Metaiodobenzylguanidine (MIBG) labelled with iodine-131 (I-131) has become a well established therapeutic tool for inoperable metastastic tumours of paraganglioma. There are different pharmacological substances known to interfere with MIBG-uptake which may result in a false negative MIBG scan. We present the case of a 26-year-old male polytoxicomanic patient with metastatic paraganglioma, who underwent MIBG therapy. During earlier therapies, MIBG uptake in the metastatic lesions was very high. A post-therapeutic whole-body scan subsequent to recent I-131-MIBG therapy failed to detect the vast majority of metastatic lesions-except for two. F-18-fluorodeoxyglucose (FDG) positron emission tomography (PET) showed metastases with a similar distribution to the initial MIBG scan. The possible reasons for the discrepancy in the findings of the MIBG scans and the F-18-FDG-PET scan are discussed with special emphasis on drug intake prior to MIBG administration, increased MIBG turn-over and unknown drug mixture interference with MIBG uptake.
Renal abscesses are rare in childhood. The diagnosis is often complicated by non-specific symptoms and the typical signs of urinary tract infections are frequently absent. The currently available imaging methods are necessary and helpful for a differentiated therapeutic approach; nevertheless, cases are continuously being found in which a renal abscess is only diagnosed intraoperatively. In most patients a combined intravenous therapy including an antibiotic which is effective against staphylococci is sufficient. The therapy is supported if necessary by percutaneous abscess drainage. Open revision or even nephrectomy is rarely required.
Die transiente Läsion des Spleniums mit Diffusionsrestriktion ist eine seltene Erkrankung in der pädiatrischen Population. Diese Läsionen wurden im Rahmen von infektiösen Ereignissen sowie bei Patienten mit Epilepsie beobachtet und bislang 35 mal in der Literatur beschrieben. Ziel dieser Arbeit ist die Präsentation dieser seltenen Entität.
Die empfohlenen Dosiswerte für Thoraxaufnahmen von Neonaten entstammen der Ära vor Einführung der Digitaltechnik. Ziel war die Erprobung unterschiedlicher Belichtungsparameter zum Erreichen einer Dosisreduktion bei Anwendung der digitalen Röntgentechnik.
Durch die Durchführung von Ganzkörper-MRT Untersuchungen im Rahmen von Stagings oder Restagings bei pädiatrisch-onkologischen Patienten sowie bei der Fokussuche können CT-grafisch bedingte Strahlenexpositionen vermieden werden. Es sollte überprüft werden, ob die gezielte Vorbereitung mittels Hydro-MRT zur Verbesserung der Bildqualität und Detektion von Pathologien beiträgt.
Röntgenaufnahmen auf Intensivstationen werden im Tagdienst von MTRAs der Kinderradiologie, nachts von MTRAs der Allgemeinradiologie sowie von Intensivärzten angefertigt. Aufgrund der klinischen Bedeutung von Intensivaufnahmen und der zugrunde liegenden gesetzlichen Vorgaben seitens der Röntgenverordnung wurde die Qualität der erstellten Aufnahmen untersucht.
Nierenabszesse im Kindes- und Jugendalter sind selten. Die Diagnosestellung wird durch unspezifische Symptome oft erschwert. Häufig fehlen die typischen Zeichen einer Harntraktinfektion. Die heute verfügbaren bildgebenden Verfahren sind für ein differenziertes therapeutisches Vorgehen notwendig und hilfreich. Dennoch gibt es immer wieder Fälle, bei denen erst intraoperativ die Diagnose eines Nierenabszesses gestellt wird. Bei den meisten Patienten ist eine i.v.-Kombinationstherapie unter Einschluss eines staphylokokkenwirksamen Antibiotikums, ggf. unterstützt durch eine perkutane Abszessdrainage, ausreichend wirksam. Selten sind eine offene Revision oder gar eine Nephrektomie erforderlich.
Zielstellung: Der hochauflösende Ultraschall (US) mit modernen Geräten eignet sich neben der konventionellen Abdomenübersichtsaufnahme für die Diagnostik der Nekrotisierenden Enterokolitis (NEK). In der Literatur ist im Rahmen einer NEK wenig über den Vergleich bezüglich der Abgrenzbarkeit einer Pneumatosis intestinalis (Pi) im US gegenüber der Abdomenübersichtsaufnahme bekannt. Ziel dieser Studie ist es, die beiden bildgebenden Modalitäten in Bezug auf den Nachweis einer Pi miteinander zu vergleichen. Material und Methoden: Es wurden die Untersuchungen aller Säuglinge mit klinischem Verdacht einer NEK (geblähtes Abdomen und blutige Stühle) während eines 10-jährigen Zeitraumes von 2003 bis 2013 analysiert, um sonographische (Pi, portal-venöse Luft, Darmwandveränderungen, Darmdistension, freie Flüssigkeit) sowie röntgenologische Zeichen (Pi, Darmdistension, Pneumoportogramm, freie Luft) einer NEK zu vergleichen. Ergebnisse: Von 79 Säuglingen mit V.a. NEK konnte bei 29 diese Verdachtsdiagnose gesichert werden. Bei 15 der 29 Säuglinge konnte eine Pi in der Abdomenübersichtsaufnahme nachgewiesen werden. 9 dieser 15 Patienten wiesen darüber hinaus auch im US eine Pi auf, wobei bei 3 Säuglingen aufgrund von Meteorismus oder nicht durchgeführtem US keine Aussage über das Vorliegen einer Pi gemacht werden konnte. Bei 3 der 15 Patienten konnte hingegen eine Pi im US nicht abgegrenzt werden, wohl aber der Nachweis von portal-venöser Luft oder von Luftbläschen im Leberparenchym als indirektes Zeichen von aus der Darmwand mesenterial fortgeleiteter Luft. Zusätzlich zu den 15 Säuglingen konnte man bei weiteren 6 Patienten am Ende des Untersuchungszeitraums eine Pi nicht im Röntgen, sondern nur im US nachweisen, was zudem mit der Einführung eines Gerätes mit verbesserter Auflösung (iU22, Philips) einhergeht. Zudem konnten bei 10 der 29 Patienten im US freie Flüssigkeit und bei 6 Darmwandveränderungen nachgewiesen werden. Im Röntgen zeigten sich bei 22 Säuglingen Darmdistensionen, bei jeweils 3 Patienten konnte freie Luft und ein Pneumoportogramm abgegrenzt werden. Schlussfolgerung: Unter guten Schallbedingungen und mit einem hochauflösenden US-Gerät zeigt sich ein suffizienter Nachweis einer Pi direkt durch hyperechogene Reflexe oder indirekt durch fortgeleitete portal-venöse bzw. hepatische Luft. Die Sonografie kann darüber hinaus eine Pi sensitiver nachweisen, bevor sie sich radiographisch in der Abdomenübersichtsaufnahme manifestiert. Dies stützt die zunehmende Bedeutung der Sonografie im Rahmen der Diagnostik einer NEK neben der konventionellen Röntgendiagnostik.
Das kaudale Regressionssyndrom (KRS) ist eine seltene angeborene Fehlbildung der unteren Wirbelsäulensegmente und des Neuralrohres. Typische Symptome sind meist motorische und sensorische neurologische Defizite der unteren Extremität sowie eine verminderte Blasen- und Darmkontrolle. Eine auffällig erscheinende Sakralregion in Kombination mit einem Gestationsdiabetes der Mutter sind weitere Auffälligkeiten, die an ein kaudales Regressionssyndrom denken lassen sollten. Es wird über ein Kleinkind berichtet, bei dem als ungewöhnliche Erstmanifestation eines KRS ein akuter Harnverhalt aufgetreten ist.
Caudal regression syndrome is a rare and sporadic congenital developmental defect of the lower spinal segments and the neural tube. Movement disorders and sensory neurological deficits of the lower extremities in conjunction with impaired bladder and bowel control are the major symptoms. Abnormal visual aspects of the sacral region in combination with maternal gestational diabetes are further diagnostic indications. This article reports the unusual case of a toddler presenting with acute urinary retention as the initial symptom of caudal regression syndrome.
Hintergrund: Die progressive Leberfibrose ist eine bedeutsame Manifestationsform der zystischen Fibrose im Kindesalter. Die korrekte Diagnose einer potentiellen Leberbeteiligung im Rahmen von CF stellt eine besondere Herausforderung dar. Die sonographische Elastometrie mittels ARFI stellt in diesem Zusammenhang einen neuen, nicht-invasiven diagnostischen Ansatz zur Charakterisierung der mechanischen Eigenschaften des Leberparenchyms dar.

AIM:The aim of the present FDG PET study was to evaluate the prognostic value of the standardized uptake value (SUVmax) of head and neck cancer (HNSCC) with respect to the chemotherapy response and tumor recurrence.METHODS:The FDG PET findings of 40 patients with HNSCC were compared with the final histopathology results after removal of the primary tumor and surgical neck dissection. The clinical T staging was based on clinical examinations and computed tomography was used for assessment of bone involvement. The pretreatment baseline SUVmax of the primary tumor were correlated with the intra-arterial chemotherapy response prior to the tumor resection and the frequency of tumor relapse.RESULTS:The median SUVmax of tumors which did not relapse was 3.4, compared to a SUVmax of 4.7 for tumors with local tumor relapse (p=0.36, n.s.). Regarding chemotherapy response, the tumor SUVmax was significantly lower in cases with complete remission (CR) (median 2.6, n=11) compared to those with stable disease (5.8, n=10), (p=0.002). Whereas no tumor with CR after chemotherapy relapsed except stage IV tumors, tumor relapse was observed in both a stage II and a stage IV tumor without chemotherapy response.CONCLUSIONS:In patients with HNSCC the tumor SUVmax seems to be a useful prognostic indicator for assessing the clinical chemotherapy response, but did not correlate significantly with the recurrence risk. Thus, in tumors with higher SUVmax alternative chemotherapy regimes have to be discussed.
Objective To investigate uterotubal transport by means of hysterosalpingoscintigraphy (HSSG) in women with and without endometriosis.Design A prospective observational study.Setting University Hospital, Department of Obstetrics and Gynaecology, Division of Reproductive Medicine and Gynaecologic Endocrinology with 350 in vitro fertilisation (IVF)/intracytoplasmic sperm injection (ICSI) cycles and 400 intrauterine insemination (IUI) cycles/year.Population Cases included 56 infertile women with laparoscopic proven endometriosis and patent fallopian tubes. Twenty-two women with partners suffering from male factor infertility served as controls.Methods A diagnostic cycle incorporating HSSG was performed. Subsequently, patients underwent either four cycles of timed intercourse (TI) or IUI in order to achieve pregnancy. If pregnancy did not occur, IVF or ICSI was performed.Main outcome measures Evaluation of uterotubal transport capacity in women with endometriosis and healthy controls.Results Patients suffering from endometriosis (group I) showed a significant reduction in physiologic uterotubal transport function: While 20 patients (36%) had ipsi- or bilateral uterotubal transport, there was pathological uterotubal transport contralateral to the dominant follicle or a complete failure of transport capacity (negative HSSG) in 36 patients (64%). In the controls (group II), transport function was significantly different: 15 of 22 patients (68%) revealed ipsi- and bilateral tubal demonstration, while 5 patients (22%) showed contralateral transport and 2 patients (10%) showed negative HSSG (P= 0.01). Twenty-three pregnancies were observed (pregnancy rate: 29%). Eleven out of 14 (79%) women with ipsi- or bilateral tubal transport function fell pregnant by means of TI or IUI. In seven of nine patients (78%) with a failure in tubal transport, pregnancy was achieved by IVF/ICSI, despite acceptable semen parameters (P= 0.01).Conclusions Endometriosis is significantly associated with a reduction in physiologic uterotubal transport capacity compared with controls. This resulted in diminished pregnancy rates even in women with normozoospermic partners. Therefore, IVF/ICSI may be required even when fallopian tubes are patent or semen quality is normal.
This clinically oriented review presents main principles of metabolism of cardiac muscle, pathophysiology of myocardial hibernation and stunning, as well as methodological principles of positron emission tomography (PET) of the heart with (18)F-fluoro-2-deoxyglucose ((18)F-FDG). Diagnostic and prognostic value of (18)F-FDG PET and scintigraphic sings of disturbed myocardial viability, contractility and metabolism are also described. Efficacy of (18)F-FDG PET is compared with other imaging methods such as radionuclide, ultrasound and radiological. Literature data and clinical cases demonstrate importance of preoperative diagnosis of hibernating myocardium in patients with ischemic heart disease. (18)F-FDG PET is a basic method of detection of potentially reversible pathological states of the heart (hibernation and stunning); it has high sensitivity and specificity as well as predictive power in relation to forthcoming course of ischemic heart disease. This noninvasive method of investigation provides unique information on severity of ischemic heart disease for stratification of patients in risk groups and selection of candidates for coronary artery bypass surgery or cardiac transplantation.
Purpose: Prerequisite for lymphoscintigraphy-guided sentinel Iymph node (SLN) biopsy in head and neck cancer patients has been a previously untreated primary tumor to avoid disturbance of the lymphatic flow. We evaluated the feasibility in a multimodality treatment approach to oral and oropharyngeal cancer comprising presurgical intra-arterial chemotherapy. of the primary, tumor.Patients and Methods: In a prospective study of 35 consecutive patients with primary oral and anterior oropharyngeal cancer T1-4N0 treated with superselective intra-arterial chemotherapy with high-dose cisplatin, SLN scintigraphy was performed on the day of surgery, including gamma camera imaging before and hand-held gamma probe identification of the SLN during surgery. SLN, biopsy was perfomed in 50 neck sides, followed by radical resection of the primary in the same session. Elective neck dissection was not carried out. The pattern of lymphatic drainage was recorded and compared with the literature on well-known patterns found in lymphographic studies and metastatic disease in neck dissection specimens of patients without presurgical treatment.Results: Drainage patterns of AN related to different primary; locations mapped the mentioned comparison patterns. SLN drainage patterns gave strong evidence that there are SLN in levels 3 and 4 with direct drainage from the primary. After a median observation time of 24 months, no patients had a neck relapse.Conclusions: The results suggest that intra-arterial chemotherapy of the primary tumor does not significantly alter lymphatic drainage and that SLN biopsy following intra-arterial chemotherapy is feasible and offers valid results comparable to those obtained from patients without presurgical treatment.
