Kolondivertikel und das Auftreten von adenomatösen Polypen sind ein im Alter zunehmendes Phänomen. Die aktuelle Datenlage gibt widersprüchliche Angaben, ob Patienten mit Divertikulose ein erhöhtes Adenomrisiko aufweisen, her. Eine positive Korrelation hätte möglicherweise Auswirkungen auf Empfehlungen zur Durchführung von Vorsorgeuntersuchungen. Das Ziel der Studie war es daher, zu überprüfen, ob Patienten mit Divertikulose ein erhöhtes Risiko für das Auftreten von Adenomen haben. Es handelt sich um eine offene, prospektive, duozentrische Beobachtungsstudie.
Die Beurteilung der Papilla duodeni major während einer Ösophago-Gastro-Duodenoskopie ist eingeschränkt, bestehen hier mehrere Schwierigkeiten. Bisher wird zur kompletten Darstellung der Papille ein Seitblickduodenoskop empfohlen, das jedoch vor allem in Praxen und kleinen Krankenhäuser nicht immer vorhanden ist und außerdem Erfahrung und eine spezielle Ausbildung benötigt. Ziel der Studie sollte der diagnostische Nutzen der Kappen-assistierten ÖGD (KA-ÖGD) im Vergleich zu einer konventionellen ÖGD (K-ÖGD) zur Darstellung und Beurteilung der Papilla duodeni major evaluiert wurden.
Background and aims: Anastomotic strictures are well-known complications after bilioenterostomy. Endoscopic procedures are usually not possible in patients with a bilioenterostomy. Hence, percutaneous transhepatic biliary drainage (PTBD) has become the treatment of choice for the management of these patients. The main goal of the present study was to analyze the long-term follow-up of PTBD in such patients.Methods and patients: Between January 1996 and December 2006, 44 patients with benign anastomotic stricture after bilioenterostomy were identified by an analysis of the PTBD database, hospital charts, and cholangiograms.Results: In 27/44 patients the percutaneous transhepatic biliary drain was successfully removed after 19.9 +/- 16.1 months (treatment success in 61.4%). During a mean follow-up of 53.7 +/- 28.4 months after removal of the drain, no evidence was found of recurrent strictures in these patients. Ten out of 44 patients carry permanent drains (22.6% of patients with ongoing treatment, mean follow-up 46.4 +/- 54.7 months) without the option for further surgery owing to concomitant disease (n = 2) or because they refused further surgery (n = 8). In 7 out of 44 patients (16%) PTBD treatment was deemed to have failed and the patients underwent repeat operation.Conclusions: PTBD should be considered the treatment of choice in patients with benign anastomotic stricture after bilioenterostomy, especially after stricturing of a hepatojejunostomy.

OBJECTIVE:To assess prospectively the effectiveness of a single prophylactic dose of cefuroxim before therapeutic endoscopy, in view of the general practice not to give antibiotics routinely.PATIENTS AND METHODS:In a prospective study endoscopic retrograde cholangiopancreatography (ERCP) or percutaneous transhepatic cholangiography with drainage (PTCD) together with simultaneous stone extraction, dilatation or stent implantation were performed in 99 patients (51 men, 48 women; mean age 60.6 +/- 19.2 years). Group A (n = 49) received 1500 mg cefuroxim i.e. 30 min before the procedure, while none was given to group A patients (n = 50). Several blood cultures were taken up to 60 min after the endoscopy. The number of septicaemias (bacteraemia with fever, rigor, circulatory reactions, leukocytosis or leukopenia) were noted. Bile cultures were obtained in 56 patients with indwelling biliary drainage.RESULTS:Incidence of bacteraemia was 6.1% (3 of 49) in group A, 16% (8 of 50) in group B, but this difference is not statistically significant. The septicaemia rate was 6.1% in group A and 10% in group B (not significant). There were eleven positive blood cultures with 12 different microorganisms, Escherichia coli in four (A: n = 3). In vitro cefuroxim sensitivity was 53.3%. 25 different bacterial species were isolated from 73.2% of bile cultures, of which 53.4% were sensitive to cefuroxim and 8.2% moderately so.CONCLUSION:Although the obtained differences between the two groups were not statistically different, the reduction in bacteraemia/septicaemia rate may be of clinical use. Further studies are needed with higher dosages or antibiotic combinations to improve these results.
Hintergrund: Obstruktion des Biliären Systems kann zu schweren Komplikationen wie Cholangitis oder Pankreatitis führen. Das Verfahren der Wahl bei diesen Problemen ist die ERCP. Die Kanülierung kann jedoch manchmal aufgrund anatomischer Variationen, Tumorbedingten Verschlüssen oder entzündlichen Veränderungen sehr schwierig sein. Nadelmesser Papillotomie wie auch Precut Sphincterotomy wurde beschrieben als Verfahren, um das Ostium des Gallengangs besser sichtbar zu machen und die Kanülierung zu erleichtern, allerdings auch mit einem erhöhten Risiko von Komplikationen.
Hintergrund: Zahlreiche Studien konnten die Überlegenheit der Kapselendoskopie (KE) im Vergleich mit anderen diagnostischen Verfahren für die mittlere gastrointestinale Blutung zeigen. Dagegen ist bisher unklar, ob Patienten mit Eisenmangelanämie – unabhängig von den Zeichen einer gastrointestinale Blutung – von einer KE profitieren. Wir untersuchten daher in dieser retrospektiven Studie den diagnostischen Gewinn der KE bei Patienten mit Eisenmangelanämie und unauffälliger ÖGD und Ileo-Koloskopie. Darüber hinaus gingen wir der Frage nach, ob bei diesen Patienten die Blutungsanamnese auf pathologische Befunde in der Dünndarmschleimhaut hinweisen kann. Patienten und Methoden: In einem endoskopischen Zentrum wurden zwischen den Jahren 2002 und 2006 332 Patienten mit V.a. gastrointestinale Blutung mit der KE untersucht. Von diesen erfüllten 72 Pat. die Kriterien einer Eisenmangelanämie (Frauen (Männer): Hb <12mg/dl (<13mg/dl) und MCV <80 fl und/oder Ferritin <15µg/l (<30µg/l). Nach ihrer Blutungsanamnese wurden die Pat. in drei Gruppen eingeteilt: Kein Nachweis von Blut im Stuhl, Nachweis von okkultem oder ouvertem Blut im Stuhl. Patienten mit unklarer Blutungsanamnese wurden ausgeschlossen. Klinische und laborchemische Parameter wurden mit pathologischen Befunden im Dünndarm zwischen den drei Gruppen verglichen und statistisch analysiert. Ergebnisse: Bei 35 von 72 Pat. (49%) mit Eisenmangelanämie konnte eine potentielle Blutungsquelle im Dünndarm gefunden werden. 42% der Pat. ohne Blutungszeichen, 52% mit okkulter und 58% mit ouverter Blutung hatten einen pathologischen Befund in der KE. Es konnte weder ein signifikanter Unterschied zwischen der absoluten Anzahl der Pat. mit positivem Befund in der KE noch zwischen den unterschiedlichen Diagnosen wie Ulkus, Erosion, Angiodysplasie, Tumor oder Polyp gefunden werden. Allein die akute Blutung trat signifikant häufiger in der Patientengruppe mit ouverter Blutung auf. Zusammenfassung: Bei rund der Hälfte der Pat. mit Eisenmangelanämie deckt die KE potentielle Blutungsquellen im Dünndarm auf. Interessanterweise trifft dies auch für Pat. ohne Blutungsnachweis im Stuhl zu.
A 61-year-old man was transferred from another hospital because of recurrent cholestasis. He had undergone a Whipple’s operation for pancreatric cancer 27 months previously. Because of a stenosis of the biliodigestive anastomosis (tumor recurrence was suspected, but not histologically confirmed) a first, uncovered, metal stent was inserted percutaneously 1 year after surgery. At 9 months after that, a second metal stent (now a covered version) was placed to treat stent occlusion. We performed percutaneous puncture and drainage of the biliary system, and the second metal stent dislocated distally into the small bowel during dilation (Figure [1]). The first stent was firmly embedded in the biliary duct. To avoid further complications, removal of the second dislocated stent was planned. The endoscope could not reach the biliodigestive anastomosis. A flexible Terumo guide wire was therefore introduced deeply into the small bowel via the percutaneous tract, was grasped with a forceps using a pediatric colonoscope and was pulled out of the mouth. A Teflon guide wire was then substituted for the Terumo wire. A 15-mm dilation balloon catheter (CRE-balloon; Boston Scientific, Ratingen, Germany) was inserted perorally over this guide wire, and pushed forward until it reached the dislocated stent (Figure [1]), with the help of manipulation at both ends of the wire. After placement inside the stent, the balloon was inflated and extracted through the mouth together with the stent, without any problems (Figures [2] and [3]). Treatment was then continued with a Yamakawa-type prosthesis, as planned. This case represents a further example of the usefulness of the combined endoscopic and percutaneous approach in difficult biliary situations; otherwise surgery would have been the only alternative.
Das von-Willebrand-Syndrom soll eine Ursache für Blutungen aus Angiodysplasien (AD) sein. Ein erworbener Defekt des von-Willebrand-Faktors (vWF) wird pathogenetisch mit einer Aortenstenose, chronischer Niereninsuffizienz, Tumor- und Autoimmunerkrankungen in Zusammenhang gebracht. Wir untersuchten systematisch hämostaseologische und klinische Parameter bei Patienten mit Blutungen aus AD. Patienten und Methoden: 22 Patienten (12 Frauen, mittl. Alter 68J) mit gastrointestinaler Blutung aus AD wurden eingeschlossen. Bei allen Patienten wurden eine ÖGD, Ileokoloskopie, Kapsel-Endoskopie und Echokardiographie durchgeführt. Die Thrombozytenfunktion wurde in vitro mit Hilfe der Verschlusszeit in einem Vollblut-Test, aktiviert durch Kollagen/Epinephrin bzw. Kollagen/ADP gemessen (Platelet Function Analyser, PFA). Ferner wurden Quick, aPTT, Thrombinzeit, Faktor XIII, Faktor VIII: C und vWF (Ristocetin-Cofaktor-Aktivität und vWF-Ag) bestimmt. Ergebnis: Die Patienten hatten eine durchschnittliche Blutungsanamnese von 5,7J, ein medianes minimales Hb von 7,0mg/dl und einen Transfusionsbedarf von 2,5 EK. Es wurden im Median 3 AD mit einem Durchmesser von 4mm diagnostiziert. 5% der AD waren im Magen, 55% im oberen Dünndarm, 10% im mittleren Dünndarm, 5% im unteren Dünndarm und 25% im Kolon lokalisiert. 13/22 Patienten (60%) hatten eine pathologische Thrombozytenfunktion im PFA-Test (PFA-Koll/Epi.: median 266 sec (Norm: <165) und/oder PFA-Koll/ADP: median 149 sec (Norm: <118)). Thrombozytenzahl (Median 280 G/l), Quick (Median 101%), aPTT (Median 32 s), Thrombinzeit (Median 16 s), Faktor XIII (Median 113%) und Faktor VIII-Koagulationsaktivität (Median 165%) waren bei diesen Patienten normal. Diese hämostaseologische Konstellation weist auf einen funktionellen Defekt des vWF hin. 10/22 Patienten (45%) hatten eine Aortenstenose. Bei 7 Patienten lagen sowohl eine Aortenstenose als auch verlängerte PFA-Verschlusszeiten vor. 3 Patienten waren an einer Niereninsuffizienz erkrankt (2 mit pathol. PFA-Werten). Zusammenfassung: Fast 2/3 aller Patienten mit blutenden AD hatten eine Störung der Thrombozytenfunktion mit Hinweis auf einen funktionellen Defekt des vWF. Als Ursache eines erworbenen von-Willebrand-Syndroms kommt bei der Hälfte der Patienten eine Aortenstenose in Frage.
Hintergrund: Patienten mit chronischer Pankreatitis leiden häufig unter chronischen Schmerzen, die eine dauerhafte Schmerztherapie notwendig machen. Ursache dieser Schmerzen sind zumeist Veränderungen des Pankreasgangs wie impaktierte Steine, Stenosen oder die Kombination aus Steinen und Stenosen. Seit mehreren Jahren wird versucht diese Problematik durch endoskopische Verfahren zu bekämpfen. Langfristig besteht bislang Ungewissheit über die Effektivität dieser endoskopischen Therapie. Ziel: In dieser prospektiven Studie sollte bei Patienten mit chronischer Pankreatitis das Outcome endoskopischer Interventionen bezüglich Schmerzfreiheit und Lebensqualität untersucht werden. Methoden: Bei 19 Patienten mit chronischer Pankreatitis wurde zunächst mithilfe bildgebender Verfahren wie CT, MRT, MRCP, Sonographie oder Endosonographie die Morphologie des Pankreasgangs beurteilt. Einschlusskriterien waren neben den klinischen Zeichen einer chronischen Pankreatitis und chronischer Schmerzen ein Aufstau des Ductus Wirsungianus durch Steine oder Stenosen. Eine Evaluation der klinischen Beschwerden wurde 6 und 12 Monate nach Beginn der Schmerztherapie durchgeführt. Ergebnisse: Von den 19 in die Studie eingeschlossenen Patienten hatten 12 Patienten eine Stenose, 7 Patienten hatten die Kombination aus einem Stein und einer Stenose. Bei allen Patienten wurde nach endoskopischer Papillotomie eine Dilatation der Stenose mit anschließender Stenteinlage, sowie bei Patienten mit Pankreasgang-Konkrementen eine Steinextraktion mit dem Dormia-Körbchen durchgeführt. Die Dauer der Stenttherapie betrug im Durchschnitt 6 Monate. Bei 11/19 (58%) Patienten konnte nach Beginn der endoskopischen Therapie eine komplette Schmerzfreiheit erzielt werden, bei 6/19 (32%) Patienten kam es zur deutlichen Verbesserung der klinischen Beschwerden und nur bei 2/19 (10%) Patienten konnte keine signifikante Verbesserung der Schmerzsymptomatik erzielt werden. Bezüglich der Einnahme von Schmerzmedikamten konnte ebenfalls ein gutes Ergebnis erzielt werden. Vor Beginn der endoskopischen Therapie benötigten 7/19 (37%) Patienten eine dauerhafte Schmerzmedikation, nach endoskopischer Intervention waren es lediglich 4/19 (21%). Insgesamt waren 17/19 (89%) der in die Studie eingeschlossenen Patienten nach 12 Monaten mit dem Gesamtergebnis bezüglich Schmerzreduktion und Verbesserung der Lebensqualität zufrieden, lediglich 2/19 (11%) Patienten waren mit dieser Therapie unzufrieden. Nur ein Patient wurde einer operativen Therapie zugeführt. Zusammenfassung: Bei Patienten mit chronischer Pankreatitis stellt eine endoskopische Therapie bestehend aus Steinextraktion, Ballondilatation und abschließender Stenteinlage eine sichere, effektive und dauerhaft erfolgreiche Therapie zur Verbesserung der klinischen Beschwerden und der Lebensqualität dar.
In magnetic resonance imaging (MRI) the trend is to move towards higher magnetic field strengths. This tendency is driven by the possibility to obtain better signal to noise ratios. One recent development is diffusion tensor imaging (DTI). As water diffusion is less hindered along the length of the axon than its cross section DTI allows for the investigation of the white matter microstructure. For example such histological correlates as the size cross sectional density, organization of axons and degree of myelination can be studied with this method. 13 preterm infants at term equivalent age and 6 healthy term newborn controls were studied. Median GA in the preterm group was 31 weeks, median postmenstrual age (PMA) = 41 weeks when MR was performed. Term controls were examined at a median postnatal age of 2 days. Each subject underwent a single MRI examination on a 3 Tesla Trio Siemens scanner. MR protocol included a DTI sequence with 6 non-collinear diffusion weighting directions. From the raw data a diffusion tensor was estimated with linear least square fitting and from the tensor a scalar measure of anisotropic diffusion was calculated. This measure, called fractional anisotropy (FA), has been used to compare the groups. We present results demonstrating the feasibility of DTI at 3 Tesla in neonatal subjects. We found Anisotropy values which were not significantly different between preterm infants at term-equivalent age and healthy term controls in Corpus callosum splenium and White matter. However, values in the posterior limb of internal capsule (PLIC) were significantly higher for the preterm group (p= 0.0017). Conclusions: Diffusion tensor imaging of neonatal subjects can be safely carried out with high quality results on 3 Tesla. Anisotropy values in PLIC are significantly higher for preterm infants at term-equivalent age as compared with term control infants in the studied group.
Submukosale Tumoren (SMT) des oberen Verdauungstrakts sind häufig endoskopische Zufallsbefunde. In der Regel umfasst das bisherige Procedere Läsionen größer 2–3 cm früh-elektiv zu resezieren, während Patienten mit kleinerem SMT in ein Überwachungsprogramm eingeschlossen werden und erst bei Größenzunahme eine Resektion erwogen wird. Die Datenbasis als Rationale für dieses Vorgehen ist jedoch spärlich.
Background The accuracy of ERCP-based brush cytology or forceps biopsy for tissue diagnosis is relatively low (usually not exceeding 70%). By contrast, reported accuracy rates for EUS-guided FNA of pancreatobiliary masses are over 80%. This prospective study compared these two modalities for the first time in the diagnosis of indeterminate biliary strictures and pancreatic tumors. Methods Fifty consecutive patients (29 men, 21 women; mean age 62.1 years) with obstructive jaundice in whom a tissue diagnosis was required were included. During ERCP, intraductal specimens were obtained with a forceps and with two different types of brush (conventional and spiral suction) in random order. During EUS, only visible mass lesions or localized bile duct wall thickening were aspirated (22-gauge needle), with at least two passes yielding material sufficient for assessment. A cytopathologist was not present in the procedure room to evaluate specimen adequacy. The reference methods were surgery, other biopsy results, follow-up until death, or the conclusion of the study (mean follow-up 20 months). Results The final diagnoses were malignancy, 28 (16 pancreatic, 12 biliary), and benign biliary stricture, 22. Sensitivity and specificity for ERCP-guided biopsy were 36% and 100%, respectively; for ERCP-guided cytology (when using conventional and spiral suction brushes), 46% and 100%, respectively; and for EUS-guided FNA, 43% and 100%, respectively. If the punctured lesions are considered (n = 28) alone, the sensitivity of EUS-guided FNA was 75%. In general, sensitivity was better for ERCP-based techniques in the subgroup biliary tumor (ERCP 75% vs. EUS 25%), whereas EUS-guided biopsy was superior for pancreatic mass (EUS 60% vs. ERCP 38%). Conclusions For biliary strictures, combined ERCP- and EUS-guided tissue acquisition seems to be the best approach to tissue diagnosis. From a clinical standpoint, it appears reasonable, when a tissue diagnosis is required, to start with ERCP if biliary malignancy is suspected and with EUS when a pancreatic tumor is thought to be the cause of a biliary stricture.
Der größte Teil der Pankreaskarzinome ist bei Diagnosestellung aufgrund der lokal fortgeschrittenen Erkrankung und/oder wegen Fernmetastasen inoperabel. Die meisten Pankreaskarzinome sitzen im Pankreaskopf. Das Hauptsymptom ist die extrahepatische Cholestase. Die entscheidende therapeutische Maßnahme ist die Herstellung der Gallengangsdränage entweder in das Duodenum endoskopisch oder perkutan transhepatisch, perkutan nach außen oder durch eine biliodigestive Anastomose. In der Mehrzahl der Fälle erfolgt die Platzierung eines Gallengangsstents über die ERC, nur in Ausnahmefällen ist eine PTCD erforderlich. Plastikstents okkludieren schneller als Metallstents und müssen etwa alle 3 Monate gewechselt werden. Liegt eine Duodenalobstruktion mit einer Stenosesymptomatik vor, ist ein operativer Bypass in Form einer Gastrojejunostomie ggf. mit biliodigestiver Anastomose indiziert. In einzelnen Fällen kann die Tumorstenose durch Einlage eines Duodenalstents überbrückt werden. Ziel dieser Maßnahmen ist die Gewährleistung einer möglichst hohen Lebensqualität für den Patienten. Ein Abwägen von endoskopischer und chirurgischer Palliation muss für den Patienten individuell entschieden werden.

BACKGROUND:In the diagnosis and treatment of biliary disorders, establishing percutaneous transhepatic biliary drainage (PTBD) is an invasive procedure that can potentially lead to infectious complications in both the short and long-term. We therefore prospectively analysed the time course and spectrum of biliary bacteria in patients undergoing PTBD.METHODS:Forty-nine patients (19 F, 30 M; mean age 64 years) with malignant (65%) or benign (35%) biliary disorders were included, 20 of whom had a newly established PTBD (group A), while the remaining 29 had already had their PTBD in situ (group B) for a mean of 8 months. Bacteriological analyses of bile and blood were carried out, and clinical symptoms and laboratory values were obtained.RESULTS:Biliary bacteria were found in 60% of cases during the initial PTBD placement, and 24 h later this rate had already increased to 85%; two or more microorganisms were found in 40% initially and in 70% after a few days. At later PTBD exchanges, bacteriobilia was found in 100%, with all patients harbouring multiple organisms. Whereas the initial spectrum was mixed, Escherichia coli and enterococci (97% each), Klebsiella (73%) and Bacteroides species (37%) later predominated; Candida increased initially from 15% to 80%, but later decreased to 30%. Clinical signs of cholangitis were observed in 30% initially (no sepsis), but decreased to 6% at later exchanges.CONCLUSIONS:Bacteriobilia is initially a frequent, and later a regular, event in PTBD; however, clinically significant complications are rare during the long-term course and limited to the initial, more invasive, phase of PTBD. A knowledge of the bacterial spectrum is important for selecting appropriate antibiotic coverage if complications arise and/or major interventions such as surgery are planned.
Percutaneous transhepatic biliary drainage (PTBD) is an established means of treating biliary disease after endoscopic retrograde cholangiopancreatography (ERCP) has failed [1]. Arterial bleeding from a PTBD tract is very uncommon as a late complication [2] [3] - in contrast to the early phase after PTBD placement, during which this may rarely occur [4] [5] [6]. We present here the case of a 74-year-old man with a 2-year history of malignant biliary stricture (Bismuth IV) who received treatment with a unilateral 16-Fr Yamakawa-type prosthesis to drain the right lobe of the liver. More than a year after the creation of the tract and after several uneventful exchanges of the Yamakawa-type prosthesis, the patient presented prior to schedule because of fever and an inability to flush the prosthesis. He also reported melena. Laboratory investigations showed an alkaline phosphatase level of 1291 U/l (normal range 60-180), γ-glutamyltransferase 394 U/l (normal range < 28), bilirubin 13.7 mg/dl (normal range < 1.2), and C-reactive protein 7.9 mg/dl (normal range < 0.5); leukocytes were normal.
Tumor masses in the area between the esophagus and the tracheobronchial tree can lead to complications involving both systems, mainly strictures and compressions. Malignant esophageal strictures are nowadays often treated by insertion of a metal stent which, however, can cause airway compression especially in the proximal area. We present here a new method of creating a Y-stent out of two self-expandable tracheal nitinol stents, utilizing fiberbronchoscopy, in a 55-year-old woman with advanced colon cancer metastastic to the mediastinum. The endo-Y-stent technique can be performed with the patient under sedation and having topical anesthesia. The opening through which the second tracheal stent must be placed for the Y construction is created by laser. In this case, the patient suffered from airway compression which was efficiently relieved by this method. Within a short time the endo-Y-stent provides effective restoration and maintenance of airway patency in patients with tumor compression in the region of the esophagus and airway, and in those with airway compression following esophageal stenting. Expertise in both stent implantation and laser application is, however, mandatory.
