目的:探讨消眩止晕片治疗颈性眩晕痰瘀内阻证的疗效及对氧化应激和血清内皮素(ET)-1,5-羟色胺(5-HT)和一氧化氮(NO)水平的影响.方法:筛选符合要求就诊的颈性眩晕患者118例为研究对象,参照数字表法随机分为对照组和治疗组各59例.对照组采用正骨联合针刺治疗.治疗组在对照组治疗的基础上口服消眩止晕片,5片/次,3次/d.两组患者均进行3周的连续治疗.比较两组患者颈性眩晕症状与功能评估量表(ESCV)评分、眩晕障碍量表(DHI)评分、椎-基底动脉血流速度、生活质量及临床疗效.检测两组患者血清超氧化物歧化酶(SOD),丙二醛(MDA),ET-1,5-HT和NO水平.结果:治疗后,治疗组ESCV评分显著高于对照组,DHI评分明显低于对照组(P<0.01).治疗组治疗后患者左椎动脉、右椎动脉及基底动脉血流速度均显著高于对照组(P<0.01).治疗组患者的临床总有效率为98.25%,高于对照组的83.93% (P <0.05).治疗后,治疗组患者SF-36量表各项评分均显著高于对照组(P<0.01).治疗后,治疗组患者血清SOD,NO水平均显著高于对照组,MDA,ET-1和5-HT水平均明显低于对照组(P<0.01).结论:在正骨联合针刺治疗的基础上,加服消眩止晕片治疗颈性眩晕疗效显著,调节患者体内氧化应激及血清ET-1,5-HT和NO水平可能与其临床疗效有关.
目的 探讨某院2型糖尿病(T2DM)患者SLC47A1 rs2289669位点基因多态性与二甲双胍降糖疗效的相关性.方法 以180例新发T2DM患者为研究对象,用PCR-限制性片段长度多态性(PCR-RFLP)的方法检测基因多态性位点突变在研究人群中的分布频率.研究人群口服二甲双胍单药治疗T2DM,随访12个月,根据研究人群基因型分组,分析糖化血红蛋白(HbA1c)治疗前后变化情况与各基因型的关系.结果 SLC47A1 rs2289669 AA型、GA型和GG型患者HbA1c在治疗前后变化差异有统计学意义(P<0.05).结论 SLC47A1 rs2289669基因多态性位点突变与T2DM患者二甲双胍降糖疗效有关联.
The Vickrey's bottleneck model adopted a trip-based approach,which cannot consider the linkage between the commuter's activity schedule and trip.This paper extends the Vickrey's bottleneck model to address the departure time choice in the morning peak by introducing an activity-based bottleneck model.This model explicitly considers the commuter's trade-off between the utility received by activities at home and at work and the disutility of travel between activity locations.The optimal time-varying toll and step toll are then investigated by using the activity-based bottleneck models with constant and linear marginal activity utility functions.The optimal toll solutions are also compared with the traditional Vickrey's bottleneck model.The results show that the curve of the optimal solution for the time-varying toll is not piecewise linear but piecewise quadratic when the marginal activity utility is linear.The traditional Vickrey's bottleneck model overestimates the queuing delay at the bottleneck,the step toll level,and the start time and end time of the morning peak period.The optimal step tolls under the trip-based bottleneck model and the bottleneck model with constant marginal activity utility are half of the maximum value of the optimal time-varying tolls and exactly eliminate half of the total queuing delay at the bottleneck.The trip-based bottleneck model and the bottleneck model with constant marginal activity utility underestimate the role of step tolls in removing the bottleneck queues compared to the bottleneck model with the linear marginal activity utility.
OBJECTIVE:To analyze the utilization and variation tendency of oral hypoglycemic agents in Wuhan area,and to provide reference for drug selection and enterprise production and sale. METHODS:The utilization of oral hypoglycemic agents in 34 hospitals from Wuhan area during 2011-2013 was analyzed statistically in respects of consumption sum,DDDs and DDC etc. RESULTS:The consumption sum and DDDs of oral hypoglycemic agents increased by 20% and 7% respectively year by year. Acarbose had a market share of over 40%. The drug use frequency of metformin ranked the second place only to acarbose. DPP-4 inhibitor had a promising future,with a consumption sum proportion of more than 10%,while the market share of traditional Chi-nese medicine had fallen below 3%. Except for repaglinide,gliclazide,glimepiride,Xiaoke pills,etc.,the selling of other medica-tions are consistent with the user. CONCLUSIONS:The utilization of oral hypoglycemic drugs is basically rational in 34 hospitals from Wuhan area during 2011-2013.
OBJECTIVE:To observe the spermicidal effect of alcohol extracts from different ratios of Sophora flavescens Ait/Chinese Bulbul in vitro.METHODS:Semen samples aseptically obtained by masturbation and prepared by density gradient centrifugation from 15 healthy men were incubated in the alcohol extracts from 9 different ratios of Sophora flavescens Ait/Chinese Bulbul for 20 seconds, 2 minutes and 4 minutes. Then the motility and movement parameters of the sperm were detected by computer-assisted semen analysis, and the minimal effective concentrations of the instant spermicidal effect of the extracts were determined.RESULTS:At the ratio of 3:1, the extract at 0.5 mg/ml significantly inhibited the sperm motility and other sperm movement parameters VCL, VSL, VAP, ALH, WOB and MAD, as compared with the control group. The minimal effective concentration of the instant spermicidal effect of the extracts was 3.5 mg/ml at 3:1.CONCLUSION:The alcohol extracts from Sophora flavescens Ait and Chinese Bulbul at the ratio of 3:1 have the best spermicidal effect in vitro.
OBJECTIVE:To study the antifertility and anti-inflammatory effects of compound Kucen gel in vivo. METHODS:As antifertility experiment, we randomly divided 60 female SD rats into six groups of equal number: normal saline, blank gel, low-, medium- and high-dose compound Kucen gel (0.05, 0.10 and 0.15 g/g), and positive control (4% nonoxynol gel) to receive intravaginal administration of 200 microl of respective agent, followed by copulation with male rats in a 1:1 ratio. At 12 days after successful mating, the female rats were dissected for calculation of the embryos and the rate of contraception. As an anti-inflammatory trial, we established a mouse model of inflammation by applying xylene to the pinna, and equally randomized 60 Kunming mice to six groups as in the former experiment. We determined the degrees and average rates of swelling inhibition in the left ear. RESULTS:High-dose compound Kucen gel achieved a fertility-inhibition rate of 100% in the female rats, the number of embryos significantly lower than in the normal saline group (0.00 +/- 0.00 vs 11.00 +/- 2. 00, P < 0.05), but with no statistically insignificant difference from that of the positive control (0.00 +/- 0.00, P > 0.05). High-dose compound Kucen gel also markedly suppressed swelling in the left ear of the mice, with an inhibition rate of 52.3%, the average swelling degree significantly lower than in the normal saline group (10.17 +/- 2.56 vs 21.32 +/- 3.17, P < 0.01), but not remarkably different from that of the positive control (8.53 +/- 1.89, P > 0.05). CONCLUSION:Compound Kucen gel, with its strong antifertility and anti-inflammatory effects, deserves further study and clinical application.
