More and more applications of artificial intelligence technologies are made in biomedical software and equipment. These applications are multiple: intelligent alarms, intelligent monitoring, diagnosis support, …. Several different knowledge representation schemes are already in use: decision trees, first-order logic expert systems, calculations (mathematical modeling), trained neural network simulations, …. All these techniques have their own preferred field of application, and they do not overlap. Building a complete diagnosis support tool would require the use of several of these techniques. The problem is therefore communication between these very different systems and the complexity of the composite result. This paper describes the Think! formalism: a unified symbolic-connectionist representation scheme which tries to subsume some of the precited formalisms. Being able to integrate these knowledge representation schemes in a single model enables us to use existing knowledge bases and existing knowledge extraction techniques to make them communicate and work together.
Le registre EPECIM a évalué la pertinence des stratégies préhospitalières de reperfusion et leur délai de mise en route dans une grande métropole du sud de la France. Registre prospectif qui inclut 194 syndromes coronariens avec sus-décalage persistant du segment ST (SCA ST+) pris en charge par un SMUR de Marseille entre juin 2003 et décembre 2004. Une stratégie d’angioplastie primaire est proposée à 136 patients (70 %), une fibrinolyse 19 fois (10 %). Pour 39 patients (20 %) aucune stratégie de reperfusion n’est débutée en SMUR. L’accès direct à la salle de coronarographie ne concerne que 101 patients (52 %), 78 patients sont admis en USIC (40 %). Les délais moyens d’évolution des symptômes sont de 162 minutes au moment de la prise en charge, la médicalisation préhospitalière dure en moyenne 49 minutes. La fibrinolyse est débutée 26 minutes en moyenne après le début de la médicalisation contre 66 minutes pour une angioplastie avec accès direct en salle de coronarographie et 110 pour une angioplastie sans accès direct. Une stratégie de reperfusion peut toujours être débutée dans les délais recommandés. L’angioplastie, quand un accès direct en salle de coronarographie est possible, est souvent la meilleure stratégie. La fibrinolyse reste une bonne alternative pour les patients dont les symptômes évoluent depuis moins de trois heures. Les délais de prise en charge doivent être réduits par un appel direct et précoce du centre 15. Des efforts restent à faire pour faciliter l’accès aux structures de cardiologie interventionnelles. The EPECIM registry evaluated the pertinence of prehospital reperfusion strategies and their delays of starting in a big city in the south of France. This prospective registry include 194 cases of acute ST-segment myocardial infarction (STEMI) medicalised by the SMUR of Marseille between June 2003 and December 2004. A primary angioplasty strategy has been proposed to 136 patients (70%), a fibrinolysis in 19 cases (10%). For 39 patients (20%) no reperfusion strategy was carried out in SMUR. The direct access to the cath lab concerned 101 patients (52%), the other admissions were mostly at the cardiologic intensive units (78 admissions, 40%). The medium delays of symptoms evolution was 162 min, the prehospital medicalisation was 49 min long. The fibrinolysis was started 26 min after the onset of the symptoms versus 66 min for an angioplasty with direct access to the cath lab and 110 min for an angioplasty without direct access. A reperfusion strategy can always begin in the recommended delays. The angioplasty, when direct access is possible, is often the best solution. The fibrinolysis is a good alternative for the patients with three hours’ short symptoms. The treatment delays must be reduced by a direct and premature call of the 15 centers. The access of the interventional cardiology centers must be facilitated.
