Les traumatismes du lit de l’ongle chez l’enfant sont fréquents, principalement par écrasement. La suture du lit unguéal est classiquement effectuée par des points de fil résorbable. L’objectif était de comparer les résultats à moyen terme de l’apparition de dystrophie unguéale après suture du lit unguéal de façon classique ou en utilisant la colle 2-octylcyanoacrylate (Dermabond®, Ethicon) chez les enfants pris en charge pour traumatisme digital par écrasement. Cette étude rétrospective monocentrique incluait les enfants de moins de 15 ans opérés d’une suture du lit de l’ongle entre 2012 et 2017 avec un recul minimal de 6 mois. La durée opératoire et le type de suture étaient colligés. Au dernier recul étaient recherchés — une dystrophie unguéale, une hypoesthésie pulpaire, des dysesthésies, une douleur (spontanée, à la palpation, au chaud, au froid). L’aspect esthétique final était noté par les parents de 0 à 10. Nous avons évalué 46 sutures chez 42 enfants d’âge moyen 4,2 ans (1–11) lors de la chirurgie (13 perdus de vus), avec un recul moyen de 17 mois (6–69). Vingt-deux ont été suturés à la colle biologique, 24 par suture traditionnelle. Il n’y avait pas de différence statistiquement significative entre les 2 groupes concernant l’apparition de dystrophie majeure (3 vs 0, p = 0,10), de dysesthésies pulpaires (3 vs 2, p = 0,66), d’hypoesthésie pulpaire ou de douleurs (aucune pour les 2 groupes). L’aspect final esthétique moyen était noté par les parents à 8,7, sans différence significative entre les 2 groupes (8,4 vs 9, p = 0,06). La durée opératoire était significativement plus longue dans le groupe suture traditionnelle (10,2 min vs 20,3 min, p < 0,001). Les dystrophies retrouvées dans le groupe colle correspondaient à une hypertrophie de l’hyponychium chez 2 enfants, dont une était en relation probable avec une onychophagie. Nos résultats confirment ceux de la littérature chez l’adulte et dans certaines séries de faible effectif chez l’enfant. La suture au Dermabond® présente les avantages d’un gain de temps opératoire, et de soins postopératoires facilités par l’absence de fils. La suture à la colle chirurgicale 2-octylcyanoacrylate (Dermabond®) d’une plaie du lit de l’ongle après traumatisme par écrasement est une alternative possible à une suture classique, tant sur le plan esthétique que fonctionnel.
Osteochondritis dissecans of the patella (OCDP) is rare in adolescents. The objective of this study was to evaluate functional results of OCDP management in adolescents with mosaicplasty and to analyze proper integration of osteochondral grafts via MRI.
Multiple Hereditary Exostosis (MHE) is an automosal dominant skeletal disorder that begins in childhood. Birth prevalence is estimated at 1/50,000. Three distinct genetic loci are known to cause MHE: one on chromosome 8, and two others on chromosomes 19 and 11. Mutations of the germ line in the EXT tumour suppressor gene family have been identified in MHE. The loci on chromosomes 8 and 11 have been associated with malignant transformation. Bony exostoses are benign osteocartilaginous growths that start close to growth plates, inducing metaphyseal remodeling, bony deformities, and asymmetric long bone growth. Frequent orthopaedic manifestations include short stature, limb-length discrepancy, femoroacetabular impingement, coxa valga with hip dysplasia, genu valgum, ankle valgus, and forearm deformities. The authors focused this study on points which do not make the unanimity in the literature: do surgical procedures alter or not the natural history of exostoses especially for forearm and ankle deformities, frequency and preventive attitude for spinal stenosis, risk of malignant change? Forearm deformities: Characteristic forearm deformities are present in 30-60% of patients with hereditary multiple exostoses. Characteristic forearm deformities include a shortened forearm bowing, an ulnar club hand, and sometimes a radial heas subluxation or dislocation. Patients often suffer for aesthetic reasons. There is no consensus regarding optimal therapy. Some authors recommend "aggressive" early operative surgery to prevent or reduce progression of deformity and functional impairment (or both). Correction of deformity and improvement of function are not necessarily associated. However, it emerges from recent studies that these surgical procedures, judged in a subjective and objective way, do not produce any predictable functional improvement. These studies showed that the gain obtained by surgery did not reach improvement especially for pronosupination but provide only cosmetic satisfaction. Deformations, from natural evolution of the MHE or from residual defects of surgical correction, are well tolerated in the adulthood. Ulna lengthening is currently very contrversial. Ankle deformities: They occur in about 50% of affected individuals. Relative shorthening of the fibula and obliquity of the distal tibial epiphysis result in valgus deformities of the ankle. Surgical procedures for correction include excision of exostosis, lengthening of the fibula, tibial osteotomy, and hemiepiphyseal stapling of the distal tibial physis. Prophylactic surgical procedures in children that are designed to improve alignment in the ankle may be justified because of the high rate of osteoarthritis in individuals with MHE. Spinal stenosis: Recent reports noted a high rate of intracanal exostoses in MHE patients (68%) with medullary compression in 27%. So, systematic spinal screening with clinical examination, magnetic resonance imaging or computed tomography is warranted in these children at the age of 10 to avoid a possible neurological complication. If surgeon decides not to remove an intracanal lesion, he must very regularly control the child with MRI. However, it seems logical to remove any exostose in contact with nervous elements, as the neurological recovery is always favorable and the risk of recurrence very low. Malignant changes: Information and education are essential. Malignant change in children is exceptional. Any modification of size or the appearance of a pain in the adulthood on a known exostose has to bring the patient to consult. Percentage of malignant change is probably about 2%. 80% of tumors develop on the axial skeleton. If some exostoses are clinically easy to detect, others as axial skeleton and pelvis are more difficult to detect. Bone scan and MRI can help in this supervision. Strong uptake at Technetium bone scan is highly suspect of malignant change. It does not seem possible to identify a risk group. The annual risk of malignant transformation being 0,1% between 30 and 50 years, it will be logical to propose a screnning for patients with MHE. Surgical prophylactic resection of the exostoses is not recommended. The rate of complications after exostose resection is about 13%.
La dysplasie épiphysaire hémimélique (DEH) est une pathologie rare touchant l’enfant et le jeune adolescent. Elle correspond à des excroissances épiphysaires multiples unilatérales (1 à 4) et dans la majorité des cas localisées sur un seul hémicorps. Les auteurs rapportent leur expérience de neuf cas de DEH ayant bénéficié d’une résection précoce et complète des lésions. Ils présentent les manifestations cliniques, discutent la valeur de l’imagerie par résonance magnétique (IRM) et de l’intérêt d’une résection précoce de ces lésions. Cette série rétrospective comporte neuf patients. L’âge moyen au diagnostic variait de 1 à 12 ans. Tous les patients présentaient une tuméfaction augmentant progressivement de volume. Une déformation angulaire a été retrouvée dans deux cas. Tous les patients ont été opérés (huit fois par une excision isolée) puis suivis cliniquement et radiologiquement. La durée moyenne de suivi a été de 5,6 ans (2–10,5 ans). Tous les patients ont eu une évolution favorable au plus long recul. Il n’a pas été noté de pont d’épiphysiodèse, de déformation angulaire ou de récidive. Un patient avec une atteinte distale médiale de l’épiphyse fémorale a développé quatre mois après la chirurgie une calcification en dehors de la zone atteinte initialement par la tumeur. La lésion était non évolutive à deux ans de recul. Un patient avec une lésion épiphysaire du tibia proximal a présenté, en postopératoire, une paralysie régressive du nerf fibulaire commun avec une récupération en trois mois. L’IRM est utile pour trouver un éventuel plan de clivage entre l’épiphyse et la tumeur. Les auteurs conseillent, chaque fois que ce plan de clivage est visualisé, de faire une résection précoce de ces ossifications. Attendre une éventuelle complication ou une augmentation de taille de la tumeur ne leur semble pas logique. Bien entendu, cette résection ne sera pas possible si aucun plan de clivage n’est isolé sur l’IRM. Niveau IV, étude clinique rétrospective.
Le traitement des luxations congénitales de hanche irréductibles (LCHI) à la naissance, souvent classées à tort tératologiques est très controversé. L’option de les abandonner pour une réduction chirurgicale à un âge plus avancé est la plus admise.
Background: Dysplasia epiphysealis hemimelica (DEH) is a rare developmental bone disorder with hemimelic involvement of one or more epiphysis. We report on nine new cases and discuss the clinical manifestations, the value of MRI, and the results of complete and early surgical resection of these lesions.Materials and methods: In this retrospective study, nine patients with a diagnosis of DEH were evaluated. Age at presentation ranged from 1 year to 12 years. The main complaint at diagnosis was a swelling bony mass. Angular deformities were recorded in two patients. All patients were surgically treated and followed up clinically and by imaging. Eight patients underwent excision only.Results: The average follow-up was 5.6 years (range, 2-10.5 years). All patients had a good outcome without related symptoms. No epiphysiodesis, angular deformity or recurrence was observed. One patient with femoral lesion involving the distal medial part of the epiphysis developed, four months after surgical excision, a calcification outside the area of total excision. This calcification did not increase in size at two years follow-up. Another patient with lateral involvement of the proximal tibial epiphysis presented a postoperative nervous complication. Spontaneous nervous recovery occurred three months after surgery.Discussion: MRI was useful to find a potential plane of cleavage between the epiphysis and the pathological tissue. We recommend early removing ossifications when a cleavage plane is identified. Waiting a possible complication or increasing of size does not seem logical. La reconstruction diaphysaire après résection d’une tumeur maligne demeure un challenge difficile. La technique de la membrane induite décrite par Masquelet, initialement indiquée dans les pertes de substance d’origine traumatique ou septique, commence à être appliquée dans cette indication. Nous rapportons trois cas de lyse massive de la greffe après reconstruction fémorale, nécessitant une ou plusieurs reprises chirurgicales chez deux garçons de trois et six ans et une fille de neuf ans. Cette complication pourrait s’expliquer par le délai important entre les deux temps opératoires, le greffon, les contraintes varisantes propres à la localisation fémorale ou le défaut de stabilité de l’ostéosynthèse. L’application de cette technique dans les tumeurs osseuses a montré cependant des résultats préliminaires satisfaisants : c’est la localisation fémorale qui semble spécifiquement exposer à la lyse du greffon osseux. La recherche de l’origine de cette complication est fondamentale pour adapter la technique ou en modifier les indications.
Bone reconstruction after surgical resection of bone malignancies in children remains a difficult challenge. Induced-membrane reconstruction as described by Masquelet et al. was originally reported in traumatic or septic bone defects and is now adapted to this field. We report here three cases of massive femoral graft resorption requiring surgical revision in two boys aged 3 and 6 years and a 9-year-old girl. Hypotheses include the long delay between the two stages, nature of the bone graft, high varus loads specific to this location, and lack of stability of the fixation. This technique has recently provided promising preliminary results when applied to the field of bone tumours. However, reconstruction of the femur seems to be specifically associated with a risk of graft resorption. Identification of the origin of this major complication is needed to amend the technique or its indications.
Thirty-six cases of femoral lengthening using the Albizzia nail were performed. The indication for lengthening was a congenital malformation, sequellae of trauma, of infection, of radiation therapy, short stature, and vascular malformation. The mean age of the patients was 16 years, the average lengthening achieved was 4.7 cm, and the follow-up period averaged 5.8 years. We found that bone consolidation was achieved faster than with external fixation. The patient's comfort during lengthening as well as the speed of functional restoration also improved. In three cases, the program failed, in six the lengthening was achieved with a second procedure, and eight patients required one ratcheting or more under general anesthesia. In our experience, the Albizzia nail is a simple and effective solution for uncomplicated femoral lengthening.
Le kyste osseux essentiel (KOE) et le kyste osseux anévrysmal (KOA) sont des lésions osseuses lytiques, bénignes, rencontrées le plus souvent chez l'enfant et l'adolescent. Le KOE est une lésion, kystique, à contenu liquidien pouvant être unicamérale ou partiellement cloisonnée. Le KOA peut toucher tous les os de l'organisme, mais le plus souvent la métaphyse des os longs. Ses localisations préférentielles sont l'humérus proximal puis le fémur proximal. Le KOA est une lésion expansive et hémorragique qui dans sa forme habituelle présente une translocation caractéristique. Les KOA secondaires représentent 30 % des cas, correspondent à une lésion réactionnelle qui se développe sur une autre lésion osseuse généralement bénigne. Le KOA est une lésion métaphysaire excentrée, soufflante à contenu liquidien souvent multiloculée qui peut toucher tous les os de l'organisme. En IRM l'aspect de niveaux liquides est évocateur. Le diagnostic différentiel entre KOE et KOA est important car leur pronostic et leur traitement sont différents. L'évolution du KOE se fait spontanément vers la guérison entre l'adolescence et l'âge adulte. Le principal risque du KOE est celui de fracture. Son traitement peut se résumer à l'injection intrakystique de corticoïdes dans les formes non menaçantes. En cas de risque de fracture notamment au col fémoral, le traitement peut nécessiter un curetage et un comblement avec une greffe ou un substitut osseux associé à une ostéosynthèse. Le KOA est une lésion potentiellement plus agressive qui présente un risque de destruction osseuse. Le diagnostic doit toujours être confirmé par biopsie après IRM en repérant les zones charnues car un ostéosarcome télangiectasique peut prendre l'aspect d'un KOA. Le traitement peut se faire par sclérothérapie intrakystique à l'alcool. En cas de risque fracturaire, ou sur une localisation vertébrale, un traitement chirurgical éventuellement précédé d'une embolisation peut être nécessaire. Le risque de transformation maligne des kystes osseux est anecdotique.Simple bone cyst or unicameral bone cyst (UBC) and aneurysmal bone cyst (ABC) are lytic benign bone lesions, usually encountered in children and adolescent. UBC is a cystic lesion liquid filled, unicameral or partially separated. UBC can involve all skeleton but usually the long bone metaphysis and primarily proximal humerus and femur. The classic ABC is an expansive and haemorrhagic tumour with a caracteristic translocation. Secondary ABC represents 30% of ABC and have no translocation. It is developed in reaction to another lesion, usually benign. ABC aremetaphyseal, excentrated, bulging, containing liquid, and may develop in all bones of the skeleton. In MRI, the liquid level image is usual. It is mandatory to distinguish ABC from UBC, because their evolution and treatment are different. UBC spontaneously disappear between adolescence and adulthood. The main concern in UBC is the risk of pathologic fracture. UBC could be cured by cortico-steroid injections. When there is a risk of fracture, especially in the femoral neck, surgery with curettage, filling with bone substitute or graft and osteosynthesis are recommended. ABC are more aggressive lesions with a risk of bone destruction. The diagnosis must be assessed by a biopsy because a telangiectasic sarcoma can mimic an ABC. Sclerotherapy with alcohol is an effective treatment. In spinal ABC and in aggressive lesions with a risk of fracture, surgical treatment should be preferred, after a pre operative embolization. The risk for malignant transformation is very low, excepted in case of radiotherapy.
Purpose The active or aggressive character in certain localisations of aneurysmal bone cysts in children requires either curettage with a considerable recurrence rate or a radical segmental excision, raising complex reconstructive challenges. Cyst maturation with subsequent ossification may be observed either spontaneously or after incisional biopsy. Patients Five new cases of active aneurysmal bone cysts (ABCs) with healing of the cyst after biopsy alone are reported. All patients had no treatment of the cyst after the biopsy. Results In two cases, the lesion initially increases in size immediately after the biopsy, and it is only secondarily that the lesion decreases in size. Four out of five cases of the spontaneous healing occurred in pelvic bone. The cysts healed after, respectively, 36, 24, 12, 32 and 12 months. Conclusions The emergence of these new cases of spontaneous healing encourages promoting clinical and radiological supervision after biopsy in selected cases. Unfortunately, it is impossible to predict a possible aggressive behaviour in ABCs. Then, if the lesion is quickly aggressive with clinically and radiologically increasing size after biopsy, it would be illogical and dangerous to let this ABC evolve. It would be necessary to treat it without delay. On the other hand, if the lesion moderately increased after the biopsy, it is possible to wait and observe the patient during a period of 5 months for a possible healing, if the ABC localisation is not dangerous. Of course, if the lesion does not increase in size after biopsy, there is no delay to treat it.
Kingella kingae est un bacille Gram négatif appartenant à la famille des Neisseriaceae. Sa responsabilité dans les infections ostéoarticulaires est de description relativement récente.Patients et méthodes. - Nous rapportons une série de huit malades pris en charge au CHU de Rouen pour une infection ostéoarticulaire à K. kingae. Il s'agissait de six filles et deux garçons.Résultats. - L'âge moyen au moment du diagnostic était de 30,6 mois. Six enfants étaient âgés de moins de trois ans. L'atteinte était articulaire six fois et osseuse deux fois. Les deux tiers des patients étaient fébriles au moment du diagnostic. Le bilan biologique était peu perturbé. Tous les enfants ont eu des prélèvements à visée diagnostique. Les prélèvements ont été ensemencés directement au bloc opératoire sur flacon d'hémoculture. La culture positive à K. kingae était issue dans sept cas du prélèvement local et dans un cas d'une hémoculture. Tous les enfants, en attendant les résultats de l'enquête bactériologique, ont reçu une bi-antibiothérapie par voie intraveineuse. Le relais per os s'est toujours fait par de l'amoxicilline. La durée moyenne du traitement antibiotique était de 33 jours dont dix par voie intraveineuse. Six patients ont un recul supérieur à 18 mois. L'évolution s'est toujours faite vers la guérison.Discussion. - Le présence de K. kingae dans la flore nasopharyngée habituelle, en particulier les amygdales, permet sa diffusion hématogène à la faveur d'une altération de la muqueuse oropharyngée par diverses affections. Différents types d'infections à K. kingae ont été décrits. Les plus fréquentes sont les infections ostéoarticulaires, les endocardites et les atteintes oculaires.Conclusion. - Les infections ostéoarticulaires à K. kingae ne présentent aucune particularité par rapport aux infections à germes usuels. La sensibilité de K. kingae aux antibiotiques, notamment l'amoxicilline, permet le relais per os très rapide (après l'identification du germe et en l'absence d'autre localisation notamment endocardique) et ainsi la réduction de la durée du traitement antibiotique. K. kingae est un germe très fragile, l'examen direct est souvent négatif, la culture est difficile et l'identification requiert un observateur averti. L'utilisation de flacons d'hémocultures ensemencés directement au bloc opératoire permet une mise en culture rapide et favorise l'isolation de K. kingae par un effet de dilution d'éventuels inhibiteurs.Kingella kingae osteoarticular infections in children: eight new cases.Kingella kingae is a Gram-negative bacillus which belongs to the Neisseriaceae family. Its involvement is osteoarticular infections is relatively recent.Methods and results. - We report eight cases of Kingella kingae osteoarticular infections that have been diagnosed at the paediatric surgical centre of Rouen University Hospital since October 1995. Six boys and two girls (mean age: 30.6 months) presented with osteomyelitis in six cases and arthritis in two. Only 75% of patients had a fever at time of diagnosis. The biological findings were slightly modified. All samples were obtained from blood, bone or joint fluid. These samples were systematically inoculated into a blood culture tube. Positive Kingella kingae culture was achieved in seven local samples in one blood culture. All children received two antibiotics via intravenous injection while waiting for the bacteriologic results. Later, the antibiotic treatment (amoxicilline)_was given per os. The mean duration of treatment was 33 days. Patients were given intravenous treatment for a period of only ten days. Six patients were followed up for a period of more than 18 months and outcome was always uneventful.Discussion. - Kingella kingae is usually present in the nasopharyngeal mucosa and spreads in the blood due to various infections. Different types of Kingella kingae infection have been reported with a large frequency of osteoarticular infection.Conclusion. - This type of infection does not present any unusual characteristics as compared to other osteoarticular infections. Because of its antibiotic sensitivity treatment duration could be reduced. Kingella kingae is a fragile microbe and its culture is often difficult; therefore, it is important to use blood culture tubes to inoculate joint fluid and bone samples.
Les résultats à 20 ans de recul après ligamentoplastie du croisé antérieur sont rares. Cette étude a été réalisée afin d'étudier la prévalence de la gonarthrose à plus de 20 ans de recul d'une ligamentoplastie du croisé antérieur et de déterminer les facteurs de risque. Hypothèse : Les lésions méniscales, le niveau d'activité, le délai traumatisme–chirurgie, l'indice de masse corporelle, la laxité résiduelle, le positionnement des tunnels et les lésions cartilagineuses traumatiques influencent l'évolution vers la gonarthrose.Cette série rétrospective multicentrique regroupait 182 patients. Elle a été réalisée dans le cadre d'un symposium SOFCOT dont deux tiers des effectifs étaient des femmes. La technique chirurgicale était arthroscopique dans 82 % des cas et un transplant de type os–ligament patellaire–os était utilisé dans 92,8 % des cas. L'âge moyen à la chirurgie était de 26 + 7 ans. Les scores cliniques recueillis étaient l'IKDC objectif et subjectif, le KOOS. Les signes radiographiques d'arthrose étaient classés selon l'IKDC. Les facteurs prédictifs d'arthrose étudiés étaient l'âge, le sexe, l'IMC, le niveau d'activité, le délai traumatisme–chirurgie, les méniscectomies, les chondropathies, le positionnement des tunnels et la laxité résiduelle.Au dernier recul, pour l'IKDC objectif il y avait 48 % de genoux normaux (A), 35 % d'anormaux (B) et 17 % de pathologiques (C et D). Le score IKDC subjectif moyen était de 82,7 + 13,1. Une arthrose modérée à sévère était présente dans 29 % des cas. Les facteurs de risque de gonarthrose identifiés étaient : la méniscectomie médiale ou latérale, la laxité résiduelle, l'âge à la chirurgie > 30 ans et la pratique d'un sport comprenant du pivot. La méniscectomie était un facteur essentiel de survenue d'arthrose (17 % si les ménisques étaient préservés et 46 % en cas de méniscectomie). Enfin le taux de re-rupture était de 13 %.La reconstruction du LCA donne des résultats satisfaisants sur la stabilisation du genou mais le devenir cartilagineux dépend principalement du statut méniscal. La laxité séquellaire a également un rôle favorisant la gonarthrose.IV, étude rétrospective de cohorte.
Elbow instability is a common feature after medial epicondyle fractures, displaced or not, even in the absence of dislocation. Undisplaced or minimally displaced fractures often have an underestimated degree of instability secondary to unrecognised capsuloligamentous and muscular injuries. The purpose of this retrospective study was to analyze and to assess objectively the results of the surgical treatment of these acute injuries.
La scintigraphie osseuse peut-elle se substituer aux classifications radiographiques conventionnelles (Catterall et Herring) pour différencier les formes bénignes et sévère de LCP ? et traiter plus tôt les formes graves ? Le but de ce travail est d'étudier la corrélation entre la scintigraphie et les classifications radiographiques. Étude prospective à partir d'une série de 63 hanches pathologiques, le diagnostic est établi sur des critères cliniques, radiographiques et scintigraphiques. Parmi ces 59 enfants, 49 garçons et 10 filles, 32 enfants avant l'âge de 5 ans, 20 entre 4-9 ans et 7 après l'âge de 9 ans ; 6 enfants présentaient une forme bilatérale. La classification scintigraphique a été de Conway (stade A existence d'un pilier externe, stade B absence d'un pilier externe, stade C conversion d'un stade A initial vers un stade B après 4-6 mois d'évolution) et compléter par la classification radiographique de Herring et Catterall. Un suivi clinique régulier a été instauré avec consultation orthopédique, contrôle radiologique et scintigraphique tous le 4 mois pendant un an. Une évolution selon le stade A de Conway a été observée dans 31 cas. Parmi eux 29 ont été classé Herring A ou B et 22 cas Caterrall 2. L'évolution selon le stade B de Conway a été observée dans 29 cas. Parmi eux 5 cas ont été classé Herring B, 25 cas Herring C et 27 cas Caterrall 3 ou 4. Trois enfants ont évolué selon un stade C, ceux-ci ont été classés Herring B et Caterrall 4. Le pilier externe apparaît aux alentours du 5e mois d'évolution. La scintigraphie donne une information pronostique qui précède d'environ trois mois les classifications radiographiques (Catterall - Herring). Ces quelques mois gagnés sont précieux pour différencier les formes graves des bénignes et envisager si nécessaire un acte chirurgical, avant que la tête fémorale ne soit déjà déformée. La scintigraphie osseuse est un élément précieux de la conduite à tenir dans la maladie de LCP, elle apporte précocement une information pronostique de grande valeur, permettant d'orienter au plus tôt la prise en charge thérapeutique. We report what we believe to be the first case to be presented in the literature of a giant cell tumor (GCT) arising in the seventh cervical vertebral body in a girl of almost 10 years of age. The only similar-reported case occurred in the cervical spine of an adolescent. Cervical location of this tumour is rare: spinal cases usually arise in the lumbar or thoracic spine. The patient presented to the emergency department with the following neurological symptoms: reduction of tricipital reflex and lack of straight in the right upper limb. Radiographs revealed lysis of the C7 vertebral body with anterior subluxation of C7 on T1. Further investigations (MRI, CT scan, Total Whole Body Scintigraphy) showed neurovascular involvement (the vertebral artery and C7 and T1 nerve roots were incorporated within the tumoural mass) without secondary lesions. An open biopsy at C6-T1 level was performed; diagnosis was histological: mononucleated spindle cells and osteoclast-like multinucleated cells were identified. Treatment consisted of surgical resection, vertebral stabilisation and chemotherapy. Twenty-four months after surgical resection and chemotherapy, the girl is well without evidence of local or systemic recurrence. Radiation therapy was not performed to avoid the risk of secondary sarcomatous degeneration.
Between 1990 and 1998, 110 knee arthroscopies were performed in children. We analyzed the epidemiology and diagnostic data and studied the correlation between clinical and radiographic findings and the final diagnosis after arthroscopy in order to establish a consensus on use of knee arthroscopy in children.
PURPOSE OF THE STUDY:Between 1990 and 1998, 110 knee arthroscopies were performed in children. We analyzed the epidemiology and diagnostic data and studied the correlation between clinical and radiographic findings and the final diagnosis after arthroscopy in order to establish a consensus on use of knee arthroscopy in children.MATERIAL AND METHODS:We made a retrospective analysis of 110 knee arthroscopies performed in children, classing the patients in three age groups: 0-5 years, 5-10 years, 10-17 years. Clinical and radiological findings were compared with the arthroscopy findings.RESULTS:One or more arthroscopies were performed in 56 boys and 48 girls. Mean age at the time of the procedure was 12 years 4 months. There were 11 children aged 0-5 years, 14 aged 5-10 years and 85 aged 10-17 years. The main pathology was arthritis in the 0-5 year and 5-10 year age groups. Trauma was more frequent in the older children. Knee arthroscopy was found to be normal in 19 cases.DISCUSSION:For most surgery teams, arthroscopy is indicated for arthritis of the knee. Arthroscopy may also be needed for hemarthrosis. In these contexts, arthroscopy is both a diagnostic and therapeutic procedure. Our analysis demonstrates that emergency arthroscopy is only warranted for free floating osteochondral fractures and fractures of the tibial articular surfaces, with the exception of the tibial spines. Arthroscopy may be performed later in other cases after careful physical examination and radiographic series. We had 19 normal arthroscopies and 10 that showed femoropatellar chondropathies and plicas that could explain knee pain. We recommend arthrography before arthroscopy to avoid unnecessary procedures.CONCLUSION:Arthritis of the knee is an excellent indication for arthroscopy. Painful and acute hemarthrosis requires attentive physical exams and x-rays before making the decision for surgery.
