Between September and November 1988, six children (aged 4-17 months) from a parish in Upper Bavaria fell ill with a haemolytic-uraemic syndrome. The illness had been preceded by a gastroenteritis with at times haemorrhagic stools. All patients needed peritoneal dialysis for acute renal failure. A 12-month-old girl was left with statomotoric developmental impairment due to cerebral involvement, but the other five children were cured. In the first four children to be admitted stool examinations for the common enteritis-causing microorganisms had been negative. But in the following two, specific gene probes demonstrated enterohaemorrhagic E. coli O157: H- with formation of verotoxin 2 (Shiga-like toxin II). This group of microorganisms must be taken into account in the diagnosis of haemorrhagic enterocolitis, because they can cause severe complications.
Eine Herzbeteiligung ist bei der fazioskapulohumeralen Muskeldystrophie (FSHD) im Gegensatz zu anderen Muskeldystrophien nicht näher beschrieben. Wir berichten über eine 71-jährige Frau mit progredienter Herzinsuffizienz bei FSHD, die bei einer der beiden ebenfalls an dieser Erkrankung leidenden Töchter molekulargenetisch nachgewiesen worden ist. Die Verstorbene zeigte autoptisch neben Veränderungen an der quergestreiften Muskulatur, die auch fokale entzündliche Infiltrate insbesondere an der Zwerchfellmuskulatur einschließen, eine Beteiligung der Herzmuskulatur. Histologisch gleicht das Bild am Herzen dem einer primären Kardiomyopathie, jedoch ohne Erhöhung der Herzmasse. Sowohl klinisch als auch morphologisch ist der Tod dieser Patientin Folge der Herzerkrankung
Cardiac involvement is well known in a number of skeletomuscular diseases but not in facio-scapulohumeral muscular dystrophy (FSHD). We report on a 71 year old woman with progressive cardiac insufficiency in FSHD, which was also confirmed by molecular analysis in one of the two daughters affected by the disease. Autopsy of the deceased patient showed the typical changes in skeletal muscles including focal inflammatory infiltrates in the diaphragm and, in addition, cardiac muscular involvement. The histological changes resembled those seen in primary cardiomyopathy despite the normal muscle mass volume. Both clinically and morphologically, the cardiac disease was the cause of death in this patient with FSHD.
Desmoids are tumours of the connective tissue cells with aggressive growth. Frequent recurrences of these tumours have often been described. Genetic and endocrine factors as well as local tissue biomechanics are thought to be responsible for the pathogenesis. This case report should contribute to the discussion about possible pathogenetic factors.
Neuropathologische Untersuchungen sollen dazu beitragen,die Ursachen der zentralnervösen Komplikationen nachHerzoperationen aufzudecken und die Rate der postoperativenneurologisch-kognitiven Störungen zu reduzieren. Unter diesemAspekt haben wir die Gehirne von 262 nach herzchirurgischenOperationen (Bypass-, Klappenoperationen undHerztransplantationen) Verstorbenen untersucht. Es finden sichZirkulationsstörungen (Makro- und Mikroblutungen, Infarkte,Subarachnoidalblutungen, hypoxämische Hirnschäden) in 128 Fällen(49%), davon kommen 33 Fälle als Todesursache in Betracht(12,6%). Als Ursachen der Infarkte sind neben einerstenosierenden Arteriosklerose von Hirnarterien undThromboembolien (Operationsgebiet, Myokardinfarkte) in seltenenFällen Fettembolien (2), Fremdkörperembolien (1) undMikroembolien durch Megakaryozyten zu nennen. Weiterhin bestehenentzündliche Veränderungen in 17 Fällen, davon meistSeptikopyämien (12 Fälle) durch Pilze oder Bakterien, und in 5Fällen ließen sich Gliaknötchen als mögliches Substrat einerviralen oder Autoimmunencephalitis (Bickerstaff) nachweisen. AlsNebenbefund fand sich noch ein Morbus Alzheimer (37 Fälle, 14%des Materials) bei älteren Patienten, zum Teil mit einerAmyloidangiopatie, jedoch nicht als Todesursache und nicht alsUrsache größerer Hirnblutungen. Da die Arbeit eineAutopsiestudie darstellt, ist eine Übertragung der gefundenenVeränderungen auf die Gesamtgruppe der operierten Herzpatientenmit neurologisch-kognitiven Störungen nur bedingt möglich ist.Auf jeden Fall spielen im eigenen Material umfangreichemikroembolische Ereignisse (Fettembolie, Fremdkörperembolie),wie sie oft in der Literatur angegeben werden, keineentscheidende Rolle. Es finden sich aber, in unterschiedlicherHäufigkeit in den einzelnen untersuchten Operationsgruppen,besonders nach Klappenoperationen, Mikroblutungen der weißenSubstanz, die nicht zum Tode führen. Sie könnten, ebenso wie diegelegentlich auftretenden Gliaknötchen, nach Resorption undVernarbung postoperativ bei den Patienten neurologisch-kognitive Störungen verursachen.
Neuropathological studies may contribute to the discovery of central nervous system complications after heart surgery and thus help to reduce the incidence of postoperative neurological or cognitive disturbances. We examined the brains of 262 such patients operated for coronary bypass, valve replacement, or heart transplantation. Circulatory disturbances (macro- and microhemorrhages, infarcts, subarachnoid hemorrhages, and hypoxemic brain damage) were present in 128 cases (49%), as the cause of death in 33 cases (12.6%). The infarcts were caused by local arteriosclerosis of brain arteries, arterial emboli originating from the operative sites or myocardial infarctions, or by fat emboli, foreign body emboli or megakaryocytic capillary emboli in rare cases. Inflammatory disturbances were present in 17 cases and consisted of fungal or bacterial septicopyemic changes (12) or of glial nodules (5) as the substrate of a viral or autoimmunencephalitis (Bickerstaff). An incidental finding was Alzheimer's disease in 37 cases (14% of the material) of elderly patients, often associated with cerebral amyloid angiopathy but not as cause of death or cause of macroscopic brain hemorrhage. Since we have conducted an autopsy study, there is a limitation to transfer the documented changes to the total group of post-cardiac surgery patients with neurologic and cognitive deficits. Contrary to some previous reports, histologically overt microembolic phenomena do not seem to play a major role in our material. On the other hand, careful scrutiny revealed non-fatal white matter microhemorrhages of varying frequency in the different groups, especially after valve operations. These as well as the occasional glial nodules, after resorption and microscarring, could well be the cause of slight neurologic and cognitive impairments.
Zusammenfassung Ziel : In einzelnen Veröffentlichungen wird ein Zusammenhang zwischen der Einnahme sehr hoher Enzymdosen und einer fibrosierenden Kolonopathie vermutet. Ziel dieser sonografischen Studie war es daher, auffällige Darmwandbefunde bei CF-Patienten festzustellen und einen möglichen Zusammenhang mit der Enzymsubstitution und klinischen Symptomatik zu überprüfen. Methode und Patienten : Bei 51 CF-Patienten erfolgte eine sonografische Messung der Darmwanddicke im terminalen Ileum und Colon ascendens. Ein Darmwandreflex >2 mm wurde als verdickt definiert. Ergebnisse : Bei 17,6% der Patienten fand sich eine Darmwandverdickung im Ileum, bei 35,3% im Colon ascendens. Patienten mit einer Enzymsubstitutionsdosis bis 6000 E/kg und Tag wiesen häufiger Darmwandverdickungen auf als diejenigen mit Dosen zwischen 6000 und 15.000 E/kg und Tag, so daß sich eine negative Korrelation zwischen der Höhe der Enzymsubstitutionsdosis und der Wanddicke ergab ( p <0,05). Schlußfolgerung : Sonografisch lassen sich bei CF-Patienten Darmwandverdickungen im Ileozäkalbereich nachweisen, wobei diese in einem Enzymdosisbereich bis 15.000 E/kg und Tag negativ korrelieren. In diesem Dosisbereich scheint kein erhöhtes Risiko für eine fibrosierende Kolonopathie zu bestehen.
Zusammenfassung Hintergrund: Die ultraschallgesteuerte hydrostatische Reposition einer intestinalen Invagination ermöglicht im Unterschied zu anderen Therapiekonzepten eine Reposition ohne Strahlenbelastung. Gezeigt werden sollte, daß dieses Verfahren gleichwertige Erfolge bietet. Methode: Alle Invaginationen, diagnostiziert an der Kinderklinik der TU München von 1989–1999, wurden retrospektiv analysiert und durch Zusammenstellung der wichtigsten Veröffentlichungen die verschiedenen Therapiekonzepte verglichen. Ergebnisse: Von 158 Invaginationen (Alter der Patienten: 2–130 Monate) lösten sich 10 spontan, 9 mußten primär operiert werden. 87,1% aller Repositionsversuche führten zum Erfolg, bei Berücksichtigung nur der ileokolischen Invaginationen sogar 95,1%. Die Erfolgsrate betrug bei Rezidivinvaginationen 79% bei einer Häufigkeit von 9,5%. Komplikationen traten selten auf: Letalität 0%, Perforationsrate 0,7%. Diese Resultate entsprechen den Ergebnissen der ultraschallgesteuerten hydrostatischen Reposition in der Literatur. Schlußfolgerung: Die ultraschallkontrollierte hydrostatische Reposition bietet neben der höheren Erfolgsrate und der niedrigeren Rate an Komplikationen im Vergleich zu allen anderen Therapiekonzepten weitere Vorteile. Daher sollte dieses Verfahren bevorzugt werden, zumal mit der Farbdopplersonografie eine neue Möglichkeit der nichtinvasiven Durchblutungskontrolle als prognostischer Parameter besteht.
Zusammenfassung Fragestellung: Es soll geklärt werden, inwieweit ein präoperatives EKG bei Kindern für die Risikoabschätzung vor einer Narkose notwendig ist. Methode: Wir überprüften die Narkoseprotokolle von 3030 Kindern, bei denen im Jahr 1990 in unserer Klinik eine Narkose durchgeführt wurde, und stellten die Komplikationen bei diesen Narkosen den Ergebnissen der präoperativen EKGs gegenüber. Außerdem werteten wir Fragebögen zu diesem Thema aus, die wir 1991 an 51 große Kinderkliniken in Deutschland verschickt haben. Ergebnisse: Bei 3030 Narkosen traten 93 Komplikationen auf, von denen 67 nicht kardial bedingt waren. Von den 26 kardialen Komplikationen waren nur 4 behandlungsbedürftig (AV-Block 2°, Blutdruckabfall, Schock). Von 180 präoperativ geschriebenen EKG waren 152 normal; 8 zeigten harmlose Normvarianten, 16 wurden wegen bekannter Herzfehler durchgeführt. Keines dieser Kinder hatte Komplikationen bei der Narkose. Die Fragebögen wurden von 36 der 51 angeschriebenen Kinderkliniken sorgfältig ausgefüllt zurückgeschickt. In den meisten Kliniken wird nur bei Hinweisen auf eine mögliche oder bestehende Herzerkrankung ein präoperatives EKG geschrieben. Schlußfolgerung: Es gibt bei Kindern keine Indikation für ein präoperatives EKG als Routinemaßnahme. Nur wenn sich aus Anamnese und körperlicher Untersuchung Hinweise auf eine Herzerkrankung ergeben, ist ein EKG und – bei Bedarf auch ein Echokardiogramm – eine sinnvolle Untersuchung.
Objective:The aim of this study is to examine whether a routine preoperative ECG is necessary in children to assess the risks in anaesthesia and surgery.Methods: We examined the records of 3030 children anaesthetized in our hospital in 1990 and compared records of complications with preoperative ECGs. We also evaluated questionnaires on this subject which we had sent to 51 large pediatric hospitals in Germany in 1991.Results: During 3030 anaesthetic procedures there were 93 complications, 67 of which were not of cardiac origin. Of the 26 cardiac complications only 4 needed therapy (AV-Block 2 degrees, fall in blood pressure). Preoperative ECGs were recorded in 180 cases: 152 were normal,8 showed harmless deviations from normal and 16 were recorded because of known cardiac defects. None of the 180 children developed complications during anaesthesia. Our questionnaire was returned completed carefully by 36 of the 51 hospitals. ln most hospitals a preoperative ECG is only recorded when heart disease is known or suspected.Conclusion: Routine preoperative ECGs are unnecessary in children unless there are clinical symptoms of heart disease or heart disease is suspected.
Oxycardiorespirographies, recording arterial oxygen saturation (SaO2), breathing movements, heart rate and ECG with a mean recording time of 22.3 hours, were performed on 85 preterm (mean postconceptional age: 38 weeks) and 81 term infants (mean postconceptional age 42.4 weeks). 83% of the preterm infants showed periodic breathing (PB), in 97% of them this was accompanied by periodic variations of arterial oxygen saturation (PVO). Periodic breathing occurred in 61% of the term infants, 84% of them showed PVO during periodic breathing. The mean variation of oxygen saturation was between 92.8 and 96.8% (+/- 1.7) for preterm and between 92.9 and 96.0% (+/- 2.2) for term infants. In some infants the peak to peak amplitude of the SaO2 cycles was up to 22%, sometimes a further fall of SaO2 occurred. There was a strong correlation of the PVO both at the beginning and end of the episode as well as with the PB-cycle periodicity itself. The fall of the oxygen saturation occurred 3.1 to 7.8 s after the beginning of the first apnea of an episode of periodic breathing, the minimum SaO2 was reached approximately 4.2 to 8.6 s later. This periodic rapid fall of SaO2 from a high oxygenation level cannot be explained by the apneas of a rather short duration during periodic breathing. It is discussed that PVO during periodic breathing may be caused by an ideopathic right to left shunting across fetal circulation pathways which occurs intermittently and periodically. This mechanism could-via patterns of reaction exhibited during the fetal and neonatal time period-lead to acute hypoxemia, as found in apparently life threatening events (ALTE) and as postulated in sudden infant death (SID).
The type II collagenopathies represent a group of chondrodysplasias sharing clinical and radiological manifestations which are expressed as a continuous spectrum of phenotypes, ranging from perinatally lethal to very mild conditions. Their common molecular bases are mutations in the type II collagen gene (COL2A1). We describe one case of lethal platyspondylic dysplasia, Torrance type, and a variant of lethal Kniest dysplasia, neither of which has been reported as a type II collagenopathy. Biochemical studies of cartilage collagens and morphological analysis of cartilage sections suggest that abnormalities of type II collagen structure and biosynthesis are the main pathogenetic factors in both cases. Thus, the phenotypic spectrum of type II collagenopathies might be greater than hitherto suspected.
Objective: To examine to what extent the condition of chronic malnutrition of anorexia nervosa leads to changes of the heart.Methods: Using 1- and 2-dimensional echocardiography we collected parameters to determine the size and function of the heart, and the thickness of the myocardium of 81 patients with anorexia nervosa. Thereover we investigated the occurence of mitral valve prolapse and pericardial effusion. In a cross-sectional sample of hospitalised patients we put the measured heart parameters in relation to body weight. Subsequently we conducted a longitudinal study of 44 of these 81 patients to find out how these parameters changed with increasing body weight.Results: In the cross-sectional sample we found that the parameters always remained within normals, provided normals were related to body weight. This also implied to patients with extreme underweight. A mitral valve prolapse was detected in 6 of 81 patients (7,5 %), and a pericardial effusion in 16 of 81 patients (20 %).In the longitudinal study the cardiac parameters increased with increasing body weight, but to a lesser degree than weight. There were no changes for the mitral valve prolapse whereas the number and size of pericardial effusion clearly decreased.Conclusion: Chronic malnutrition in anorexia nervosa leads to significant changes of the heart. These are, however, less significant than weight loss. The mitral valve prolapse ist not dependent on underweight. With increasing body weight most of the cardiac parameters tend to normalize albeit at slower pace than body weight. None of these changes were dangerous for the patients even for those with extreme underweight. Therefore a specific treatment was never found to be necessary.
Two siblings and their mother developed afebrile generalized lymphadenopathy. The lymph nodes were movable and painless. During the course of the illness, the mother and one child developed an uncharacteristic rash. Increased titers of human herpes virus type 6 (HHV-6) antibodies were found in all three family members and in an unrelated patient with lymphadenitis colli. The enlarged lymph nodes decreased in size within several weeks. We speculate these symptoms to be caused by an infection with this lymphotropic virus.

At the beginning of the illness convulsions with preference to the right side were noticed, which were consistent with an encephalitis (on top to a suspected pre/perinatal lesion) and resulting in spastic triplegia. Nuclear magnetic resonance imaging and cranial computertomographic results showed severe, predominantly left-sided cerebral lesions. In addition there was clinical and biochemical evidence of an associated hepatitis. Human herpesvirus-6 has been identified as the cause of exanthema subitum. In addition, the virus is known to cause other clinical entities (lymphadenopathy, febril seizures, hepatitis, postinfectious chronic fatigue a.o.) and has been identified in brain tissues. Our observations show that the course of exanthema subitum can be complicated by affection of the liver and central nervous system. At present it is impossible to estimate the clinical outcome in our patient.
