De nombreux requérants d’asile et réfugiés présentent des signes de traumatisme. Une condition primordiale pour que les personnes concernées puissent entreprendre une nouvelle vie est qu’elles reçoivent un traitement. Des centres de thérapie spécialisés offrent à cet effet des thérapies ciblées pour les victimes d’actes de torture, de guerre et de persécution. Dans ces centres, le recours à l’interprétariat communautaire est considéré comme faisant partie intégrante des structures et des processus, et le travail des interprètes est reconnu comme un service de soutien essentiel dans le processus médical de guérison.
Traumatic elbow dislocations are among the most common injuries in sport climbing. They occur most frequently in bouldering (a climbing discipline with strong upward trend often performed indoors) due to the typical low-height backward fall into crashpads. There is still no data about the functional outcome and return to sport of this typical bouldering injury. All Patients with elbow dislocations due to a bouldering associated fall between 2011 and 2020 were identified retrospectively in our level I trauma centre. Trauma mechanisms, injury types and therapies were obtained. Follow-up was performed with an online questionnaire including sports-related effects, return to sport and the Elbow Self-Assessment Score (ESAS). 30 patients with elbow dislocations after bouldering accidents were identified. APR-246 (Eprenetapopt/PRIMA-1Met) is a very potent anti-cancer drug in clinical trials and was initially developed as a p53 refolding agent. As an alternative mode of action, the elevation of reactive oxygen species (ROS) has been proposed. Through an in silico analysis, we investigated the responses of approximately 800 cancer cell lines (50 entities; Cancer Therapeutics Response Portal, CTRP) to APR-246 treatment. In particular, neuroblastoma, lymphoma and acute lymphocytic leukemia cells were highly responsive. With gene expression data from the Cancer Cell Line Encyclopedia (CCLE; n = 883) and patient samples (n = 1643) from the INFORM registry study, we confirmed that these entities express low levels of SLC7A11, a previously described predictive biomarker for APR-246 responsiveness. Combining the CTRP drug response data with the respective CCLE gene expression profiles, we defined a novel gene signature, predicting the effectiveness of APR-246 treatment with a sensitivity of 90% and a specificity of 94%. We confirmed the predicted APR-246 sensitivity in 8/10 cell lines and in ex vivo cultures of patient samples. Moreover, the combination of ROS detoxification-impeding APR-246 with approved HDAC-inhibitors, known to elevate ROS, substantially increased APR-246 sensitivity in cell cultures and in vivo in two zebrafish neuroblastoma xenograft models. These data provide evidence that APR-246, in combination with HDAC-inhibitors, displays a novel potent targeted treatment option for neuroblastoma patients.
Der zukünftig fortschreitende Anspruch der evidenzbasierten Medizin macht die flächendeckende Erfassung von funktionellen Therapieergebnissen nach unfallchirurgischen Behandlungen erforderlich. Insbesondere die subjektive, patientenbasierte Ergebniszufriedenheit steht im Mittelpunkt. Patient reported outcome measures (PROM) sind ein geeignetes Mittel, um diese Ergebnisse valide, kostengünstig und zeiteffizient zur erfassen. Zudem bieten moderne technische Möglichkeiten vielversprechende Chancen, um speziell die unfallchirurgisch wichtige Erhebung der spezifischen Gelenkfunktion, einschließlich Bestimmung des Bewegungsumfangs, zuverlässig, z. B. mithilfe des Smartphone-Einsatzes, zu bestimmen.
Zusammenfassung Hintergrund Seit ihrer Zulassung in Deutschland im Juli 2019 erfreuen sich E‑Scooter zunehmender Popularität. Diese steigende Beliebtheit und die einfache Zugänglichkeit der E‑Scooter für jedermann haben jedoch auch zu teils schweren Unfällen geführt. Ziel der Arbeit Das Ziel dieser Studie war es, die Art und Schwere der Verletzungen, die in direktem Zusammenhang mit der Nutzung von E‑Scootern in einer deutschen Millionenstadt stehen, zu analysieren und hieraus Schlüsse für zukünftige Sicherheitskonzepte und Verhaltensregeln zu ziehen. Methodik Alle Patienten, die sich aufgrund eines Unfalls mit einem E‑Scooter in der interdisziplinären Notaufnahme des Klinikums rechts der Isar, Universitätsklinikum der Technischen Universität München, zwischen dem 01.07.2019 und dem 01.04.2020 vorstellten, wurden erfasst und sowohl demografische Daten als auch Informationen zu Traumamechanismus und den entstandenen Verletzungen dokumentiert. Ergebnisse Im oben genannten Zeitraum wurden prospektiv 60 Patienten erfasst, wovon 34 (56,7 %) Männer waren. Durchschnittlich waren die Patienten 34,7 Jahre (18 bis 73 Jahre) alt. Unter Alkoholeinfluss fuhren 22 Patienten (36,7 %); ein Helm wurde lediglich von einer Person getragen. Verletzungen des Kopfes waren mit Abstand am häufigsten, gefolgt von Verletzungen der oberen und der unteren Extremität (Radiusköpfchenfraktur n = 5, Riss-Quetsch-Wunden an Fuß/Sprunggelenk n = 8). 2 Patienten (3,3 %) waren schwer verletzt (ISS ≥16) Diskussion Mit zunehmender Beliebtheit der E‑Scooter steigt auch die Anzahl an Verletzungen. Am häufigsten ist die Kopfregion betroffen, weshalb zukünftig eine Helmpflicht sinnvoll erscheint. Zudem sollten eine breitere Informationskampagne und strengere polizeiliche Kontrollen im Hinblick auf die Vielzahl an alkoholisierten Unfallopfern erfolgen.
The European Extremely Large Telescope (ELT) is a 39-m Class telescope with active and adaptive capability included into the telescope being developed by the European Southern Observatory (ESO). The Telescope Secondary (M2) and Tertiary (M3) Mirrors are 4-metre class Zerodur mirrors close to 3.2 Tons that are passively supported by Cells with an 18 points axial whiffletree and a warping harness system that allows to correct low order deformations of the Mirrors. Laterally the Mirrors are supported on 12+2 points by Lateral Supports. In addition, the Cells have alignment capabilities by means of a high precision hexapod. SENER has been contracted by ESO for the design, construction, validation and delivery of the ELT Secondary Mirror (M2) and Tertiary Mirror (M3) Cells. The Cells' mechanisms guarantee the alignment of the Telescope during observation while correcting optics deformations. In this process, a high precision hexapod will be responsible for aligning and tracking the mirrors and an active structure will be used to compensate errors in the mirrors' surface. These are large-size critical elements of the Telescope that require extremely high precision levels to give the Telescope optimal image quality. This manuscript describes the preliminary design and key aspects of the ELT M2 and M3 Mirrors Cells mechanisms, in particular the Mirror Suppor.
Hintergrund/Einleitung: Die p53-Familienmitglieder p63 und p73 weisen nicht nur eine strukturelle Homologie zu p53 auf, sondern können wie p53 Zellzyklusarrest und Apoptose, die entscheidenden Mechanismen der p53 Tumorsuppression, induzieren. Hinsichtlich der Apoptose-Induktion nach DNA-Schädigung wird eine Abhängigkeit der p53-Familienmitglieder untereinander vermutet; allerdings sind die exakten Apoptose-Signalwege / -Zielgene von p63 und p73 noch unbekannt.
Hintergrund/Einleitung: Funktionelle Inaktivierung von p53 führt zur Apoptose- und damit verbundenen Therapieresistenz von malignen Tumoren. Flores et al. (Nature vol. 416, 560) konnten kürzlich zeigen, dass p53 zur Apoptoseinduktion nach DNA-Schädigung p63 und p73 benötigt. Es liegen bisher keine Daten vor, inwieweit p63 und p73 bei einer p53-rekonstituierenden Gentherapie berücksichtigt werden sollten.
Hintergrund/Einleitung: p63 kann, ebenso wie die beiden anderen Familienmitglieder, p53 und p73, Apoptose auslösen. Die molekularen Mechanismen der Apoptose-Induktion durch p63 sind bisher ungeklärt.

Bisherige Arbeiten unserer Arbeitsgruppe haben gezeigt, dass der Status der p53-Familie eine zentrale Rolle für Prognose und Therapieansprechen des Hepatozellulären Karzinoms (HCC) spielt. TAp73 ist – wie p53– u.a. ein bedeutender Vermittler der Apoptose. Im Gegensatz zur pro-apoptotischen Wirkung der TA-Isoform führt die Expression von DeltaNp73 (DNp73) beim HCC zu Apoptosehemmung und Chemoresistenz und geht mit einer ungünstigen Prognose einher. Ziel der vorliegenden Arbeit war die Identifizierung und Charakterisierung TAp73- und DNp73- regulierter microRNAs (miRs) und deren funktioneller Bedeutung für das Therapieansprechen des HCC.
An acute Hepatitis B Virus (HBV)-infection is characterized by a vigorous, polyclonal and multi-specific CD8+ T cell response. CD8+ T cells are the main effector cells responsible for viral clearance and disease pathogenesis during an acute HBV-infection. CD95 Ligand (CD95L) expression is generally restricted to a few cell types, e.g. T cells, macrophages and NK cells. We have previously shown that an intact CD95 signalling pathway is required for elimination of HBV-infected hepatocytes. Furthermore, we have identified an AP–1 element in the CD95L promoter which is essential for the transactivation of the CD95L gene. The aim of this study was to analyze CD95L transactivation and -expression in HBV-infected hepatocytes. Adenoviral transfer of HBV constructs into the hepatoma cell line HepG2 resulted in the transactivation, transcription and expression of CD95L. Four different CD95L promoter constructs (CD95L –36, –220, –536, and –1204/+100) have been used to identify the binding site responsible for CD95L transactivation in HBV-infected hepatocytes. However, there was no particular difference in the induction of the different constructs. This led to the hypothesis that no binding site between –36 and –1204 functions as the major binding site responsible for CD95L transactivation in HBV-infected hepatocytes. Instead, inhibition of JNK resulted in a significant inhibition of CD95L transactivation. Since HBV infection resulted in the phosphorylation of JNK (Jun-NH2 kinase) and the subsequent activation of the transcription factor AP–1, we postulate that CD95L transactivation is triggered by Thr183/Tyr185 phosphorylation of JNK. We characterized the molecular mechanisms resulting in CD95L expression in HBV-infected hepatocytes. In this study we provide evidence that HBV infection leads to the phosphorylation of JNK, AP–1 dependent transactivation of CD95L and finally CD95L expression, suggesting a potential CD95L activation independent of CD8+ T cells in HBV infection.
MicroRNAs (miRNAs) are small endogenous noncoding RNAs that were shown to be essential posttranscriptional regulators of gene expression. MiRNAs most notably members of the miR–34 family, have been shown to be subject to transcriptional regulation by wild-type p53. We have established a relevant role for the p53 family in the induction of apoptosis, therapeutic response and prognosis of hepatocellular carcinoma (HCC). TAp73–like wild-type p53- is a mediator of apoptosis. The dominant negative isoform of p73 (DNp73)–on the contrary–is antiapoptotic and enhanced expression of DNp73 correlates with reduced survival of HCC patients.
Bcl-2 family proteins serve as critical regulators in apoptosis, either promoting or inhibiting cell death via the mitochondrial apoptosis signaling pathway. Wildtype p53 promotes apoptosis via transcription-dependent and transcription-independent mechanisms. The latter may be mediated by direct translocation of p53 to the mitochondria where it leads to outer mitochondrial membrane permeabilization (OMMP) by forming an inhibitory complex with the anti-apoptotic Bclxl protein.
Drug Prescribing for Patients with Chronic Kidney Disease in General Practice: a Cross-Sectional Study
MicroRNAs (miRs) sind kleine, nicht codierende RNA-Moleküle, die die Genexpression auf post-transkriptioneller Ebene regulieren und als Tumorsuppressoren oder Onkogene fungieren können. Ein Beispiel für Tumor-suppressive Funktionen von miRs ist die Transaktivierung von miR-34a durch Wildtyp (wt) p53, die zur Induktion von Apoptose und Zell-Zyklus-Arrest führt. Die Rolle von wt p53 und TAp73 in der Aktivierung von miRs beim Hepatozellulären Karzinom ist nicht untersucht. Bisherige Arbeiten unserer Arbeitsgruppe haben gezeigt, dass der Status der p53-Familie eine zentrale Rolle für Prognose und Therapieansprechen des Hepatozellulären Karzinoms spielt. Ziel der vorliegenden Arbeit war die Identifizierung und Charakterisierung wt p53- und TAp73- induzierter miRs, die essentiell für die durch die p53-Familie vermittelte DNA-Schädigungs-Antwort (DNA damage response) und damit relevant für das Therapieansprechen beim Hepatozellulären Karzinom sind.
Multizelluläre Tumor Spheroide (MCTS) stellen ein gut etabliertes 3-D in vitro Modellsystem dar, welches die pathophysiologische in vivo Situation in Tumormikroregionen simuliert. Insbesondere ist es mithilfe von MCTS möglich Tumor-, Immunzell-Interaktionen zu analysieren. Eine akute Hepatitis-B-Virus (HBV)-Infektion ist durch eine starke, multispezifische CD8+ T-Zellantwort gekennzeichnet. Der genaue molekulare Mechanismus der T-Zell vermittelten Immunantwort ist noch nicht vollständig geklärt. Wir konnten bisher zeigen, dass ein intaktes CD95-Apoptose-System essentiell für die Elimination HBV-infizierter Hepatozyten ist. Ziel dieser Arbeit war es, die CD95-vermittelte Apoptose von HBV-infizierten Leberzellen durch CD95 Liganden-exprimierende Lymphozyten näher zu charakterisieren.
Die p53-Familie, besteht aus den transkriptionell aktiven (TA-) Isoformen p53, TAp63 und TAp73, die Apoptose induzieren, und dominant negativen (DN-) Isoformen, die die TA-vermittelte Apoptose inhibieren. Wir konnten zeigen, dass die Imbalance der TA- und DN-Isoformen der p53-Familie bei Überwiegen der DN-Isoformen zu Entstehung, Progression und Therapieresistenz des Hepatozellulären Karzinoms (HCC) führt. Darüber hinaus konnten wir zeigen, dass die Expression von Myeloid cell leukemia-1 (Mcl-1), ein anti-apoptotisches Mitglied der Bcl-2 Familie, mit einer verminderten Apoptosesensitivität und Chemoresistenz im HCC einhergeht.
