AcutePRN are described in this review, two uncommon forms of treatable neuropathies. They associate a proximo-distal sensory-motor attack whose mode of appearance is acute or chronic. Both forms are dysimmune. Their treatments are based on immunomodulation or immunosuppression for chronic forms. The sequence of assessment and therapeutic methods are explained in light of recent consensus data. Treatments aim at better analgesia and early intervention through rehabilitation.
Measles, also known as the “first disease”, is a common childhood illness that can occur in adults as well. Sometimes leading to extremely serious complications. Highly contagious, this infectious disease is caused by the measles virus, a morbillivirus belonging to the family Paramyxoviridae. Typical symptoms of measles include fever, a strong feeling of general malaise and rash on large areas of the body. During infection, a high fever appears twice. Complications such as otitis and pneumonia are relatively common. The incubation period, which is the time between infection and onset of disease, usually lasts eight to ten days. Getting measles immunizes for life and protects against any new infection with the measles virus. There is a preventive vaccination, recommended in the Vaccination Plan. It is a reportable disease.
Depuis le déclenchement de la pandémie Covid-19 déclarée par l'Organisation Mondiale de la Santé le 11 mars 2019, le monde a adopté des mesures préventives visant à affaiblir la chaine de transmission interhumaine. Toutefois, des pics de contamination ont été observé le long des deux dernières années, le plus récent étant celui de la saison hivernale 2021 avec la nouvelle variante B.1.1.529 Omicron désignée préoccupante à cause de sa haute transmissibilité et son échappement au spectre d'anticorps neutralisants. Il s'agit d'une étude prospective descriptive sur une période de 03 mois de décembre 2021 au février 2022 ayant comme objectif de mettre le point sur l'émergence du nouveau variant Omicron L'ensemble des tests diagnostiques réalisés ont révélé 5442 cas positifs, dont 77% était par RT-PCR constitués de 96% de souches Sdrop positives dont 179 ont été hospitalisé correspondant à 5,36 %, toutefois le taux moyen d'hospitalisation toute souche confondue était de 5.81%. 57 % de l'ensemble des patients hospitalisés avait un statut non conforme au protocole de vaccination anti-COVID-19, ce même statut vaccinal s'estimait à 66% pour les patients admis en réanimation dont l'âge moyen était de 72 ans, alors que des facteurs de gravité ont été retrouvé chez 94% parmi eux. Le taux de létalité étant de 1,3%, un pic de décès a été enregistré en fin janvier 2022, avec un délai d'aggravation clinique et paraclinique entre 10-15 jours. Au terme de cette étude et en comparant avec les deux vagues précédente Alpha et Delta, la 3ème vague Omicron s'est distinguée en termes de temps par une durée plus courte de 8 semaines, un nombre de cas positif moins important, et un taux d'admission en réanimation n'excédant pas les 1,9 % par rapport à 3% durant la vague Delta. Tuberculosis is a chronic infectious disease that affects millions of people. The system most affected is the respiratory system. Nodular hepatic TB without involvement of other organs is not common. Its clinical manifestations are not specific. The differential diagnosis includes primary and metastatic malignancies of the liver. We report a case of nodular hepatic TB revealed by molecular biology in a 69-year-old diabetic patient without any sign of TB impregnation and highlighting the contribution of molecular biology in the diagnosis and rapid management of this rare entity.
Context: Morocco has adopted the protocol combining hydroxychloroquine or chloroquine with azithromycin to treat patients with Covid-19, mainly based on Chinese studies and those of the IHU Mediterranee team in Marseille. The objective of this study is to assess the effectiveness of this association in a Moroccan population. Methods: We conducted a prospective descriptive analytical study at the center of virology, infectious and tropical diseases to assess the efficacy of hydroxychloroquine associated with azithromycin for the treatment of patients with Covid-19. All patients hospitalized at the center between March 16, 2020 and May 16, 2020 were included in this study with a positive RT-PCR result for SARS-Cov2. Results: 125 patients were hospitalized during this period meeting the inclusion criteria. All patients benefited from the combination of hydroxychloroquine and azithromycin. We studied the clinical, biological and CT parameters that could be associated with early viral clearance with a negative RT-PCR on D6 and D8. The average age at diagnosis is 37.98 +/- 12.13 years. 121 patients (96.8%) are cured after 10 days of treatment. 32 patients (25.6%) had a viral clearance on D6 and D8. We did not find any factors significantly associated with early viral clearance. Conclusion: Our study is inconsistent with work published by other teams regarding early viral clearance in patients treated with the combination of hydroxychloroquine and azithromycin, although clinical healing has been achieved in the majority of patients.
Tick-borne relapsing fever is a zoonosis caused by spirochetes of the genus Borrelia, transmitted by hematophagous ticks. This life-threatening condition is still misdiagnosed. We report a case of tick-borne relapsing fever in a 54 year-old man in Morocco with a history of hiking, who was hospitalized for an isolated fever. On admission, the clinical examination showed no specific signs. The laboratory assessment showed inflammatory syndrome with disturbance of the liver function. The blood culture was sterile. Serology was negative for HIV and Lyme disease, as were all the rest of the paraclinical tests. The patient's course was marked by spontaneous resolution of the fever and then, seven days later, the onset of a new episode of fever. Malaria was suspected and a blood sample was sent to the parasitology department. The thick smear and blood smear stained with May-Grünwald Giemsa showed the presence of spirochetes. Tick-borne relapsing fever was diagnosed and the patient responded favorably to treatment by cycline drugs. This disease is poorly known and should be considered in cases of presence of intermittent fever without a specific clinical-laboratory picture, according to epidemiological context. The diagnosis is confirmed by the presence of spirochetes on thick smear and blood or cerebrospinal fluid smear stained with MGG.
La pneumonie chronique idiopathique à éosinophiles (maladie de Carrington), connue de longue date, s’inscrit dans le cadre des pneumopathies à éosinophiles sans cause déterminée. Le diagnostic repose sur des caractéristiques cliniques, des infiltrats pulmonaires associés à une éosinophilie circulante et/ou alvéolaire. Le poumon à éosinophiles est le résultat de la sécrétion par la cellule Th2 de l’interleukine-5 qui entraîne le recrutement et l’activation des éosinophiles. La thyroïdite d’Hashimoto, maladie auto-immune spécifique d’organe, est la conséquence d’une rupture de la tolérance centrale et périphérique du fait de facteurs génétiques et environnementaux. L’association entre les deux pathologies n’a jamais été décrite dans la littérature. Cet article fait le point sur la physiopathologie, le diagnostic, le traitement d’une association exceptionnelle entre une maladie rare, la maladie de Carrington et la thyroïdite d’Hashimoto.
La maladie de Castleman (MC) est une affection rare, caractérisée par l’existence d’une forme localisée et d’une forme multicentrique. L’évolution peut être émaillée par l’émergence de tumeurs malignes.
Infectious pulmonary complications, in particular in the context of endemic tuberculosis, were first described in patients with Crohn's disease treated with an anti-TNF alpha drug. Interstitial lung disease (ILD) is a rare, little known, often severe complication. We describe the case of a 42-year-old patient who was being treated with infliximab (an anti-TNF alpha drug) for severe Crohn's disease. The patient was hospitalized complaining of subacute dyspnea, fever, a dry cough and lower thoracic pain during the previous two years. Imaging showed bilateral interstitial pneumonitis. Microbiological samples were negative. In the absence of treatment with a known pneumotoxic drug or other causes, a diagnosis of diffuse ILD due to the anti- TNF alpha drug was suggested. Corticotherapy resulted in a spectacular remission. Based on our experience, we discuss the diagnostic and etiologic difficulties encountered in a patient with Crohn's disease who developed ILD while being treated with infliximab and the pathophysiological mechanisms and the therapeutic management in such cases. Interstitial pulmonary complications must be considered before prescribing an anti-TNF alpha drug for patients with Crohn's disease. (C) 2014 Elsevier Masson SAS. All rights reserved.
Carcinoid tumors of the fundus are rare neuroendocrine tumors. Most of them occur in patients with atrophic gastritis of autoimmune origin. They are relatively indolent and commonly originating from enterochromaffine cells in the gastrointestinal tract. Endoscopic or surgical resection is recommended. Among the physiopathological mechanisms, hypergastrinemia following achlorydia is predominant. These tumors are usually detected on histological examination from endoscopic biopsies and are known complication of pernicious anemia. Conversely, microcytic anemia and iron deficiency can be the initial symptoms.We report a case of a 55 years old man with iron deficiency anemia related to a pernicious anemia complicated by gastric carcinoid tumors.These tumors often latent, are most frequently developed from enterochromaffin-like cells in response to hypergastrinemia. Usually, evolution is benign and surgical or endoscopic resection recommended.
Les complications infectieuses sont rarement observées au cours de la sarcoïdose, seules quelques mini-séries ou cas isolés ont été décrites. Le traitement corticoïde et la fibrose pulmonaire ont contribué à l’apparition de ces infections, mais de façon inconstante. La lymphopénie, fréquente dans la sarcoïdose, n’est pas un facteur déterminant. La nocardiose pulmonaire, de plus en plus fréquente, est une infection opportuniste survenant principalement chez les patients immunodéprimés. Le tableau clinique, évoluant par poussées et rémissions, peut être aigu, subaigu ou chronique. Nous décrivons le cas d’une nocardiose pulmonaire, infection rare, chez une patiente suivie pour sarcoïdose avec fibrose pulmonaire sous corticothérapie. Lors du bilan d’une dyspnée fébrile, il est mis en évidence une pneumopathie à Nocardia spp. sur prélèvement endobronchique. L’évolution est favorable après six mois d’antibiothérapie par cotrimoxazole. La nocardiose pulmonaire est exceptionnelle au cours de la sarcoïdose et survient principalement au cours d’un traitement par corticoïdes, dans un contexte de lymphocytopénie T-CD4. La sarcoïdose en elle-même ne représente pas un facteur de risque infectieux.
Les tumeurs carcinoïdes du fundus sont des tumeurs neuroendocrines rares qui surviennent souvent au cours des gastrites atrophiques d’origine auto-immune. Les mécanismes physiopathologiques sont en faveur d’un rô1e prédominant de l’hypergastrinémie secondaire à l’achlorhydrie. Le diagnostic est souvent endoscopique. L’association à la maladie de Biermer, anémie macrocytaire, a été fréquemment rapportée dans la littérature. Une carence en fer avec anémie microcytaire peuvent, en revanche, apparaître de façon précoce et être un mode de révélation de la maladie.
INTRODUCTION:Tocilizumab is a humanized monoclonal antibody directed against interleukin-6 receptor and is beginning to be reported as effective in some cases of Still's disease refractory in adults (ASD). ASD is rare, heterogeneous, with unpredictable evolution. The distal destructive arthritis represents a possible complication.PATIENT:We report an unusual case of adult-onset Still's disease with severe distal interphalangeal destructive arthritis with refractory early and prolonged remission after the first tocilizumab infusion.CONCLUSION:Tocilizumab can be used in patients with refractory ASD after failure or intolerance of conventional treatments.
