Fatigue is a prevalent and disabling symptom in Sjögren’s disease (SjD), yet its pathogenesis remains poorly understood and inadequately managed. This narrative review addresses a critical gap in knowledge by conceptualizing SjD-related fatigue as a bidimensional phenomenon driven by both biomedical and psychosocial mechanisms, and framed within the model of persistent somatic symptoms (PSS). In this narrative review, findings from immunology, neuroendocrinology, psychology, and clinical research were synthesized to examine predisposing, triggering, and maintaining factors contributing to SjD-related fatigue. Particular attention was given to studies exploring immune dysregulation, neuroimmune signaling, autonomic imbalance, and psychological traits. Findings indicate that biomedical contributors—such as pro-inflammatory cytokine (e.g., interleukin [IL]-1β, IL-36α), neuroendocrine dysfunction, dysbiosis, and genetic predispositions—interact with psychosocial vulnerabilities, including early-life adversity, personality profiles, maladaptive coping strategies, and mood disorders. These interactions foster self-reinforcing loops of symptom persistence. Notably, current treatments targeting isolated domains yield limited success. Nonetheless, integrated strategies—ranging from biologics and cytokine modulators to exercise, cognitive behavioral therapy, and neuro-modulatory interventions—have demonstrated promise, particularly when aligned with the PSS framework. This narrative review underscores the need for a paradigm shift in fatigue management, advocating for multidimensional assessment and personalized, multimodal interventions. By illuminating the duality of SjD-related fatigue, this narrative review provides a roadmap for more effective diagnosis, research, and patient-centered care.
There is a wide spectrum of complications related to tattooing, among them local and regional granulomatous reactions. We report the well-documented case of a a 46-year old male patient who displayed an acute pseudolymphomatous reaction to the red pigment of a tattoo, followed by a systemic granulomatosis involving the skin, lymph nodes, kidneys, meninges, heart, and ultimately lungs and pleura with a refractory, although indolent, course over more than 10 years. The chronological and spatial sequences of clinical and pathological findings, including mineralogic studies, support the hypothesis of a multi-organ granulomatosis induced by an unknown antigen displayed in the red tattoo ink, in an individual genetically and immunologically predisposed to granulomatous inflammation, rather than a more trivial sarcoidosis with tattoo involvement.
Les troubles fonctionnels affectent fréquemment l’appareil cardiorespiratoire. La douleur thoracique non cardiaque touche plus de 10 % de la population et est très souvent associée à des troubles anxiodépressifs, notamment au trouble panique. Le syndrome de tachycardie orthostatique posturale (POTS), plus rare, est caractérisé par une forte augmentation de la fréquence cardiaque en position debout, sans hypotension orthostatique. Ses mécanismes physiopathologiques incluent une neuropathie autonome « partielle », une hypovolémie et une activation noradrénergique, mais il est aussi associé aux troubles anxieux et au déconditionnement à l’effort. Les troubles respiratoires fonctionnels, notamment le syndrome d’hyperventilation, s’accompagne de manifestations variées allant des crises de « tétanie » aux symptômes gastro-intestinaux. Leur prévalence est élevée, surtout chez les femmes et les personnes anxieuses, et particulièrement en cas de maladie respiratoire associée. Le diagnostic du syndrome d’hyperventilation repose sur le questionnaire de Nijmegen. Sa physiopathologie n’implique pas nécessairement une hypocapnie mais relève de cercles vicieux incluant une anxiété anticipatoire activant le système nerveux autonome. Les troubles fonctionnels cardiorespiratoires sont souvent liés entre eux et à d’autres troubles somatiques fonctionnels et ont en commun une dysrégulation du système nerveux autonome. Les facteurs psychologiques, cognitifs et comportementaux jouent un rôle central dans leur persistance. Si chacun a ses spécificités, ils ont tous une causalité circulaire complexe. Leur prise en charge repose actuellement avant tout sur une rééducation physique et des traitements symptomatiques comme les bêtabloquants. La place des thérapies cognitives et comportementales mériterait d’être davantage explorée dans ces troubles.
Functional disorders affecting the cardiorespiratory system are common. Non-cardiac chest pain affects more than 10% of the population. It can result from gastro-oesophageal reflux or chest wall disorders but is also very often associated with anxiodepressive disorders, particularly panic disorder. Postural orthostatic tachycardia syndrome (POTS), a rarer entity, is characterized by a sharp increase in heart rate without orthostatic hypotension when standing. The pathophysiological mechanisms of POTS include 'partial' autonomic neuropathy, hypovolaemia and noradrenergic dysregulation, but it is also closely linked to anxiety disorders and deconditioning to exertion. Functional respiratory disorders, particularly hyperventilation syndrome, are accompanied by various manifestations ranging from tetany to gastrointestinal symptoms. They are highly prevalent, especially among women and people suffering from anxiety, and particularly in the case of associated respiratory disease such as asthma. The diagnosis of hyperventilation syndrome is based on the Nijmegen questionnaire. The pathophysiology does not necessarily involve hypocapnia but is related to cerebral conditioning phenomena and vicious circles linked to anticipatory anxiety. Cardiorespiratory functional disorders are often linked to each other and to other functional somatic disorders and have in common a dysregulation of the autonomic nervous system. Psychological, cognitive and behavioural factors play a central role in their persistence. While each has its specificities, all have a complex circular causality. Their management currently relies primarily on physical rehabilitation, often associated with symptomatic treatments such as beta-blockers. The place of cognitive and behavioral therapies would be well worth exploring in these disorders.
Objectif Répertorier en France les terrains de stage agréés pour le diplôme d’études spécialisées (DES) de Médecine interne et Immunologie clinique (MIIC). Méthode Tous les coordonnateurs/trices locaux du DES de MIIC ont été contactés afin d’identifier les terrains de stage agréés pour le DES de MIIC dans leur subdivision. Résultats Nous avons répertorié 244 terrains de stage agréés, dont 87 (35,7 %) en centre hospitalier universitaire (CHU). En fonction des régions/subdivisions, le nombre des terrains de stage agréés pour le DES de MIIC varie beaucoup. Les terrains de stage de CHU et hors CHU ont l’agrément pour la phase socle dans 85 % (74/87) et 63,7 % (100/157) des cas, pour la phase d’approfondissement dans 94,3 % (82/87) et 84,1 % (132/157) des cas et pour la phase de consolidation dans 80,5 % (70/87) et 52,2 % (82/157) des cas, respectivement. Au cours du semestre d’hiver 2024, le nombre des postes occupés par rapport au nombre de postes ouverts au choix est plus important en CHU que hors CHU : en phase socle en CHU 133/149 (89,3 %) postes sont occupés versus 64/105 (61 %) hors CHU ; en phase d’approfondissement en CHU 150/169 (88,8 %) sont occupés versus 64/131 (48,9 %) hors CHU ; en phase de consolidation 77/87 (88,5 %) sont occupés versus 36/65 (55,4 %) hors CHU. Conclusion Trente-cinq pour cent des terrains de stage sont en CHU. En fonction des régions/subdivisions, le nombre des terrains de stage agréés pour le DES de MIIC varie beaucoup. Près de la moitié des terrains de stage hors CHU n’ont pas l’agrément pour la phase de consolidation. Au cours du semestre d’hiver 2024, près de la moitié des postes d’internes ouverts hors CHU n’étaient pas pourvus.
BACKGROUND:Following the results of the MANDARA trial, this real-life study aimed at comparing the effectiveness and safety profile of mepolizumab versus benralizumab in a European EGPA cohort. METHODS:We conducted a retrospective observational comparative study including EGPA patients, who received mepolizumab or benralizumab at the asthma dose. Patients were matched 1:1 by sex, age, BVAS and oral corticosteroid (OCS) dosage at the treatment initiation (T0). Complete response (CR) and partial response (PR), disease activity, OCS, pulmonary parameters, eosinophil count, relapses, and safety outcomes were also compared at 3, 6 and 12 months. RESULTS:Patients treated with mepolizumab or benralizumab (n = 88 each) were matched: 57 % were females, median age was 54 years (IQR 45-60), median OCS dose 10 (7.5-12.5) and 10 (7-13) mg/day, median BVAS 4 (2-7) and 3 (2-8), respectively. 45.4 % of patients in the mepolizumab group and 51.1 % in the benralizumab group achieved CR or PR at T3, with CR steadily increasing during follow-up for both treatments. At T12, a higher CR rate was found in the benralizumab group (48.1 % vs 32.4 %, p = 0.005). No differences in BVAS, OCS, and respiratory parameters were observed between groups at the different timepoints. Throughout the follow-up, both treatments reduced eosinophil count, although a deeper reduction was found in the benralizumab group at all timepoints (p < 0.0001). Safety profile was comparable between patient groups. CONCLUSION:Mepolizumab and benralizumab showed comparable overall effectiveness and safety in EGPA. However, benralizumab achieved a higher CR rate at T12, and a deeper peripheral eosinophil reduction.
OBJECTIVE:To compile a list of approved training sites for the Residency Training Program [Diplôme d'Études Spécialisées] in Internal Medicine and Clinical Immunology (DES-MIIC) in France. METHOD:All local coordinators of the DES-MIIC were contacted to establish the list of approved internship sites for the MIIC DES within their geographical subdivision. RESULTS:We listed 244 approved training sites, of which 87 (35.7%) were in university hospitals. Depending on the region/subdivision, the number of approved training sites varied significantly. Internship sites within and outside of university hospitals were approved for the core-phase in 85% (74/87) and 63.7% (100/157) of cases, for the "development phase" in 94.3% (82/87) and 84.1% (132/157) of cases and for the consolidation phase 80.5% (70/87) and 52.2% (82/157) of cases, respectively. The vacancy rate for selection was lower in university hospitals than in general hospitals with ratios of 1.5 (89.3% vs. 61%), 1.8 (88.8% vs. 48.9%) and 1.6 (88,5% vs. 55.4%) - for respectively the core, development and consolidation phases. CONCLUSION:Thirty-five percent of internship sites were located in University hospitals. Depending on the region/subdivision, the number of approved training sites for the DES-MIIC varies greatly. Half of internship positions are vacant outside of University hospitals, just as nearly half of training sites in these same hospitals lack approval for the "consolidation phase".
BACKGROUND:No specific description of monoclonal gammopathies of undetermined significance (MGUS)-associated angioedema due to acquired C1 inhibitor deficiency (AAE-C1-INH) has been reported yet. OBJECTIVE:To describe the biological and clinical characteristics, evolution, and response to treatment of MGUS-associated AAE-C1-INH. MATERIALS AND METHODS:We conducted a French national retrospective observational study on MGUS-associated acquired angioedema spanning a 30-year period. RESULTS:Forty-one patients with MGUS-associated AAE-C1-INH at diagnosis were included; 68% displayed anti-C1-INH antibodies. The monoclonal component was an IgM in 24 patients, IgG in 11, and IgA in 6 patients. The mean age at first angioedema attack was 63 years (standard deviation [SD] = 13 years) and at diagnosis 66 years (SD = 11 years). A total of 88% patients benefited from acute attack treatments, and 77% from long-term prophylaxis, either danazol, tranexamic acid, or lanadelumab. Median follow-up was 7 years, during which 14 patients (33%) evolved into well-defined malignant hemopathies. Fifty percent of patients were given a hematological treatment, either rituximab alone, indicated by recurrent attacks of angioedema in patients with AAE-C1-INH with anti-C1-INH antibodies, or validated combinations of chemotherapies, indicated by evolution into a lymphoma in 7 patients and a myeloma in 3 patients. Fifteen patients (35%) were in clinical complete remission of angioedema at last visit, of whom 60% had an undetectable serum monoclonal immunoglobulin. CONCLUSIONS:Complete remission of AAE-C1-INH is correlated to complete remission of the underlying hematological malignancy, as defined by an undetectable serum monoclonal immunoglobulin. In our MGUS-associated acquired angioedema cohort, we recorded an incidence of evolution into hematological malignancy of 4% per patient-year. It is therefore crucial to conduct full hematological workup during follow-up at an annual rate, and earlier if AAE relapses or if acute attack frequency increases.
Introduction L’angiœdème dû à un déficit acquis en C1 inhibiteur (AOA-C1-INH) est une pathologie rare, représentant 10 % des angiœdèmes dus à un déficit en C1INH, et fréquemment associé à un lymphome indolent ou à une gammapathie monoclonale de signification indéterminée (MGUS). Les plus grandes séries décrivent moins d’une centaine de patients. Le diagnostic repose sur des angiœdèmes récurrents après l’âge de 30ans, l’absence d’antécédents familiaux d’angiœdème et un C1INH fonctionnel inférieur à 50 % de la valeur de référence. Une diminution du C1q et des anticorps anti-C1-INH (Abs) sont observés dans un sous-groupe de patients. En France, ces analyses du complément peuvent être réalisées dans différents laboratoires d’immunologie, et les séries publiées ont fourni des données indépendamment du laboratoire. Les corrélations cliniques avec les paramètres du complément sont mal connues. Patients et méthodes Nous avons étudié rétrospectivement les dossiers médicaux des patients inclus dans le registre national français AOA-C1-INH qui avaient été diagnostiqués dans le laboratoire d’immunologie du centre national de référence de l’angiœdème à kinines (CREAK), et analysé les corrélations clinicobiologiques au moment du diagnostic et au cours du suivi. Résultats L’analyse a porté sur 104 patients. L’évaluation du complément a été réalisée 0,52 (0,12–1,44) ans après le premier épisode d’angiœdeme. Un lymphome était présent chez 54 patients (52 %) et une MGUS chez 22 patients (21 %).Au diagnostic, on observait une baisse de C1-INH pondéral chez 89 (86 %) patients, de C1q chez 77 (91 %) et de C4 chez 85 (94 %). Des anticorps anti-C1INH étaient présents chez 47 (45 %) patients. Les patients atteints de lymphome avaient une protéine C1INH plus basse, et C1q et C4 étaient plus fréquemment abaissés (45/47 vs 32/38 et 50/51 vs 35/39, respectivement). Les patients avec une protéine C1INH normale avaient plus souvent des anticorps anti-C1-INH (80 vs 40 %), et une forme idiopathique (47 vs 24 %).Une évaluation de suivi était disponible pour 59 patients (57 %), après une durée médiane de 3ans (0,5–10). La normalisation du complément est corrélée à la rémission clinique de l’angiœdème (7/8 en rémission biologique vs 4/20), et au traitement d’une affection sous-jacente : parmi 14 patients traités avec succès pour un lymphome, une rémission à la fois du complément et de l’angiœdème a été observée chez 5 patients, et une rémission de l’angiœdème sans normalisation du complément chez 4 patients. Parmi les 5 patients traités par rituximab pour un AAE-C1INH avec anticorps anti-C1INH, sans lymphome, un seul a eu une amélioration de l’angiœdème, et aucun n’a eu de disparition des anticorps anti-C1INH. Conclusion Les caractéristiques du complément au moment du diagnostic de l’AAE-C1INH dépendent de la maladie hématologique sous-jacente : il semble exister deux sous-groupes, avec une plus grande implication de la voie classique et une moindre positivité des anticorps anti-C1-INH chez les patients atteints de lymphome par rapport aux formes idiopathiques ou associées aux MGUS. Il existe une corrélation clinicobiologique : la normalisation du complément est associée à la disparition des crises d’angiœdème. Le traitement du lymphome entraîne une rémission clinicobiologique, mais le rituximab n’est pas efficace dans les AAE associés à une MGUS ou idiopathiques malgré la présence d’anticorps anti-C1INH. La persistance d’une valeur basse de C1INH chez des patients en rémission de lymphome et de l’angiœdème suggère que d’autres mécanismes sont impliqués dans la survenue des crises.
Health anxiety (HA) is a frequent problem (up to 20% of consultants in the context of secondary care) responsible for decreased well-being, disability, somatic and psychiatric complications, which contributes to high healthcare expenditures at the population level. It is likely, if not definitely established, that the prevalence of ACS is increasing, and this can partly be explained by the growing medicalization of society in general, the appetite of Media for health issues, and the uncontrolled use of the Internet (which can lead certain vulnerable subjects to cyberchondria). The pandemic of COVID-19 could have contributed to it, at least by the significant increase in the overall level of psychological distress in the population it has caused, although this has not formally been demonstrated to date. The diagnosis of ACS is easy, as soon as its assessment is considered as a mandatory part of any medical consultation. Certain intuitive attitudes of doctors, such as reassurance, prove to be iatrogenic for the patient with HA. The management of HA can be facilitated by an acculturation of physicians to cognitive conceptions of anxiety in general and HA in particular. HA is effectively treatable by certain psychotherapy and in the first place cognitive and behavioral therapies (CBT), but the availability of trained therapists and accessible at a lower cost is sorely lacking, particularly in France.
L’anxiété centrée sur la santé (ACS) est un problème fréquent (jusqu’à 20 % des consultants en contexte de soins secondaires) responsable d’altération de la qualité de vie, d’invalidité, de complications somatiques et psychiatriques, et contribuant à des dépenses de santé élevées à l’échelle de la population. Il est probable, sinon formellement établi, que la prévalence de l’ACS soit en augmentation, et cela peut en partie être expliqué par la médicalisation croissante de la société en général, l’appétit des media pour les questions de santé, et l’usage incontrôlé d’Internet (pouvant conduire certains sujets vulnérables à la cyberchondrie). La pandémie de Covid-19 pourrait y avoir contribué, au moins par l’augmentation significative du niveau global de détresse psychique dans la population qu’elle a engendrée, bien que cela ne soit pas formellement démontré. Le diagnostic de l’ACS en est aisé, dès lors que son évaluation est considérée comme devant faire partie de toute consultation médicale. Certaines attitudes intuitives des médecins, comme la réassurance, s’avèrent iatrogènes face aux personnes ayant un niveau élevé d’ACS. Sa prise en charge peut en être facilitée par une acculturation des somaticiens aux conceptions cognitives de l’anxiété en général et de l’ACS en particulier. L’ACS s’avère traitable efficacement par certaines psychothérapies et en première place les thérapies comportementales et cognitives (TCC), mais la disponibilité de thérapeutes formés et accessibles à moindre coût fait cruellement défaut, particulièrement en France.
INTRODUCTION:Severe acute respiratory syndrome coronavirus 2 is a viral respiratory infection that can cause systemic disorders and lead to death, particularly in older people. Proton pump inhibitors (PPIs) increase the risk of enteric and lung infections. Considering the broad use of PPIs in older people, the potential role of PPIs in COVID-19 could be of dramatic significance. The objective of our study was to evaluate the link between PPIs and severe COVID-19 in older people. METHOD:We performed a retrospective cohort study, including all patients aged ≥65, hospitalised for a diagnosis of COVID-19. Epidemiological, clinical and biological data were extracted and we performed an Inverse Probability of Treatment Weighing method based on a propensity score. RESULTS:From March 2020 to February 2021, a total of 834 patients were included, with a median age of 83 and 52.8% were male. A total of 410 patients had a PPIs prescription, 358 (87.3%) were long-term PPIs-users and 52 (12.7%) were recent PPIs-users. Among PPIs-users, 163 (39.8%) patients developed severe COVID-19 versus 113 (26.7%) in PPIs-non users (odds ratio (OR) = 1.59 [1.18-2.14]; P < 0.05). Moreover, the double dose PPI-users had a higher risk of developing severe COVID-19 (OR = 3.36 [1.17-9.66]; P < 0.05) than the full dose PPI-users (OR = 2.15 [1.22-3.76]; P < 0.05) and the half dose PPI-users (OR = 1.64 [1.13-2.37]; P < 0.05). CONCLUSION:Our study reports evidence that the use of PPIs was associated with an increased risk of severe COVID-19 in older people.
We read with interest the publication by Wang and colleagues on the off-label use of Mepolizumab for the treatment of eosinophilic cystitis in a 77-year-old patient [1].The authors presented another patient with idiopathic eosinophilic cystitis successfully treated with mepolizumab [1].

Introduction Les lymphomes T périphériques sont des hémopathies lymphoïdes issues d’une prolifération clonale de lymphocytes T ou Natural Killer matures non primitivement cutanés. Ces lymphomes sont rares et représentent 5 à 10 % de l’ensemble des lymphomes malins de l’adulte. Leurs présentations cliniques sont hétérogènes et leur pronostic sombre. Les internistes sont parfois confrontés à des patients présentant des symptômes atypiques et/ou faisant évoquer des maladies d’allure « systémique » chez qui on diagnostique finalement un lymphome T. La difficulté à poser le diagnostic définitif entraîne un retard diagnostique et pourrait avoir des conséquences délétères pour le patient et sa survie. Notre objectif était de décrire les manifestations cliniques « non typiquement hématologiques » ayant révélé le lymphome T, ainsi que le cheminement diagnostique, dans le but d’optimiser la démarche clinique et de réduire le délai diagnostique. Une attention particulière a été accordée aux investigations réalisées et à leur pertinence (apport diagnostique), et à d’éventuelles corrélations entre le type histologique du lymphome T et les symptômes initiaux. Patients et méthodes Nous avons réalisé une étude observationnelle rétrospective, à partir des données du PMSI des services de médecine interne et d’hématologie du CHU de Saint-Étienne ainsi que les registres d’anatomopathologie. Nous avons inclus les patients majeurs chez lesquels un diagnostic de lymphome T a été posé entre 2010 et 2021. Nous avons exclu les patients présentant un mycosis fungoïde, un syndrome de Sezary et un lymphome T cutané CD8+. Les données cliniques et biologiques présentées par les patients au moment du diagnostic ainsi que les examens d’imagerie et à visée cyto-histologique réalisés pour établir le diagnostic ont été recueillis. Résultats Nous avons analysé les données de 66 patients. Histologiquement on retrouvait un lymphome T périphérique sans autre spécificité (PTCL-NOS) chez 42,4 %, un lymphome T angio-immunoblastique (TFHL-AI) chez 24,2 %, un lymphome anaplasique à grandes cellules (ALCL) primitivement cutané chez 7,6 % et un ALCL ALK- chez 6 % des patients. Les autres sous-types histologiques représentaient moins de 5 % des patients. Seulement 15 % des patients présentaient l’association d’adénopathies et de signes B au premier contact et 39 % au moment du diagnostic. On retrouvait une anasarque chez 30 % des patients. Nous décrivons également certains patterns cliniques atypiques non rapportés dans la littérature chez de petits effectifs de sujets, notamment la présence d’œdèmes du visage associés à une anémie hémolytique auto-immune (AHAI) (n=3) ; l’association d’une hyperéosinophilie et d’œdèmes péno-scrotaux (n=2/3) ; et l’association d’un syndrome sclérodermiforme avec une neuropathie périphérique et des arthralgies (n=2). Cinq patients présentaient des manifestations de polyarthrite oedémateuse précédant parfois de plusieurs années le diagnostic de lymphome. Les délais diagnostiques observés s’avèrent extrêmement variables, allant de 4jours à 1802jours avec une médiane à 49jours. La survie au sein de notre cohorte n’était pas bonne avec 66,7 % de décès et une survie moyenne après le premier contact de 1434jours. Conclusion Nous rapportons dans cette cohorte des manifestations cliniques et biologiques extrêmement variées sans corrélation avec type histologique, avec notamment la présence de symptômes atypiques habituellement rapportés aux anciennes lymphadénopathies angio-immunoblastiques (THF-LAI de la nouvelle classification OMS) dans une proportion non négligeable de patients présentant un lymphome T périphérique non spécifié (PTCL-NOS). Les délais diagnostiques observés s’avèrent extrêmement variables, allongés en présence de ces manifestations atypiques qui semble corrélées à un délai plus long avant la réalisation d’un PET-scanner et des biopsies. Néanmoins il n’est pas clair que les retards diagnostiques aient un impact majeur sur le pronostic, globalement péjoratif, de ces lymphomes. Les biopsies d’adénopathies, masses, ou organes pleins à l’aiguille ont une très faible rentabilité et retardent inutilement la biopsie chirurgicale dans cette série. Les biopsies ostéomédullaires (BOM) n’ont pas une bonne rentabilité, particulièrement en l’absence de cytopénies, et leur réalisation avant la biopsie chirurgicale et le bilan d’extension n’a pas d’intérêt.
