Little information is provided in the literature describing an efficient reduction technique for pelvic ring disruption. The aim of this study is to assess the use of the transcondylar traction as a closed reduction technique for vertically unstable fracture-dislocations of the sacro-iliac joint. Twenty-four pelvic ring disruptions were treated with attempted closed reduction followed by percutaneous screw fixation. Transcondylar traction was used as a closed reduction technique. Closed reduction to within 1 cm of residual displacement was obtained in all cases. No incidence of infection, digestive, cutaneous, or vascular complications occurred. We observed secondary displacement in three patients. Correction of the vertical displacement is better achieved when performed within 8 days after the trauma. Two posterior screws and a complementary anterior fixation is typically required to avoid further displacement in case of sacral fractures. However, an open approach should be preferred in both cases of crescent iliac fracture-sacroiliac dislocation and transforaminal fracture associated with peripheral neurological deficit. A vertical sacral fracture should make the surgeon more wary of fixation failure and loss of reduction.
The mini-incision anterior approach in total hip replacement is not new, but uses a shorter incision than the traditional Hueter approach, typically only 6-8 cm in length. Despite its size, the single anterior incision allows good exposure. It is very atraumatic, preserves muscles and tendons, and allows the patient early mobilisation and fast postoperative recovery. Although, a special table (e.g., a Judet table) and specific tools (e.g., a curved reamer) are needed to perform hip replacement via the mini-anterior approach, any kind of hip prosthesis (cemented or uncemented) can be implanted. As there is a significant learning curve in mastering the mini-incision anterior approach, surgeons are advised to start with a longer incision and then to decrease its length with increasing experience.
For several years, special attention has been paid to minimally invasive prosthetic surgery by surgeons, prosthetic implant manufacturers, the media and patients, especially in English-speaking countries.
Pour comprendre certaines coxarthroses, il faut introduire la notion de mouvement. Un conflit n'existe que dans les mouvements extremes, il est suffisamment frequent pour etre a l'origine d'un nombre tres important de coxarthroses precoces. Il existe deux types de conflit anterieur de hanche: Conflit avec effet came car la tete du femur n'est pas parfaitement ronde et comporte une bosse / Conflit avec effet tenaille, la tete du femur est bien ronde mais c'est le col du femur qui vient, en fin de flexion, buter sur le labrum anterieur. Souvent ces deux types de conflit sont souvent associes. Il s'agit souvent de patients jeunes qui pratiquent des sports ou l'articulation est soumise a de grande amplitude en flexion. Dans le cas le plus caricatural, la simple flexion de hanche impose la mise en rotation externe. Sur la radio de face, on cherche une tete pas tout a fait ronde en crosse de revolver, et une retroversion du cotyle. La notion de conflit anterieur repose sur un faisceau d'argument: Clinique: diminution de la rotation interne, douleur inguinale en flexion, adduction et rotation interne. Radio: de face: tete phallique. absence d'offset, coxa vara, retroversion du cotyle / de profil chirurgical : meplat a la jonction tete col, voire tumefaction. Scanner: retroversion de la partie superieure du cotyle, faible anteversion du femur. IRM: modification de signal sur le col du femur a la jonction tete col, lesion du bourrelet, modification du signal de l'os sous chondral sur la partie anterieure du cotyle ou sur la corne posterieure. Chirurgicalement il faut faire disparaitre le conflit soit en creant un sillon a la jonction tete col soit en diminuant la retroversion du cotyle. Cette chirurgie a ete developpee en par Reinhold Ganz. Nous avons propose un geste chirurgical un peu moins invasif qui semble interessant dans les cas ou les modifications morphologiques ne sont pas majeures. Sur 57 cas, 41 se jugent satisfaits ou tres satisfaits. Aucun patient n'est tres mecontent. Aucune necrose de hanche n'a ete observee. Ce travail doit nous faire insister sur la prevention et le depistage de ce probleme d'anomalie de couple tete col / cotyle.
