The mental health risks of internal migrants have become increasingly prominent, yet traditional diagnosis and treatment pathways face structural barriers including the uneven distribution of medical resources, insufficient service accessibility, high time costs, and the stigma surrounding mental health that prevent them from adequately addressing the mental health problems of this population. Cultural participation, as a low-threshold, low-stigma daily practice, may provide preventive support before mental health conditions deteriorate through fostering social connection and cultivating a sense of belonging and identity. Although public cultural services are not designed exclusively for internal migrants, their universally accessible and daily-embedded institutional characteristics may offer this population an alternative pathway for mental health support that complements the barriers to formal help-seeking. The Public Cultural Service Guarantee Law of the People’s Republic of China, as the foundational legislation in this field, establishes a normative framework centered on guaranteeing basic cultural rights and interests, enriching spiritual and cultural life, and advocating healthy and civilized activities. Thus it provides normative space for public cultural services to undertake health promotion functions. In practice, however, resource allocation and performance assessment remain heavily oriented toward physical indicators such as the number of venues, facility size, and event frequency, with insufficient attention paid to whether service content matches users’ actual needs, whether access arrangements accommodate diverse groups, and whether participation processes are conducive to social interaction and emotional regulation. Under this logic, internal migrants are mostly regarded as service recipients rather than subjects of cultural participation, constraining the release of the mental health support potential that public cultural services inherently possess. Accordingly, the cultural participation opportunities, expression rights, and identity recognition needs of internal migrants should be incorporated into the framework of legal protection, and public cultural services should be transformed from general cultural supply into institutionalized public resources oriented toward mental health prevention.
This study aimed to evaluate the prognostic factors influencing the survival of patients with lung cancer identified from a lung cancer screening cohort in the community. A total of 25,310 eligible participants were enrolled in this population-based prospective cohort study, derived from a community lung cancer screening program started from 2013 to 2017. Survival analyses were conducted using the Kaplan–Meier method and the log-rank test. Cox proportional hazards regression models were utilized to identify prognostic factors, including demographic characteristics, risk factors, low-dose CT (LDCT) screening, and treatment information. The screening cohort identified a total of 429 patients with lung cancer (276 men, 153 women) during the study period. The 1-year, 3-year, and 5-year survival rates were 74.4
Metabolic syndrome (MS) is a well-established risk factor for chronic kidney disease (CKD), especially in hypertensive patients. These patients are more likely to exhibit additional components of MS, further elevating their risk of developing CKD. This study aimed to investigate the association between the components of MS and the risk of CKD in hypertensive patients. This retrospective cohort study included 51,470 hypertensive patients who participated in the national basic public health services for hypertension management in Minhang District, Shanghai, China. Cases of CKD were identified through linkage with elderly physical examination statistics. Multivariable-adjusted Cox proportional hazard models were used to assess the relationship between the number of MS components and the risk of CKD. Additionally, the study explored the associations between different combinations of MS components and CKD. During the 3-year follow-up period, the incidence of CKD was significantly higher in hypertensive patients with MS compared to those without MS (33.77 vs. 25.67 per 1000 person-years). This study aims to explore the predictive value of adiposity measures and lipid-related indices for metabolic syndrome (MetS) in Chinese elderly people at high risk of stroke. A total of 7,693 community-dwelling individuals aged 60 years and older in Shanghai were included. Through ROC curve analysis, it was found that indices such as triglyceride glucose-waist circumference (TyG-WC), triglyceride glucose-waist-to-height-ratio (TyG-WHtR), and Lipid accumulation product (LAP) showed excellent performance in predicting MetS, with TyG-WC being the strongest predictor in both males (AUC = 0.90) and females (AUC = 0.90). These indices integrate information on both lipid metabolism and body fat distribution, allowing for more accurate identification of MetS. The study also revealed that ABSI had poor predictive performance. These findings suggest that the combined use of these indices may help identify MetS earlier and more accurately in elderly people at high risk of stroke, providing a basis for clinical intervention. Future research should validate the generalizability of these indicators in more diverse populations and explore their potential integration into clinical workflows.
ObjectiveTo explore the cumulative incidence of diabetes mellitus and its influencing factors among the diabetes mellitus high-risk groups in Minhang District of Shanghai, and to provide a basis for the development of community-based diabetes mellitus early prevention and treatment strategies in Shanghai.MethodsThe data of this study were collected from the screening project of diabetes mellitus high-risk groups in Minhang District of Shanghai, which was carried out from 2016‒2017. A total of 12 278 people were screened, of which 10 442 were at high risk based on the results of blood glucose diagnosis except those with diabetes. People diagnosed with abnormal fasting blood glucose, impaired fasting blood glucose and abnormal glucose tolerance were classified into the abnormal blood glucose group, and those with normal fasting blood glucose were classified into the normal blood glucose group. In 2023, the cumulative incidence of diabetes mellitus in the 2 groups was followed up, furthermore, the influencing factors of diabetes mellitus were analyzed.ResultsA total of 8 774 cases within the high-risk groups were followed up in 2023, of which 808 cases progressed to diabetes. Of the 8 774 cases, the cumulative incidence of diabetes mellitus in the abnormal blood glucose group (n=2 163) was 16.37% (354/2 163), and that in the normal blood glucose group (n=6 611) was 6.87% (454/ 6 611), and the difference in cumulative incidence of diabetes mellitus between the 2 groups was statistically significant (χ2=175.88, P<0.001). A history of impaired glucose regulation (OR=2.828, 95%CI: 2.177‒3.675), family history of type 2 diabetes (OR=1.294, 95%CI: 1.047‒1.600), hypertension (OR=1.268, 95%CI: 1.083‒1.485), dyslipidemia (OR=1.205, 95%CI: 1.003‒1.448), overweight/obesity (OR=1.526, 95%CI: 1.300‒1.792) were positively correlated with the cumulative incidence of type 2 diabetes, while female gender (OR=0.785, 95%CI: 0.675‒0.914) and sedentary lifestyle (OR=0.506, 95%CI: 0.374‒0.685) were negatively correlated with the cumulative incidence of type 2 diabetes.ConclusionBlood glucose monitoring should be strengthened in diabetes mellitus high-risk groups with an impaired glucose regulation history, a family history of type 2 diabetes, hypertension, and dyslipidemia, so as to achieve early detection of diabetes. Timely intervention measures should be taken for high-risk population with unhealthy lifestyle or overweight/obesity.
Both body mass index (BMI) and family history of cancer are established risk factors for female breast cancer. However, few studies explored the potential interaction between both factors. We assessed the association of BMI and its interaction with family cancer history on the risk of female breast cancer in Shanghai, China. Based on a population-based prospective cohort study started from 2008 to 2012 with 15,055 Chinese female participants in Minhang district, Shanghai. Cox regression models were used to estimate the association of BMI and its interaction with a family history of cancer on breast cancer risk. The additive interaction was evaluated by the relative excess risk due to interaction (RERI) and the attributable proportion due to interaction (AP), and the multiplicative interaction was assessed by the product term (BMI* family history of cancer) in the Cox regression model. Compared with BMI of < 24 kg/m2 and no family history of cancer, women with BMI of ≥ 24 kg/m2 and a family history of cancer had a higher risk for breast cancer with HR 2.06 (95% CI 1.39, 3.06). There was an additive interaction between BMI and family history of cancer on breast cancer incidence, with the RERI being 0.29 (95% CI 0.08, 0.51) and the AP being 0.37 (95% CI 0.08, 0.66). The coexistence of obesity and cancer family history may exacerbate breast cancer incidence risk, highlighting the importance of weight management in women with a family history of cancer.
Frailty and sarcopenia are age-related diseases, and exhibit a concomitant relationship, as they share many common clinical features and etiological factors. Transitions within frailty status would be influenced by the presence of sarcopenia. Investigating their association to devise efficacious intervention and management strategies for geriatric patients is imperative, given their potentially unfavorable outcomes. In this study, the literature on sarcopenia and frailty was screened in the Web of Science core collection database over the past 30 years to ascertain the link between them through bibliometric analysis and the exploration of disease-related molecular pathways within the GeneCards and OMIM databases was conducted. Per inclusion and exclusion criteria, 3889 literature sources were selected for subsequent analysis. Keywords, including “cirrhosis” and “postoperative complications,” represent the current and potential future research trends and focal points in this field. Moreover, 63 common potential targets between the two diseases were identified. Their pathogenesis involved cellular aging and endocrine metabolism regulation pathways, including AMPK, cell senescence, and the endocrine resistance pathway. This study identified an intimate correlation between frailty and sarcopenia in pathogenesis, prevention, and treatment measures, establishing a foundation for exploring shared prevention and treatment strategies for these two disorders.
Aims To assess the independent and combined impacts of visit-to-visit fasting blood glucose variability (VVV-FBG) and mean fasting blood glucose level (M-FBG) on all-cause mortality. Materials and methods This prospective cohort study included 48 843 Chinese patients with type 2 diabetes. Cox proportional hazards regression models were used to calculate hazard ratios (HRs) and 95% confidence intervals (CIs) to evaluate the association of VVV-FBG and M-FBG with all-cause mortality. The potential nonlinear associations were examined using restricted cubic splines, and additive interaction was evaluated using relative excess risk due to interaction (RERI). Cox generalized additive models (CGAMs) and bivariate response surface models were further used to assess the combined effects of VVV-FBG and M-FBG. Results A total of 4087 deaths were observed during a median follow-up of 6.99 years. Compared with patients with values at the 5th percentile of average real variability (ARV) and M-FBG, we observed a 23% and 38% increased risk of premature deaths among those with values at the 95th percentile of ARV (HR 1.23, 95% CI 1.10, 1.37) and M-FBG (HR 1.38, 95% CI 1.26, 1.51), respectively. The interaction between glycaemic variability (ARV) and M-FBG was significant on both the additive scale (RERI 0.80 [0.29, 1.32]) and the multiplicative scale (HR 1.90 [1.10, 3.28]). High VVV-FBG and high M-FBG conferred the highest risk of all-cause mortality (HR 1.89, 95% CI 1.64, 2.17), compared to low VVV-FBG and low M-FBG. The CGAMs showed significant synergistic effects between glycaemic variability and M-FBG (P < 0.05). Moreover, a bivariate surface plot showed that risk of death increased more rapidly in type 2 diabetes patients with lower M-FBG combined with lower VVV-FBG. Conclusions The coexistence of high glycaemic variability and high glucose level might exacerbate the independent risk of premature mortality in type 2 diabetes patients, highlighting the importance of achieving normal and stable glucose levels simultaneously in the management of glucose.
Backgrounds Fasting blood glucose (FBG) variability may make an impact on adverse events in patients with diabetes mellitus. However, the association between long-term changes in FBG and cancer remains unclear. We aimed to investigate this association in a large-scale longitudinal study. Methods Data were collected from 46 761 patients with type 2 diabetes mellitus aged 20-80 years who participated in the Diabetes Standardized Management Program in Shanghai, China. We adopted four indicators, including standard deviation (SD), coefficient of variation (CV), variation independent of the mean (VIM), and average real variability (ARV) to describe FBG variability. Adjusted multivariable Cox regression analyses and restricted cubic splines were used to investigate the association between long-term FBG variability and cancer risk. We also determined the interactive effect of FBG variability with hypertension and FBG-mean with hypertension on cancer risk, respectively. Results In this study, we confirmed 2218 cancer cases (51.1% male) over a median follow-up of 2.86 years. In the multivariable-adjusted models, participants in the highest quartile of FBG variability had an increased risk of cancer compared with those in the lowest quartile. The nonlinear association was found when using FBG-VIM, FBG-ARV, and FBG-SD in restricted cubic spline plots. There was a significant interaction effect of FBG variability with hypertension on cancer, whereas the effect of FBG-mean with hypertension did not attain significance. Conclusions Our retrospective cohort study demonstrated a positive association between the long-term changes in FBG and cancer risk in patients with type 2 diabetes mellitus. FBG variability may independently predict cancer incidence.
Objectives This study aims to explore the long-term trend of fasting blood glucose (FBG) among urban patients with type 2 diabetes mellitus (T2DM) and the impacts of the Chinese Spring Festival on their glycemic control in urban China. Methods The general information and longitudinal monitoring data of patients with T2DM in Minhang District, Shanghai China from 15 December 2006 to 31 December 2015 were collected. The FBG records were grouped into three periods, namely, the preholiday period (2 months right before the Chinese Spring Festival), the holiday period (from 28 December to 7 January of the lunar calendar year), and the postholiday period (2 months after the Chinese Spring Festival). The Mann-Kendall trend test and Cochran-Armitage trend test were occupied to explore the long-term trend, and paired t -test and chi-square (χ 2 ) test were used to determine the differences in glycemic level and control rate between the preholiday and postholiday periods, respectively. Results From 2007 to 2015, the glycemic control rate in patients with T2DM showed an upward trend ( P < 0.001), and the FBG level showed a decreasing trend ( P = 0.048). After the Chinese Spring Festival, the glycemic control rate decreased significantly ( P < 0.001), and the FBG level increased significantly ( P < 0.001) compared to those during the preholiday period. The incidence of hypoglycemia increased during holidays. Patients who were aged 60–69 years, overweight or obese, with hypertension, with a disease duration of <3 years, or with poor glycemic control in one previous year were more likely to be affected by the holiday. Conclusion Chinese Spring Festival is a key point for glycemic control of patients with T2DM in China. Intensive holiday-specific diabetic healthcare needs to be further improved, and community-based interventions should be developed and implemented to control the possible holiday effects.
Introduction China has the largest population of diabetic patients (about 116 million) in the world. As a novel model of the fat index for Chinese people, the Chinese visceral adiposity index (CVAI) was considered a reliable indicator to assess the dysfunction of visceral fat. This study aimed to explore the dose–response relationship between CVAI and type 2 diabetes mellitus (T2DM) in the Chinese population, considering CVAI as a continuous/categorical variable. Method Baseline and follow-up data were collected from waves 2011 and 2015, respectively, of the China Health and Retirement Longitudinal Study (CHARLS). Multivariate logistic regression models were used to explore the relationship between CVAI and T2DM. We built three models to adjust the possible effect of 10 factors (age, gender, education level, location, marital status, smoking status, drinking status, sleep time, systolic blood pressure (SBP), and diastolic blood pressure (DBP)) on the outcome. The restricted cubic splines were used to examine possible non-linear associations and visualize the dose–response relationship between CVAI and T2DM. Results A total of 5,014 participants were included, with 602 (12.00%) T2DM patients. The last CVAI quartile group (Q4) presented the highest risk of T2DM (OR, 2.17, 95% CI, 1.67–2.83), after adjusting for all covariates. There was a non-linear (U-shaped) relationship between the CVAI and the risk of T2DM (p for non-linear <0.001) in the restricted cubic spline regression model. CVAI was a risk factor of T2DM when it exceeded 92.49; every interquartile range (IQR) increment in the CVAI was associated with a 57% higher risk of developing T2DM (OR = 1.57, 95% CI = 1.36–1.83) after adjusting for potential confounders. The area under the receiver operating characteristic curve (AUC) (95% confidence interval) for CVAI was 0.623, and the optimal cutoff point was 111.2. There was a significant interaction between CVAI and gender by stratified analysis. Conclusion CVAI was closely associated with the risk of T2DM and might possibly be a potential marker in predicting T2DM development. The outcome suggested that it might be better to maintain CVAI within an appropriate range.
The aim of our study was to examine the association between body mass index (BMI) and the risk of cardiovascular disease (CVD)-specific mortality among Chinese adults with hypertension by sex. This study included 212,394 adult hypertensive patients aged 20-85 years registered in the records of Minhang District during 2007-2018. Cox proportional hazards regression was performed to evaluate the association between BMI and CVD-specific mortality among Chinese adults with hypertension. There were 14,029 deaths over an average of 8.24 years (range, 0.19-11.96 years). The multivariable-adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) across BMI categories (< 18.5 kg/m2, 18.5-24.9 kg/m2 [reference group], 25.0-29.9 kg/m2, and ≥ 30 kg/m2) for CVD-specific mortality were 1.37 (1.22-1.53), 1.00 (reference), 0.95 (0.90-1.01), and 1.21 (1.04-1.40) in males, and 1.44 (1.31-1.59), 1.00 (reference), 0.96 (0.91-1.01), and 1.04 (0.92-1.17) in females. A U-shaped relationship was observed between BMI and CVD-specific mortality (overall association P< 0.001; non-linearity P< 0.001). This association was attenuated in old age. This study revealed a U-shaped relationship between BMI and CVD-specific mortality among hypertensive men and women. In older people, overweight and obesity are potential factors that reduce the risk of CVD death.
Background Relationship between BMI and all-cause mortality in patients with hypertension remains controversial. This study aimed to evaluate the time-varying association between BMI in patients with hypertension and all-cause mortality. Methods This population-based cohort study included 212,394 Chinese adults with hypertension from 2007 to 2015 and was followed up until death, loss-to-follow-up, or December 31, 2018. According to the World Health Organization criteria for Asians, BMI was categorized into five groups: underweight (BMI < 18.5 kg/m 2 ), normal weight (18.5–22.9 kg/m 2 ), overweight (23–24.9 kg/m 2 ), class I obesity (25–29.9 kg/m 2 ) and class II obesity (BMI ≥ 30 kg/m 2 ). Cox model was used to estimate the time-varying association of BMI on the risk of mortality by including the interaction term between BMI and time using restricted cubic spline. Results Compared with normal weight, underweight and class II obesity were associated with higher mortality (Hazard ratio [HRs] at 1 and 10 years of follow-up: 1.51 [95% CI: 1.39–1.65], and 1.27 (1.15–1.41) for underweight, respectively; 1.08 (0.96–1.21), and 1.16 (1.03–1.30) for class II obesity, respectively). However, overweight and class I obesity were associated with lower mortality, although the protective effects gradually attenuated over time (HRs at 1 and 10 years of follow-up: 0.85 (0.81–0.90), and 0.96 (0.91–1.02) for overweight, respectively; 0.80 (0.76–0.84), and 1.04 (0.99–1.10) for class I obesity, respectively). Conclusions We found increased mortality among hypertensive patients with underweight and class II obesity while decreased mortality with overweight and class I obesity was observed during the first 5 years of follow-up. Management efforts for hypertension may target controlling body weight in a reasonable range for patients, and probably more attention should be given to underweight patients.
目的 应用双变量响应模型分析高血压人群中体质指数(body mass index,BMI)和年龄两个因素对心血管死亡(cardiovascular death,CVD)的共同作用.方法 使用上海市闵行区2007-2015年高血压患者随访数据,建立可加COX比例风险模型,分析BMI、年龄与CVD之间的非线性关联以及两变量的共同作用.结果 BMI、年龄与CVD的关联分别呈"U"型和"J"型.不同年龄,BMI对CVD的影响略有差异.青年和老年群体的死亡风险随BMI的增加而降低,中年群体的死亡风险与BMI的关联略微呈"U"型.结论 双变量响应模型是一种全面了解两变量共同作用的有效的方法.
BackgroundThe relationship between body mass index (BMI) and mortality in hypertension patients remains controversial. This study aimed to evaluate the association and the time-varying effects of different BMI categories on the risk of all-cause mortality in hypertension patients. MethodsThis retrospective cohort study was conducted among 212,394 Chinese people with hypertension. All deaths were identified based on Shanghai Vital Statistics. Cox model combined with time-by-covariate interactions was used to estimate the association and the time-varying effects of BMI on the risk of all-cause mortality. The potential non-linear effects across follow-up period for BMI were examined by the application of restricted cubic spline (RCS).ResultsOverall, 31,130 deaths occurred (14.7%) within an average follow-up of 8.24 years. Underweight (<18.5 kg/m2) showed a progressively weakening negative effect on all-cause mortality over time. For both sexes, overweight (23.0-24.9 kg/m2) and class I obesity (25.0-29.9 kg/m2) showed protective effects within 5 years after registration, but these became insignificant in later years. There was no significant difference in the effect on all-cause mortality between class II obesity (≥30.0 kg/m2) and normal weight. in the elderly patients, overweight, class I obesity and class II obesity had continuous protective effects on mortality.ConclusionsAlthough the effect of baseline body mass index on the risk of all-cause mortality varied at different follow-up periods, underweight persistently remained a risk factor for all-cause mortality in hypertension, whereas overweight and class I obesity had protective effects. Thus, in the long-term management of hypertension, more attention should be given to underweight patients.
Few prospective cohort studies have investigated associations between environmental tobacco smoke (ETS) and other cancer sites, in addition to lung cancer. We assessed these associations in a population-based prospective cohort study started from 2008 to 2011 with average of 9.1 years of follow-up, in Minhang district, Shanghai, China. The study included a total of 23,415 participants (8388 men, 15,027 women) and 205,515 person-years. Epidemiological data were collected by a standardized questionnaire including ETS exposure. Newly diagnosed patients with primary cancers and deaths were identified by record linkage system with the Shanghai Cancer Registry and Shanghai Vital Statistics. Hazard ratios (HRs) and their 95% confidence intervals (CIs) were estimated using Cox proportional hazards regression models, adjusting for potential confounders. During the study period, a total of 1462 patients with diagnoses of primary cancers were identified. Among all participants and non-smokers, ETS was associated with an increased risk of all smoking-related cancers (all: adjusted HR: 1.23, 95% CI: 1.05-1.43 and non-smokers: 1.24, 1.02-1.49), lung cancer (1.29, 0.98-1.71 and 1.27, 0.91-1.77), and stomach cancer (1.86, 1.21-2.85 and 1.75, 1.05-2.91), respectively. Furthermore, associations for lung and stomach cancers were the strongest among non-smoking females. The joint effects of both ETS and active smoking were strongest for all cancers, all smoking-related cancers, lung cancer, and stomach cancer. No clear interactions were observed. These results suggest that ETS exposure may increase the risk of smoking-related cancers in a Chinese population. Further studies on the relationship between ETS exposure and specific cancer sites are warranted to replicate our findings.
BACKGROUND:Long-term glycemic variation in diabetes patients may have contributed to cancer incidence.AIM:In this study we aimed at the association between annual glycemic variation and the risk of cancer in Chinese patients with type 2 diabetes mellitus (T2DM).METHODS:Subjects of this study were from an established population-based cohort of T2DM patients in Minhang District of Shanghai, China. Incident cancer were obtained from the Shanghai Cancer Registry. Glycemic variation was evaluated using the annual fasting glucose coefficient of variation (FG-CV), which was used as a time-dependent variable in a Cox regression model to estimate the associations with the cancer risk. Restricted cubic splines were used to explore potential non-linear associations.RESULTS:A total of 2,140 incident cancers (1100 men and 1040 women) were identified from the 46,202 diabetes patients during 12-year follow-up. The annual FG-CV remained significantly associated with an increased risk of cancer, even after adjusting for the annual mean FG level. A significant non-linear association was found in male T2DM patients, and a significant linear association in female patients.CONCLUSIONS:The positive association of the annual FG-CV with the risk of cancer in T2DM patients indicate the importance to stabilize the FG level.
OBJECTIVE:To investigate the relationship between blood pressure variability (BPV)and target-organ damage in elderly patients with hypertension.METHODS:A total of 197 elderly patients were included in the study. The participants were divided into two groups: those with hypertension (n=146) and those without hypertension (Control, n=51). The 24 h systolic and diastolic blood pressure variability (24 h SBPV/DBPV),24 h average systolic and diastolic blood pressure (24 h SBP/DBP), day time systolic and diastolic blood pressure variability (d SBPV/DBPV),and night time systolic and diastolic blood pressure variability (n SBPV/DBPV)were measured. The hypertensive group was further divided into low and high variability groups according to the 50th percentile of 24 h SBPV. The carotid artery intima-media thickness (IMT), left ventricular mass index (LVMI), 24 h microalbuminuria (MA) and basic clinical and laboratory parameters were compared among the groups. The correlations between blood pressure variability and IMT, LVMI, and MA were analyzed with multivariable regression analyses.RESULTS:The patients with essential hypertension had significantly higher 24 h SBPV (P < 0.001) and d SBPV (P < 0.05) than those without hypertension. The patients with higher blood pressure variability had greater incidence of plaque and cardiovascular disorder, as well as IMT, LVMI and MA (P < 0.001). The prevalence of diabetes mellitus (DM) differed significantly among the three groups of participants (P < 0.05). The multivariable regression analysis showed that 24 h SBPV was independently correlated with IMT, LVMI and MA.CONCLUSION:Elderly patients with hypertension have high BPV, which is an important independent predictor of target-organ damage.
The objective of this study was to assess the capacity of different criteria of metabolic syndrome (MetS) to identify risks of coronary heart diseases (CHDs) and related changes of adipocytokines in postmenopausal women. A cross-sectional study was carried out in 225 community-dwelling, elderly postmenopausal Chinese women (age, 66.77+/-5.09 years) without hormone replacement therapy (HRT). Baseline data such as blood pressure, body mass index (BMI), serum lipid profiles, and fasting glucose were analyzed, and insulin sensitivity was estimated via the homeostasis model assessment for insulin resistance. Serum tumor necrosis factor alpha (TNFalpha), interleukin-6 (IL-6), and adiponectin were measured simultaneously. The prevalence of MetS identified by the Third Report of the National Cholesterol Education Programme Expert Panel on Detection, Evaluation, and Treatment of High Blood Cholesterol in Adults, the International Diabetes Federation (IDF), the Chinese Diabetes Society (CDS), and the Japanese Society of Internal Medicine (JPN) were 27.31%, 37.34%, 23.29%, and 13.65%, respectively. No significant differences of baseline data were found among different MetS groups, except for a significant higher waist circumference in the JPN-MetS group as compared with other MetS groups. The prevalence of confirmed CHD in the four MetS groups were 26.2%, 18.6%, 26.9%, and 32%, respectively. Odds ratios for CHD were 1.905 (95% CI=1.273-2.851), 1.208 (95% CI=0.778-1.876), 1.997 (95% CI=1.238-3.221), and 2.336 (95% CI=1.119-4.876), respectively. The JPN-MetS group had higher levels of TNFalpha and IL-6, whereas the CDS-MetS group correlated better with lower adiponectin levels. The IDF definition for MetS is the most sensitive one with regard to metabolic disorders, whereas JPN and CDS definitions correlate better with CHD and changes of adipocytokines among the four criteria studied.
