Cefepime/enmetazobactam is a new β-lactam/β-lactamase inhibitor combination with broad-spectrum activity against multidrug-resistant Enterobacterales, including OXA-48-producing isolates. Furthermore, cefepime and enmetazobactam have demonstrated similar and excellent intrapulmonary penetration, supporting the use of this new antibiotic combination in the treatment of hospital-acquired pneumonia. This study evaluated the in vivo efficacy of cefepime/enmetazobactam in a murine neutropenic pneumonia model infected with various OXA-48-producing K. pneumoniae strains. Mice were subcutaneously administered with cefepime (100 mg/kg/q2h), alone or combined with enmetazobactam (30 mg/kg/q2h), or intraperitoneally with meropenem (100 mg/kg/q2h) at 2 h post-infection. Mice were euthanized at 26 h post-infection for bacterial enumeration in lungs and spleen. A robust growth was achieved in untreated control mice. Cefepime alone or meropenem had no effect on reducing the bacterial burden in lungs after a 24-h period of treatment. The addition of enmetazobactam to cefepime resulted in a 2-log10 CFU/g bioburden reduction in lungs compared to 26-h controls for all strains, including the strain harboring the highest MIC (= 8 µg/mL) to cefepime/enmetazobactam. When changes of bacterial burden were assessed relative to 2-h controls, bacterial stasis was observed. These data highlight the limited in vivo activity of meropenem against OXA-48-producing Enterobacterales despite in vitro susceptibility. Conversely, cefepime/enmetazobactam with a human-mimicking regimen demonstrated a significant antibacterial effect in the pneumonia model induced by three OXA-48-producing K. pneumoniae strains, compared with cefepime or meropenem at 24 h post-infection. Therefore, cefepime/enmetazobactam may be a new alternative for lung infections due to Enterobacterales producing OXA-48. IMPORTANCE:Third-generation cephalosporin-resistant Klebsiella pneumoniae with extended-spectrum β-lactamases as principal resistance determinants are classified as critical priority pathogens. Their increasing occurrence has led clinicians to widely use carbapenems. Accordingly, carbapenem resistance in Klebsiella pneumoniae has spread in recent decades across several countries, and OXA-48-like carbapenemases are one of the main determinants of carbapenem resistance in Enterobacterales. Cefepime/enmetazobactam is a novel β-lactam/β-lactamase inhibitor combination that demonstrated excellent intrapulmonary penetration, supporting its use in the treatment of pneumonia. This study examined the efficacy of enmetazobactam, in combination with cefepime, compared to carbapenems for OXA-48-producing Klebsiella pneumoniae in a 24-h murine neutropenic pneumonia model. The combination showed a bacteriostatic effect using the 2-h controls as reference. Compared to 24-h controls, and to cefepime or meropenem monotherapies, the co-administration of enmetazobactam with cefepime demonstrated a pronounced in vivo bactericidal activity against cefepime-non-susceptible K. pneumoniae isolates with cefepime/enmetazobactam MICs up to 8 µg/mL in this model.
IntroductionLe céfépime/enmétazobactam (FEP/ENM) est une nouvelle association β-lactamine/inhibiteur de β-lactamase ayant une activité à large spectre contre les entérobactéries multirésistantes (incluant les isolats producteurs d'OXA-48). Cette association a déjà démontré une meilleure efficacité que la combinaison pipéracilline/tazobactam dans le traitement des infections urinaires compliquées (Kaye, 2022). Notre étude vise à évaluer l'efficacité in vivo du FEP/ENM dans un modèle de pneumonie chez la souris neutropénique infectée à K. pneumoniae (Kp) OXA-48.Matériels et méthodesTrois souches cliniques de Kp OXA-48 et produisant une CTX-M-1 (Kp 549 et Kp 235) ou une CTX-M-15 (Kp 246) ont été utilisées dans ce modèle. Des souris CD1 neutropéniques ont été infectées par voie intranasale (8 Log10 UFC/animal). Elles ont été traitées en souscutané soit par du FEP seul (100mg/kg), soit par du FEP associé à de l'ENM (30 mg/kg) ou par voie intrapéritonéale par du méropénème (MEM, 100 mg/kg) 2h après l'infection. Tous les traitements ont été administrés toutes les 2h selon les résultats pharmacocinétiques (PK) obtenus. Les souris ont été euthanasiées 26h après l'infection pour évaluer la charge bactérienne dans les poumons et la rate.RésultatsToutes les souches sont résistantes au FEP (CMI de 16 à >256 µg/mL). L'ajout d'ENM à une concentration fixe de 8µg/mL réduit drastiquement ces CMI (0,5–4–8 µg/ml pour Kp 549, Kp 235 µg/mL et Kp 246, respectivement). Seule la souche Kp 549 est sensible au méropénème.Les paramètres PK obtenus chez la souris ayant reçu du FEP à 100mg/kg ou de l'ENM à 30mg/kg sont les suivants : t1/2-FEP=∼30min, Cmax-FEP=∼110µg/mL, AUC0-24-FEP =∼876µg.h/mL et t1/2-ENM=∼15min, Cmax-ENM=∼20-25µg/mL, AUC0-24-ENM =∼40 µg.h/mL. Les simulations pharmacodynamiques suggèrent d'utiliser une dose toutes les 2h lorsque le FEP est combiné à l'ENM pour obtenir un %T> CMI suffisant (∼ 40% T>2µg/ml).Quelle que soit la souche testée, la charge bactérienne pulmonaire des animaux témoins est élevée 26h post-infection (∼ 9,5 Log10 UFC/g). Le MEM ou le FEP seuls n'ont montré aucune efficacité sur les trois souches testées. L'ajout d'ENM au FEP induit une réduction bactérienne significative dans les poumons (- 2 Log10 UFC/g), y compris sur la souche Kp 246 (CMI = 8 µg/ml). De plus, une réduction bactérienne significative est observée dans les rates des animaux recevant le régime FEP/ENM toutes les 2h.ConclusionCes données mettent en évidence l'activité in vivo limitée du méropénème contre les entérobactéries productrices d'OXA-48 malgré une sensibilité in vitro. À l'inverse, un traitement FEP/ENM administré toutes les 2h s'associe à un effet antibactérien significatif dans ce modèle de pneumonie à Kp OXA-48/CTX-M. ABSTRACT Carbapenem-resistant Acinetobacter baumannii (CRAB) is an emerging cause of hospital-acquired pneumonia (HAP). Preclinical large models are warranted to predict the efficacy and the resistance profile of anti-infectives and mimic how they will be used in the human treatment of CRAB-HAP. Here we reported on the development of an Acinetobacter baumannii experimental pneumonia model in immunocompromised rabbits, receiving a 48-h human-simulated regimen. The efficacy of meropenem (2 g/q8h i.v. over prolonged 3-h perfusion), rifampin (25 mg/kg/q8h, i.v.), or the combination of meropenem and rifampin was assessed in rabbits infected with the carbapenem-susceptible ATCC 17978 reference strain or the CRAB Turc 2 clinical strain. The emergence of rifampin mutants was also investigated. Meropenem demonstrated a strong pulmonary bacterial reduction in animals infected with the ATCC 17978 strain (unlike the CRAB strain). The high rifampin dosage was associated with a 1.3 Log 10 bacterial killing on average but induced the emergence of high-level resistant mutants in 80%–100% of animals, depending on the strain. The adjunction of rifampin to meropenem did not improve the bioburden in the lungs but partially reduced the number of animals exhibiting resistant mutants, whatever the tested strain. However, this adjunctive treatment was insufficient to overcome the emergence of resistance since mutation prevention concentration-related pharmacodynamic indices were unattainable at this dose. This CRAB pneumonia rabbit model represents an innovative tool to evaluate the efficacy of new or existing therapies and will provide informative data on how they can meet the resistance pharmacodynamic targets, which now need to be investigated before deciding on clinical therapeutic regimens. IMPORTANCE Within intensive care unit settings, carbapenem-resistant Acinetobacter baumannii (CRAB) has emerged as a frequent cause of hospital-acquired pneumonia (HAP) with poor clinical outcomes. This multidrug-resistant pathogen remains very challenging to study in clinical trials, and the U.S. Food and Drug Administration highlighted the limitations of existing small animal models for evaluating antibacterial or prophylactic strategies against such critical infections. These limitations include the difficulty in anticipating the risk of the emergence of resistance during treatment. Here we developed a new Acinetobacter baumannii pneumonia rabbit model using high inoculum. We demonstrated the emergence of resistance with rifampin, an existing antibiotic debated as a rescuing option to treat CRAB infections; and even intensified rifampin doses failed to close the mutant selection window. This CRAB pneumonia rabbit model represents a valuable tool to evaluate the efficacy of new or existing therapies and provides supportive data in antimicrobial resistance pharmacodynamics.
ABSTRACT The in vivo efficacy of ceftaroline and rifampin alone or in combination was investigated in an experimental rat model of methicillin-resistant Staphylococcus epidermidis (MRSE) osteomyelitis. After a 3-day treatment, the mean tibial bacterial load was significantly reduced in animals receiving rifampin alone or combined with ceftaroline (1.89 and 1.68 log 10 CFU/g, respectively, P < 0.0001), whereas ceftaroline alone did not show any significant reduction at this early stage. After a 7-day treatment, the mean tibial bacterial load was reduced for ceftaroline alone compared to control animals (3.6 log 10 CFU/g versus 4.4 log 10 CFU/g, respectively, P < 0.05) and reached the limit of detection in almost all animals receiving rifampin alone or combined with ceftaroline ( P < 0.0001). Finally, after a 14-day treatment the efficacy of ceftaroline alone was enhanced ( P < 0.0001), and no MRSE was recovered from the tibias of animals receiving rifampin alone or combined with ceftaroline. No emergence of rifampin resistance was observed, regardless of the treatment group (monotherapy or combined therapy) or the timepoint of interest. Ceftaroline administered with rifampin could represent a valuable therapeutic option in the management of MRSE osteomyelitis. IMPORTANCE Methicillin-resistant Staphylococcus epidermidis (MRSE) contributes to a high percentage of orthopedic infections, and their treatment represents a huge challenge. The present study aimed to evaluate the efficacy of ceftaroline alone or combined with rifampin in a rat MRSE osteomyelitis model and the bone penetration of ceftaroline. A ceftaroline monotherapy showed a significant bacterial reduction in infected bones after a 7-day period of treatment. The combination ceftaroline plus rifampin leveraged rifampin’s bactericidal activity, shortening the duration of positive culture in infected animals. These results suggest that ceftaroline and rifampin combination therapy could represent a valuable therapeutic option for human MRSE osteomyelitis and deserves further preclinical and clinical evaluation.
Introduction: Emergency departments (ED) are pivotal for antibiotic prescription, of which the appropri- ateness and consequences have rarely been assessed.Methods: A retrospective study included patients referred to the ED and hospitalized with an advocated diagnosis of infection. Day-0 (ED initial prescription) and day-2 (reevaluation) antibiotic therapies were graded as optimal (if fully following the guidelines in terms of molecule, dose, and route of administra- tion), adapted (if the prescribed molecule was microbiologically active but not recommended as first-line treatment, or in case of a wrong dose), or inadequate (other situations). The primary endpoint was onset of an unfavorable event (death, transfer to intensive care unit, or re-hospitalization). Prognosis factors associated with survival without unfavorable event were assessed by multivariate analysis.Results: We included 484 patients. Optimal, adapted, and inadequate initial prescriptions concerned 328 (67.8 %), 110 (22.7 %) and 46 (9.5 %) patients respectively. Compared with an optimal prescription, an ini- tial adapted prescription was associated with a poorer prognosis (HR=1.95, CI95% [1.18-3.22]; p=0.01). Reevaluation was performed in 436 (90.1 %) patients. After reevaluation, optimal, adapted, and inade- quate prescriptions concerned 326 (74.8 %), 64 (14.7 %), and 46 (10.5 %) patients respectively. After ree- valuation, and as compared with optimal prescription, inadequate prescription was significantly associated with unfavorable events (HR=3.52, CI95% [1.42-8.72]; p=0.003).Conclusion: Antibiotics are frequently prescribed in EDs. Antibiotic prescription has got to be optimal, and not simply adapted, so as to be associated with significant clinical benefit.

La dalbavancine est un lipoglycopeptide fortement lié à l’albumine et utilisé dans le cadre des infections des tissus mous à bactéries Gram+. Au vu de ses caractéristiques microbiologiques et pharmacocinétiques, elle est aussi employée pour prévenir et/ou traiter les infections ostéoarticulaires (IOA) à Gram+. Dans ce cadre, le schéma posologique n’est cependant pas établi et reste variable selon les centres prescripteurs. Notre objectif a consisté à étudier l’exposition en dalbavancine chez les patients traités pour une IOA, notamment chez les patients présentant une insuffisance rénale (IR) en raison d’une élimination rénale partielle de la dalbavancine. Il s’agit d’une étude rétrospective monocentrique. Des dosages de dalbavancine plasmatiques ont été réalisés à différents instants de prélèvement et différentes covariables ont été colligées (c.-à-d., poids, taille, DFG estimés par CKD-EPI). Les expositions retrouvées chez nos patients ont été comparées à celles retrouvées lors des études cliniques grâce au modèle de pharmacocinétique de population publié par Buckwalter et al. Cette comparaison s’est décomposée en 2 étapes. La première étape a consisté à comparer l’exposition de nos patients à des patients présentant les mêmes valeurs de covariables et le même schéma posologique. Lors de la seconde étape, les expositions déterminées chez nos patients ont été comparées à celles retrouvées dans une population de patients ayant une fonction rénale normale et le même schéma posologique. Les 17 patients inclus dans l’étude avaient un DFG moyen de 99 mL/min/1,73 m 2 (52–131 mL/min/1,73 m 2 ). Au total, 92/95 prélèvements plasmatiques ont été retenus; les 3 prélèvements rejetés présentaient des valeurs aberrantes au vu du plan expérimental. Après une perfusion de 1500 mg, les concentrations moyennes observées à j14 et j21 ont été, respectivement, de 23,5 mg/L (14,3–32,6 mg/L) et de 17,0 mg/L (8,8–24,9 mg/L). De façon générale, les concentrations mesurées chez les 17 patients ont été en adéquation avec les valeurs attendues dans une population identique à la leur (c.-à-d., même surface corporelle DFG et posologie). Par conséquent, ces patients ne diffèrent pas de ceux qui ont permis la construction du modèle publié par Buckwalter et al. Par ailleurs, les concentrations mesurées chez les 17 patients ont été en adéquation avec les valeurs attendues dans une population d’individus traités selon un même schéma posologique, ayant une même surface corporelle mais une fonction rénale normale (c.-à-d., DFG de 100 mL/min). Les patients inclus dans l’étude n’ont pas présenté de sur- ni de sous-exposition. Par conséquent, il ne semble pas y avoir de phénomène d’accumulation pouvant conduire à des effets indésirables chez les patients présentant une IR modérée. De fait, aucun ajustement posologique ne semble nécessaire. La dalbavancine étant fortement liée à l’albumine (> 95 %), la liaison aux protéines plasmatiques est un paramètre qu’il faudra également documenter dans cette population.
Background: Debate continues regarding the usefulness and benefits of wide prescription of antibiotics in patients hospitalized with coronavirus disease 2019 (COVID-19). Methods: All patients hospitalized in the Infectious Diseases Department, Dijon University Hospital, Dijon, France between 27 February and 30 April 2020 with confirmed COVID-19 were included in this study. Clinical, biological and radiological data were collected, as well as treatment and outcome data. An unfavourable outcome was defined as death or transfer to the intensive care unit. Patient characteristics and outcomes were compared between patients who did and did not receive antibiotic therapy using propensity score matching. Findings: Among the 222 patients included, 174 (78%) received antibiotic therapy. The univariate analysis showed that patients who received antibiotic therapy were significantly older, frailer and had more severe presentation at admission compared with patients who did not receive antibiotic therapy. Unfavourable outcomes were more common in patients who received antibiotic therapy [hazard ratio (HR) 2.94, 95% confidence interval (CI) 1.07–8.11; P = 0.04]. Multi-variate analysis and propensity score matching indicated that antibiotic therapy was not significantly associated with outcome (HR 1.612, 95% CI 0.562–4.629; P = 0.37). Conclusion: Antibiotics were frequently prescribed in this study and this was associated with more severe presentation at admission. However, antibiotic therapy was not associated with outcome, even after adjustment. In line with recent publications, such data support the need to streamline antibiotic therapy in patients with COVID-19.
Les manifestations de la COVID-19 vont d'un syndrome grippal à un syndrome de détresse respiratoire aiguë nécessitant un séjour prolongé en réanimation. Compte tenu de la hausse conséquente de l'activité au moment du pic de l'épidémie, notamment en réanimation, de nombreuses décisions de limitation thérapeutique ont été prises, soulevant des problématiques éthiques. La question se pose donc de savoir si, au vu du débordement des services hospitaliers, nous n'avons pas été plus restrictifs qu'habituellement concernant ces limitations. Tous les patients hospitalisés dans notre service pour une COVID-19 confirmée par PCR entre le 26 février et le 30 avril 2020 ont été inclus. De façon systématique, le niveau d'engagement thérapeutique était défini dès l'entrée du patient dans le service. Pour chaque patient, nous avons recueilli, via le dossier médical informatisé, les antécédents, le mode de vie (dont l'autonomie), la clinique, la biologie et l'imagerie initiale ainsi que l'évolution. Nous avons comparé les patients limités et non limités afin d'évaluer les caractéristiques des différentes populations et l'impact d'une limitation de soins. Sur la période, 222 patients étaient inclus dont une majorité d'hommes (123, 55 %) avec un âge médian de 70,1 ans. Une limitation thérapeutique a été décidée pour 67 patients (32 %). La décision était prise dans la majorité des cas par les médecins du service (49 patients, 73 %) suivis par les réanimateurs (13 patients, 19 %). Les patients limités étaient significativement plus âgés (80 vs 63,3 ans, p < 0,001), avec plus de comorbidités comme le témoigne les scores de Charlson avec et sans l'âge significativement plus élevés respectivement à 6 (vs 3, p < 0,001) et 2 (vs 1 p < 0,001). Les patients limités étaient significativement plus atteints de maladies cardiovasculaires (55 patients [82 %] vs 74 patients [59 %], p < 0,001), de troubles cognitifs (36 patients [54 %] vs 11 patients [8 %], p < 0,001). Parmi ceux qui vivaient à domicile, les patients limités étaient plus dépendants (46 patients [70 %] vs 14 patients [10 %], p < 0,001). La présentation initiale était plus sévère chez les patients limités comme l'atteste le NEWS2 (6 vs 5, p = 0,03) avec plus de classe NEWS2 de risque élevée (36 [54 %] vs 48 [34 %], p = 0,012). Les patients limités présentaient significativement plus de confusion (26 [39 %] vs 10 patients [7 %], p < 0,001) et de dyspnée (50 [75 %] vs 85 patients [59 %], p = 0,024). Leur évolution était plus souvent défavorable (décès) que les patients non limités (décès ou transfert en réanimation) (33 [49 %] vs 31 patients [21 %], p < 0,001). Dans cette étude, les patients limités dès leur prise en charge initiale étaient plus âgés, avec plus de comorbidités dont plus de troubles cognitifs et présentaient un tableau initial plus sévère, avec une évolution plus souvent défavorable. Ces données semblent concorder avec les pratiques de décision d'engagement thérapeutique en dehors d'une période épidémique.
La résistance croissante aux antibiotiques atteint désormais des niveaux alarmants à l'échelle mondiale. De nouveaux mécanismes de résistance émergent et se propagent, compromettant notre capacité à traiter certaines pathologies infectieuses courantes. Normalement réservés aux infections à bacilles Gram négatif (BGN) difficiles à traiter, les carbapénèmes voient leurs résistances augmenter, conduisant l'Organisation mondiale de la santé (OMS) à déclarer la lutte contre les souches d'entérobactéries, de Pseudomonas aeruginosa et d'Acinetobacter baumanii résistantes aux carbapénèmes comme prioritaire à partir de 2017. Les traitements efficaces contre les infections à BGN résistants aux carbapénèmes (CR-BGN) sont peu nombreux et les données cliniques sur ces traitements parcellaires. Cette analyse présente l'épidémiologie des patients infectés ou colonisés par des pathogènes à CR-BGN en France. Cette étude rétrospective multicentrique réalisée à partir des dossiers médicaux de 6 hôpitaux français incluait tous les patients adultes admis entre avril 2017 et mars 2018 ayant présenté une infection et/ou une colonisation documentée à CR-BGN. Les données collectées comprenaient les résultats microbiologiques et les caractéristiques démographiques des patients. Au total, 29 339 patients ont été inclus, représentant 36 168 BGN dont 2086 (6 %) étaient résistants aux carbapénèmes. L'âge moyen des patients était 58 ans, 60 % étaient des femmes. Les entérobactéries représentaient 89 % des souches (majoritairement E. coli, 56,4 % et Klebsiella spp, 11,4 %), Pseudomonas spp. 9,4 %, Acinetobacter spp. 1 % et Stenotrophomonas spp. 1,1 % des infections à BGN, toutes sensibilités confondues. La distribution des CR-BGN était différente avec 43 % d'entérobactéries, 29 % de Pseudomonas spp., 18,6 % de Stenotrophomonas spp et 1,3 % d'Acinetobacter spp. Les infections à BGN sont fréquentes en France, parmi lesquels 6 % sont résistants aux carbapénèmes. L'analyse des données réelles de prévalence dans une population de patients pris en charge en milieu hospitalier apparaît indispensable à optimisation de la politique d'utilisation rationnelle des antibiotiques.
ABSTRACT The rapidly increasing number of PCR-based SARS-CoV-2 tests as COVID-19 control measures has been leading not only to the discovery of positive results in asymptomatic individuals, but also to the re-testing of persons having contracted (proven or unproven) COVID-19 or having previously tested positive. In the face of interrogations as to the interpretation and management of these types of situations, and given the need to weigh collective and individual risks, we have carried out a review of the literature aimed at responding to the six following questions. Can PCR-based SARS-CoV-2 testing lead to false positives? For how long do the results remain positive? For how long can the results be associated with an actual risk of transmission / with infectivity in a symptomatic person? Is a newly positive PCR-based SARS-CoV-2 test result following one or more negative test results to be associated with a risk of transmission? Is a positive PCR-based SARS CoV-2 test result to be associated with a genuine risk of transmission / with infectivity in an asymptomatic person? Does the presence of antibodies detected by serology attest to control of the infection and consequently to non-contagiousness, even in the event of positive PCR-based SARS-CoV-2 test results? Having been initiated prior to the recently issued recommendations of the Haut Conseil de la Sante Publique (French public health council), this review is aimed at synthesizing practical proposals liable to evolve according to the advancement of scientific knowledge and the overall epidemic context.
La propagation du Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) responsable de la Coronavirus Disease 2019 (COVID-19) a provoqué une crise sanitaire majeure. Si les signes cliniques, biologiques et scanographiques du COVID-19 ont été largement rapportés, on manque d'informations sur les patients ayant une suspicion de COVID-19 qui s'est révélée négative. L'utilisation de techniques de diagnostic avec un résultat rapide permet une orientation de plus en plus précoce, mais leur non-disponibilité et/ou les potentiels faux négatifs rendent nécessaires une meilleure distinction des patients avec une suspicion de COVID-19 non confirmée (SCNC) de ceux atteints de COVID-19. L'objectif de cette étude est de décrire et de comparer ces deux populations. Ceci doit permettre d'optimiser l'orientation et la gestion des patients suspects de COVID-19 requérant une hospitalisation pour limiter le risque de perte de chances. Une partie des patients avec indication d'hospitalisation pour suspicion de COVID-19 a été incluse rétrospectivement, du 23 février au 28 avril 2020. Le statut de SCNC était retenu lorsque la RT-PCR COVID-19 était négative et que le diagnostic final retenu était autre. Les données cliniques, biologiques et scanographiques, ainsi que les diagnostics finaux ont été recueillis. Ces données ont été comparées à celles des patients COVID-19 hospitalisés sur la même période. Parmi les 152 patients SCNC hospitalisés inclus, la moitié avait plus de 70 ans. Les comorbidités principales étaient l'hypertension artérielle (n = 63 ; 41,5 %), les maladies pulmonaires chroniques (n = 32 ; 21,1 %) et les cardiopathies ischémiques (n = 21 ; 13,8 %). Parmi les diagnostics finaux, 65 avaient une maladie infectieuse (42,7 %) : 38 infections respiratoires (25 %), 12 bactériémies (7,9 %) et 7 infections urinaires (4,6 %). Treize avaient une décompensation cardiaque (8,6 %). Ils avaient eu en moyenne 1,3 (0,7) RT-PCR. Comparés aux 222 patients COVID-19, les patients SCNC étaient plus souvent fumeurs (n = 28 [27,2 %] vs n = 8 [4,4 %], p < 0,001). Ils présentaient cliniquement moins de fièvre (n = 78 [51,3 %] vs n = 169 [76,1 %], p < 0,001), moins de toux (n = 76 [50,7 %] vs n = 146 [63,8 %], p < 0,003) et moins de signes digestifs (n = 18 [11,8 %] vs n = 66 [29,7 %], p < 0,001). Les taux de fibrinogène (g/L) et de CRP (mg/L) étaient significativement plus bas chez les SCNC avec des médianes respectives à 5,3 (4–6,5) vs 6,0 (5,3–6,8), p < 0,001, et 57,3 (13–126) vs 83,7 (37,7–127), p = 0,008. Leur taux médian de lymphocytes (G/L) était plus haut : 1,1 (0,7–1,7) vs 0,9 (0,6–1,4), p = 0,03. Il s'agit, à notre connaissance, de la première étude observationnelle comparative des patients avec indication d'hospitalisation pour suspicion de COVID-19. L'absence de symptomatologie respiratoire, de fièvre et de signes digestifs doit faire rechercher un diagnostic différentiel. Les signes respiratoires « isolés » associés au tabagisme chronique pourraient expliquer la surreprésentation des fumeurs chez des sujets fragiles faisant redouter par excès une COVID-19, plus qu'un rôle protecteur intrinsèque. La perte de chance engendrée par la suspicion de COVID-19 doit être explorée plus largement.
Introduction. - Carbapenems are broad-spectrum antibacterial molecules. Imipenem-cilastatin and meropenem are the two main molecules used in French healthcare services. Objective. - We aimed to evaluate the relative strengths and weaknesses of these two molecules by considering their pharmacokinetic, pharmacodynamic, microbiological, and clinical properties. We demonstrated that imipenem-cilastatin and meropenem are not alike. Method. - Review of the literature by querying the MEDLINE network. Results. - Imipenem-cilastatin is the first marketed molecule of the carbapenem class. It is more effective against Gram-positive cocci. Its stability does not allow for long infusions and its main adverse effect on the central nervous system limits its use. Meropenem is more effective against Gram-negative bacilli. Its stability and its milder adverse effects distinguish it from imipenem-cilastatin. Conclusion. - Meropenem is preferred for daily use in healthcare services when carbapenems are to be used. Introduction La COVID-19 est une maladie virale emergente decouverte fin 2019 en Chine. La France fait partie des pays les plus touches avec pres de 30 000 deces attribuables au virus, contrastant avec la mortalite dans d’autres pays. La question se pose donc de savoir s’il existe des caracteristiques et des facteurs pronostics propre a la population francaise touchee par l’epidemie de COVID-19. Nous avons donc repris les dossiers de nos patients hospitalises dans notre centre afin d’en effectuer une analyse descriptive et de mettre en evidence des facteurs pronostics d’evolution defavorable. Materiels et methodes Nous avons inclus tous les patients hospitalises dans notre service pour une COVID-19 confirmee par PCR entre le 26 fevrier et le 30 avril 2020. Les patients admis via un service de reanimation etaient exclus. Les donnees cliniques, biologiques, radiologiques, ainsi que les donnees concernant l’evolution ont ete recueillies via le dossier medical informatique pour chaque patient. Une analyse comparative entre les patients ayant presente un evenement defavorable (transfert en reanimation ou deces) ou non a ete realisee afin d’etablir des facteurs pronostics. Resultats Sur les 222 patients inclus, 123 etaient des hommes et l’âge median etait de 70,1 ans. Cent soixante-seize patients (79 %) avaient au moins un facteur de risque de CoVID-19 grave dont les plus frequents etaient une maladie cardiovasculaire (133 patients, 60 %) et un âge superieur a 75 ans (132 patients, 59 %). Seuls 8 patients presentaient un tabagisme actif. La duree mediane entre les premiers symptomes et l’hospitalisation etait de 6 jours. Les signes cliniques retrouves etaient le plus frequemment de la fievre (166 patients, 76 %), de la toux (146 patients, 66 %) et une dyspnee (137 patients, 62 %). Treize patients (6 %) avaient une anosmie, 17 patients (8 %) une agueusie et 66 patients (30 %) des troubles digestifs. La biologie montrait une lymphopenie moyenne a 0,9 G/L et une CRP moyenne a 83,7 mg/L. Un scanner etait compatible dans 90 % des cas (147 patients) avec majoritairement une atteinte moderee (53 patients, 32 %). La mortalite observee etait de 36 deces soit 16 %. Vingt-huit patients (13 %) ont ete transferes en reanimation. Le delai moyen avant un evenement defavorable etait de 4 jours, soit 10 jours depuis le debut des symptomes. La duree moyenne d’hospitalisation sans evenement defavorable etait de 6 jours. Les patients presentant une evolution defavorable etaient significativement plus âges (77,7 ans vs 67,7 ans, p Conclusion Dans cette cohorte decrivant les caracteristiques epidemiologiques de patient atteint du COVID-19, ceux ayant une evolution defavorable semblent avoir a l’entree en hospitalisation, une presentation clinique plus severe avec un NEWS2 plus eleve, un syndrome inflammatoire plus important et une atteinte scanographique plus etendue. Les facteurs pronostics retrouves et le delai de survenu d’evenements defavorables semblent coincider avec ce qui est rapporte dans les autres cohortes.
La COVID-19 est une infection virale pouvant se manifester par un syndrome grippal bénin, une pneumonie voire un syndrome de détresse respiratoire aiguë justifiant une hospitalisation. Actuellement, les surinfections bactériennes semblent peu fréquentes. L’intérêt d’une prescription systématique d’antibiotiques dans cette éventualité est donc en question. Nous avons donc comparé nos patients hospitalisés pour une COVID-19 ayant bénéficié ou non bénéficié d’une antibiothérapie. Nous avons inclus de façon rétrospective tous les patients hospitalisés de notre service de maladies infectieuses pour une COVID-19 confirmée par PCR entre le 26 février et le 30 avril 2020. Les patients admis via un service de réanimation ou de soins intensifs étaient exclus. Les données cliniques, biologiques, radiologiques, l’évolution clinique ainsi que la prescription éventuelle d’antibiotiques ont été recueillies via le dossier médical informatique pour chaque patient. Une comparaison entre les patients ayant reçu ou non une antibiothérapie a été réalisée. Sur les 222 patients inclus, 174 (78 %) ont bénéficié d’une antibiothérapie. L’amoxicilline–acide clavulanique représentait 55 % (95 patients) des prescriptions, suivie par les C3G (25 patients, 30 %). La pipéracilline–tazobactam ne représentait que 5 % (9 patients) des antibiotiques de première ligne. Aucune co-infection bactérienne n’a été documentée. Les patients ayant eu des antibiotiques étaient significativement plus âgés (71,5 vs 65,3 ans, p = 0,026) et 144 patients sous antibiotique (83 %) avaient au moins un facteur de risque de COVID-19 grave (vs 32 patients [67 %], p = 0,015), notamment une maladie cardiovasculaire (111 patients [64 %] vs 22 patients [46 %], p = 0,025). Cliniquement, les patients ayant bénéficié d’une antibiothérapie avaient plus de fièvre (143 patients [82 %] vs 26 patients [54 %], p < 0,001) et de dyspnée (118 patients [68 %] vs 19 patients [40 %], p < 0,001). Le score NEWS2 était significativement plus élevé à j0 chez les patients avec antibiotiques (6 vs 2, p < 0,001). Biologiquement, les patients ayant eu des antibiotiques avaient une CRP significativement plus élevée (94 mg/L vs 29 mg/L, p < 0,001). Enfin, les patients avec antibiotiques avaient une atteinte significativement plus grave au scanner (p = 0,014). Concernant le devenir des patients, les patients ayant eu des antibiotiques avaient une évolution significativement plus défavorable (60 patients [34 %] vs 4 patients [8 %], p < 0,001). Les patients ayant bénéficié d’une antibiothérapie étaient plus âgés, plus à risque de forme grave comme le témoigne le score NEWS2 et avaient plus de comorbidités. Cela explique probablement la prescription d’antibiotiques afin de ne pas méconnaître une surinfection bactérienne chez ces patients graves et fragiles. L’évolution était cependant plus défavorable que sans antibiotiques, ce qui est probablement expliqué par la sévérité initiale plus importante de ces patients. Au vu de ces résultats et de l’absence de co-infection documentée, l’antibiothérapie ne semble pas avoir d’intérêt au cours de la CoVID-19, comme l’indique les dernières recommandations du haut conseil de la santé publique. Des études cliniques supplémentaires sont nécessaires pour évaluer l’impact d’une antibiothérapie.
This study investigated the in vivo efficacy of three bacteriophages combined compared with linezolid in two mouse models (nondiabetic and diabetic) of Staphylococcus aureus foot infection. In both models, a single injection of bacteriophages in the hindpaw showed significant antibacterial efficacy. Linezolid was as effective as bacteriophages in nondiabetic animals but ineffective in diabetic animals. These findings further support preclinical and clinical studies for the development of phage therapy.
Aims The EURO-ENDO registry aimed to study the management and outcomes of patients with infective endocarditis (IE). Methods and results Prospective cohort of 3116 adult patients (2470 from Europe, 646 from non-ESC countries), admitted to 156 hospitals in 40 countries between January 2016 and March 2018 with a diagnosis of IE based on ESC 2015 diagnostic criteria. Clinical, biological, microbiological, and imaging [echocardiography, computed tomography (CT) scan, F-18-fluorodeoxyglucose positron emission tomography/computed tomography (F-18-FDG PET/CT)] data were collected. Infective endocarditis was native (NVE) in 1764 (56.6%) patients, prosthetic (PVIE) in 939 (30.1%), and device-related (CDRIE) in 308 (9.9%). Infective endocarditis was community-acquired in 2046 (65.66%) patients. Microorganisms involved were staphylococci in 1085 (44.1%) patients, oral streptococci in 304 (12.3%), enterococci in 390 (15.8%), and Streptococcus gallolyticus in 162 (6.6%). F-18-fluorodeoxyglucose positron emission tomography/computed tomography was performed in 518 (16.6%) patients and presented with cardiac uptake (major criterion) in 222 (42.9%) patients, with a better sensitivity in PVIE (66.8%) than in NVE (28.0%) and CDRIE (16.3%). Embolic events occurred in 20.6% of patients, and were significantly associated with tricuspid or pulmonary IE, presence of a vegetation and Staphylococcus aureus IE. According to ESC guidelines, cardiac surgery was indicated in 2160 (69.3%) patients, but finally performed in only 1596 (73.9%) of them. In-hospital death occurred in 532 (17.1%) patients and was more frequent in PVIE. Independent predictors of mortality were Charlson index, creatinine > 2 mg/dL, congestive heart failure, vegetation length > 10 mm, cerebral complications, abscess, and failure to undertake surgery when indicated. Conclusion Infective endocarditis is still a life-threatening disease with frequent lethal outcome despite profound changes in its clinical, microbiological, imaging, and therapeutic profiles.
