Doses were measured in-vivo at the entrance using semiconductor detectors for patients with head and neck tumors. Regular measurements started on January 1st and continued till June 30th, 2001. Then the evaluation of the discrepancies between the measured and calculated doses was made, and it resulted in the changes in the protocol of in-vivo dosimetry made effective by February 1st, 2002. The collection of the measurements was censored by January 30th 2003. The number of patients in the two groups was 285 (1st) and 407 (2nd), respectively. The results exhibited the not-Gaussian distribution of the measured doses in both groups. The average number of dose checks per patient increased from 4.9 to 6.0 (1st vs. 2nd group). The mean relative difference between the measured and calculated doses was: -1.5% vs. -0.5%, whereas the standard deviation (1 SD) decreased from 6.1% to 5.6%. The Mann-Whitney U test detected a significant difference between the 1st and the 2nd group (p=0.00000), which justified the conclusion that the changes implemented in the protocol improved dose delivery accuracy and reproducibility of irradiation.

Odnosiły się one nie tylko do aspektów technicznych ale i strukturalno-organizacyjnych. Działania w zakresie zapewnienia jakości oraz systematyczna i prawidłowa kontrola jakości pracy ludzi oraz aparatów zapewnia rzetelną i prawidłową terapię za pomocą promieniowania jonizującego. Usuwa wszelkie możliwe niezgodności jakie mogą powstać podczas napromieniania. Powoduje, że pacjent jest poddany terapii w sposób bezpieczny i zgodny z wymogami leczenia choroby. Ponadto gwarantuje sprawność i skuteczność funkcjonowania aparatu. Wynika to bowiem z faktu, posiadania przez pracowników ściśle i jasno sprecyzowanych schematów działania w określonych sytuacjach, co w dużej mierze eliminuje możliwość popełnienia błędu, generującego jednoznacznie pojawienie się zagrożenia. Celem pracy jest opracowanie procedury kontroli i pomiarów procesu radioterapii w Wielkopolskim Centrum Onkologii. Powołany przez Dyrektora Wielkopolskiego Centrum Onkologii, Zespół ds. Zapewnienia i Kontroli Jakości w Radioterapii, za jeden z celów poprawy świadczeń medycznych przyjął opracowanie procedury kontroli i pomiarów procesu radioterapii. Wtym celu przeanalizowano etapy pomiarów i kontroli procesu radioterapii jak również przestudiowano istniejące akta prawne oraz literaturę przedmiotu. Wynikiem prac Zespołu było opracowanie procedury kontroli i pomiarów procesu radioterapii. Proces zapewnienia jakości, a wraz z nim kontrola jakości pozwala na sprawne, skuteczne i wydajne użytkowanie aparatury. Ogranicza jej awaryjność i czas przestoju z powodu uszkodzeń. Praca aparatu i personelu jest bowiem stale monitorowana i kontrolowana według wcześniej przyjętych procedur i instrukcji. Natomiast w przypadku stwierdzenia odchyleń od założonego standardu podejmowane są działania, umożliwiające usunięcie problemu zanim się on pojawi.
to evaluate the feasibility of treatment method (intraperitoneal administration of radiolabelled monoclonal antibody) for patients with gastric cancer. a total of 15 eligible patients, with histologically proven adenocarcinoma of the stomach or gastro-oesophageal junction, who had undergone resection to remove the primary tumour mass, were enrolled in the study. Eight patients were treated with the study drug and 7 were enrolled into the standard care control arm. mean blood radioactivity peaked between 36 and 48 hours, with a mean peak value of approximately 15% ID. Data previously reported on intraperitoneal Y-HMFG1 treatment of patients with ovarian cancer show peak values of 25–35% ID seen at 48 hour post-injection. The present results require confirmation in a large population of gastric cancer patients, but suggest that the transfer of radioactivity into the blood pool is decreased compared with the ovarian cancer patients. a HAMA response was detected in all patients after treatment, and there is some suggestion that is follows a bi-phasic pattem. If as hypothesised, a HAMA response provides a boost to the immune system, leading to a potential longer term benefit, then the HAMA response seen in this study following pemtumomab treatment is encouraging.

Technika leczenia polami płaszczowymi polega na napromienianiu węzłów chłonnych położonych w okolicy nadprzeponowej stanowiącej najczęstszą lokalizację ziarnicy złośliwej.
Współczynnik TAR i SAR pozwalają na obliczenie dawek dla pól nieregularnych lub zawierających osłony poprzez podzielenie ich na wiele pól i policzenie wkładu do całkowitej dawki. Pomiarów na Theratronie 780E dokonano za pomocą fantomu wodnego PTW z oprogramowaniem Mephysto. Pomiary rozpoczęto od najmniejszego pola 5×5 cm i od powierzchni wody. Komora jonizacyjna typu 2271 (0,6 mm) była połączona z dawkomierzem firmy Nuclear Enterprise. Pomiary w wodzie zakończono na największym polu 35×35 cm. Głębokość maksymalna 30 cm. Następnie dokonano w tych samych warunkach pomiarów w powietrzu. Zachowano tę samą geometrię pomiarów. Pomiarów dokonano z nasadką build up, aby zachować równowagę elektronową. Zamierzone współczynniki TAR i SAR dla źródła 60Co porównano z danymi literaturowymi. Dla małych pól, do 10 cm i na głębokości 0,5 cm uzyskano zgodność w granicach 1%. Dla pól większych różnice wzrosły do 3%. Wraz ze wzrostem głębokości pomiaru zgodność malała. Przy dużych głębokościach, powyżej 15 cm rozbieżności w wynikach osiągnęły 40°f. Uzyskane dawki dla tej samej głębokości i pól od pola zerowego do pola 35×35 cm zachowują charakterystykę zgodną z danymi literaturowymi. Wykonane pomiary były zgodne z literaturowymi. Krzywe opisujące zależność TAR i SAR od wielkości pola, osiągają dla dużych pól charakterystyczne plateau. Z analizy wyników wynika, że coraz bardziej uwidaczniają się różnice dla większych głębokości. Okazało się, że zbliżając się z komorą jonizacyjną do dna fantomu, coraz więcej promieniowania rozproszonego od jego dna i stołu terapeutycznego docierało do dozymetru. Dane literaturowe nie uwzględniają tych przyczynków. Promieniowanie pochodzące od stołu powinno być uwzględnione, gdyż w praktyce medycznej pacjent leży na stole terapeutycznym.
We have designed phase I/II human melanoma gene therapy clinical protocol. The aim of the study was to actively immunize HLA-A1 and/or HLA-A2-positive patients with melanoma with an admixture of irradiated autologous tumor cells and allogeneic melanoma cells genetically engineered to secrete IL-6 and sIL-6R in order to elicit or enhance specific and nonspecific anti-melanoma immune responses to autologous tumor cells to eradicate distant melanoma lesions. Irradiation of autologous and allogeneic tumor cells is a key step in preparation of cellular vaccine because of two major reasons, (i) it inhibits cell proliferation which is crucial in the case of autologous cells which may form a tumor; (ii) it increases melanoma vaccine immunogenicity. The aim of the study was to estimate the optimal dose of ionizing radiation which will provide sterilization of both autologous and allogeneic melanoma cells and will ensure cytokine secretion. Human melanoma cells (Mich-1) were transduced with IL-6 and sIL-6R cDNA using double copy bicistronic retroviral vector. Parental and transduced cells were seeded at in six-well tissue culture plates and were irradiated with 10, 50, 100 and 200 Gy. Secretion of both recombinant proteins into culture was analyzed before and 24, 48, 72, 96 h and 6, 7, 10 and 12 days following irradiation. At the same time adherent cells were enumerated, evaluated for viability and proliferation. At 24, 48, 72 and 96 h postirradiation specific IL-6 and sIL-6R mRNA levels were analyzed. Irradiation of gene modified cells inhibited their proliferation in the dose dependant manner. Dose of 50 Gy sufficiently affected cell proliferation, however, for safety reasons we decided to use the dose of 100 Gy for vaccine preparation. Irradiation did not inhibit secretion of IL-6 and sIL-6R. In contrary, on a per cell basis it significantly increased their secretion which lasted 12 days postirradiation. Interestingly, we did not observe dose or time dependent differences in specific mRNA cellular levels suggesting that increased secretion of both proteins is regulated not on the transcriptional but rather on the posttranscriptional level. Taking all these facts into account we concluded that irradiation of tumor cells may provide an effective and safe approach for gene-modified vaccine preparation.
X-ray's doses in the pelvis area was published and discussed in this paper. On the CT scan base, doses was calculated by the treatment planning system Target 2 plus. The analysis of dose distribution was performed on 26 patients with the postsurgical cancer of endometrium. The treatment was combined teletherapy and brachytherapy techniques.
An analysis of three types of detectors used for dose calculation and verification during total body irradiation (TBI), procedure is presented. Doses were traced in ten points representative for body anatomy on beam entry and exit. To verify the calculated doses, a set of semiconductor, thermoluminescent and ionization detectors has been applied. Application of each type of detector has been discussed and their response to dose in TBI condition evaluated. The accuracy of dose determination between 2.6% to 3.3% on entry and 5.8% to 7.7% on exit was achieved depending on the type of detector.
