Introduction Le syndrome de Felty, défini par la triade polyarthrite rhumatoïde (PR) séropositive, neutropénie et splénomégalie, est une entité rare (1 % des PR). Sa prise en charge, quand elle s’impose, n’est pas codifiée, en particulier en cas d’échec du méthotrexate. Quelques cas rapportent une efficacité des biothérapies, notamment du rituximab. Dans les cytopénies auto-immunes, on juge généralement l’efficacité du rituximab à 6 mois. Nous rapportons ici le cas d’une patiente présentant un syndrome de Felty avec neutropénie symptomatique chez qui le rituximab a permis d’obtenir une réponse hématologique complète retardée. Une patiente de 67 ans suivie pour une PR depuis 30 ans, traitée par méthotrexate puis léflunomide, développait en 2011 une neutropénie fluctuante, parfois inférieure à 500/mm3, sans complication infectieuse. Le léflunomide fut interrompu. La neutropénie se corrigea temporairement. L’hydroxychloroquine fut arrêté après quelques mois pour intolérance. En 2014, la neutropénie devenait profonde (< 500/mm3) et permanente, associée à une anémie hémolytique auto-immune modérée (10g/dL) avec un test direct à l’antiglobuline positif à IgG à deux croix. L’imagerie révélait une splénomégalie (15cm). L’immunophénotypage lymphocytaire B et T, le myélogramme et la BOM ne retrouvaient ni hémopathie maligne, ni prolifération LGL, ni granulomatose. En 2016, la patiente était opérée en urgence d’une sigmoïdite diverticulaire perforée abcédée. Le G-CSF (filgrastim) normalisait transitoirement le taux de polynucléaires neutrophiles (PNN). En février 2018, du rituximab était administré (500mg j1 j15) à visée rhumatologique (PR active sous corticothérapie) et hématologique (AHAI corticodépendante à 7–10mg/j). La rate mesurait 19cm, les PNN étaient à 160/mm3. L’amélioration rhumatologique était observée dès 3 mois mais à 12 mois la patiente restait neutropénique (autour de 200/mm3). Des injections semestrielles de rituximab furent poursuivies. Après 34 mois de traitement, on notait une ascension du taux de PNN (1640/mm3) et une diminution de la splénomégalie (16cm). Le rituximab fut espacé à 1 injection annuelle fin 2021. Un an plus tard, la PR restait contrôlée (DAS28 à 2,03), les PNN étaient à 2540/mm3, l’hémoglobine à 13g/dL. La physiopathologie du syndrome de Felty est partiellement élucidée. Elle mettrait en jeu un blocage de la granulopoïèse par la production d’anticorps anti-GCSF et l’apoptose intramédullaire induite par des cytokines pro-inflammatoires (interleukine-8, TNFα et interferonϒ) et par les lymphocytes T CD8 cytotoxiques, associée à une destruction périphérique des polynucléaires neutrophiles médiée par des anticorps antigranulocytes et complexes immuns. Dans la littérature, nous avons identifié 15 cas de syndrome de Felty traités par Rituximab. Après une médiane de suivi de 10 mois, une réponse hématologique était constatée pour la majorité des patients, dans un délai médian de 12 semaines, avec 4 patients sur 15 sous schéma d’entretien. Une inefficacité à 6 mois est rapportée dans 3 cas seulement. Deux patients n’ayant pas eu de traitement d’entretien rechutaient (à 5 et 9 mois) et furent retraités de nouveau efficacement par rituximab. Cette efficacité suggère une implication des lymphocytes B CD20+ dans la physiopathologie du syndrome de Felty. Au vu du peu de données disponibles dans la littérature à l’origine d’un très probable biais de publication, ces résultats doivent néanmoins être considérés avec prudence. Concernant notre cas, la réponse hématologique était beaucoup plus retardée, soit 2 ans après l’introduction du rituximab et soutenue depuis 2 ans. Conclusion Le syndrome de Felty grève le pronostic des patients atteints de PR par le surrisque infectieux et son traitement est mal codifié. Le rituximab semble être une option thérapeutique. Notre observation montre que son efficacité peut être très retardée, et qu’il faut savoir persévérer avant de conclure à une éventuelle inefficacité. D’autres thérapies ciblées, comme les anti-TNFα et le tocilizumab, ont également été proposées, d’efficacité variable sur un faible effectif. Leur place dans la stratégie thérapeutique reste à déterminer.
Diagnosis of sciatica mainly relies on pain reproduction by stretching of the lumbar roots since neurological examination and medical history are usually not sufficient to guarantee diagnosis. The Lasègue test is the most popular method, which starts with the straight leg-raising test (SLR). However it is not perfect, and is not always well performed or interpreted. Passive ankle dorsiflexion at the end of the SLR (Bragard test) is more sensitive, but can also remain normal in some cases of sciatica. Other stretching tests can help to recognise lumbar root damage in patients with poorly defined pain in a lower extremity: firstly, the Christodoulides test, i.e. reproduction of L5 sciatic pain by a femoral stretch test; secondly, the Slump test, performed on a patient in a sitting position, by slowly extending their painful leg then passively bending their neck (or the opposite); and thirdly, the Bowstring test, which requires, at the end of the Lasègue test, once the knee has been slightly flexed, pressing on the course of the peroneal and/or tibial nerves in the popliteal fossea to try and reproduce the exact pain felt by the patient. The combination of all these tests takes less than 2minutes, and could improve both the sensitivity and specificity of the physical examination for the diagnosis of sciatica. This article is a review of the limitations of the Lasègue/SLR tests and of the efficacy of these other tests for stretching the lumbar roots.
L’interrogatoire et l’examen neurologique ne suffisent souvent pas au diagnostic de sciatique radiculaire, sur lequel repose surtout la reproduction de la douleur par la mise en tension des racines. Celle-ci est réalisée d’ordinaire seulement par la manœuvre de Lasègue, dont le premier temps, jambe tendue, est dénommé straight-leg-raising (SLR). Le test de Lasègue n’est toutefois pas parfait, ni toujours bien réalisé et interprété. La dorsiflexion de la cheville en fin de test (test de Braggard) peut être plus sensible. Toutefois, ces deux tests peuvent être parfois faussement négatifs, si bien que d’autres tests de mise en tension radiculaire peuvent aider à poser le diagnostic positif d’une souffrance sciatique en présence de douleurs atypiques d’un membre inférieur. Il s’agit premièrement du signe de Christodoulides, qui correspond à la reproduction d’une douleur de topographie L5 lors de la réalisation d’un test de Léri (femoral stretch test) ; deuxièmement, du signe de Slump, qui consiste sur un patient assis, à étendre la jambe douloureuse à l’horizontale puis à fléchir passivement le cou (ou l’inverse) ; et troisièmement, du signe de la corde de l’arc, qui consiste lors de la manœuvre de Lasègue, une fois le genou un peu fléchi après avoir atteint le degré maximal d’élévation passive du membre douloureux, à appuyer sur le trajet des nerfs péroniers et tibiaux dans le creux poplité pour reproduire ou non la douleur connue du patient. La combinaison de tous ces tests pourrait beaucoup améliorer tant la sensibilité que la spécificité de l’examen clinique des sciatiques.
Introduction : Cette etude avait pour objectif de determiner la place de la tritherapie (methotrexate-sulfasalazine hydroxychloroquine) dans la prise en charge des rhumatismes inflammatoires chroniques (RIC). Patients et methodes : Il s’est agi d’une etude retrospective sur une periode de 12 mois (du 1er fevrier 2016 au 31 janvier 2017), portant sur les dossiers des patients traites par une tritherapie pour un RIC au centre hospitalier universitaire (CHU) de Nantes. Resultats : Quatorze patients ont ete inclus dans l’etude. Leur âge moyen etait de 59,36 ans et le sex ratio etait de 0,75. Le methotrexate etait la premiere alternative de traitement (100%). La maintenance moyenne de la monotherapie etait de 40,36 mois, et le motif de changement par la tritherapie etait la perte d’efficacite therapeutique (100%). La tritherapie etait maintenue en moyenne pendant 18,79 mois avant d’etre remplacee par une biotherapie. Cinq patients ont garde leur traitement car ils etaient stabilises et toleraient bien la tritherapie (35,71%). Quatre patients (28,57%) etaient stabilises sous tritherapie mais le traitement a ete arrete pour intolerance digestive (3/4) et cutanee (1/4). Un echec therapeutique a ete observe chez quatre patients (28,57%). Un refus secondaire de la tritherapie a ete note chez une personne (7,14%). Conclusion : La tritherapie a ete prescrite apres echappement therapeutique de la monotherapie et etait efficace chez deux-tiers des patients. Nous avons observe un arret therapeutique pour intolerance medicamenteuse chez environ un tiers des patients. L’indication de la tritherapie doit etre elargie, notamment pour pallier a l’accessibilite reduite de la biotherapie. Background The rebound effect after stopping treatment with denosumab may be associated with rapid loss of the gains in bone mineral density achieved with treatment, high levels of bone remodeling markers, the occurrence of vertebral fractures, and even hypercalcemia. Case presentation A 64-year-old osteoporotic Caucasian woman suffered from a fracture of her second lumbar vertebra in 2004. From January 2005, she was treated with denosumab for 9 years, with good densitometry results for her hip and lumbar areas, and no fractures over the last 6 years of treatment. Ten months after the treatment with denosumab was stopped, a cascade of vertebral fractures, including some in unusual locations (third thoracic vertebra), and multiple rib fractures in a context of hypercalcemia, suggested possible malignancy. A complete evaluation, including systemic, biological, and biopsy analyses, ruled out this hypothesis. The hypercalcemia was associated with normal plasma phosphate and vitamin D concentrations, and a high parathyroid hormone level, with an abnormal fixation of the lower lobe of the thyroid on sesta-methoxy-isobutyl-isonitrile scintigraphy. Histological analysis of the excised parathyroid tissue revealed hyperplasia. The associated thyroidectomy (goiter) led to the discovery of a thyroid papillary microcarcinoma. Conclusions We consider the consequences of this rebound effect, not only in terms of the major loss of bone density (return to basal values within 3 years) and the multiple disabling fracture episodes, but also in terms of the hypercalcemia observed in association with apparently autonomous tertiary hyperparathyroidism. Several cases of spontaneous reversion have been reported in children, but the intervention in our patient precluded any assessment of the possible natural course. The discovery of an associated thyroid neoplasm appears to be fortuitous. Better understanding of the various presentations of the rebound effect after stopping treatment with denosumab would improve diagnostic management of misleading forms, as in this case. Bisphosphonates could partially prevent this rebound effect.
L’arthrose est la plus fréquente des pathologies ostéo-articulaires. Son incidence croissante avec l’âge fait de la population gériatrique une population particulièrement exposée aux formes symptomatiques. La principale conséquence, du fait des douleurs et limitations, est la perte d’indépendance fonctionnelle qui entraîne le sujet âgé dans un cercle vicieux entretenu par la perte musculaire et la dénutrition jusqu’à la chute, dont l’arthrose est un facteur de risque majeur. La prise en charge de l’arthrose du sujet très âgé prend en compte sa douleur et raideur souvent mésestimées, mais aussi son état de base avec ses comorbidités, les limitations préexistantes (fonctionnelles comme cognitives), les paramètres diététiques, musculaires, thérapeutiques, environnementaux et éthiques afin de compléter la prise en charge thérapeutique médicamenteuse, chirurgicale ou de rééducation selon les possibilités. Nous nous limiterons ici aux arthroses primitives périphériques du sujet âgé.Osteoarthritis (OA) is one of the most disabling arthritic condition. Aging is a major risk factor and represent one of the three OA phenotypes currently identified, including mechanical or obesity-related osteoarthritis. In the very elderly, loss of function and pain due to osteoarthritis are primary concerns. Other conditions related to osteoarthritis are associated with aging, as malnutrition and sarcopenia. Management of osteoarthritis in the oldest old require a global evaluation and care. This evaluation emphasizes ethical concerns after a look at the cognitive impairment and functional disability, taking into account often-underestimated pain and stiffness. Assessment of muscular and nutritional parameters seem to be as important as the rheumatologic evaluation in this frail population. Symptomatic care is the best option in the impaired elderly whereas global care with a multidisciplinary approach is preferred in the frail population. Individualized decision making after global assessment is one of the specificities of the very elderly care because of the major risks of safety concerns, especially with pharmacological therapies.
OBJECTIVES:To assess RAPID3 in various rheumatologic conditions, and the impact of pain catastrophising on RAPID3. METHODS:A set of questionnaires, including RAPID3 (0-30) and pain catastrophising score (0-52), was given to all outpatients seen in a one-month period: 518 patients fulfilled the questionnaires, including 127 RA (42% taking biologics), and 135 SpA (58% taking biologics). RESULTS:Mean pain catastrophising was 18.5±12.5, and 19% of patients could be classified as catastrophisers (>30). Higher RAPID3 scores were observed in the 33 osteoarthritis of lower limbs (16.44±5.20), 10 fibromyalgia (15.52±5.53), 47 back-pain (14.88±5.17), 17 osteoarthritis of upper limbs (13.61±7.42), and 38 tendinopathies (12.85±4.38). Lower RAPID3 were observed in the 135 SpA (12.79±6.03), 127 RA (12.18±6.30), 27 miscellaneous disorders (9.83±6.28), 7 entrapment neuropathies (9.81±4.51), 19 systemic connective tissue disorders (8.26±7.04) and 58 osteoporosis (7.85±6.95). Much higher RAPID3 scores were observed in the 19% with high pain catastrophising scores, whatever the conditions, and lower scores in the 15% with disablement benefits. RAPID3 was not associated with age or disease duration, but strongly correlated with daily fatigue, poor sleep, and length of daily pain. CONCLUSIONS:Thanks to progress made in RA and SpA treatment, higher RAPID3 scores were mostly observed in other rheumatic conditions, but co-morbidities and pain catastrophising might contribute to floor effects when assessing rheumatic disorders with RAPID3, hindering the recognition of low disease activity in some RA of SpA patients.
We report here a case of multiple vertebral osteonecroses with intrasomatic gaseous dissection (Kümmell's disease) occurring 1 year after the end of a 10-year course of denosumab treatment for osteoporosis without fractures. Histomorphometry and bone remodeling markers revealed major bone resorption and the persistence of an inhibition of bone formation. The presence of multiple empty lacunae in the bone provided evidence for high levels of osteocyte apoptosis. Osteocytes direct bone resorption (via the RANK/RANK-L/osteoprotegerin system) and formation (Wnt system, with SOST and DKK1) pathways. The vertebral osteonecrosis in our case may, therefore, have resulted from osteocyte apoptosis, decompensated by the sudden reactivation of bone remodeling after the cessation of denosumab treatment.
L’objectif principal de cette étude était de comparer l’efficacité des infiltrations anesthésie locale et corticoïdes versus anesthésie locale et sérum physiologique dans le traitement des anomalies transitionnelles de la charnière lombosacrée (ATLS). Les patients présentant une lombalgie unilatérale cliniquement liée à une ATLS ont été randomisés pour infiltration en double insu lidocaïne et sérum physiologique (LSP) versus lidocaïne et cortivazole (LC) sous scanner. Le critère principal étudié était l’évolution de l’EVA moyenne des 24 dernières heures à 4 semaines. Seuls 16 patients ont été randomisés et 15 étudiés (8 LSP et 7 LC). Il n’existait pas de différence significative entre les 2 groupes sur l’EVA moyenne à S4. Par contre si on regardait l’évolution de l’ensemble des patients, on notait une amélioration significative de l’EVA à S4 par rapport au baseline : EVA 5,52 ± 0,99 à S0 et 3,86 ± 2,55 à S4 (p = < 0,05) ; cette efficacité se maintenait à S12. On notait également une amélioration significative de l’indice Eifel et des différents items du Dallas. Aucun effet secondaire n’a été constaté. Dans les douleurs chroniques évoquant une origine ATLS avec néoarticulation de type II ou IV de la classification de Castellvi une infiltration peut être proposée car elle peut améliorer la douleur et la fonction durablement sans préjuger du mécanisme de l’amélioration.
Objective: The primary objective of this study was to compare the efficacy of local injection of a local anesthetic with a glucocorticoid versus a local anesthetic with saline to treat low back pain due to lumbosacral transitional vertebras (LSTV) with a pseudoarticulation. Methods: A randomized placebo-controlled double-blind study was conducted in patients with unilateral low back pain ascribed clinically to LSTV. Patients were randomized to lidocaine plus saline (LS group) or lidocaine plus cortivazol (LC group) injected locally under computed tomography guidance. The primary outcome measure was the 24-hour mean visual analog scale (VAS) score for low back pain 4 weeks after the injection. Results: Of 16 randomized patients, 15 were included in the analysis, 8 in the LS group and 7 in the LC group. The mean VAS pain score at week 4 was not significantly different between the two groups. In the two groups pooled, the mean VAS pain score decreased significantly from baseline to week 4, from 5.52 +/- 0.99 to 3.86 +/- 2.55 (P <= 0.05). The difference remained significant at week 12. Significant improvements occurred in the EIFEL disability index and items of the Dallas Pain Questionnaire. No adverse events were recorded. Conclusion: In patients with chronic low back pain consistent with a symptomatic LSTV type II or IV in the Castellvi classification, a local injection of lidocaine with or without cortivazol may provide sustained improvements in pain and function. Background Facet joint-mediated pain has been identified as a common cause of lumbar pain. Steroid injections are currently made to treat them, they are usually performed under fluoroscopy or computed tomography guidance. Ultrasound (US) is also appropriate to study facet joints and some studies showed feasibility and efficacy of facet joint injections performed under US-guidance through a transversal approach. On a longitudinal view, facet joints are easy-to-identify as a series of lumps with the joint capsule appearing as a thin hypoechoic line that envelops the joint. Objectives Considering the good visibility of these joints and their capsule on the longitudinal view, we studied the feasibility of US-guided facet joint injections using a longitudinal inline approach. Methods Patients referred to our rheumatology department to receive facet joint injections under fluoroscopy were included. To realize the injection, we first located the accurate lumbar level on a longitudinal median view going through the spinous processes. Facet joints were identified as previously described placing the probe 2–3 cm away from the median line. Then, the needle was inserted to reach the hypoechoic line corresponding to the capsule or, if not visible, the top of the lump formed by the inferior articular process of the superior vertebra overlying the superior articular process of the vertebra below it. When we obtained the bone contact, we injected iodinated contrast medium followed by cortivazol. Finally, we made a lumbar X-ray to analyze the needle position and the quality of the arthrography. The first objective was to assess the number of injections realized in front of the joint. For secondary objectives, we assessed the number of accurate arthrography, the duration of the procedure and the occurrence of adverse events. During US examination, the visibility of the capsule and the presence of osteophytes were collected. Results Thirty-eight patients have been included by two operators. We excluded four patients because of a poor visibility of the spinal structures. Mean age was 58,4 years (range, 30–82) and mean BMI was 25,2 kg/m2 (range, 18–34). US showed osteophytes in 42% and the joint capsule was inconstantly visible (25%). One-hundred and forty-four injections were performed (72 at the L4-L5 level and 72 at the L5-S1 level) and 141 X-ray were analyzed. One-hundred and twenty-three injections (87%) were accurately realized in front of the joint, i.e. in front of the inferior articular process of the superior vertebra. However, a proper arthrogram was obtained in only 35 cases (25%). Mean procedure duration was 8.5 minutes for four injections. Six patients (18%) reported transient pain exacerbation and no severe complication occurred during the first month after the procedure. Conclusions With a longitudinal inline approach, US-guided facet joint injections were feasible and 87% injections were realized right in front of the joint. However, we obtained a correct arthrogram in only 25%. The depth of the target-point, the inconstant visualization of the capsule and the obliquity of the needle probably explain this result. The loosening of total joint replacement is a frequent complication after total hip arthroplasty (THA). Septic osteolysis should always be feared. We report here a rare case of hip arthroplasty infection due to Mycobacterium bovis after Bacillus Calmette-Guerin (BCG) therapy in a context of bladder cancer. A 70-year-old man was referred to the department of rheumatology in September 2015, because of a total functional disability of the left lower limb. He underwent an un-cemented left total hip arthroplasty in 2003 for the treatment of osteoarthritis. We noted a bladder carcinoma treated by BCG instillation in 2013. Concerning his lower-limb, the activity-related pain has progressively intensified. No fever, weight loss, night sweat or other constitutional symptoms had been noticed. The medical examination showed an isolated painless mass in the inguinal fold. Initial x-ray showed a large trochanteric bone defect and a medial femoral cortex osteolysis. A CT-scan revealed a mass located between the acetabulum and the femoral vasculo-nervous package. A puncture of the mass yielded a hematic and purulent material. On day 14 of incubation, a BCG Mycobacterium bovis strain developed on a specific mycobacterial culture. Tuberculosis vaccine is an effective intravesical immunotherapy for superficial urothelial bladder carcinoma. Most patients experience irritative voiding. Flu-like symptoms may occur and are resolved with symptomatic treatment. Osteoarticular complications are rare. Only 8 cases of prosthetic infections due to Mycobacterium bovis are reported. There is no predictive factor that could help identify patients at risk for developing BCG infection. Most of patients present sub-acute or chronic joint pain, with a great difficulty and a long delay for clinicians to make the diagnosis. In our case, the pseudo-tumoral aspect associated with the loosening of the THA showed many similarities with polyethylene wear induced osteolysis with a classic inflammatory granuloma. A regimen of antituberculous drugs was essential alongside with the removal of the mass. Peri-prosthetic infection due to Mycobacterium bovis is a very rare complication of BCG therapy but clinicians should be able to evocate this diagnosis.
Nous avons évalué des objectifs personnalisés prédéfinis avec le patient lors d’une séance d’éducation thérapeutique dans la polyarthrite rhumatoïde (PR). Au total, 54 PR ont été randomisées éducation thérapeutique au patient (ETP) versus mise en liste d’attente (LA). L’évaluation finale comparative a porté sur la résolution des 3 problèmes prédéfinis (EVA). Cinquante-quatre patients ont été évalués à 6 mois. Le critère principal a été résolu pour l’ensemble des 3 thèmes choisis à 76,9 % dans le groupe ETP et 42,4 % dans le groupe LA. Parmi les autres critères évalués positivement et significativement, on retrouve : moins de corticothérapie, plus d’ergothérapie, plus de demande d’aide sociale, plus d’activité physique, les connaissances pour reconnaître une poussée de PR et comment la gérer. Par contre, les connaissances sur les traitements n’ont pas été différentes dans les 2 groupes, de même que les scores RAPID, la fatigue, la raideur, la dépression, l’observance, le nombre de consultations et d’hospitalisations. La satisfaction des patients est excellente (entre 85,3 % et 93,9 %). Cette étude illustre bien la place et la valeur de l’ETP pour résoudre les problèmes paramédicaux spécifiques de chaque PR, notamment en rapport avec ses conséquences physiques et psychosociales, la grande satisfaction des patients qui en découle, et son caractère indépendamment complémentaire de la prise en charge purement médicale de la PR. Une ETP personnalisée est susceptible de mieux répondre aux problèmes spécifiques des patients dans la PR.
L’échographie est couramment utilisée en rhumatologie pour étudier et guider les infiltrations des articulations périphériques. Cette technique peut également fournir des informations utiles sur l’anatomie du rachis lombaire. Des études ont montré que l’examen échographique du rachis était un outil efficace pour faciliter la réalisation de l’anesthésie épidurale. L’objectif de cette étude était de déterminer si le choix du niveau optimal d’infiltration par échographie pouvait faciliter l’injection épidurale de corticoïdes dans les cas où l’infiltration était présumée difficile (IMC>30 kg/m2, âge>60 ans ou scoliose lombaire).
Les fractures de côtes non traumatiques sont probablement largement sous-estimées, alors qu’il s’agit de la 2e cause de fracture ostéoporotique périphérique. Elles peuvent être répertoriées en 5 chapitres : 1) fractures liées à une ostéoporose, sachant que certains facteurs de risque habituels d’ostéoporose sont plus à rechercher, comme l’ostéoporose cortisonique, l’hyperparathyroïdie, et l’anorexie mentale chez les patient(e)s les plus jeunes. Ces fractures ostéoporotiques sont 5 fois moins coûteuses qu’une fracture de hanche ; 2) fractures de fatigue, qui seront à rechercher surtout chez des rameurs, golfeurs, ou adeptes des sports de lancer ; 3) fractures pathologiques, d’origine essentiellement tumorale ; 4) fractures par ostéomalacie, dont le diagnostic repose en fait souvent sur l’imagerie ; 5) enfin, fractures de côtes en rapport avec des traumatismes non avoués ou discrets, comme chez l’alcoolique, après une toux rebelle, ou dans le syndrome de Silverman chez le petit enfant. Dans ce dernier contexte, le diagnostic différentiel avec l’ostéogenèse imparfaite reste difficile, mais les fractures de côtes sont très rarement révélatrices d’une ostéogenèse imparfaite, alors qu’elles sont parmi les plus fréquentes des fractures dans le syndrome de Silverman. Le diagnostic positif de fractures de côtes reste difficile. Les clichés obliques semblent utiles, mais le scanner est référence. Toutefois, l’échographie est un autre moyen simple et performant d’en assurer le diagnostic. Contrairement aux fractures traumatiques, il n’est pas rapporté de complications particulières aux fractures de côtes non traumatiques, en dehors des formes tumorales. Le traitement reste antalgique, mais les techniques algologiques proposées n’ont pas été validées.
Les fractures spontanées du sternum restent rares. Leur diagnostic peut être différé quand les patients sont d’abord vus dans un contexte d’urgence thoracique. Le rhumatologue doit redresser le diagnostic, grâce parfois à la simple palpation quand celle-ci ou de minimes mouvements reproduisent électivement la douleur sternale. On peut classer ces fractures en 5 chapitres. Les fractures sternales par ostéoporose primitive sont rares, et peuvent être facilitées par une hypercyphose induite par la survenue préalable de plusieurs fractures vertébrales dorsales cunéiformes, du fait de la solidarité entre la cage thoracique et le sternum. Les fractures par ostéoporoses secondaires (corticothérapie ou anorexie) sont plus volontiers spontanées. Des fractures de fatigue du sternum ont été rapportées après la pratique intensive du golf, du trampoline ou du « bodybuilding ». Les fractures pathologiques sont d’origine essentiellement tumorale, même si l’ostéomalacie peut aussi induire au sternum des fractures de Looser-Milkman. Des antécédents de massage cardiaque (fractures secondaires quasi constantes), ou de manœuvres de Heimlich, sont en principe facilement retrouvés à l’anamnèse, mais des crises d’épilepsie peuvent ne pas être remémorées. Enfin, un syndrome de Silverman est souvent non avoué chez les proches d’un enfant, voire adulte, maltraité. Le diagnostic radiographique des fractures du sternum peut être difficile : scanner, mais aussi échographie permettent alors de visualiser la fracture. Il n’y a pas de complications rapportées dans les formes non traumatiques et non tumorales. Le traitement reste antalgique, la pose chirurgicale de plaques ne se discutant qu’en cas de pseudarthrose.
Objective: We have evaluated customized objectives, predefined during a therapeutic education session for rheumatoid arthritis (RA).Methods: Fifty-four RA patients were randomised into patient therapeutic education (PTE) group versus waiting list (WL). The final comparative evaluation involved solving 3 predefined problems.Results: Fifty-four were evaluated after 6 months. The main criterion was defined for all three of the chosen themes at 76.9% in the PTE group and 42.4% in the WL group. Among the other positively evaluated criteria were: less corticotherapy, more occupational therapy, more demand for social aid, more physical activity, knowledge of the recognition of an RA attack and how to cope with it. On the other hand, knowledge of the treatments did not differ between the 2 groups nor did the RAPID scores, fatigue, stiffness, depression, compliance, number of consultations and hospitalisations. Patient satisfaction was excellent (between 85.3 and 93.9%).Conclusion: This study is a good illustration of the position occupied and value of PTE in solving the problems specific to each RA case, the resulting high level of patient satisfaction and its independently complementary aspects relative to the purely medical treatment of RA. UNLABELLED:Ultrasound (US) is widely used in rheumatology to study and guide injection of peripheral joints. It can also provide useful information about the anatomy of the lumbar spine. Studies have shown that US examination of the spine was a useful tool to help perform epidural anaesthesia. The purpose of the study was to determine if the selection of the optimum puncture level by US may facilitate epidural steroid injection in case of presumed difficult puncture (BMI>30 kg/m(2), age>60 years or lumbar scoliosis). METHODS:We performed a prospective randomized controlled study. Eighty patients were randomized in two groups: US group (n=40) which underwent a pre-procedure spinal US to determine the optimal lumbar level for injection or control group (n=40) for which the level of injection was determined by palpation. Primary endpoint was the pain during the procedure assessed using the Visual Analogue Scale (VAS). RESULTS:We found a positive correlation between depth of the epidural space and BMI (P<0.001) and a negative correlation between size of the interspinous spaces and age (P<0.01). Visibility of the epidural space was not altered by obesity or age. We observed a trend toward a reduction in pain intensity during the procedure in the US group compared to the control group with a mean difference at -0.94 [-1.90; 0.02] but the difference was not significant (P=0.054). CONCLUSION:US of the lumbar spine was feasible in patients with lumbar conditions even in obese and old ones and allowed the visualization of the epidural space. However, pre-procedure US examination did not reduce pain during the procedure.
L’échographie ostéoarticulaire s’est développée de manière considérable dans le milieu rhumatologique depuis une dizaine d’année. Elle constitue maintenant une aide importante au quotidien tant sur le plan diagnostic que pour guider nos gestes infiltratifs. Cet examen est quasi exclusivement utilisé pour l’étude des articulations périphériques alors que la pathologie rachidienne représente une activité non négligeable en rhumatologie. L’étude de la littérature montre que l’examen échographique du rachis lombaire est faisable. Il est bien entendu indispensable d’utiliser un matériel adapté et d’apprendre l’écho-anatomie du rachis. Dans cette revue, nous proposons dans un premier temps un examen systématique du rachis lombaire. Cet examen permet d’explorer les différentes structures anatomiques du fascia thoraco-lombaire en surface jusqu’à la partie postérieure des vertèbres en profondeur. Sont ainsi visibles les ligaments, les muscles érecteurs du rachis, les articulations articulaires postérieures et les apophyses transverses. Nous montrons ensuite qu’il est possible de réaliser des infiltrations des articulaires postérieures, péri-radiculaires, des ligaments ilio-lombaires et de réaliser un échorepérage avant infiltration épidurale. Les applications diagnostiques sont pour le moment plus limitées mais l’étude et la description systématique d’images pathologiques permettra d’évaluer l’intérêt de l’échographie dans cette indication. Il existe certes des limites techniques à l’exploration du rachis lombaire de part la profondeur des structures étudiées empêchant d’obtenir une résolution fine des images. Cependant, l’amélioration technique des sondes et des machines d’échographie et la formation d’un grand nombre de praticiens ne pourra que favoriser le développement de cette technique dans un futur proche.
