Introduction Air pollution is a key modifiable environmental determinant of cardiovascular health. Particulate matter (PM2.5 and PM10) and nitrogen oxide (NO2) are among the main studied pollutants, and are likely to induce multiple cardiovascular outcomes. However, no study on their effects has utilized single-photon emission computed tomography myocardial perfusion imaging (SPECT MPI), a widely used, non-invasive technique for the diagnosis, prognosis, and therapeutic management of coronary artery disease. Objective This study aimed to investigate for the first time the associations between long-term exposure to air pollution and SPECT MPI abnormalities. Method Patients referred to Grenoble University Hospital for SPECT MPI between 2017 and 2020 were prospectively enrolled in the Evaluation of Prognostic Value of SPECT-imaging Myocardial Perfusion (EVAPERF) cohort study. Moderate-to-severe myocardial ischemia was defined as left ventricular myocardial ischemia (LVMI) above 10%. Long-term exposures to PM2.5, PM10, and NO2 at each participant's residential address, defined as the annual average prior to the SPECT date, were estimated using data from a high temporal and spatial resolution air pollutant dispersion model (air quality monitoring system [AQMS]), (10-meter). Associations were assessed using logistic regression models, and potential modifying effects by sex, body mass index, and smoking status were explored. Results A total of 1585 participants were prospectively included (mean age 68±12 years; 63% male, among them, 148 (9.3%) presented moderate-to-severe myocardial ischemia. The adjusted odds ratios of moderate-to-severe ischemia for each interquartile range (IQR) increase in exposure to PM2.5, PM10 and NO2 were 1.39 (95% confidence interval [CI]: 1.07–1.80; P=0.013), 1.33 (95% CI: 1.01–1.75; P=0.042), and 1.22 (95% CI: 0.96–1.57; P=0.10), respectively (IQRs: 3.3, 4.3, and 10.0μg/m3, respectively). Associations with PM10 and PM2.5 appeared stronger in women than men, with interaction P-values of 0.11 and 0.077, respectively. Conclusion This study provides imaging-based evidence supporting that chronic exposure to particulate air pollution may contribute to increased risk of myocardial ischemia, particularly in women.
Objectif Le but de cette étude est d’évaluer l'intérêt pronostique d'une nouvelle méthode d'indice de résistance microcirculatoire (IMR) dérivé de l'angiographie; l'AngioIMR chez les patients présentant un infarctus du myocarde avec sus décalage du segment ST (STEMI). Introduction Plusieurs méthodes de mesure de l'IMR dérivé de l'angiographie ont été développées. L'AngioIMR est une nouvelle méthode permettant de mesurer l'IMR dérivé de l'angiographie sans guide et sans hyperhémie (1). La valeur pronostique de cette méthode n'est pas connue. Méthodes Les patients présentant un STEMI avec réalisation d'une angioplastie primaire étaient prospectivement enrôlés entre janvier 2015 et août 2018. L'AngioIMR était calculé en utilisant une simulation de pression et de computation des fluides immédiatement après l'angioplastie primaire. Un seuil d'AngioIMR supérieur à 40 était retenu pour identifier une dysfonction coronaire microvasculaire. Le critère de jugement était la mortalité toute cause ou l'insuffisance cardiaque (MACE). Résultats Au total 178 patients ont été enrôlés (Age moyen 65.0±12.8 ans, 74 % d'hommes). Un AngioIMR> 40 était retrouvé chez 72 patients. Durant un suivi médian de 2,9 (2,3-6,9) années, le critère de jugement principal était retrouvé chez 56 patients. En analyse de Kaplan-Meyer, le risque de MACE était significativement plus élevé chez les patients avec AngioIMR> 40 (log-rank P <0,01). L'AngioIMR>40 était significativement associé à la survenue du critère de jugement principal (70% vs 27%; Hazard ratio 4.519; 95% CI: 2.550- 8.009; p<0,0001) et ce de manière indépendante (Hazard ratio 4.282; 95% CI: 2.325-7,886; p<0,0001). Il existait une valeur pronostique incrémentale comparée à un modèle de prédiction du risque clinique et d'imagerie (C-index 0,84 vs 0,79; p=0,04) (fig.1). Conclusion Dans le STEMI, une dysfonction microcirculatoire évaluée par AngioIMR prédit un pronostic péjoratif.
The risk assessment for ventricular arrhythmia in patients with ischemic cardiomyopathy is, at the present time, unspecific. Left ventricular (LV) entropy is a measure of myocardial heterogeneity derived from single photon emission computed tomography (SPECT). This study assesses the utility of LV entropy for predicting cardiovascular events in patients with ischemic cardiomyopathy. Patients with ischemic cardiomyopathy referred for implantable cardioverter-defibrillator (ICD) implantation were included retrospectively. Their LV entropy was measured on SPECT. Patients were followed from implantable cardioverter-defibrillator placement to detect arrhythmic events (appropriate implantable cardioverter-defibrillator therapy and sustained ventricular arrhythmia), end-stage heart failure events (congestive heart failure, transplantation, or ventricular assist device placement), and cardiac death. Forty patients (mean age 68.1 ± 9.4 years, 75% men, LV ejection fraction 29.7 ± 4.6%) were followed for 22.8 ± 12.9 months. Of the forty patients, three (7.5%) experienced arrhythmic events, one died of advanced heart failure after receiving the appropriate shock, and three others (7.5%) experienced end-stage heart failure events. On univariate analysis, age (HR: 1.02 [0.99; 1.04]; P = 0.176); diabetes (HR: 0.30 [0.05; 1.68]; P = 0.17), CRT-D (HR: 0.06 [0.007; 0.53]; P = 0.01), NYHA class (HR: 7.6 [2.00; 29.5]; P = 0.003) and ischemia (HR: 1.07 [0.96; 1.18]; P = 0.19) were significant predictors of cardiac events. On multivariate analysis, CRT-D (HR: 0.06 [0.04; 0.89]; P = 0.04) remained a significant predictor of cardiac events. LV entropy was not a predictor of cardiac events. In patients with ischemic cardiomyopathy, LV entropy measurement is not a predictor of life-threatening ventricular arrhythmia. However, CRT-D is a strong protector of ventricular arrhythmia in ischemic cardiomyopathy.
Myocardial perfusion reserve (MPR) is a validated diagnostic and prognostic value by Positron Emission Tomography (PET) scans. However, PET availability stays limited in certain countries as France. Gamma cameras with cadmium-zinc telluride detector (CZT-SPECT) are more available and allow the evaluation of this MPR. However, there is no comparison of its results with the interventional diagnostic procedure. Twenty-seven patients with suspected of epicardial or microcirculatory coronary involvement were prospectively included. All patients benefited from an evaluation of the MPR by CZT SPECT as well as an interventional diagnostic procedure with measure of the Fractionnal Flow Reserve (FFR), coronary flow reserve (CRF) and index of microcirculation resistance (IMR). Correlation between MPR and CFR was measured and sensitivity and specificity to detect severe coronary microcirculation disease (CMVD) defined by FFR > 0.8, CFR < 2 and IMR > 25. Coronary angiography was performed in 27 patients, with a complete functional evaluation (FFR, CFR, IMR) on 33 vessels. Two patients had significant stenosis (50-90% stenosis and FFR < 0.8). MPR was correlate with CRF measurement (r = 0.41; P = 0.017). The sensitivity and specificity per patient to detect severe CMVD were 71% and 65%, respectively. Sensitivity and specificity of MPR by territory were 87% and 76% respectively. MPR was correlated with CRF and presented satisfactory diagnostic performance to discriminate, in the absence of significant stenosis, patients free from patients with severe CMVD.
Les ponts intra-myocardiques siègent le plus souvent sur l'artère interventriculaire antérieure (IVA) et sont asymptomatiques dans la plupart des cas. Cependant, ils pourraient avoir un rôle dans la physiopathologie de la cardiomyopathie de stress. Nous décrivons un cas où le diagnostic différentiel entre cardiomyopathie de stress et infarctus du myocarde a été difficile, dans un contexte de pont intra-myocardique. Une femme de 70 ans a été adressée dans le service des soins intensifs de cardiologie du CHU de Grenoble pour suspicion d'infarctus ambulatoire. Elle avait ressenti plusieurs épisodes de douleur thoracique faisant suite à une activité physique intense la semaine précédente. L'ECG montrait un rythme sinusal avec un aspect QS dans les dérivations inférieures (DII, DIII, aVF) et un sus-décalage du segment ST dans le territoire antérieur (V1-V5). Il y avait une élévation en plateau du taux de troponine T et l'échocardiographie trans-thoracique (ETT) montrait une altération de la fraction d'éjection ventriculaire gauche (FEVG) à 45%, associée à une akinésie de l'ensemble des segments apicaux, et une hyperkinésie des segments basaux. La coronarographie n'a pas retrouvé de thrombus, de dissection ou de sténose coronaire, mais un pont intra-myocardique de l'artère IVA (Figure 1), faisant de la cardiopathie de stress le diagnostic le plus probable, conforté par l'aspect angiographique du ventricule gauche. Afin de confirmer le diagnostic, une IRM myocardique a été réalisée pendant son hospitalisation, comme ceci est suggéré dans les cas de syndromes coronariens aigus sans sténose résiduelle serrée. Contre toute attente, l'IRM montrait une plage de réhaussement tardif de Gadolinium, de type sous-endocardique, dans la région apicale, suggérant le diagnostic final d'infarctus du myocarde compliquant un pont intra-myocardique de l'IVA. Par ailleurs, l'IRM a permis de mettre en évidence la présence d'un thrombus intra-ventriculaire gauche tapissant la paroi infarcie, qui n'avait pas été détecté en ETT ou en ventriculographie (Figure 2A et 2B). Or, des cas de prises de contraste tardives transitoires ont été décrits dans la cardiopathie de stress, correspondant à une augmentation transitoire de la matrice extracellulaire. Une IRM cardiaque de contrôle a donc été réalisée quelques mois plus tard, montrant une disparition complète de la prise de contraste et du thrombus, ainsi qu'une normalisation de la fonction ventriculaire gauche sans séquelle, permettant de porter le diagnostic final de cardiomyopathie de stress. Ce cas clinique rappelle qu'une prise de contraste tardive transitoire en IRM est possible dans les premiers jours après une cardiopathie de stress. Par ailleurs, il montre l'importance de l'imagerie multimodale pour le diagnostic étiologique des syndromes coronariens aigus sans sténose résiduelle serrée, ainsi que la nécessité de répéter l'IRM dans les cas les plus complexes, tels que le diagnostic différentiel entre cardiomyopathie de stress et infarctus du myocarde.
Coronary microvascular dysfunction (CMD) is associated with a higher rate of major adverse cardiovascular events (MACEs) in diabetic patients. Liraglutide, a glucagon-like peptide-1 analog, decreases MACEs and all-cause mortality in this population. The present paper aims to study the effects of liraglutide on heart function and myocardial perfusion entropy (MPE) as a surrogate marker of coronary microvascular dysfunction (CMVD) in a rat model of type-2 diabetes. Thirty-nine rats were divided into three groups, control (CTL), diabetic (T2D) and liraglutide (DT2-LIR). T2D and T2D-LIR rats were fed a high-fat diet and injected with 35 mg/kg streptozotocin intra-peritoneally (IP) to induce diabetes. T2D-LIR rats were injected with a subcutaneous liraglutide solution for four weeks. Heart structure and function were assessed with trans-thoracic-doppler echocardiography and CMVD was assessed with a novel 201 Thallium SPECT-technique, measuring MPE, under stress conditions. Histological analysis was later performed. T2D rats had a significantly lower left ventricular (LV) ejection fraction (LVEF) versus CTL (64.5 ± 4.3% vs. 74.4 ± 9.2% P < 0.01), higher indexed LV mass (5.5 ± 1 g/m2 vs. 3.8 ± 0.7 g/m2 P < 0.001) and increased LV end diastolic diameter (LVEDD) (8.7 ± 0.4 mm vs. 7.5 ± 0.6 mm P < 0.001). Liraglutide increased LVEF (72.2 ± 6.9% vs. 64.5 ± 4.3% P < 0.05), LVSF (43.4 ± 6.6% vs. 36.9 ± 3.3% P < 0.05) versus T2D rats. MPE was significantly increased in T2D rats (7.59 ± 0.5 vs. 7.09 ± 0.5 P < 0.05), and there was no difference between T2D rats and T2D-LIR rats. In accordance with MPE analysis, CD31 immunohistochemistry analysus indicated a significant decrease in LV capillary density in TD2 animals, which was not improved by liraglutide. Liraglutide is associated with an improvement in heart systolic function and reduced LV hypertrophy in type 2 diabetic rats but only has a small specific effect on CMVD.
We assessed the prognostic value of myocardial perfusion heterogeneity (entropy) in patients with normal myocardial perfusion as estimated from a dual isotope high-speed protocol (Thallium-201 stress and technetium- 99m-sestamibi rest) performed on a semiconductor camera. The prognostic value of normal myocardial perfusion scintigraphic imaging has been widely established for over twenty years but the clinical value of perfusion heterogeneity quantification beyond perfusion imaging as routinely performed is unknown. From May 2011 to September 2012, 1159 patients with a normal scan or a non-significant perfusion abnormality (SSS < 4) and a normal left ventricular ejection fraction (> 45%) were retrospectively identified. This study reports an interim analysis of 443 patients. A dual isotope high-speed protocol was performed on a semiconductor camera following exercise testing (37%) or pharmacological stress (63%). Myocardial perfusion images were then used to quantify myocardial perfusion heterogeneity as estimated by stress and rest image entropy. The primary endpoint was the occurrence of a cardiovascular event (cardiovascular death, non-fatal myocardial infarction, coronary angiography with or without revascularization, and stroke). The primary endpoint occurred in 73 patients during follow-up (3.3 ± 0.4 years). In a multivariate analysis, age (HR 1.04 [1.01; 1.06] P < 0.0001) and stress entropy (HR: 1.6 [1.004; 2.56], P = 0.04) were independent predictors of cardiovascular events. In survival analysis, patients with elevated entropy (i.e. increased heterogeneity) (> 4.06) underwent more events than those with a lower entropy (< 4.06) (log rank = 0.04). Myocardial perfusion heterogeneity (entropy) determined from thallium-201 stress perfusion images acquired on a semiconductor camera represent an independent prognostic factor of cardiovascular events with additive value over the classical interpretation of images from patients with normal myocardial perfusion scintigraphy. Myocardial perfusion heterogeneity (entropy) might represent a surrogate of coronary microvascular dysfunction.
Prasugrel and Ticagrelor, two P2Y12 receptors antagonists commonly used for the treatment of acute coronary syndrome, are suspected to exert anti-inflammatory effects, which may decrease atherosclerotic lesions. Moreover, Ticagrelor, through its interaction with adenosine metabolism, may have supplemental positive effect on inflammation and atherosclerosis. We evaluated in this study the effects of these two molecules on atherosclerotic lesions and inflammation on a murine model of atherosclerosis. Firstly, twelve C57BL/6 mice treated by vehicule ( N = 4), Prasugrel ( N = 4) or Ticagrelor ( N = 4) were used to evaluate the comparability of the anti-platelet effects by aggregometry. Then, twenty-one Apolipoprotein E deficient mice (ApoE -/-) fed a paigen diet (1.25% cholesterol, 0.5% cholic acid, 15% fat) were treated with vehicule ( N = 7), Prasugrel 5 mg/kg/day ( N = 7), or Ticagrelor 180 mg/kg/day ( N = 7) during 6 weeks. Anti-atherosclerotic effects of these molecules were evaluated in vivo using 99mTc-cAbVCAM1-5, an imaging agent evaluating the expression of the inflammatory marker Vascular Cell Adhesion Molecule-1 (VCAM-1). Maximum platelet aggregation was lower in C57BL/6 mice treated by Prasugrel and Ticagrelor (8%), than in mice treated by vehicule (70%). 99mTc-cAbVCAM1-5 uptake in the aortic root evaluated by in vivo Single Photon Emission Computed Tomography (SPECT) imaging (2.1 ± 0.6%ID/mm 3 vs 2.2 ± 0.7ID/mm 3 vs 1.9 ± 0.7ID/mm 3 ), ex-vivo biodistribution (2.9 ± 0.8%ID/g vs 2.5 ± 0.4%ID/g vs 2.6 ± 0.5%ID/g), and autoradiography (3.9 ± 1.0%ID/g vs 4.2 ± 1.0%ID/g vs 4.0 ± 0.6%ID/g) were not different between the control group, Prasugrel group and Ticagrelor group respectively. Ex vivo analysis confirmed that plaque surface and lipid content were identical between the three groups. Neither Prasugrel nor Ticagrelor has anti-atherosclerotic effects in this specific model of ApoE -/- mice fed a Paigen diet.
La TEP/TDM à la 18F-Choline (FCH) est actuellement considérée comme l’examen diagnostique de référence en médecine nucléaire chez des patients en récidive biochimique (RB) de cancer de prostate, alors que la scintigraphie osseuse (SO) est controversée dans cette indication, en particulier lorsque le PSA est inférieur à 10 ng/mL. Les objectifs de l’étude étaient de déterminer les performances diagnostiques de la SO avec double TEMP/TDM systématique en comparaison à la TEP/TDM à la FCH pour la détection des métastases osseuses chez des patients présentant une RB de cancer de prostate, indépendamment du taux de PSA, et d’identifier une sous-population pour laquelle la SO avec TEMP/TDM avant la TEP/TDM à la FCH pourrait être pertinente. Les patients adressés pour une RB de cancer de prostate et qui ont bénéficié d’une SO couplée à une TEMP/TDM double champ systématique et d’une TEP/TDM à la FCH dans un délai de trois mois ont été rétrospectivement inclus. Les examens ont été classés comme positifs, équivoques ou négatifs pour l’atteinte osseuse par deux médecins nucléaires en aveugle. La FCH était considérée comme le gold standard. Dans la perspective de réaliser la SO avec TEMP/TDM avant la TEP/TDM à la FCH, les SO équivoques étaient considérées comme négatives dans l’analyse. De janvier 2011 à novembre 2017, 115 patients consécutifs en RB de cancer de prostate ont été évalués. La sensibilité, spécificité, valeur prédictive positive et négative de la SO avec TEMP/TDM étaient de 92,9 % [76,5–99,1], 98,9 % [93,8–100], 96,3 % [81–99,9] et 97,7 % [92–99,7] respectivement. Les patients présentant une SO avec TEMP/TDM positive avaient un score de Gleason (p < 0,01) et un taux de PSA pré-test 12,5 (4,4–18,6) ng/mL versus 4,4 (2,1–6,9) ng/mL, p < 0,001) significativement plus élevés, ainsi qu’un temps de doublement du PSA (PSAtd) significativement plus court (3,8 (2,1–6) versus 7,6 (3,7–16,7) mois, p < 0,001). Un PSAtd ≤ 6 mois était significativement associé avec la positivité de la SO avec TEMP/TDM (p < 0,01), en particulier dans le sous-groupe de patients présentant un PSA ≤ 10 ng/mL (OR = 4,4, p = 0,03). La SO avec TEMP/TDM a de bonnes performances pour le diagnostic des métastases osseuses dans la RB du cancer de prostate par rapport à la TEP/TDM à la FCH. Cependant, les patients pour lesquels cette stratégie apparaît pertinente se limitent désormais à ceux qui ont un haut volume tumoral métastatique osseux (8 % de nos patients).
This paper presents a segmentation process of the myocardium, endocardium and epicardium surfaces of the heart from 3D SPECT images to compute a heterogeneity index. This index represents the distribution of the activity in the myocardium. Because of the low resolution of SPECT images, the thickness of the myocardium is 1 to 4 voxels and a sub-voxel accuracy is therefore needed. The segmentation process is based on the minimization of an energy by dynamic programming after a coarse segmentation to define the center surface of the myocardium. A gaussian mixture is fitted on the data to ensure subvoxel accuracy. The heterogeneity index is compared to the reference index on 58 SPECT images and the segmentation is visually validated on 300 SPECT images by a clinician.
