Zusammenfassung. Wir stellen den Fall einer 81-jährigen Patientin mit Fieber unklarer Ursache bei bekannter chronisch inflammatorischer demyelinisierender Polyneuropathie (CIDP) und monoklonaler Gammopathie unklarer Signifikanz vor. Nach langer Hospitalisation konnte die Diagnose eines diffus grosszelligen B-Zell-Lymphoms erst nach einer Splenektomie gestellt werden. Die Abklärungen bei Diagnosestellung und die Nachsorgeuntersuchungen bei einer monoklonalen Gammopathie werden diskutiert.
Zusammenfassung Anamnese und klinischer Befund Wir berichten über einen 62-jährigen Patienten mit trockenem Husten seit 3 Monaten, Fieber, einer Allgemeinsymptomatik und erhöhten Entzündungswerten. Untersuchungen Im Vorfeld ließ sich trotz umfangreicher Diagnostik keine infektiöse, neoplastische oder rheumatologische Diagnose stellen. Die Bronchoskopie mit bronchoalveolärer Lavage zeigte eine Lymphozytose. Im 18F-FDG-PET/CT fiel eine vermehrte Glukoseaufnahme in der gesamten Aorta mit ihren primären Ästen auf. Diagnose, Therapie und Verlauf Wir diagnostizierten eine Großgefäßvaskulitis, vereinbar mit einer extrakraniellen Variante einer Riesenzellarteriitis (RZA). Nach Etablierung einer immunsuppressiven Therapie fand sich ein gutes klinisches Ansprechen und eine Regredienz der Entzündungswerte. Folgerung Bei älteren Patienten mit Fieber, Husten und Allgemeinsymptomatik sollte differenzialdiagnostisch an eine RZA gedacht werden, auch wenn „klassische" Symptome fehlen. Respiratorische Symptome kommen als alleiniges Erstsymptom in ca. 4 %, zusammen mit klassischen Symptomen in ca. 9 % der Fälle mit RZA vor.Bei der Abklärung unklarer Entzündungszustände kommt dem 18F-FDG-PET/CT eine immer größere Bedeutung zu.
History and admission findings We report on a 62-year-old male patient with dry cough for 3 months, constitutional symptoms and elevated inflammatory markers. Investigations Despite extensive diagnostics no infectious, malignant or rheumatologic disease could be found. Bronchoscopy with bronchoalveolar lavage showed lymphocytosis. 18-FDG-PET/CT demonstrated increased metabolic activity of the aorta and its primary branches. Diagnosis, treatment and course We diagnosed large vessel vasculitis consistent to a subtype of giant cell arteritis (GCA) without cranial manifestation. Immunosuppressive therapy resulted in prompt resolution of symptoms and normalizing of inflammatory markers. Conclusions Elderly patients with unexplained fever, cough and constitutional symptoms should be investigated for GCA, even when classic symptoms are absent. Respiratory symptoms occur in about 4 % as initial and only presenting manifestation of GCA and in about 9 % along with classical symptoms. In cases with unclear focus of inflammation 18-FDG-PET/CT is becoming more and more important as a diagnostic tool.
Bei einer Rauchgasexposition ist an die Moglichkeit einer Mischintoxikation mit CN zu denken und bei Verdacht auf eine Zyanidintoxikation fruhzeitig eine Therapie zu beginnen. Andererseits sollten jedoch auch immer noch weitere toxische Substanzen in Betracht gezogen werden.
AIMS Our goal was to determine the relative contributions of demographic variables, drugs, comorbidities, and weather conditions on serum calcium (Ca) and phosphate (Pi) in patients admitted to a tertiary referral center. METHODS For 12,667 consecutive patients admitted to the Kantonsspital St. Gallen, drug history on admission, age, sex, body weight, ICD-10 diagnoses, and laboratory data were extracted from electronic medical records. Weather parameters prior to hospital admission were also integrated in a regression analysis. RESULTS Serum Ca was normally distributed with a median (interquartile range) of 2.3 (2.2/2.4) mmol/L. In contrast Pi showed a right tailed distribution of 1.0 (0.9/1.2) mmol/L. Ca was increased in postmenopausal women. Solar radiation prior to admission was associated both with higher Ca and higher Pi. Lower blood pressure was associated with lower Ca and higher Pi. In addition Ca increased by 0.017 mmol/L per g/L increase of albumin (p < 0.0001). CONCLUSIONS Serum Ca and Pi at hospital admission are highly dependent on patient characteristics, drugs, and comorbidities. In particular, we found higher Ca in postmenopausal women. The commonly applied albumin correction formula of Payne (0.025 mmol/L Ca per g/L albumin) may overestimate the effect of albumin; we propose using 0.017 mmol/L Ca per g/L albumin or measurement of free (ionized) Ca.
Zusammenfassung. Wir präsentieren den Fall eines aufgrund einer rheumatoiden Arthritis mit Azathioprin und Prednison immunsupprimierten 66-jährigen Mannes, der wegen Fieber, Schüttelfrost, Dyspnoe und Myalgien hospitalisiert wurde. Es waren alle Kriterien eines SIRS erfüllt bei jedoch weder laborchemisch noch bildgebend (inkl. Echokardiografie) nachweisbarem Entzündungs- oder Infektfokus. Obwohl es unter empirischer antibiotischer Therapie zu einer raschen Symptomregredienz kam, trat nach Wiederbeginn mit Azathioprin die gleiche Symptomatik wie bei Eintritt auf. Dies führte uns zur Diagnose einer Hypersensitivitätsreaktion auf Azathioprin mit SIRS.
Die Hemmkörperhämophilie ist eine seltene Erkrankung mit hoher Mortalität. Sie wird durch Autoantikörper meist gegen den Gerinnungsfaktor VIII verursacht und ist häufig idiopathisch. Klinisch imponieren Weichteilblutungen. Diagnostisch von Bedeutung ist eine isoliert verlängerte aPTT, niedrige Faktor-VIII-Aktivität und Antikörper gegen den Faktor VIII. Die Therapie besteht einerseits aus einer Gerinnungsaktivierung unter Umgehung des Faktors VIII mittels rekombinantem Faktor VIIa (NovoSeven®) oder aktivierter Prothrombinkomplex (FEIBA®) sowie einer immunsuppressiven Therapie (Kortikosteroide, Cyclophosphamid, Rituximab) zur Elimination des Antikörpers.
Acquired hemophilia is a rare but severe bleeding disorder caused by autoantibodies mostly against factor VIII. Clinically it mostly presents with soft tissue and muscular bleeding. Diagnostics include a prolonged aPTT, antibodies against FVIII and a reduced FVIII titre. Acute bleeding is controlled with recombinant factor VIIa (NovoSeven(®)) or activated prothrombin complex (FEIBA(®)), as both bypass FVIII in the coagulation pathway. Treatment to reduce the inhibitor consists of immunosuppression with corticosteroids, cyclophosphamid and can be extended with rituximab.Abstract available from the publisher.
OBJECTIVES:Considerable variation in serum sodium (Na) and chloride (Cl) is found in patients at hospital admission. Our goal was to quantify the respective impact of drugs, comorbidities, demographic factors and weather conditions on serum Na and Cl. DESIGN AND METHODS:For 13277 consecutive patients without terminal kidney disease admitted to the Department of Internal Medicine of the Kantonsspital St. Gallen drug history on admission, age, sex, body weight, ICD-10 diagnoses, and laboratory data were extracted from electronic medical records. Weather parameters prior to hospital admission were also integrated in a multivariate regression analysis. RESULTS:Both serum Na and Cl showed an asymmetric left-tailed distribution. Median (interquartile range) Na was 138 (136/140) and Cl 104 (101/106). The distribution of sodium in patients with one or more risk factors for SIADH was best explained by the presence of two populations: one population with a similar distribution as the unexposed patients and a smaller population (about 25%) shifted to lower sodium levels. Lower weight, lower blood pressure, kidney dysfunction, fever, and diabetes were associated with both lower Na and Cl. Higher ambient temperature and higher air humidity preceding admission were associated with both higher Na and Cl values. CONCLUSIONS:Na and Cl at hospital admission are highly influenced by ambient weather conditions, comorbidities and medication. The bimodal distribution of Na and Cl in persons exposed to risk factors for SIADH suggests that SIADH may only affect a genetically distinct vulnerable subpopulation.
Wir stellen den Fall eines 77-jährigen Mannes vor, bei dem ein unklares Fieber auftrat, das trotz detaillierten Abklärungen zunächst unklar blieb. Erst die Serologie brachte am Schluss die Diagnose eines Q-Fiebers. Retrospektiv passen die Anamnese und die Befunde gut zu einem akuten Q-Fieber.
We present the case of a 77 year old man with fever of unknown origin. Despite a thorough assessment in hospital the diagnosis could only be made after discharge when positive results for C. burnetii serology revealed acute Q-fever. However, retrospectively history and clinical findings matched well with acute Q-fever.Abstract available from the publisher.
Patients with severe respiratory failure (SRF) are often in need for rapid ventilatory support. Acute NIV is frequently limited to the intensive care unit (ICU). We hypothesized that initiation of NIV in the ED may benefit these patients. In a multidisciplinary consensus we created the following ED NIV protocol: - Patients admitted to the ED with SRF defined as either respiratory rate (RR)>25/min, SpO2<92% despite 6L/min supplemental oxygen and/or hypoventilation (paCO2>7.4kPa). - Initiation of NIV by respiratory therapists (RTs) within 30 min after calling theRTs - NIV in the ED was limited to office hours (otherwise standard care) Results: Within 6 months, 55 patients qualified. 44 cases were assessed on the ED, 31 during office hours. 2 did not receive NIV (improvement or abdominal distress). Main reasons for SRF were COPD, pulmonary edema and pneumonia. RR, acidosis and hypercarbia improved in all but 2 patients within 60 min of NIV (RR 30+/-8/min to 19+/-6/min, pH 7.32+/-0.11 to 7.39+/-0.07, and pCO2 8.13+/-2.21kPa to 6.32+/-2.26kPa). 13 patients were placed on ICU, 4 had been intubated. 5 of 13 patients with a no-resuscitation order died. An important indication in such cases was palliation of dyspnea. Conclusion: NIV on the ED resulted in substantial and rapid clinical benefit, even in palliative settings. NIV in SRF bridges optimal patient management between ED, ICU and medical ward. ![Figure][1] [1]: pending:yes
We report the case of a 73 year old patient with a COPD. A severe exazerbation was treated with glucocorticoids and Quinolones. A few days later both ankles were tender and painful. Ultrasound examination showed bilateral Achilles tendon ruptures. Diagnostics, etiology and therapeutic options are discussed.
Wir berichten über einen 73-jährigen Patienten mit schwerer Exazerbation einer chronisch-obstruktiven Pneumopathie. Unter Therapie mit Glukokortikoiden und sequenziell zwei verschiedenen Chinolonen kam es zu einer beidseitigen Achillessehnenruptur. Wir diskutieren die Häufigkeit dieser Komplikation, die diagnostischen Methoden und die möglichen Therapieoptionen.
