Background Developmental dysplasia of the hip (DDH) is among the most common congenital musculoskeletal disorders and may lead to early hip osteoarthritis if left untreated. Modified Pemberton acetabuloplasty is an established, growth-preserving osteotomy technique for surgical correction in childhood. However, robust clinical long-term outcomes with follow-up periods exceeding two decades have not yet been published. The aim of this study was to retrospectively evaluate functional and patient-reported outcomes after an average follow-up of 25.6 years. Methods In this monocentric retrospective cohort study, 63 patients (92 hips) were included from an original cohort of 331 patients with 507 operated hips who had undergone modified Pemberton acetabuloplasty between 1990 and 1995. In 2018, clinical follow-up was performed in 54 patients; all participants completed standardised questionnaires, including the Harris Hip Score (HHS), WOMAC, and EQ-5D. Data analysis was descriptive. Results The mean follow-up period was 25.6 years. The average HHS was 95.8 points, the mean WOMAC index 7.2 points, and the EQ-5D-VAS 89.3/100. A total of 81% of patients were free of symptoms, and 83.3% achieved excellent functional results. Overall, 96.8% rated the surgical outcome as good or very good, and 92% would choose to undergo the procedure again. No patient required total hip arthroplasty. No major postoperative complications occurred. Conclusion Modified Pemberton acetabuloplasty demonstrates stable functional outcomes, low symptom levels, and high health-related quality of life more than 25 years after surgery. The very high patient satisfaction and the absence of secondary procedures underscore the long-term clinical effectiveness of this growth-preserving technique.
Background:Intertrochanteric rotational osteotomy (ITO) is a joint-preserving option in selected patients with symptomatic femoral torsional abnormalities. Functional results beyond 15 years are scarce. This study describes the long-term functional and patient-reported status after isolated ITO; it is not designed to demonstrate efficacy or superiority. Methods:A monocentric retrospective cohort of 60 patients was evaluated 16.6 years after isolated rotational or derotational osteotomy. Functional outcomes were assessed using HHS, WOMAC, EQ-5D-5L, and clinical examination. Exploratory multivariable regression was used to assess potential associations with functional and patient-reported outcome parameters. Results:More than 75% of patients achieved good to excellent outcomes (mean HHS 85 ± 15). Mean WOMAC score was 11 ± 13, and mean EQ-5D-5L index was 0.843 ± 0.189. No conversion to total hip arthroplasty was observed during follow-up. Female sex was associated with higher WOMAC scores and lower EQ-5D-5L values. Conclusion:In this retrospective single-arm cohort, isolated intertrochanteric rotational or derotational osteotomy was accompanied by stable functional and patient-reported values over a mean of 16,6 years, with no conversion to total hip arthroplasty during follow-up. Sex and the documented rotational correction angle were associated with selected patient-reported outcomes. Because of the retrospective design, the absence of a control group, the absence of preoperative score values, and the absence of standardised radiological torsion and follow-up imaging, the data do not support any statement on efficacy, on prevention of degenerative change, or on superiority over alternative approaches. These results describe the long-term functional status of a clinically documented cohort.
BACKGROUND:Peripheral nerve blocks are an integral part of the multimodal analgesia for total knee arthroplasty (TKA). The adductor canal block (ACB) gained popularity for its motor-sparing effect. This study evaluated ACB alone or combined with local infiltration analgesia (LIA), in comparison to LIA alone in TKA patients. METHODS:A systematic review and meta-analysis of randomized controlled trials (RCTs) was performed. The MEDLINE and PMC using PubMed, Cochrane Library, and Scopus databases were searched for RCTs evaluating ACB alone or with LIA compared to LIA alone after TKA published up to December 18, 2024. The random-effect model was used for both the standardized mean difference (SMD) of continuous parameters, as well as for the odds ratio of binary parameters. Postoperative pain scores assessed by a visual analog scale or a numerical rating scale at 24 and 48 hours constituted the primary outcome. Secondary outcomes were opioid consumption, knee range of motion (ROM), length of hospital stay, and postoperative nausea and vomiting (PONV). The risk of bias was assessed with the risk of bias tool 2 (RoB-2). RESULTS:In total, 26 studies (2,400 patients) were included. Static and dynamic pain scores at 24 and 48 hours in the ACB+LIA group compared to LIA alone were lower (SMD24rest = -0.54, 95% confidence interval [CI], -0.80 to -0.28, P < .00001; SMD24activity = -0.85, 95% CI, -1.37 to -0.32, P = .001; SMD48rest= -0.26, 95% CI, -0.49 to -0.03, P = .02; SMD48activity = -0.66, 95% CI, -0.96 to -0.36, P ≤ .0001). ACB alone reduced pain during activity but not at rest compared to LIA (SMD24activity = -0.93, 95% CI, -1.88 to 0.02, P = .05; SMD48activity = -1.10, 95% CI, -2.03 to -0.17 P = .02). Reduced opioid consumption (P = .005) and greater ROM at 24 hours (P = .02) were observed with ACB+LIA. Regarding the remaining outcomes, no differences were observed in opioid consumption, ROM, and hospital stay. ACB patients experienced lower PONV rates (P = .05). The GRADE framework rated evidence certainty from moderate to very low with a moderate to high bias according to the RoB-2. CONCLUSION:Adding ACB to LIA improves pain scores at 24 and 48 hours compared to LIA alone.
Im Jahr 2022 wurden im deutschen Endoprothesenregister über 300.000 totale Hüft- und Knieprimärendoprothesen erfasst. Die ständig steigende Anzahl von Eingriffen führt zu einer Zunahme der absoluten Anzahl möglicher Komplikationen. Die Menge an Daten, die auf nationaler Ebene weltweit verfügbar sind, ermöglicht es, Risikofaktoren für postoperative Komplikationen zu bestimmen. Die an den deutschen Leitlinien beteiligten Fachgesellschaften legen großen Wert auf modifizierbare Risikofaktoren, da ein angemessenes Management dieser Faktoren die Wahrscheinlichkeit postoperativer Komplikationen verringern kann. Einer der häufigsten veränderbaren Risikofaktoren ist Fettleibigkeit, deren Prävalenz in den meisten Industrieländern konstant hoch bleibt. Die Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften in Deutschland im Bereich der Orthopädie fokussiert sich auf veränderbare Risikofaktoren wie Adipositas, Diabetes mellitus und Rauchen. Zu diesem Zweck werden in den Leitlinien klare Anweisungen zum Umgang mit ihnen gegeben. Speziell bei Adipositas wird in den Leitlinien für Knie- und Hüftendoprothetik empfohlen, bei einem BMI > 30 kg/m2 eine Gewichtsabnahme anzuraten. Ein BMI ≥ 40 kg/m2 wird in den Leitlinien als „relative Kontraindikation“ betrachtet. In beiden Leitlinien wird die zentrale Rolle der partizipativen Entscheidungsfindung betont. Letztendlich bleibt die besonders kritische individuelle Abwägung von Risiko und Nutzen bei jedem adipösen Patienten entscheidend.
Introduction:Although one of the most successful orthopedic procedures, total hip arthroplasty (THA) conveys a not negligible risk for complications. Case Report:A 72-year-old patient after a THA received an operative revision 2 times due to persistent hematoma and early periprothetic infection. Post-operative renal failure requiring hemodialysis has been caused by a histologically confirmed oxalate nephropathy (ON). Post-operative antibiotic treatment altering the gut microbiome has been assumed as the most likely cause of ON. It is the first presented case of ON as a post-operative complication after a hip arthroplasty revising operation. Conclusion:Acute post-operative renal injury conveys significant danger for the patients. An interdisciplinary approach is needed as many cases require complex diagnostic and therapeutical procedures offered by experts on the field.
Purpose:The aim of this systematic review and meta-analysis was to evaluate the effectiveness of augmented reality (AR) techniques in acetabular cup positioning during total hip arthroplasty (THA). Methods:The Cochrane Central Register of Controlled Trials, MEDLINE and EMBASE databases were systematically searched until December 12, 2024 according to the PRISMA 2020 guidelines for prospective and retrospective studies comparing AR techniques to conventional ones regarding acetabular cup positioning. Anteversion, inclination angles, blood loss, operation time and postoperative complications comprised the outcomes. Bias assessment was performed with the RoB 2 tool for prospective studies and ROBINS-I for retrospective ones. Results:In total, 9 studies were included in the systematic review and 8 in the meta-analysis. The target error according to postoperative radiographs or computer scan for both anteversion and inclination angles was significantly more precise in the 346 THAs of the AR group (Zinclination = 3.10, P inclination = 0.002, Std. mean difference = -0.52 [95 % CI: 0.85 to -0.19] and Zanteversion = 2.44, P anteversion = 0.01, Std. mean difference = -0.57 [95 % CI: 1.03 to -0.11]) with a significant substantial heterogeneity (I2 inclination = 79 %, Pinclination<0.0001 and I2 anteversion = 89 %, P anteversion < 0.00001), when compared to the 395 THAs of the conventional group. Regarding blood loss, operation time and postoperative complications, no significant differences were observed. The risk of bias was high among the retrospective studies. Conclusion:The different types of studies, as well as their individual way of assessing the acetabular cup angles are the main limitations of the meta-analysis. The results provide a strong sign of improved acetabular cup positioning with the help of AR systems, while maintaining a favorable safety profile. Prospero registration number:CRD42024609350.
BACKGROUND:In the year 2022, more than 300,000 total hip and knee primary arthroplasties were recorded in the German Arthroplasty Registry. The ever-increasing number of interventions naturally leads to an increase in the absolute number of possible complications. The abundance of internationally available data makes it possible to determine risk factors for postoperative complications. RISK FACTORS:Professional societies involved in the German guidelines in the field of orthopedics place great emphasis on modifiable risk factors, as their adequate management can consequently reduce the likelihood of postoperative complications. One of the most common modifiable risk factors is obesity, with its prevalence remaining consistently high in most developed countries. In the field of orthopedics, the Association of the Scientific Medical Societies in Germany focuses on modifiable risk factors including obesity, diabetes mellitus and smoking. OBESITY:In the case of obesity in particular, the guidelines for knee and hip arthroplasty recommend a weight loss attempt for patients with a BMI > 30 kg/m2. A BMI ≥ 40 kg/m2 is considered a "relative contraindication" in the guidelines. Shared decision making is emphasized in both guidelines. Ultimately, the particularly critical individual assessment of risk and benefit for each obese patient remains crucial for the final decision.
Der plantare Fersenschmerz ist ein Überbegriff für zahlreiche Krankheitsbilder der Fersenregion und ein häufiger Konsultationsgrund in der orthopädischen Sprechstunde. Er umfasst eine Vielzahl von Pathologien unterschiedlicher Ätiologie. Die exakte Diagnose des plantaren Fersenschmerzes erfordert genaue anatomische, pathophysiologische sowie -morphologische Kenntnisse. Erschwert wird die Diagnosestellung nicht nur durch die engen anatomischen Beziehungen, sondern auch durch eine teilweise undifferenzierte Terminologie der Krankheitsbilder dieser Region. Die Plantarfasziitis stellt mit Abstand die häufigste Ursache für den plantaren Fersenschmerz dar, daneben können Nervenkompressionssyndrome, Stressfrakturen, Fibromatosen und Rupturen der plantaren Faszie ähnliche Beschwerden hervorrufen. Der differenzialdiagnostischen Betrachtung kommt eine besondere Bedeutung zu, da diese therapeutische Konsequenzen nach sich zieht. Im Folgenden sollen die Ursachen des plantaren Fersenschmerzes mit Schwerpunkt auf Diagnose und differenzialdiagnostische Abgrenzung dargestellt werden.
The orthopedic community is divided on the question of whether modern technologies in arthroplasty improve outcomes. Therefore, the aim of this initiative of the working group on intraoperative imaging and technology integration (Arbeitsgemeinschaft intraoperative Bildgebung und Technologieintegration, AGiTEC) is to initiate the collection of additional data for the scientific evaluation of modern technologies. To what extent are modern technologies currently used and which implementations are planned? Do the members of the German Society for Orthopedics and Trauma Surgery (DGOU) consider the acquisition of additional data for scientific assessment necessary? Members of the DGOU were asked via an e‑mail survey about the distribution and projected introduction of modern technologies in arthroplasty. They were also asked whether sufficient data were collected for scientific evaluation and whether acquisition of additional data in studies or arthroplasty registries were considered necessary. Of the 7923 probands surveyed, 428 completed the questionnaire in full (5.4%). It was found that individual implants and navigation are currently the most frequently used (31% and 29%, respectively). The largest increases in the next 2 years are projected for virtual reality and robotics (+30% and +23%, respectively), 85% of respondents indicated that insufficient data were collected for scientific evaluation, and 89% each requested initiation of multicenter studies and inclusion of technologies in the arthroplasty registry. The results of this study should motivate the scientific community, industry, and those responsible for the arthroplasty registries to collect and analyze data for the scientific assessment of modern technologies.
A total of 26 feet of 26 patients with calcific Achilles tendinosis were treated with midline incision of the tendon. In all feet calcific tendon parts were resected. In 10 (38%) feet, a prominent dorsal spur was resected, in 12 feet (38%) retrocalcaneal bursa, and in 24 (92%) feet a calcaneal tuberosity. Mean follow-up was 34.5 months (range 2-64 months). Preoperative Manchester-Oxford Foot Questionnaire (MOXFQ) score was 58.2 (±8.1) and postoperatively the score was 22.75 (±6.0). In all, 7 (26.9%) patients stated delayed wound healing; 1 suffered from deep vein thrombosis. Shoe problems were reported by 50% of patients, and 23.1% suffered from par- or dysesthesia. No revision surgery was required.

Dieser Artikel beschreibt den Weg einer orthopädischen Klinik im Rahmen eines maximalversorgenden Klinikums zur elektronischen Patientenakte.
Behandlung der kalzifizierten Enthesiopathie der Achillessehne über einen transtendinösen Zugang mit optionaler Resektion des Bursa subachillea, des dorsalen Fersensporns und der Tuberkante (Haglund-Exostose). Kalzifizierte Enthesiopathie, dorsaler Fersensporn, Insertionstendinopathie. Allgemeine Kontraindikationen für chirurgische Eingriffe, Infekt. Hautschnitt medial paraachillär. Präparation bis auf die Achillessehne, Eröffnung des Paratenons. Mediane Inzision der Achillessehne und plantare Ablösung vom kalkanearen Ansatz unter Erhalt der medialen und lateralen Zügel. Darstellen und Resektion der Exostose am dorsalen Kalkaneus. Präparation und Resektion der intratendinösen Kalzifikationen der Achillessehne. Optional Resektion der Bursa subachillea als auch der dorsoproximalen Tuberkante. Refixation der Achillessehne mittels transossärer Nähte oder Fadenanker. Zwei Wochen 20 kg Teilbelastung in 30 Grad Plantarflexion im Orthesenstiefel an Unterarmgehstützen. Anschließend 2 Wochen 15 Grad schmerzadaptierte Belastung, dann 2 Wochen in Neutralstellung des oberen Sprunggelenks schmerzadaptierte Belastung im Orthesenstiefel. Thromboseprophylaxe bis zur Vollbelastung. Bei 26 Füßen von 26 Patienten wurde zwischen Juli 2015 und September 2021 eine mediane Inzision der Achillessehne bei chronischer, therapieresistenter Insertionstendinopathie durchgeführt. In allen Fällen erfolgte die Resektion der kalzifizierten Sehnenanteile über einen transtendinösen Zugang. In 10 (38
Introduction: Evidence concerning the role of isolated lateral retinacular release (LRR) for lateral patellar compression syndrome (LPCS) dates back at least three decades. Appropriate indications, execution and outcomes still remain unclear and controversial. The present investigation analyzed the midterm result of isolated and arthroscopic LRR for LPCS in a cohort of patients who underwent such procedure at our institution. Material and methods: Patients undergoing isolated arthroscopic LRR for LPCS were identified retrospectively from our electronic database. All procedures were performed by two experienced surgeons. Patients with bony and/or soft tissues abnormalities, patellofemoral instability, moderate to severe chondral damage were not included. Patients with previous surgeries were not included, as were those who underwent combined interventions. Clinical scores and complications were recorded. Results: 31 patients were recruited in the present investigation. The mean follow-up was 86.0 ± 22.8 months. The mean age of the patients at the index operation was 34.2 ± 13.1 years. A total 55% (17 of 31) were women, and 58% (18 of 31) had involved the right knee. The mean hospitalization length was 3.5 ± 1.4 days. At a mean follow-up of 86.0 ± 22.8 months, the numeric rating scale (NRS) was 1.2 ± 0.8, the Kujala score was 91.3 ± 11.3, the Lysholm score was 93.1 ± 15.0, and the Tegner score was 5.0 ± 1.8. At the latest follow-up, 9 of 31 (29.0%) of patients experienced compilations. One patient (3.2%) had a post-operative hemarthrosis which was managed conservatively. Six patients (19.4%) reported a persistent sensation of instability, without signs of patellar dislocation or subluxation. One patient underwent an arthroscopic meniscectomy, and another patient an anterior cruciate ligament (ACL) reconstruction. Conclusion: isolated arthroscopic lateral retinacular release for lateral patellar compression syndrome is feasible and effective, achieving satisfying results at more than seven years following the procedure.
Behandlung der chronischen plantaren Fasziitis mit optionaler Dekompression des ersten kalkanearen Astes des Nervus plantaris lateralis (Baxter-Nerv) bei Nervenkompressionssyndrom und Abtragung eines plantaren Kalkaneussporns. Chronische plantare Fasziitis, Kompression des ersten kalkanearen Astes des N. plantaris lateralis (Baxter-Nerv). Allgemeine Kontraindikationen für chirurgische Eingriffe, Infekt. Hautschnitt an der medialen Ferse am Übergang zur Sohlenhaut. Darstellung der ursprungsnahen plantaren Faszie am medialen Kalkaneus. Teilablösung der medialen, plantaren Faszie knochennah unter Erhalt des lateralen Anteils. Optional Abtragung des plantaren Kalkaneussporns (Fersensporn). Bei gleichzeitiger Kompression des ersten kalkanearen Astes des N plantaris lateralis (Baxter-Nerv) darstellen des M. abductor hallucis. Eröffnen der oberflächlichen Faszie. Retraktion des Muskels. Darstellen der tiefen Faszie. Kerbung der Abduktorfaszie und Dekompression. Zwei Wochen 20 kg Teilbelastung im Verbandsschuh an Unterarmgehstützen. Anschließend schmerzadaptierte Belastung für 4 Wochen im Konfektionsschuh mit starrer Sohle. Thromboseprophylaxe bis zur Vollbelastung. Bei 32 Füßen von 27 Patienten wurde zwischen April 2014 und Oktober 2017 ein offenes partielles Release der plantaren Faszie mit Dekompression des ersten kalkanearen Astes des N. plantaris lateralis durchgeführt. In 24 Fällen erfolgte zusätzlich die Abtragung eines planetaren Kalkaneussporns. Von den operierten Patienten waren 16 weiblich (59 %) und 11 männlich (41 %). Das Durchschnittsalter lag bei 43,3 Jahren (24–68 Jahre). Postoperative Kontrollen erfolgten nach 6 Wochen und abschließend im Durchschnitt nach 25,6 Monaten (12–35). Der präoperative MOXFQ-Score lag bei 52,5 (±9,0), der postoperative MOXFQ-Score betrug zum letzten Follow-up 31,3 (±4,1). Postoperativ klagten nach 6 Wochen 6 Patienten über eine unveränderte oder schlechtere Beschwerdesymptomatik (18,8 %). Über geringfügige Komplikationen wie Schwellung, protrahierter Wundheilungsverlauf, temporäre Taubheit im Bereich der Ferse oder schmerzhafte postoperative Mobilisation klagten 8 Patienten (25 %). Keine der genannten Komplikationen war revisionsbedürftig.
Die individuelle Knieendoprothetik hat sich mittlerweile zu einer weltweiten Routineoperation an einigen Zentren etabliert. Bislang fehlte insbesondere Literatur zum klinischen Outcome, der die hypothetischen Vorteile der individuellen Versorgung belegt. Ziel der vorgelegten Studie ist es, die aktuelle Literatur thematisch zu sortieren und aufzuarbeiten. Die hier präsentierten Studien zeigen, dass die individuelle Knieendoprothetik mittlerweile ein gereiftes Verfahren mit vielversprechenden Ergebnissen in Hinblick auf Achskorrektur, natürlichere Kinematik, Patientenzufriedenheit und Kostenneutralität darstellt. Diese Entwicklung hat dazu geführt, dass das Orthopaedic Data Evaluation Panel (ODEP), als Beratungsgremium des National Health Service in Großbritannien, die Conformis-Prothesen bereits in 2017 mit einem 3A-Rating versehen hat.
Die Arthrose des Knieund Hüftgelenks sind Volkskrankheiten mit einer zunehmenden Tendenz aufgrund der Alterung der Bevölkerung. Trotz kontinuierlicher Weiterentwicklung der Operationstechniken, des Designs und der Materialbeschaffenheit der Implantate ist dennoch die Standzeit aufgrund verschiedener Faktoren limitiert. Bei Implantatlockerung kommt nur der Austausch der gelockerten Implantatanteile infrage. Die Sineplastik oder auch die Arthrodese sind keine guten Alternativen. Da immer mehr Implantate eingesetzt werden, steigt auch die Anzahl der erforderlichen Revisionsoperationen. Diese Eingriffe sind neben der Belastung für den Patienten auch eine finanzielle Belastung des Gesundheitssystems. Daher ist es weiterhin das Ziel, die Implantate näher an die physiologische Situation anzupassen und die Standzeit zu verbessern. Da die knöcherne Anatomie an Knieund Hüftgelenk anlagebedingt, entwicklungsbedingt oder auch durch operative Eingriffe oder Verletzungen durchaus sehr variabel sein kann, liegt eine Individualisierung der Implantate nahe. Neben dem Standzeitthema hat sich ebenso gezeigt, dass trotz des grundsätzlich sehr erfolgreichen Operationsverfahrens des Gelenksersatzes eine gewisse Anzahl vonPatientenmit demerreichten Ergebnisnicht zufrieden ist undpostoperativ Beschwerden behält. Dieses Phänomen ist vor allem beimKniegelenksund weniger beim Hüftgelenksund Schultergelenksersatz zu verzeichnen. Je nach Literatur liegt dieser Prozentsatz bei bis zu 15–20%. Diese Erkenntnis hat dazu geführt, dass patientenimmanente Faktoren bei der Versorgung stärker in den Vordergrund gerückt sind. Insbesondere beim Kniegelenk ist die komplexe Kinematik des Gelenks als bedeutsamer Faktorerkanntworden.Untersuchungenzeigen zweifelsfrei auf, dass vorkonfektionierte Standardimplantate bei einem hohen Prozentsatz der Patienten die natürliche Kniekinematik nicht ausreichend nachahmen. Für das Hüftgelenk gilt dies ebenso bei ca. 15%der Patientenmit Deformierungen des proximalen Femurs. AuchdieserAspekt unterstreicht die notwendige Individualisierung von Implantaten nicht zwingend bei allen Patienten flächendeckend, aber eben bei den Patienten, die radiologisch auffällige knöcherne Verhältnisse zeigen (beim Kniegelenk z.B. sehr unterschiedliches dorsales oder distales Offset, beim Hüftgelenk z.B. das Champagnerglasfemur). Zuletzt muss auch anerkannt werden, dass die Implantation von künstlichen Knie-, Hüftund Schultergelenken sehr komplexe Eingriffe sind, die ein hohes Maß an operativemGeschick, Erfahrung und Genauigkeit voraussetzen. Zu allen Gelenken liegen hinreichend Hinweise vor, dass die Implantationsgenauigkeit aufgrund verschiedener Faktoren aber nicht bei allen Operationen umzusetzen ist, bzw. umgesetzt werden kann. Es ist auch bekannt, dass z.B. Bandinstabilitäten beim Kniegelenk, Implantatpositionsprobleme am Hüftgelenk und auch am Schultergelenk zu vermeidbarenFrührevisionenführen,die,wieschon oben erwähnt, sowohl Patient als auch das solidarische Gesundheitssystem belasten. Auch dies führt zur Erkenntnis, dass individualisierte Implantatemit entsprechenden Schablonen und Ausrichthilfen eine verbesserteGenauigkeit erzielen könnten und damit die Notwendigkeit von Frührevisionen vermeiden helfen.
