Introduction: The URC PARADICT-O initiated the COVIDOR study; clinical research project in collaboration with the infectious diseases department of the CHRO (Orléans Regional Hospital Centre), the CHU of Tours and the University of Paris Saclay. More than 25% of the territorial agents were concerned by psychoactive drug abuse (alcohol, tobacco, cannabis or others…). Materials and methods: The addiction treatment pro-gram was a Level 2 program structured into four functional units. We decided in 2022 to transform the addiction unit in a level 3 program with PARADICT-O support with the university hospital program. Results: We include 7696 patients in 2019 and we reached 16 394 patients in 2025. We doubled the number of addictive patients in 7 years. Conclusions: In June 2026, the new addiction university service (Pr R Serreau) could take care of the regional Val de Loire centre population with the collaboration of Tours CHU to reduce the addiction prevalence with a public health challenge.
Alcohol use disorder is a psychiatric condition characterized by excessive alcohol consumption. The drugs that are used to treat it often fail to prevent relapse. At the same time, psilocybin is increasingly being investigated for the treatment of various substance use disorder. This review aims to evaluate the results of the most recent clinical trials assessing psilocybin as a treatment for alcohol use disorder. According to these trials, psilocybin seems to reduce craving but its effect on overall alcohol consumption is less clear. There is no doubt that future trials would benefit from larger sample sizes and standardized tests.
Background: Perinatal exposure to pollutants could lead to side-effects in mothers and children, since this is a period of high vulnerability. Sparse and contradictory data are available on the impact of pollutants on breast milk. Objective: The present study consists of a scoping to review the recent research on the persistent presence of organic pollutants (POPs) and heavy metals in breast milk. Design: We performed a scoping review using two online databases to identify articles published from 1995 to 2023 on pollutants in breast milk, using the PRISMA Checklist. Results: A total of 54 articles were found relevant. POPs were present in breast milk in higher concentrations the longer the lactation period. In 3-year-old children, the presence of POPs correlated with the duration of breastfeeding. Estimated dietary exposure to Polychlorinated Biphenyls (PCBs) during pregnancy was positively associated with plasma concentrations in 3-year-old children, but not in pregnant women, suggesting transplacental transfer of these substances. Indeed, plasma concentrations were approximately 40% higher in children compared to pregnant women. In China, a clinical study assessed the risks of POPs in breastfed infants under 6 months of age. POPs decreased over time. Dichloro-Diphenyl-Trichloroethane (DDT) was present in infants living in coastal areas. Hexachlorobenzene (HCB) was the pollutant of greatest concern. Parity is a major determinant of POP levels in breast milk, as are heavy metals. Carcinogenic PAHs were found to a greater extent in obese mothers than in mothers with a normal body mass index. Discussion: POPs and heavy metals could be found in infant formula as well as maternal milk. furthers studies are needed to compare the concentrations between the two milks. Conclusions: Breastfeeding is the only food recommended by the WHO for optimal infant growth. It would seem advisable to pursue risk reduction policies to protect mothers and infants from contaminants in breast milk.

Our research addresses this gap by exploring the intersection of environmental health and lactation, situated within the broader ‘One Health’ concept, thus contributing a novel perspective to the existing body of knowledge. This scoping review aims to examine recent research on the persistent presence of organic pollutants (POPs) and heavy metals in breast milk, thereby elucidating the environmental setting’s impact on milk quality. We seek to highlight the innovative angle of our study by emphasizing the ‘One Health’ concept, which has not been thoroughly explored in the context of lactation and environmental pollutants. We performed a scoping review, consulting two online databases to identify articles published from 1995 to 2023 that reported on pollutants in breast milk, using the PRISMA checklist. This methodological approach underlines the comprehensive and up-to-date nature of our literature review, ensuring the relevance and timeliness of our findings. From a total of 54 relevant articles, findings indicate that POPs are present in higher concentrations in breast milk the longer the lactation period. These findings highlight the persistent and bioaccumulative nature of such contaminants, offering new insights into their long-term implications for maternal and infant health. This exposure does not appear time-sensitive, suggesting pollutants accumulated in maternal fat compartments can be excreted into human milk years after exposure, a novel finding that underscores the importance of considering long-term environmental exposures in lactation research. The presence of POPs and heavy metals in both infant formula and maternal milk underscores a critical need for further comparative studies to understand the health implications better. Our discussion extends the current dialogue on the safety of breastfeeding in polluted environments, providing a new framework for assessing risks and benefits. While breastfeeding remains the WHO-recommended nutrition for optimal infant growth, the findings emphasize the importance of continued risk reduction policies to protect mothers and infants from environmental contaminants in breast milk. Our conclusion calls for an integrated approach, combining public health, environmental science, and clinical practice to develop effective strategies for reducing exposure to environmental pollutants. This multidisciplinary perspective is a significant contribution to the field, paving the way for future research and policy development.
In this review, in addition to the potential cardiovascular applications of β-blockers and, more specifically, propranolol, we wanted to list the more recent applications in psychiatry as well as current knowledge on the impact of oxidative stress on propranolol hydrochloride and the oxidative stress that could be limited by the latter. In fact, a number of studies show that this molecule is modified by oxidative stress but is also able to limit it. Mention is also made to studies on the increasingly important problem of eliminating drug waste and its impact on the environment, particularly the marine environment. Given the increase in the consumption of medicines, more rigorous waste management is needed to avoid impacting biodiversity.
Objective To evaluate the SARS-CoV-2 seroprevalence among local authority workers, depending on their position and potential interaction with the public.Methods A cohort of volunteer participants was recruited among local authority workers of the Centre Val de Loire region in France, to be tested using a rapid serological test (COVID-PRESTO). The collected data were analysed by comparing different parameters including, gender, age, position held, and contact or not with the public. The study was carried out from August to December 2020 and included 3228 participants (n=3228), from 18 to 65 years old.Results The seroprevalence of SARS-CoV-2 among local authority workers was estimated at 3.04%. No significant difference could be observed according to the position held by the workers and whether they were or not in contact with the public. Nevertheless, a significant difference was observed between the different investigating centres, in correlation with the geographical location.Conclusion Contact with members of the public was not a critical parameter for SARS-CoV-2 seroprevalence as long as protective measures are applied. Among the population included in the study, childcare workers were more at risk of getting infected by the virus.Trial registration number NCT04387968
For the last two years, the SARS-CoV-2 virus spread all around the world and led to the COVID-19 pandemic. The need of methods to control the pandemic and to propose rapid and efficient diagnostic tools has emerged. In this perspective, SARS-CoV-2 rapid antigen detection tests (RADT) have been developed. We performed a retrospective study on 638 collected nasopharyngeal samples used for reference RT-qPCR diagnosis to compare the AQ + COVID-19 Ag Rapid Test" from InTec (AQ + InTec test) performance with other commercially available RADT (Abbott Panbio, Roche SDBiosensor and Siemens Clinitest). We analysed the sensitivity and specificity of the different tests and showed a better overall performance of the AQ + InTec test, which was confirmed on the SARS-Cov-2 Omicron variant. We then conducted a prospective study on 844patients, to evaluate the sensitivity and specificity of the AQ + InTec test on nasal and nasopharyngeal samples in a point of care setting. We showed that sensitivity and specificity reach acceptable criteria (respectively 94.4% and 99.6% on nasal samples) regarding the official recommendations of the MDCG 2021-21 in both symptomatic and asymptomatic patients. Overall, the results of these two studies confirm that the AQ + InTec test is a valuable tool for testing in a pandemic context with a high proportion of asymptomatic patients who are potential carriers for the SARS-CoV-2 virus and is performant on the most current circulating variant Omicron.
Here we have chosen to highlight the main natural molecules extracted from Camellia sinensis, Andrographis paniculata, and Curcuma longa that may possess antioxidant activities of interest for skin protection. The molecules involved in the antioxidant process are, respectively, catechins derivatives, in particular, EGCG, andrographolide, and its derivatives, as well as various curcuminoids. These plants are generally used as beverages for Camellia sinensis (tea tree), as dietary supplements, or as spices. The molecules they contain are known for their diverse therapeutic activities, including anti-inflammatory, antimicrobial, anti-cancer, antidiabetic, and dermatological treatment. Their common antioxidant activities and therapeutic applications are widely documented, but their use in cosmetics is more recent. We will see that the use of pharmacomodulated derivatives, the addition of co-antioxidants, and the use of various formulations enable better skin penetration and greater ingredient stability. In this review, we will endeavor to compile the cosmetic uses of these natural molecules of interest and the various structural modulations reported with the aim of improving their bioavailability as well as establishing their different mechanisms of action.
Psychedelics are used for various pathologies of the central nervous system and are currently the subject of much research, some of which relates to the compounds contained in various Psilocybe-type hallucinogenic mushrooms. It is difficult, however, to obtain and purify sufficient quantities of these compounds from fungi to carry out biological studies, hence the need to develop simple and efficient synthetic routes. We review here the various syntheses used to obtain these molecules, focusing first on the classic historical syntheses, then the use of more recent metallo-catalyzed couplings and finally the known biocatalytic methods for obtaining these molecules. Other access routes are certainly possible and should be the subject of future research given the therapeutic interest of these compounds.
This scoping review comprehensively explores the existing literature on fenugreek (Trigonella foenum-graecum), a versatile herb known for its culinary and traditional medicinal uses. The review focuses on two key aspects: the hypoglycemic mechanism of fenugreek and its impact on milk production in breastfeeding women. By systematically analyzing and synthesizing the available evidence, this review aims to enhance our understanding of fenugreek's potential applications and identify research gaps for future systematic reviews. The search strategy targeted English-language animal studies published within the last five years that specifically investigated fenugreek's hypoglycemic mechanism and its effects on milk production. A total of 15,236 articles were initially identified from databases such as PubMed, Sci finder and ScienceDirect. After a rigorous selection process, including thorough evaluation of titles, abstracts, and full-texts, a subset of articles (including those titled "Impact of Fenugreek on Milk Production in Rodent Models of Lactation Challenge" and "Fenugreek Stimulates the Expression of Genes Involved in Milk Synthesis and Milk Flow through Modulation of Insulin/GH/IGF-1 Axis and Oxytocin Secretion") were included for detailed analysis. The findings 18 articles from the included studies demonstrate promising results regarding fenugreek's potential to stimulate milk production and regulate blood glucose levels. However, further research is necessary to unravel the underlying molecular pathways, establish optimal dosage guidelines, and assess long-term side effects. Overall, this scoping review offers valuable insights for lactating women seeking to enhance milk production and individuals managing blood sugar levels, providing potential alternative approaches for supporting lactation and glycemic control.
Background: The COVID-19 pandemic has had a significant impact on organizations, health care professionals and of course patient care. The ability of health care facilities and their personnel to deliver safe, quality care was tested under particularly stressful conditions over the past two years. Health care workers represent a vulnerable population, especially during periods of high demand such as a major pandemic. This may in turn be associated with an increased risk of alcohol and substance use, abuse and disorders. There exists a correlation between psychotropic drug use and ensuing job difficulties increasing professional and emotional exhaustion among health care professionals.

Considering that, we evaluated an innovative two‐step self‐test, the AAZ COVID‐VIRO ALL IN®, switching from the classic nasal swab to a nasal sponge. We performed a multicenter study, on 124 adults and children, in a point‐of‐care setting. Sensitivity, specificity and overall acceptance of the COVID‐VIRO ALL IN® self‐test compared to reverse transcriptase polymerase chain reaction (RT‐PCR) on nasopharyngeal samples were of 93.0%, 100%, and 97.5%, respectively. We then performed a multicenter, usability study to evaluate the ease of use of COVID‐VIRO ALL IN® on 68 laypersons adults. A vast majority of participants correctly executed and interpreted the test. The usability was then specifically investigated on 40 children and teenagers, comparing COVID‐VIRO® first generation to the new COVID‐VIRO ALL IN®. They all found COVID‐VIRO ALL IN® more comfortable and easier to use. For young children, the new self‐test seems safer (less risk of trauma and no liquid exposure), and faster than saliva‐based RT‐PCR. Moreover, the COVID‐VIRO ALL IN® can easily be adapted as a multiplex self‐test for other respiratory viruses, opening new perspectives of simultaneous, rapid and massive detection of respiratory infections, especially among vulnerable populations like children and elderly people.

With that in mind, we evaluated a new innovative easy two-step self-test named COVID-VIRO ALL IN developed by AAZ that uses a sampling nasal sponge instead of a classic nasal swab. Mirroring the previous study conducted on the first generation of COVID-VIRO antigenic self-test, we first performed a multicentre, prospective study on 124 adults and children, in a point-of-care setting. Sensitivity, specificity and overall acceptance of the COVID-VIRO ALL IN antigen self-test compared to RT-PCR on nasopharyngeal samples were evaluated at 93.02%, 100% and 97,5%, respectively. We then performed a multicentre, prospective, usability study to evaluate the ease of use of COVID-VIRO ALL IN in real life on 68 laypersons, all adults. Globally, 99% of them considered the instructions material good, 98% executed the procedure well, and all of them interpreted the results correctly. The usability was then specifically investigated on 40 children and teenagers participants, comparing both COVID-VIRO first generation and the new COVID-VIRO ALL IN. All of them found COVID-VIRO ALL IN much easier to use and much more comfortable. For young children, the COVID-VIRO ALL IN self-test appears to be safer (less risk of trauma compare to nasal swabs and no liquid exposure) easier to use than classic COVID self-tests and giving immediate result which is not the case for RT-PCR done on saliva samples (currently done in routine for kids in French schools). It could be an adapted tool for future mass screening campaigns in schools or at home under adult supervision for kids from the age of 3.
To control the spread of the coronavirus disease 2019 (COVID-19) epidemics, it is necessary to have easy-to-use, reliable diagnostic tests available. The nasopharyngeal sampling method being often uncomfortable, nasal sampling could prove to be a viable alternative to the reference sampling method. We performed a multicentre, prospective validation study of the COVID-VIRO (R) test, using a nasal swab sampling method, in a point-of-care setting. In addition, we performed a multicentre, prospective, and usability study to validate the use of the rapid antigen nasal diagnostic test by laypersons. In March 2021, 239 asymptomatic and symptomatic patients were included in the validation study. Compared with reverse-transcription polymerase chain reaction on nasopharyngeal samples, the sensitivity and specificity of the COVID-VIRO (R) Antigen test combined with a nasal sampling method were evaluated as 96.88% and 100%, respectively. A total of 101 individuals were included in the usability study. Among these, 99% of the participants rated the instructions material as good, 98% of the subjects executed the test procedure well, and 98% of the participants were able to correctly interpret the test results. This study validates the relevance of COVID-VIRO (R) as a diagnostic tool from nasal specimens as well as its usability in the general population. COVID-VIRO (R) diagnostic performances and ease of use make it suitable for widespread utilization.
Orléans Métropole vient de se construire depuis 2017, elle mutualise 22 communes soit une aire urbaine de 400 000 habitants. Une stratégie universitaire est lancée afin de développer une offre de santé via les maisons de santé pluridisciplinaires (MSP) afin de couvrir le territoire en professionnels de santé dans un département qualifié de désertique dans ce domaine. Le centre 15 du SAMU, l’ARS du centre Val de Loire, le conseil de l’ordre du Loiret et la mairie d’Orléans sont parties prenantes avec l’université d’Orléans pour relever ce défi. Nous présentons un retour d’expérience depuis le 1er décembre 2013 afin de prendre en charge la santé de 6200 agents de la fonction publique territoriale. Maintien du délai réglementaire des visites médicales pour les agents en surveillance médicale renforcée à 24 mois avec triplement du nombre d’agent à prendre en charge depuis la mutualisation. Création d’une unité de recherche clinique « PARADICT-O » avec 17 professionnels de santé, un seul médecin, temps plein enseignant chercheur, ouverture de postes de technicien de recherche clinique dans un service de médecine pluridisciplinaire de santé publique, médecine du travail et santé environnementale, lutte contre les troubles du sommeil, première pathologie responsable des RPS. Le rapport coûts/bénéfices du projet : investissement de 5000 à 10 000 € par an pour des retombées de plusieurs dizaines de milliers d’euros. L’axe stratégique métropolitain principal du projet s’inscrit dans la recherche scientifique et la qualité de vie. L’axe stratégique métropolitain secondaire auquel s’inscrit le projet « PARADICT-O » est la solidarité ainsi que la soutenabilité du développement communal. L’impact du projet sur l’organisation interne de la collectivité est fort : il a permis de diminuer de 2 points le turn-over et l’absentéisme des agents passant de 9,1 à 7,05 %. À propos du service rendu, il s’agit d’une offre se substituant à une ancienne solution de médecine préventive et professionnelle.
Pour faire face à la pénurie de médecin dans le département du Loiret, le service de prévention de la mairie d’Orléans s’est organisé en créant une équipe pluridisciplinaire au 1er décembre 2013 avec les structures de secours et d’urgence du département du Loiret en suivant une procédure opératoire standard afin d’offrir une prise en charge des agents territoriaux 24 h/24, 7 jours sur 7365 jours par an, lors d’un appel à secours. Auparavant, il n’y avait qu’un seul médecin du travail assisté d’une infirmière, ils ne pouvaient faire face aux sollicitations des 3300 agents de la collectivité territoriale. Il existait de nombreux secouristes du travail, mais ils ne bénéficiaient d’aucune action coordonnée jusqu’alors. Pour faire face à la demande croissante des agents territoriaux, l’équipe pluridisciplinaire s’est enrichie de 2 psychologues cliniciens, d’une sophrologue–hypnothérapeute et de 3 IDEST. Une procédure opératoire standard a été créé avec le conseil de l’ordre des médecins, l’ARS centre Val-de Loire et le SAMU 45 afin d’articuler les actions des secouristes (SST) avec les médecins urgentistes. Le médecin de prévention, chef de service a rencontré l’ensemble des acteurs sur une période de 3 mois pour coordonner cette procédure, la valider avec eux et la mettre en pratique. Au total, 835 sauveteurs secouristes du travail sont formés au sein d’Orléans métropole et recyclés tous les deux années conformément aux recommandations de l’INRS. Depuis le 3 octobre 2014, cette procédure d’alerte à secours est en vigueur, à la fois sur Orléans mairie et elle a évolué en s’étendant aux autres communes de la métropole orléanaise (4e version). Les 835 SST se rencontrent annuellement afin d’échanger sur les difficultés rencontrées dans la pratique des urgences. Un audit a été réalisé en 2016 et a mis en évidence un taux de satisfaction à 93 %. La création d’une équipe pluridisciplinaire en médecine préventive avec l’articulation du plateau technique des urgentistes a permis de répondre au besoin croissant formulé par les agents en particulier pour la prise en charge des RPS, 1re pathologie suivie sur Orléans métropole dans un délai de moins de 3 minutes pour les urgences vitales. Face au nombre croissant de tentatives de suicide en milieu professionnel, cette procédure a permis une optimisation de la prise en charge avec un délai de réactivité raccourci. Devant les défis de la mutualisation des collectivités territoriales, cette action coercitive, coût efficace basée sur le socle de la nouvelle équipe pluridisciplinaire a été évaluée positivement et a permis de poursuivre cette action dans le respect de la législation en vigueur. To reform the unit of preventive medicine in Orleans metropole, the Director of the Administration decided to co-sign an agreement to optimize the organization of a common preventive medicine service: we made a deliberation model to permit for others towns which wanted to join us. The cost evaluated to reach the service was the cost for medical examination, regardless of the professional which included staff costs and operating costs (social benefits, costs of supplies and materials, training costs). A statement of the tasks performed for the beneficiary community was kept daily by the SMP. A strong will of the Metropolis and municipalities is to develop a synergy around health and safety at work. A joint Metropolis of Orleans, an organization chart since 1/2/2016 integrates the preventive medicine service within a Quality of Life at Work service: a multi-disciplinary service composed of prevention specialists, social workers, responsible for social action and SMP, a Department of Preventive Medicine was developed and itself multidisciplinary in accordance with the law of July 20, 2011: including doctors, nurses, psychologists, sophrologist, and a dietician. A new and active network of health is now operant with specialist correspondents. An audit is programmed in April 2018 to assess the efficacy of this network.
Le lait maternel est unique sur le plan biologique et nutritionnel. Sa composition évolue constamment pour répondre au mieux aux besoins de l’enfant jusqu’à 6 mois. La culture du biberon est en train de laisser progressivement place à celle de l’allaitement maternel comme en témoignent les dernières données épidémiologiques françaises. Si le taux d’initiation de l’allaitement maternel (AM) atteint en effet 76 à 86 % selon les régions, seuls 22 % des nourrissons restent allaités exclusivement à 6 mois. Dans les sociétés industrialisées, les jeunes mamans ont rarement vu d’autres mamans allaiter avant de le faire elles-mêmes. Avec les sorties précoces de maternité, beaucoup de femmes se trouvent désemparées face à d’inévitables petites difficultés. Les centres ressources (protection maternelle et infantile [PMI], pédiatres, généralistes, consultantes en lactation, association de soutien, etc.) vont alors avoir un rôle à part entière mais ne doivent pas diffuser des conseils inappropriés et/ou contradictoires. Combien d’interruptions d’allaitement pour une poussée de fièvre, une prescription médicamenteuse, un mamelon douloureux ou pire un renoncement à l’AM après une naissance prématurée ? C’est en se rapprochant au mieux des dernières connaissances sur la physiologie de la lactation qu’un allaitement maternel a le plus de chance de bien démarrer et de se prolonger sans douleur.
The objective of the work was to develop and validate methods for the total Hg and methylmercury (MeHg) in breast milk that could be further used to obtain first data on chemical contamination of French breast milk. For total Hg determination, the potential of two techniques, namely Advanced Mercury Analyzer (AMA) and ICP MS, was compared. For MeHg determination, ICP MS detection associated to a quantification by isotopic dilution was used and the potential of a preliminary separation by gas or liquid chromatography was evaluated and discussed. The optimization studies have shown that AMA for total Hg determination and HPLC ID ICP MS, after a preconcentration step by freeze-drying, for MeHg quantification were the most relevant methods to use for epidemiologic studies. The figures of merit for both methods were evaluated by means of accuracy profiles in terms of limits of quantification (1.82 and 1.35 mg Hg/kg dry weight, corresponding to 0.22 and 0.16 mu g Hg/kg wet basis for total Hg and MeHg, respectively), repeatability (2-11% and 3-8% for total Hg and MeHg respectively), intermediate precision reproducibility (4-12% and 4-8% for total Hg and MeHg respectively) and trueness bias (-0.1-9% and 4-0% for total Hg and MeHg respectively). The methods were then applied to 180 breast milk samples.
