de simulation. L’objectif de ce travail était de préciser les particularités radio-anatomiques, les modes d’extension des cancers du pancréas et les principes de l’irradiation conformationnelle tridimensionnelle illustrés par un cas clinique.About 7200 new cases of pancreatic adenocarcinomas are diagnosed each year in France. At the time of diagnosis, an efficient carcinologic surgery will not be possible for nearly 80 % of patients, in relation to loco-regional extension or metastatic dissemination. After surgical resection, the median survival of resected patients ranges from 12 to 20 months, with a high rate of relapses. Currently, the use of radiotherapy for patients with pancreatic cancer is controversial. In adjuvant setting, the standard treatment is six months of chemotherapy with FUFOL or gemcitabine. Chemoradiation (CRT) may improve the survival of patients with incompletely resected tumors (R1). This must be validated in a prospective trial. Neoadjuvant CRT is a promising treatment but always under evaluation. For the treatment of patients with locally advanced tumors, there is not a standart treatment. A strategy of initial chemotherapy followed by CRT for non progressive patients is under evaluation. Whereas in the first trials of CRT large fields were used, the current trend is to reduce the treated volumes to improve tolerance. The delineation of target volumes has been improved by the use of simulation CT. The aims of this work are to precise the radio-anatomical particularities, the pattern of spread of pancreatic cancer and the principles of 3D conformal radiotherapy illustrated with a clinical case.
Peu de données sont disponibles dans la littérature concernant l’administration de la chimiothérapie chez les femmes enceintes, en particulier au sujet de l’association vinorelbine-anthracyclines en néo-adjuvant. L’observation rapportée est celle d’une femme présentant un cancer du sein localement avancé, au deuxième trimestre de grossesse.
The impact cancer occurrence is variable according to the sociocultural issue, specific to each context and each area. In order to determine the psychosocial profile of Moroccan patients that have developed cancers, four studies were performed at the National institute of oncology (INO) in Rabat. These studies were prospective, included between 125 and 1,600 patients and were based on questionnaires developed by a medical oncologist, a psychologist and a sociologist. These studies were focused on the psychosocial characteristics of the Moroccan cancer occurrence on patients, the impact of cancer on the religious practice as well as the impact of cancer and its treatments on patients' sexuality. In this article, we will develop the particular characteristics of moroccan patients that were specific to their sociocultural context.
Resume L’impact de la survenue d’un cancer est variable selon le contexte socioculturel, specifique achaque contexte et a chaque region. Afin de determiner le profil psychosocial des patients marocains atteints de cancer, quatre etudes ont ete menees a l’Institut national d’oncologie (INO) de Rabat. Ces etudes etaient prospectives, incluaient entre 125 et 1 600 patients et etaient basees sur des questionnaires etablis par un panel de specialistes : oncologue medical, psychologue et sociologue. Ces etudes s’interessaient aux caracteristiques psychosociales des patients atteints de cancers marocains de maniere generale, a l’impact de la survenue du cancer sur la pratique religieuse ainsi qu’a l’impact du cancer et de ses traitements sur la sexualite des patients atteints. A travers ces etudes, des caracteristiques specifiques au contexte socioculturel marocain ont ete relevees et vont etre discutees dans cet article.

Only six cases were reported in the literature. We report the case of a 53-years-old woman with a nodule of the right breast, which appeared three months before diagnosis, and quickly increased in volume; an axillary adenopathy on the same side was also detected.The mammography showed a large opacity of almost the entire right breast and a second opacity in the axilla. An abdominal echography showed hepatic secondary localizations. A surgical biopsy was carried out and the morphological and immunohistochimical studies favoured the diagnosis of chondrosarcoma. Primary chondrosarcoma of the breast is extremely rare. However, the diagnosis cannot be retained before eliminating the possibility of a carcinosarcome or a phyllode's tumour with chondro < de differentiation. Morphology together with immunohistochimy allows the differential diagnosis of these tumours. The treatment is surgical, but in the event of metastases, the treatment remains problematic because of chimio-and radioresistance of these tumours.

Les sarcomes mammaires représentent moins de 0,1 % de l’ensemble des tumeurs malignes du sein. Les chondrosarcomes primitifs du sein sont extrêmement rares. Seulement six cas ont été rapportés dans la littérature. Nous rapportons un cas supplémentaire concernant une femme de 53 ans, présentant un nodule au niveau du sein droit augmentant rapidement de volume avec une adé nopathie axillaire du même côté. La mammographie amontré une grosse opacité occupant presque la totalité du sein droit et une deuxième opacité similaire au niveau du prolongement axillaire. L’é chographie abdominale a montré des localisations secondaires au niveau hépatique. Une biopsie chirurgicale a é té réalisée, et l’étude morphologique associée à l’immunohistochimie a conclu à un chondrosarcome. Le chondrosarcome primitif du sein est extrêmement rare. Ce diagnostic ne peut être retenu qu’après avoir éliminé un carcinosarcome et une tumeur phyllode avec différenciation chondroïde. L’étude morphologique associée à l’immunohistochimie permet de les différencier. Le traitement est chirurgical, mais en cas de métastases, la prise en charge reste problématique du fait de la chimio- et de la radiorésistance de ces tumeurs.
