Identifier les facteurs prédictifs de mortalité à long terme à partir d'une cohorte de sujets âgés de 75 ans ou plus, hospitalisés via un service d'accueil des urgences (SAU).Les variables issues de l'évaluation gériatrique standardisée des patients de la cohorte multicentrique SAFES ont été utilisées dans un modèle de Cox pour prédire la mortalité au cours des 3 ans de suivi.Cette cohorte était constituée de 1 306 patients âgés en moyenne de 85±6 ans. Au décours des 3 ans de suivi, 50 % des sujets étaient décédés. Après ajustement sur le centre d'investigation, les variables sociodémographiques (âge, sexe, niveau d'éducation et lieu de vie) et le niveau de comorbidité, les variables influençant significativement la mortalité au cours des 36 mois étaient : un état de dénutrition sévère (p < 0,03), l'existence d'une démence (p < 0,001) et/ou d'un syndrome confusionnel (p < 0,001), la présence de troubles de la marche (p = 0,001) et des antécédents récents d'hospitalisation (p < 0,001). Aucun lien significatif n'a été mis en évidence entre le risque de dépression et la mortalité (p = 0,40).À l'aide d'échelles simples, couramment utilisées en gériatrie, une approche prédictive de la mortalité à 3 ans est possible. Une correction de l'état nutritionnel, une reconnaissance et une prise en charge des troubles cognitifs ainsi qu'une rééducation fonctionnelle doivent faire partie des priorités de soin.To identify the factors predictive of long-term mortality among a cohort of subjects aged 75 years or older hospitalized from the emergency department.Variables from the standardized geriatric assessment of members of the multicenter SAFES cohort were applied to a Cox model to predict mortality over a 3-year follow-up.This cohort comprised 1306 patients with a mean age of 85±6 years. Half the patients died during the 3 years of follow-up. After adjustment for center, social and demographic variables (age, sex, educational level, and community or institutional residence) and level of comorbidity, the variables that significantly influenced mortality over the next 36 months were: severe malnutrition (p < 0.03), dementia (p < 0.001) and/or confusion (p < 0.001), walking problems (p = 0.001) and recent hospitalization (p < 0.001). No significant association was found between depression and mortality (p = 0.40).A predictive approach to mortality at 3 years is possible with simple scales widely used in geriatrics. Correction of malnutrition, recognition and management of cognitive disorders, and functional rehabilitation must be included in the priorities of care.
In the last decades, the pharmaceutical market has experienced an increase in the number of therapeutic proteins. The high activity and selectivity of these macromolecules is often achieved at the expense of complex structures, which exhibit several biophysical properties that must be carefully controlled and optimized for the successful development of these drugs as well as for guaranteeing their quality and safety. This need has motivated the application of a variety of biophysical techniques to analyze properties of therapeutic proteins and protein solutions including interactions, aggregation, solubility, viscosity, and thermal stability. After briefly summarizing currently available experimental approaches, we highlight the emerging possibilities offered by advances in microfluidic technology for the analysis of therapeutic proteins during manufacturing and formulation.
But de l'étude : L'organisation des Samu-Smur offre la possibilité d'admettre les patients directement en service spécialisé. Le but de cette étude était de vérifier que l'admission directe (AD) en service spécialisé est justifiée soit par la gravité de la pathologie soit par la thérapeutique.Patients et méthodes : L'étude a été réalisée rétrospectivement au Samu de Paris. Elle a concerné l'ensemble des patients admis en service spécialisé pour lesquels le dossier comportait : le compte rendu d'hospitalisation (CRH), la feuille de transport et le compte rendu de régulation. L'analyse de ces éléments a recherché si le diagnostic ayant motivé l'admission était exacte, si le patient présentait un critère de gravité, si le patient avait reçu un traitement spécialisé, et si la prise en charge du patient était cohérente.Résultats : En 1997, 409 (31 %) dossiers ont été étudié parmi les 1 350 patients admis en réanimation. Trois groupes de patients ont été distingués : réanimation chirurgicale ou traumatologique (n = 54), réanimation médicale ou polyvalente (n = 180), et unité de soins intensifs cardiologiques (n = 175). Le diagnostic du Smur était validé par le CHR dans 91 % des cas dans les trois groupes. La gravité patente ou potentielle était retrouvée dans 100 % des cas. La justification thérapeutique était respectivement de 96, 88 et 84 %, la cohérence de la démarche diagnostique ou thérapeutique de 94, 96 et 89 %.Discussion : La régulation des Samu-Smur a permis une AD justifiée en service spécialisé pour l'ensemble des patients de l'étude. Des études complémentaires prospectives comparant un collectif important de patients sont nécessaires pour démontrer les avantages de cette stratégie en termes de rapidité de prise en charge et de pronostic.Study design: The French system of Samu-Smur allows the admission of patients directly in intensive care unit (ICU). The aim of this study is to examine the utility of the Samu-Smur with regard to such direct admission (DA).Patients and methods: This retrospective study was performed by the Samu of Paris. Patient details were gathered from three reports : namely hospitalisation, transport and regulation reports. These were analysed to decide whether the admission diagnostic was exact, whether the patient's condition was serious, whether the prehospital treatment justified direct admission into an ICU and whether the management was coherent.Results: In 1997, 409 (31%) cases were studied among the 1,350 admitted patients in ICU. Three groups of patients were classified according to admission to surgical (n = 54), medical (n = 180), cardiological ICU (n = 175). The prehospital diagnosis was confirmed by the hospitalisation report in 91% of patients in the all three groups. The patient's condition was found to the serious in all cases. Justification of the treatment was respectively found in 96, 88 and 84% of patients. The coherence of management was confirmed in 94, 96 and 89%.Discusssion: This study has shown that Samu-Smur management lead to justifie DA in ICU for all patients in the study. Prospective studies are needed to show the advantages of this strategy in term of speed of management and outcome.
