Severe acute pancreatitis (SAP) involves dynamic shifts from inflammation to immunosuppression, where peptidomic profile evolution may reveal prognostic biomarkers. Here, the plasma peptidome of rats with taurocholate-induced SAP at 1, 3, 6, 12, and 24 h compared with controls was characterized using nLC-MS/MS. Ten peptides derived from eight precursor proteins were differentially regulated across time points. The 12-h period showed eight differentially regulated peptides, while the 6- and 24-h periods had four differentially regulated peptides, and one peptide was regulated between 1 and 3 h. Peptides derived from alpha-1-microglobulin (A1M) increased from 3 h onward, while peptides from actin showed major alterations at 12-24 h, coinciding with peak mortality (46%). Bioinformatic enrichment analyses revealed transient activation of mTOR, JAK/STAT, and cell adhesion pathways at 6 h, followed by bacterial invasion and actin cytoskeleton regulation pathways at later stages. These temporal profiles suggest an early antioxidant response and subsequent structural and infection-related remodeling. These findings suggest that A1M-derived peptides could represent potential early indicators of disease severity, although further validation in human clinical settings is required. These findings highlight the plasma peptidome as a promising tool for clinical diagnostics, providing a better understanding of SAP progression and identification of phase-specific biomarkers in pancreatitis. SUMMARY: This study reveals dynamic changes in the plasma peptidome during the progression of severe acute pancreatitis in rats. Through the identification of differentially regulated peptides, bioinformatic analysis was performed to define the altered pathways and genes, demonstrating the relationship between peptide alterations and disease progression. The 6-h time point after pancreatitis induction showed the highest number of signaling terms/pathways that characterize the inflammatory phase of the disease. In subsequent moments, the enrichment of pathways related to infection and the regulation of the actin cytoskeleton at 12- and 24-h post-pancreatitis induction suggests that this period is associated with the process of bacterial translocation and pancreatic necrosis infection. Therefore, the peptide profile and pathways may have implications for defining prognosis and early diagnosis of infection.
ABSTRACT Objectives: to describe the profile of older adults who access the internet to search for health information and identify the factors that can influence older adults’ decisions about their health based on information collected online. Methods: 391 older adults answered an online questionnaire regarding habits and satisfaction with information about health collected on the internet. Data processing involved Logistic Regression. Results: higher education reduces by 44% the likelihood of an older adult following the health recommendations on internet sites. However, social activities and self-perceived health increase the possibility of following the recommendations by 83% and 71%, respectively. The belief that the internet promotes healthy habits increases by 29.2 times the probability of an older adult following the advice. Final Considerations: knowing the profile of older adults who use the Internet can help professionals formulate public policies and build good information platforms on health and well-being.
Systemic arterial hypertension (SAH) is a multifactorial clinical condition with a high morbidity and mortality rate and low control rates. Although photobiomodulation (PBM) is capable of inducing a photobiological interaction within cells and modifying the micro and macrovascular response in situ, there is a lack of studies demonstrating its systemic effect, especially clinical and placebo-controlled studies. Thus, this study aimed to evaluate the modulatory effects on blood pressure control in hypertensive patients after transcutaneous photobiomodulation of the radial artery. For this purpose, 44 participants were randomly divided into 4 groups, one group irradiated for 60 minutes daily for 5 days, repeating after 20 days with a total of 3 cycles (Group A), and its respective placebo (Group B); The third group was irradiated for 30 minutes daily for 10 days, repeating after 20 days, also totaling 3 cycles (Group C), and its respective placebo (Group D). All participants remained with conventional drug treatment previously instituted. Before the interventions, after each cycle and at the end of them, blood pressure and heart rate were measured. Before and at the end of the interventions, blood samples were collected to measure interleukins 6 and 1beta. Statistical analysis was performed to compare the groups in each phase of the treatment, with α = 0.05. Modulatory effects were evidenced in the control of blood pressure in hypertensive patients after photobiomodulation. Group A showed a better response to the reduction of hemodynamic variables than group C. Multidisciplinary care may have increased adherence to treatment of all participants, including the placebo groups, which also showed a decrease in the values of hemodynamic variables.
Introduction. Violence as a social and public health problem, with a particular focus on its consequences, such as Post-Traumatic Stress Disorder (PTSD). According to the World Health Organization, violence encompasses the intentional use of physical force or power, threatening or harming individuals, groups or communities. Objective: To describe the effects of Cannabidiol on patients with Post-Traumatic Stress due to physical and psychological violence. Methodology: This is an observational and retrospective integrative systematic review, with the exclusion criteria: non-empirical articles, non-indexed publications. A total of 545 articles related to post-traumatic stress and cannabidiol were found and only 13 articles were selected to present the results of this study. Results and discussion: The endogenous endocannabinoid system plays a significant role in the processing and extinction of fear memory. Cannabidiol (CBD), a non-psychoactive component of Cannabis sativa, has shown potential in the treatment of anxiety disorders, including PTSD. CBD interacts with the endocannabinoid system indirectly by increasing the availability of endocannabinoids such as anandamide, which act on CB1 receptors in the brain. This activation of the endocannabinoid system may help reduce the behavioral response to aversive memories and improve fear extinction. Final Thoughts: While early research on the effects of CBD on PTSD is promising, more studies are needed to determine its efficacy and safety, particularly in victims of violence. Sources also emphasize the need for greater awareness and accessibility of cannabinoid-based treatments for those who may benefit from them.
Care protocols integrate norms, routines and procedures related to a particular health problem/condition. This study proposed the elaboration and validation of a protocol on the preservation of forensic traces in urgent/emergency health services. Methodological study with a quantitative approach. The protocol content validation was performed by 16 judges with expertise in the area of forensic sciences. The measures used to assess the degree of agreement were Percentage of Agreement, Content Validity Index (CVI) and Cronbach's Alpha. A rate of agreement with values above 80% was considered acceptable in the study; a value higher than 78% was considered for CVI calculated per item, and the minimum acceptable value for Cronbach's Alpha was 0.70. The average agreement rate of the instrument was 89.9% (±14.0), varying between the blocks of questions. Of all the items evaluated, 97.4% had an average CVI above 0.78 (78%).The sum of the items considered relevant by each evaluator resulted in an average score of 0.992 (99.2%) for the CVI of the entire protocol. In the evaluation of the degree of reliability, general Cronbach's Alpha showed a high level of internal consistency of the protocol, equivalent to 0.864. The protocol had acceptable psychometric measures of content validity, being valid for use in the preservation of forensic traces by health professionals in urgent/emergency health services.
The effects of hernia repair on testicular function remain uncertain, regardless of the technique used. Studies that analyze testicular volume and flow after hernia repair or hormonal measurements are scarce and show contradictory results. This study aimed to evaluate the impact of bilateral inguinal hernia repair on male fertility in surgical patients in whom the Lichtenstein and laparoscopic transabdominal preperitoneal (TAPP) techniques were used. A randomized clinical trial comparing open (Lichtenstein) versus laparoscopic (TAPP) hernia repair using polypropylene mesh was performed in 48 adult patients (20 to 60 years old) with primary bilateral inguinal hernia. Patients were evaluated preoperatively and 90 and 180 postoperative (PO) days. Sex hormones (Testosterone, FSH, LH and SHGB) analysis, testicular ultrasonography, semen quality sexual activity changes and quality of life (QoL) were performed. Postoperative pain was evaluated using the visual analog scale (VAS). Thirty-seven patients with aged of 44 ± 11 years were included, 19 operated on Lichtenstein and 18 operated on TAPP. The surgical time was similar between techniques. The pain was greater in the Lichtenstein group on the 7th PO day. The biochemical and hormonal analyses, testicular ultrasonography (Doppler, testicular volume, and morphological findings) and sperm quality were similar between groups. However, the sperm morphology was better in the Lichtenstein group after 180 days (p < 0.05 vs. preoperative) and two patients who underwent Lichtenstein hernia repair had oligospermia after 180 days. The QoL evaluation showed a significant improvement after surgery in the following domains: physical function, role emotional, bodily pain and general health (p < 0.05). On comparison of Lichtenstein vs. TAPP none of the domains showed statistically significant differences. No patient reported sexual changes. Bilateral inguinal hernia repair with polypropylene mesh, whether using Lichtenstein or TAPP, does not impair male fertility in terms of long-term outcomes. Trial registration: Approved by the Ethics Committee for the Analysis of Research Projects (CAPPesq) of the HC/FMUSP, Number 2.974.457, in June 2015, Registered on Plataforma Brasil in October 2015 under Protocol 45535015.4.0000.0068. Registered on Clinicaltrials.gov, NCT 05799742. Enrollment of the first subject in January 2016.
The internet emerged in Brazil in the early 90s,in states like Rio de Janeiro and São Paulo, primarily with access only for Universities and for research purposes and soon after with domestic access networks and mobile networks that went viral in all sectors and became an instrument of communication and information in all age groups, whether to meet and meet people, as well as for shopping, travel, research, among others.Along with the internet came the social media Orkut, Facebook, Whatsup, Twitter, Instagram, You Tube, Linkedin, used to look for a job, to shop, to study, to research and several other functions. On any subject, the internet offers an infinity of sites of everything you want to know just press and wait for the results. However, what has been observed in recent years has been an increase in access to sites aimed at information, as well as an increase in the number of people using this tool in order to learn a little more about various subjects.During the COVID-19 pandemic, social networks played an important role in passing on information about care and ways to prevent this condition. Therefore, this study aims to describe through a descriptive exploratory study, systematic review the role of social media as sources of information about COVID-19.
ABSTRACT Purpose: To evaluate local and systemic effects of 24-hour fasting in liver ischemia and reperfusion injury. Methods: Twenty-one adult male Wistar rats (330–390 g) were submitted to 60 minutes of hepatic ischemia followed by 24 hours of reperfusion. Before the day of the experiment, the animals fasted, but free access to water was allowed. Two groups were constituted: Control: non-fasted, that is, feeding ad libitum before surgical procedure; Fasting: rats underwent previous fasting of 24 hours. Hepatic ischemia was performed using vascular clamp in hepatic pedicle. At 24 hours after liver reperfusion, blood and tissue samples were collected. To analysis, liver lobes submitted to ischemia was identified as ischemic liver and paracaval non-ischemic lobes as non-ischemic liver. We evaluated: malondialdehyde levels, hepatocellular function (alanine aminotransferase, aspartate aminotransferase activities, and both ratio), cytokines (interleukins-6, -10, and tumor necrosis factor-alpha), hepatic ischemia and reperfusion injury (histology). Results: Malondialdehyde measured in non-ischemic and ischemic liver samples, hepatocellular function and cytokines were comparable between groups. Histological findings were distinct in three regions evaluated. Microvesicular steatosis was comparable between 24-hour fasting and non-fasted control groups in periportal region of hepatic lobe. In contrast, steatosis was more pronounced in zones 2 and 3 of ischemic liver samples of fasting compared to control groups. Conclusions: These data indicates that fasting does not protect, but it can be also detrimental to liver submitted to ischemia/reperfusion damage. At that time, using long fasting before liver surgery in the real world may be contraindicated.
Introduction: changes in their epidemiological and demographic profiles, due to low mortality rates from infectious diseases, studies have signaled a rapid increase in the number of elderly people in Brazil. This process can be observed in developed and developing countries, which generates socioeconomic, cultural, demographic and epidemiological transformations.Unfortunately, in developing countries, this fact leads to social inequalities that require planning and public policies that focus on the elderly and their particularities, including the proximity of death and injuries that can lead to early finitude.2-Objective: To understand the perceptions of the nursing team in the sector of Medical Clinic in Palliative Care with elderly people in finitude of a private hospital in the city of São Paulo that serves mostly elderly people.3-Methodology -This was field research, exploratory and descriptive with a qualitative approach, which sought to describe palliative care for the elderly in their finitude and to understand the perceptions of the nursing team in the Internal Medicine sector in palliative care for the elderly. This study was carried out in a private hospital that provides services and assistance to the predominantly elderly population with different pathologies. It had the participation of 18 professionals who provide care to the hospitalized elderly, who agreed to participate in the study by signing the Free and Informed Consent Form (TCLE) and approval of CEP CAAE: 332959914.5.0000.5494.4. Discussion -The transcription was carried out in the speech of the subjects using the words of significance in relation to the team's perception of palliative care through charts and diagrams, and later discussed in comparison to what the literature addresses in relation to the findings, through the analysis by Laurence Bardin.5.Conclusion -Through the results of this study, it was possible to describe and understand the actions and perceptions of the nursing team in the elderly in their finitude, the results of the research showed that the nursing team is not yet prepared to act in front of the elderly in a terminal phase, i.e. facing death.
Background Mineralocorticoid receptor antagonists (MRAs) are widely used for chronic central serous chorioretinopathy (cCSCR), but their effectiveness remains unclear. This research was conducted to evaluate the efficacy of this drugs for cCSCR. Methods This is a review of randomized clinical trials (RCT) comparing MRAs to placebo in adults with cCSCR, using the effects of MRAs on best-corrected visual acuity (BCVA) and adverse events as primary outcomes and the effects of MRAs on anatomical parameters as secondary outcomes: central subfield thickness (CST), subretinal fluid height (SFH) and central choroidal thickness (CCT). Our all-language online search included Medline (via PubMed), Central, Embase, Lilacs, Ibecs, and RCT registers platforms, as late as May 2021. We used the Cochrane risk-of-bias tool (version 2) to assess the methodological quality of each study and synthesized the results in meta-analyses using a random-effects model. Results The search identified 302 records, five of which were eligible, totaling 225 cCSCR patients (aged 45–62 years; M/F ratio 3.1:1) treated for 1 to 12 months with spironolactone (50 mg/day) or eplerenone (50 mg/day) vs. placebo. Moderate-certainty evidence suggests MRAs result in little to no improvement in BCVA compared to placebo (SMD 0.22; 95% CI − 0.04 to 0.48; studies = 5; comparisons = 6; participants = 218; I 2 = 0%). Very low-certainty evidence suggests that, when compared to placebo, MRAs have a very uncertain impact on adverse effects (no meta-analysis was performed), and CST (MD 18.1; 95% CI − 113.04 to 76.84; participants = 145; studies = 2; I 2 = 68%). MRAs also result in little to no difference in SFH (SMD − 0.35; 95% CI − 0.95 to 0.26; studies = 5; comparisons = 6; participants = 221; I 2 = 76%; moderate certainty) and CCT (MD − 21.23; 95% CI − 64.69 to 22.24; participants = 206; studies = 4; comparisons = 5; I 2 = 85%; low certainty). Conclusion MRAs have little to no effect on BCVA. Evidence for adverse events and CST is very uncertain. MRAs also have little to no effect on SFH and CCT. These findings should be considered when prescribing MRAs for cCSCR. This research was previous registration in the PROSPERO platform (CRD42020182601).
PURPOSE:To evaluate whether the pigeon (Columba livia) is a good model for evaluating the vestibular system involved with postural maintenance during movement. METHODS:This study maps the brainstem targets of the horizontal ampullary inputs from the vestibular periphery of the pigeon. We used biotin dextran amine (BDA) injection in horizontal semicircular canal (HSCC), immunohistochemistry for GluR2/3 and GluR4 AMPA and computerized histomorphology reconstruction. RESULTS:Our results show the same distribution pattern with ipsilateral projections to vestibular nuclear complex (VNC) from the HSCC, with the majority of labeled fibers being, long, thin, with few varicosities and many ramifications. Horizontal semicircular canal projections achieve neurons belonging to all nuclei of the VNC with exception of dorsal portion of lateral vestibular nucleus and this area express GluR2/3 and GluR4 AMPA receptors reinforcing the idea of glutamate participation in these connections. CONCLUSIONS:Pigeon is an appropriated experimental model to study of projections of HSCC and reinforcing the information that the vestibular system has strong relation with the fast responses necessary for postural control. Moreover, its phylogenetic organization apparently conservation, also seems to be a fundamental characteristic for vertebrates.
