Sarcoidosis is an inflammatory systemic disease characterized by epithelio-gigantocellular granulomatosis without cell necrosis. The diagnosis is pathologic. There are many biomarkers of sarcoidosis. The oldest and most widely used in routine medical practice is angiotensin-converting enzyme (ACE) since 1975. However, it is a marker of low diagnostic value. The other most frequently studied biomarkers of interest are chitotriosidase (CTO), soluble IL-2 receptor (rsIL-2), neopterin, lysozyme, and SAA protein. In this work, we propose a review of the serum biomarkers of sarcoidosis. The studies are heterogeneous, and the results are difficult to conclude. Pathology remains the gold standard for the definitive diagnosis. In the context of evolutionary follow-up and relapse, we can select a priori 3 markers of interest: ACE (sensitivity [Se]: from 22 to 77% and specificity [Sp]: from 54 to 99%), CTO (Se: from 45 to 89% and Sp from 70 to 100%), and rsIL-2 (Se: from 52 to 82% and Sp: from 57 to 94%). The most rational association for a particular disease phenotype has yet to be determined. Future advances will come from an in-depth knowledge of granuloma, its mechanisms of formation, growth, and regression in the context of a still partially unknown inflammatory and immune response. Recent substantial progress has been made through radiological examinations. Multi-omics technologies will undoubtedly complement the arsenal of physiopathological and diagnostic research that will make it possible to move away from pathological examination and ensure better monitoring of the disease.
La sarcoïdose est une maladie systémique inflammatoire caractérisée par une granulomatose de type épithélio-gigantocellulaire sans nécrose cellulaire. Le diagnostic est anatomo-pathologique. Les biomarqueurs de la sarcoïdose sont nombreux. Le plus ancien et le plus utilisé en pratique médicale courante est l’enzyme de conversion de l’angiotensine (ECA) depuis 1975. Il s’agit néanmoins d’un marqueur dont la valeur diagnostique est faible. Les autres biomarqueurs d’intérêts les plus souvent étudiés sont : la chitotriosidase (CTO), le récepteur soluble de l’IL-2 (rsIL-2), la néoptérine, le lysozyme et la protéine SAA. Dans ce travail, nous proposons une revue concernant les biomarqueurs sériques de la sarcoïdose. Les études sont hétérogènes et les résultats sont difficilement conclusives. L’anatomopathologie reste l’examen de référence pour le diagnostic de certitude. Dans le cadre du suivi évolutif et de la rechute nous pouvons retenir a priori 3 marqueurs d’intérêt : l’ECA (sensibilité [Se] : de 22 à 77 % et spécificité [Sp] : de 54 à 99 %), la CTO (Se : de 45 à 89 % et Sp de 70 à 100 %), et le rsIL-2 (Se : 52 à 82 % et Sp : de 57 à 94 %). Il reste à définir l’association la plus rationnelle pour un phénotype de maladie particulier. Les progrès futurs viendront d’une connaissance approfondie du granulome, de ses mécanismes de formation, de croissance et de régression dans le cadre d’une réaction inflammatoire et immune encore partiellement inconnue. Des progrès substantiels récents ont été obtenus grâce aux examens radiologiques. Les technologies multi-omiques viendront sans doute compléter l’arsenal de recherche physiopathologique et diagnostique permettant de s’affranchir de l’examen anatomo-pathologique et d’assurer un meilleur suivi évolutif de la maladie.
Introduction L’infection par le SARS-CoV-2 à l’origine de la COVID-19 peut entraîner une atteinte cardiorespiratoire parfois sévère à l’origine d’hospitalisations en réanimation et de décès chez les sujets âgés polypathologiques et/ou immunodéprimés. Au-delà des mesures d’isolement et du vaccin contre la COVID-19 certains patients ont pu bénéficier d’un traitement par anticorps monoclonaux neutralisants (AMN) associant le tixagévimab et le cilgavimab. Des recommandations très précises ont été formulées pour les patients atteints de maladies auto-immunes et/ou auto-inflammatoires, d’hémopathies malignes, greffés ainsi que les patients immunodéprimés majoritairement traités par rituximab (RTX), azathioprine, mycophénolate mofétil, inhibiteur du BTK et endoxan. À la fin de l’année 2021 le variant omicron (BA.1 puis BA.2) identifié pour la première fois en Afrique du Sud vient a remplacé le variant Delta. Nous rapportons les résultats d’une étude en vie réelle au sein d’une cohorte de patients immunodéprimés suivis en MIIC. Patients et méthodes Dès la mise à disposition d’un AMN dirigé contre le variant omicron de la COVID-19, au début de l’année 2022, nous avons mené une étude de cohorte parallèle, épidémiologique, observationnelle, prospective et comparative, en ouvert, monocentrique au sein de notre file active de 292 patients immunodéprimés. Selon les résultats de la vaccination anti-COVID les patients immunodéprimés ont été répartis en 2 bras : groupe 1 (gr.1) traités par AMN (n=35) avec un taux d’anticorps post-vaccinaux anti-Spike<264BAU/mL et groupe 2 (gr.2) contrôle (n=118) considérés comme répondeur à la vaccination. Les patients ont été suivis tous les 3 mois pendant 6 mois avec un auto-questionnaire et une sérologie quantitative des anticorps anti-SARS-CoV-2. L’objectif principal de l’étude était de comparer l’évolution des taux d’anticorps (niveau de l’immunisation) et la survenue d’évènements infectieux notamment en comparant les cas d’infections graves entre les 2 groupes. Résultats Parmi les 153 patients analysés, le taux d’anticorps médian anti-Spike était de 93,8BAU/mL [22,1–191] dans le gr.1 et de 1538,8BAU/mL [645,5–2561,5] dans le gr.2 (p<0,001). Au sein du gr.1 (non répondeurs à la vaccination traité par AMN), comparé au gr.2, il existait une proportion plus importante de patients ayant une hémopathie lymphoïde B (16 % vs 31 %) et de patients traités par RTX (1,7 % vs 25,7 %). Il n’y avait pas de différence significative concernant le taux de patients immunisés à 3 mois et à 6 mois et les taux d’anticorps [1420BAU/mL dans le gr.1 et 1113BAU/mL dans le gr.2 à 3 mois (p=0,07) puis respectivement 663BAU/mL et 888,4BAU/mL à 6 mois (p=0,16)]. En revanche, nous avons observé un taux plus élevé d’infections symptomatiques 36,4 % dans le gr.1 vs. 15 % dans le gr.2 (p=0,007) mais sans augmentation du nombre d’hospitalisation (1 : gr.1 et 2 : gr.2) ni de décès (1 : gr.1 et 0 : gr.2). Aucun cas de mauvaise tolérance cardiovasculaire au traitement, comme cela avait été évoqué au départ dans la littérature, n’a été observé tout au long de l’étude. Discussion Au cours de cette étude en vie réelle, avec un effectif modeste, les patients immunodéprimés traités par AMN, non répondeurs à la vaccination anti-COVID, accèdent tout au long du suivi de 6 mois à un taux d’anticorps anti-SARS-CoV-2 équivalents aux patients contrôles. Les patients traités par AMN ont présenté plus d’infections symptomatiques mais sans caractère de gravité. L’utilisation d’un AMN en prophylaxie pré-exposition dans le sous-groupe particulier des sujets traités par RTX apparaît intéressante avec un taux de séroconversion rapide et une réponse prolongée. Nos résultats sont conformes aux données de la littérature [1], [2]. Conclusion L’apparition d’un variant du SARS-CoV-2 échappant à la vaccination anti-COVID est possible dans un futur proche, une surveillance est effectuée en continue par l’OMS [3]. L’utilisation des AMN adaptés à la souche virale permet une protection efficace et prolongée chez les patients immunodéprimés ne répondant pas à la vaccination comme le montre les données de la littérature internationale et les résultats de notre étude. Dans ce cas il sera nécessaire d’envisager le plus rapidement possible une protection efficace des immunodéprimés à l’aide d’AMN adaptés destinés à nos patients immunodéprimés pris en charge dans nos services de médecine interne.
Acute hepatic porphyrias are inherited metabolic disorders of heme biosynthesis characterized by the accumulation of toxic intermediate metabolites responsible for disabling acute neurovisceral attacks. Givosiran is a newly approved siRNA-based treatment of acute hepatic porphyria targeting the first and rate-limiting δ-aminolevulinic acid synthase 1 (ALAS1) enzyme of heme biosynthetic pathway. We described a 72-year old patient who presented with severe inaugural neurological form of acute intermittent porphyria evolving for several years which made her eligible for givosiran administration. On initiation of treatment, the patient developed a major hyperhomocysteinemia (>400 μmol/L) which necessitated to discontinue the siRNA-based therapy. A thorough metabolic analysis in the patient suggests that hyperhomocysteinemia could be attributed to a functional deficiency of cystathionine β-synthase (CBS) enzyme induced by givosiran. Long-term treatment with vitamin B6, a cofactor of CBS, allowed to normalize homocysteinemia while givosiran treatment was maintained. We review the recently published cases of hyperhomocysteinemia in acute hepatic porphyria and its exacerbation under givosiran therapy. We also discuss the benefits of vitamin B6 supplementation in the light of hypothetic pathophysiological mechanisms responsible for hyperhomocysteinemia in these patients. Our results confirmed the importance of monitoring homocysteine metabolism and vitamin status in patients with acute intermittent porphyria in order to improve management by appropriate vitamin supplementation during givosiran treatment.
(1) Background: Congenital erythropoietic porphyria (CEP), named Günther's disease, is a rare recessive type of porphyria, resulting from deficient uroporphyrinogen III synthase (UROS), the fourth enzyme of heme biosynthesis. The phenotype ranges from extremely severe perinatal onset, with life-threatening hemolytic anaemia, to mild or moderate cutaneous involvement in late-onset forms. This work reviewed the perinatal CEP cases recorded in France in order to analyse their various presentations and evolution. (2) Methods: Clinical and biological data were retrospectively collected through medical and published records. (3) Results: Twenty CEP cases, who presented with severe manifestations during perinatal period, were classified according to the main course of the disease: antenatal features, acute neonatal distress and postnatal diagnosis. Antenatal symptoms (seven patients) were mainly hydrops fetalis, hepatosplenomegaly, anemia, and malformations. Six of them died prematurely. Five babies showed acute neonatal distress, associated with severe anemia, thrombocytopenia, hepatosplenomegaly, liver dysfunction, and marked photosensitivity leading to diagnosis. The only two neonates who survived underwent hematopoietic stem cell transplantation (HSCT). Common features in post-natal diagnosis (eight patients) included hemolytic anemia, splenomegaly, skin sensitivity, and discoloured teeth and urine. All patients underwent HSCT, with success for six of them, but with fatal complications in two patients. The frequency of the missense variant named C73R is striking in antenatal and neonatal presentations, with 9/12 and 7/8 independent alleles, respectively. (4) Conclusions: The most recent cases in this series are remarkable, as they had a less fatal outcome than expected. Regular transfusions from the intrauterine period and early access to HSCT are the main objectives.
Background Infliximab is currently recommended as a third-line treatment for refractory sarcoidosis. Data in function of clinical phenotype are currently lacking. We evaluated patients’ characteristics and responses to infliximab according to their GenPhenReSa cluster. Methods We evaluated clinical and biological characteristics of patients diagnosed with sarcoidosis who received infliximab between September 2008 and April 2019 at our centre. Results Fifty-five patients (median disease duration, 87 months) received infliximab: 48 (87%) as a second- or third-line treatment, and 7 (13%) as a first-line treatment. After a median duration of 12 months, 24 (45%) and 14 (25%) patients achieved complete and partial responses, respectively, together with a significant decrease in the number of affected organs and tapering of steroid doses. All patients with neurosarcoidosis (OR 17), 90% in group 2 (ocular-cardiac-cutaneous-CNS, OR 7.4), and approximately two-thirds of those in groups 1 (abdominal organs), 4 (pulmonary-lympho-nodal), and 5 (extrapulmonary), achieved a response, whereas patients in group 3 (musculoskeletal-cutaneous) had a treatment-failure OR of 9. Infliximab could be stopped after complete remission was achieved in 7 patients: 4 relapsed after a median of 6 months. Overall, 36% of patients experienced serious adverse events, mainly infections, which led to treatment cessation in 29% of patients and caused two deaths. Conclusions Other than patients with musculoskeletal-cutaneous involvement (group 3), infliximab led to a good response for patients with CNS (group 2) and liver (group 1) organ-predominant sarcoidosis. However, it led to serious infections and merely suspended sarcoidosis, so further research on factors predictive of relapse is needed.
Les infections sévères constituent une des principales complications des traitements par immunosuppresseurs au cours des maladies auto-immunes (MAI). Les facteurs spécifiques associés à la survenue d’infections sévères au décours d’un traitement par rituximab pour une MAI chez les personnes âgées ne sont pas connus. Grâce à une requête à partir du logiciel de prescription (CHIMIO® Computer Engineering), nous avons constitué une cohorte rétrospective monocentrique de patients âgés de plus de 75 ans ayant reçu un traitement par rituximab pour une MAI entre 2008 et 2016. Une infection sévère était définie par une infection associée au décès ou à une hospitalisation. Les données ont été recueillies rétrospectivement par retour au dossier médical. Les caractéristiques cliniques et biologiques des patients ont été recueillies à l’inclusion (date de la première perfusion de rituximab). L’analyse des facteurs de risques a été réalisée à l’aide d’un modèle de Cox cause spécifique. Parmi les 56 patients répondant aux critères d’inclusion, 29 (52 %) étaient atteints d’une cytopénie auto-immune, 18 (32 %) étaient atteints d’une vascularite (à ANCA principalement) et 9 (17 %) d’une autre pathologie (pemphigus, connectivite). Les patients étaient lourdement traités avant l’inclusion avec une dose cumulée de corticoïdes médians à 2,5g (Q25 : 1 ; Q75 : 6) et le rituximab était la première ligne thérapeutique pour seulement 12 patients (21 %). Concernant les traitements associés au Rituximab, 30 (54 %) patients étaient traités par corticoïdes, 9 (17 %) patients par un autre immunosuppresseur et 13 (30 %) n’avaient aucun traitement associé. La majorité des patients (37 patients, soit 66 %) avaient un score ADL (Activites of Daily Living) de 6/6. Seize patients (29 %) ont présenté une infection sévère avec un délai médian de 3 mois, pour un total de 23 infections. Le taux d’incidence d’un premier événement infectieux était de 22 pour 100 patients-années. Trois patients ont développé une pneumocystose au cours du suivi, tous étaient traités avec des corticoïdes associés, sans prophylaxie et ils étaient atteints d’une cytopénie auto-immune. Dix patients (18 %) sont décédés à 2 ans de la première perfusion de rituximab. L’ADL (p=0,03, effet non constant dans le temps), une albuminémie ≤ 30g/L (Rapport des risques instantanés [RRI] : 3,5 ; IC95 : 1,1 ; 12), une dose cumulée de corticothérapie (RRI pour une augmentation de 2gr : 1,1 ; IC95 : 1,0 ; 1,2) et le taux de polynucléaires neutrophiles (RRI pour l’augmentation de 2000/mm3: 1,3 ; IC95 : 1,1 ; 1,6) étaient associés à la survenue d’une infection sévère. La dénutrition et la dépendance, deux composantes de la fragilité, sont associées à la survenue d’une infection sévère chez les patients de plus de 75 ans, traités par rituximab pour une MAI. Le dépistage et la prise en charge de la fragilité doit aider le clinicien à évaluer et à améliorer le risque infectieux dans cette population de patients.
OBJECTIVE:Despite an important increase in lifespan over the last decades, patients with systemic lupus erythematosus (SLE) still have to face a high morbi-mortality, particularly related to cardiovascular diseases, infections and cancers. Such events are more commonly found during old age in the general population, raising the hypothesis of an acceleration of the aging process in SLE patients. In this pilot study, we wanted to test the hypothesis that SLE would be associated with an accelerated biological aging measured by the epigenetic clocks models.METHODS:We applied DNA methylation-based biomarkers of age in publicly available datasets of SLE patients. For every SLE patient and control included in the dataset, we calculated their epigenetic age and a measure of epigenetic age acceleration, according to Horvath's epigenetic clock model.RESULTS:We included in our analysis two distinct DNA methylation datasets of 30 subjects (among which 15 with SLE) and 55 subjects (among which 30 with SLE), respectively. In both datasets, there was a statistically significant correlation between chronological age and epigenetic age. We did not observe any statistically significant difference in the measure of epigenetic age acceleration between SLE patients and controls.CONCLUSION:We did not observe any evidence of an accelerated biological aging in SLE patients, according to Horvath's epigenetic clock model.
OBJECTIVES:Antisynthetase syndrome (ASyS) is a rare autoimmune disease. We aimed to determine clinical, biological, radiological, and evolutive profiles of ASyS patients with anti-PL7 or anti-PL12 autoantibodies.METHODS:We performed a retrospective study that included adults with overt positivity for anti-PL7/anti-PL12 autoantibodies and at least one Connors' criterion.RESULTS:Among 72 patients, 69% were women, 29 had anti-PL7 and 43 anti-PL12 autoantibodies, median age was 60.3 years, and median follow-up period was 52.2 months. At diagnosis, 76% of patients had interstitial lung disease, 61% had arthritis, 39% myositis, 25% Raynaud's phenomenon, 18% mechanic's hands, and 17% had fever. The most frequent pattern on initial chest computed tomography was non-specific interstitial pneumonia and 67% had fibrosis at last follow-up. During follow-up, 12 patients had pericardial effusion (18%), 19 had pulmonary hypertension (29%), 9 (12.5%) had neoplasms, and 14 (19%) died. Sixty-seven patients (93%) received at least one steroid or immunosuppressive drug. Patients with anti-PL12 autoantibodies were younger (p=0.01) and more frequently exhibited anti-SSA autoantibodies (p=0.01); patients with anti-PL7 autoantibodies had more severe weakness and higher maximum creatine kinase levels (p=0.03 and 0.04, respectively). Initial severe dyspnoea was more common in patients from the West Indies (p=0.009), with lower predicted values of forced vital capacity, forced expiratory volume in 1s, and total lung capacity (p=0.01, p=0.02, p=0.01, respectively) contributing to a more severe 'respiratory' initial presentation.CONCLUSIONS:The high mortality and significant numbers of cardiovascular events, neoplasms and lung fibrosis in anti-PL7/12 patients justify close monitoring and question addition of antifibrotic drugs.
Une vingtaine de cas de syndrome de Tako-Tsubo (TKS) sont décrits chez des malades suivis pour un lupus systémique, vascularite, polyarthrite rhumatoïde, et plus récemment chez des malades atteints de syndrome de Sjögren. Bien que l'on ne puisse pas exclure une association fortuite, la rareté de cette coexistence fait évoquer des mécanismes physiopathogéniques communs voire même un lien de causalité. Nous avons cherché à décrire les caractéristiques des patients présentant un TKS au cours de maladies systémiques, puis de les comparer à des TKS primitifs. Nous avons réalisé une étude rétrospective, multicentrique, observationnelle, de type appel à observations, à l'aide d'un questionnaire standardisé envoyé via les réseaux du Groupe français d'étude des vascularites (GFEV) et de la Société nationale française de médecine interne (SNFMI), afin d'identifier les patients ayant présenté un TKS au cours d'une maladie systémique (maladies auto-immunes, vascularites, syndromes auto-inflammatoires). Le diagnostic de TKS était retenu selon les critères européens de 2018, et ceux des maladies systémiques selon les critères en vigueur. Nous avons ensuite complété notre recherche de cas par une analyse exhaustive de la littérature. Enfin, nous avons comparé les TKS associés aux maladies systémiques aux TKS primitifs, extraits d'une base d'un service de cardiologie. Les variables continues ont été exprimées en médiane et leurs caractéristiques ont été comparées avec des tests t. Vingt-trois patients répondant aux critères diagnostics ont été inclus. Par ailleurs, 21 patients issus de la littérature ont été recueillis pour un total de 44 cas. Quarante (90 %) étaient des femmes. L'âge médian au diagnostic de TKS était de 67 ans. Seuls 4 (9 %) étaient porteurs d'une cardiopathie sous-jacente. Les principales maladies sous-jacentes étaient le lupus systémique pour 7 (16 %), le syndrome de Sjögren pour 6 (14 %), la polyarthrite rhumatoïde pour 6 (14 %), le syndrome des antiphospholipides pour 4 (9 %), la sclérodermie systémique pour 4 (9 %) et les vascularites associées aux ANCA pour 3 (7 %). La maladie systémique n'était compliquée d'atteinte neurovégétative que dans trois cas (7 %). Le diagnostic de TKS était porté au cours de l'évolution de la maladie systémique dans 25 cas (57 %), avec un délai médian d'évolution de 3 ans. Cependant le TKS faisait partie des manifestations faisant révéler la maladie systémique dans 17 cas (39 %) et la précédait dans 2 (4 %). Un stress émotionnel ou physique était retrouvé dans 34 cas (78 %) dont 29 (66 %) correspondaient à une poussée de la maladie. Cette poussée était traitée dans 13 cas (30 %), principalement par corticothérapie. Deux décès (4 %) sont survenus au cours de l'épisode sans lien avec le TKS. Après un suivi médian de 12 mois, tous les patients avaient récupéré une FEVG normale, 1 (2 %) avait présenté une récidive du TKS, et aucun n'était décédé d'une cause cardiaque. Les patients recueillis atteints d'un TKS associé à une maladie de système (n = 23) ont ensuite été comparés à 23 TKS primitifs. Il n'existait pas de différence significative entre l'âge au diagnostic (70 vs 76 ans, p = 0,14). La présentation était similaire entre les deux groupes, qu'elle soit clinique (symptomatiques chez 19 vs 17, p = 0,53), biologique (pic de troponine à 39 fois la normale vs 31, p = 0,54) ou échographique (fraction d'éjection du ventricule gauche à 35 vs 40 %, p = 0,010). Les traitements reçus ont été comparables (bloqueurs du système rénine-angiotensine dans 14 vs 13 cas, p = 0,77) ainsi que la nécessité d'une hospitalisation en réanimation (52 vs 61 %, p = 0,28). Les maladies de système peuvent rarement s'accompagner de TKS, notamment à l'occasion de poussées de la maladie. Bien que rare, le TKS doit ainsi être évoqué devant toute symptomatologie cardiaque au cours d'une maladie systémique. Comparés à des TKS primitifs, leur présentation clinique, biologique ou échographique n'a pas de caractère particulier. Leur pronostic semble excellent, la récupération de la FEVG étant constante, et aucun décès en lien avec une cause cardiaque n'ayant été retrouvé.
Chronic active Epstein-Barr virus (CAEBV) infection is usually a fatal disease associated with clonal proliferation of EBV-infected T or NK cells. We present the case of a 33-year-old Peruvian patient who developed a multisystem CAEBV, notably responsible for exceptional ophthalmological and renal damage. We describe the clinicopathological features of EBV-induced lymphoproliferative disorder.
INTRODUCTION:Genital vasculitis are uncommon. They may be localized or be a manifestation of a systemic vasculitis. We report a patient with a giant cell arteritis (GCA) involving uterine arteries and a literature review on genital vasculitis. CASE REPORT:A 65-year-old woman was referred to a gynecologist for a cervical intraepithelial neoplasia (CIN) associated with an ovarian mass. An unexpected diagnosis of GCA involving small to medium sized uterine arteries was made through the anatomopathological analysis while the patient was asymptomatic. Two weeks later, she presented typical cranial symptoms of giant cell arteritis (GCA). PET-scanner confirmed the diagnosis of GCA with an involvement of the ascending aorta, and the axillary and the subclavian arteries. CONCLUSION:Gynecologic vasculitis are rare and usually an asymptomatic manifestations of GCA.
Background Thanks to direct-acting antivirals, hepatitis C virus (HCV) infection can be cured, with similar rates in HCV-infected and HIV–HCV co-infected patients. HCV cure is likely to foster behavioral changes in psychoactive substance use, which is highly prevalent in people living with HIV (PLWH). Cannabis is one substance that is very commonly used by PLWH, sometimes for therapeutic purposes. We aimed to identify correlates of cannabis use reduction following HCV cure in HIV–HCV co-infected cannabis users and to characterize persons who reduced their use. Methods We used data collected on HCV-cured cannabis users in a cross-sectional survey nested in the ANRS CO13 HEPAVIH cohort of HIV–HCV co-infected patients, to perform logistic regression, with post-HCV cure cannabis reduction as the outcome, and socio-behavioral characteristics as potential correlates. We also characterized the study sample by comparing post-cure substance use behaviors between those who reduced their cannabis use and those who did not. Results Among 140 HIV-infected cannabis users, 50 and 5 had reduced and increased their use, respectively, while 85 had not changed their use since HCV cure. Cannabis use reduction was significantly associated with tobacco use reduction, a decrease in fatigue level, paying more attention to one’s dietary habits since HCV cure, and pre-HCV cure alcohol abstinence (p = 0.063 for alcohol use reduction). Conclusions Among PLWH using cannabis, post-HCV cure cannabis reduction was associated with tobacco use reduction, improved well-being, and adoption of healthy behaviors. The management of addictive behaviors should therefore be encouraged during HCV treatment.
To the Editor—We read with great attention Reddy et al’s practical guide for clinicians on the treatment of tobacco dependence in people with human immunodeficiency virus (PWH) [1], a problem that needs urgent attention [2]. We would like to provide additional elements that could enhance successful quit attempts by PWH. As Reddy et al indicated, varenicline is the pharmacological treatment of choice for tobacco dependence in PWH, and combining pharmacotherapy with behavioral therapy is more effective than either therapy alone [1]. Furthermore, PWH psychobehavioral characteristics are most probably associated with successful treatment. For instance, weight control is a commonly reported motive for tobacco use in this population [3]. Comprehensive data on these characteristics could help increase the likelihood of successful quit attempts by PWH. We conducted secondary analyses on data from the randomized controlled trial ANRS 144 Inter-ACTIV that tested the efficacy of varenicline for tobacco smoking cessation in PWH [4]. Logistic regression models were used to test for associations between clinical and sociobehavioral correlates—assessed at the screening visit (ie, week –4)—and 10-month continuous tobacco abstinence (ie, week 9 to week 48).
Introduction: Pneumococcal infections are frequent and potentially serious in patients with inflammatory diseases treated with immunosuppressants and/or biotherapies. This patient population considered to be at very high risk of infection is subject to national vaccination recommendations. The main objective of this study was to assess pneumococcal vaccine coverage in a day hospital (internal medicine and vascular disease) in patients treated with immunosuppressants. Methods: An observational, descriptive, retrospective, and single-center study. We included 150 consecutive patients for 3 months (February to April 2018). We studied pneumococcal vaccination coverage and the time elapsed between the date of vaccination with the 13-valent polysaccharide conjugate vaccine (PCV13) and the start of immunosuppressive therapy. Results: Among the 150 patients included in the study, vaccination coverage with PCV13 was 85% (127/150) and decreased to 46.7% (70/150) for the recommended vaccination schedule. Taking into account vaccine efficacy according to the date of initiation of the treatment, only 28.7% (43/150) of the patients in the study were able to benefit from an optimal complete vaccination schedule, i.e. 33.8% (43/127) among patients vaccinated with PCV13. Conclusion: Despite official recommendations, vaccination coverage against pneumococcus remains insufficient in patients under immunosuppressants and/or biotherapies. In addition to the continued training of doctors, optimizing computer prescription of vaccines in view of facilitating vaccination tracing and having vaccination carried out at the site of consultation are avenues for improvement to be considered.
Systemic immunoglobulin light-chain (AL) amyloidosis is characterized by deposition of amyloid fibrils of light chains produced by clonal CD38 + plasma cells, resulting in organ dysfunction. Cardiac involvement has a major prognostic value. Antiplasma cell chemotherapy reduces the synthesis of immunoglobulin light chains (precursors of amyloid deposits). We describe a case of AL amyloidosis in a 95-year-old patient. Our patient responded poorly to treatment with rituximab, cyclophosphamide-bortezomibdexamethasone, and rituximab-bendamustine. Finally, the anti-CD38 antibody daratumumab was associated with the best hematologic responsiveness without significant adverse effects. In conclusion, our case suggests that daratumumab is an effective and well-tolerated alternative to chemotherapy in the treatment af AL amyloidosis in very elderly patients. Les infections à pneumocoque sont fréquentes et potentiellement graves chez les patients atteints de maladies inflammatoires traitées par immunosuppresseurs et/ou biothérapies. Cette population de patients jugée à très haut risque infectieux est soumise à des recommandations vaccinales nationales [1]. L’objectif principal de cette étude a été d’évaluer la couverture vaccinale antipneumococcique (VAP) au sein d’un hôpital de jour chez des patients traités par immunosuppresseurs (IS) et/ou biothérapies (BT). Étude observationnelle, descriptive, rétrospective et monocentrique dans un service d’hospitalisation de jour de médecine interne et de médecine vasculaire. Le critère de jugement principal de l’étude était l’existence ou non d’une VAP à 13 valences (VPC13) et 23 valences (VPP23) répertoriées dans le dossier informatique de l’hôpital et lorsque cela s’avérait nécessaire appel du médecin traitant. Nous avons également renseigné le temps écoulé entre la date de la vaccination par le VPC13 et le début du traitement initial afin d’estimer l’efficacité vaccinale selon la thérapeutique d’après les dernières données de la littérature. De manière arbitraire, compte tenu du délai moyen d’efficacité de la VAP, nous avons catégorisé préalablement la population étudiée en 4 groupes : groupe 1 : vaccinés plus de 3 semaines avant l’introduction du traitement ; groupe 2 : vaccinés dans les 3 semaines avant l’introduction du traitement ; groupe 3 : vaccinés après l’introduction du traitement ou le jour même et groupe 4 : absence de vaccination tout au long de la prise en charge. Les autres variables étudiées étaient l’âge et le genre. Nous avons également recueilli l’absence de données concernant le statut vaccinal dans le dossier médical et renseigné le délai de vaccination entre le VPC13 et le VPP23 (inférieurs ou supérieur à 8 semaines). Les différents IS renseignés dans l’étude étaient les suivants : corticoïdes, méthotrexate, azathioprine, cyclophosphamide, salazopyrine, mycophénolate mofétil, léflunomide. Les BT concernées étaient : rituximab, anti-TNF alpha, ustékinumab, éculizumab et tocilizumab. Nous avons étudié la couverture vaccinale anti-pneumococcique et le temps écoulé entre la date de la vaccination par le vaccin 13-valents (VPC13) et le début du traitement immunosuppresseur. Durant la période d’inclusion de 3 mois, 443 dossiers ont été sélectionnées. Finallement, 150 patients consécutifs ont été retenus dans l’étude selon les critères de sélection. Parmi les 150 patients, le taux de vaccination par le VPC13 se répartissait de la manière suivante selon les groupes : 4 % (groupe 1) ; 17,3 % (groupe 2) ; 63,4 % (groupe 3) et 15,3 % (groupe 4). Le taux de vaccination par le VPC13 était de 85 % (127/150), 23 patients (15 %) n’ont pas été vaccinés par le VPC13 (donc non vaccinés par le VPP23 également puisque tous les patients de l’étude vaccinés par le VPP23 ont été vaccinés par le VPC13). La vaccination par le VPC13 se répartie de la manière suivante par rapport à l’introduction du traitement : 21 % avant la mise en route du traitement par IS ou BT et 63 % après le début du traitement. Soixante-dix patients ont bénéficié d’une vaccination selon le schéma vaccinal recommandé (SVR). Le taux brut de vaccination par le SVR (délai d’injection entre le VPC13 et le VPP23 égal à au moins 8 semaines) dans la population étudiée était de 46,7 % (70/150). Le taux de vaccination selon le SVR était de 55,1 % (70/127) dans la population de patients vaccinés par le VPC13. Il apparaît que 21,4 % des patients (15/70) ont bénéficié d’un SVR parmi les patients vaccinés par le VPC13 avant l’introduction du traitement IS ou BT et 78,6 % des patients (55/70) après l’introduction du traitement. Dans notre étude, aucun patient n’a reçu de VPP23 sans avoir été vacciné par le VPC13. Quatre-vingt-deux patients ont été vaccinés par le VPP23 et le VPC13 (54,7 % de la population totale de l’étude [n = 150] et 64,5 % des patients ayant bénéficié d’une première injection de VPC13 [n = 127]) mais seuls 70 ont bénéficié du SVR (VPP23 au moins 8 semaines après le VPC13). Quarante-cinq patients (35,5 %) ont été vaccinés par le VPC13 et non pas été vaccinés par le VPP23 parmi les patients vaccinés (n = 127). La couverture vaccinale contre le pneumocoque reste insuffisante chez les patients sous immunosuppresseurs et/ou biothérapies malgré les recommandations officielles. Les résultats de cette étude d’analyse des pratiques nous incite à : – poursuivre la formation des médecins ; – optimiser la prescription informatique des vaccins permettant un traçage de la vaccination ; – réaliser la première vaccination par le VPC13 sur le site de la consultation.
