Smith–Lemli–Opitz syndrome (SLOS) is an inherited metabolic disease in the cholesterol biosynthesis pathway which is characterised by accumulation of 7- and 8-dehydrocholesterol and by reduced cholesterol concentrations in all tissues and body fluids. With this study, we developed a new, rapid, robust and high-throughput tandem mass spectrometric method as routine application for the selective SLOS screening and therapy monitoring in serum and dried blood. After protein precipitation of 10 μL serum or 4.7 mm dried blood spot, the sum of 7- and 8-dehydrocholesterol (DHC) was analysed by rapid chromatography combined with tandem mass spectrometry. Method comparison with GC–MS was performed for 46 serum samples. A comparison between serum and corresponding dried blood spots for DHC and cholesterol was performed with 40 samples from SLOS patients. Concentrations of DHC and cholesterol were analysed in 2 dried blood samples from newborns with SLOS and 100 unaffected newborns. Intra- and inter-assay variabilities ranged between 3.7 and 17.7 % for serum and dried blood spots. Significant correlations between the new LC–MS/MS method and GC–MS were determined for DHC (r = 0.937, p < 0.001) and for cholesterol (r = 0.946, p < 0.001). Significant coefficients of correlation between serum and dried blood spot samples above 0.8 were calculated for both analytes. A cut-off value of 5.95 for the ratio of DHC/cholesterol (multiplied by 1000) was found to distinguish newborns diagnosed with SLOS from normal newborns in a retrospective analysis after 5 years. The developed method enables a rapid quantification of the sum parameter 7- and 8-DHC in newborns and SLOS patients under therapy in serum as well as dried blood spot samples.
Congenital hyperinsulinism (CHI), syn. nesidioblastosis, is the most frequent cause of persistent, recurrent hypoglycemia in infancy. One third of patients show a single circumscribed focus. Enucleation of the focus and the removal of all affected beta-cells with preservation of healthy tissue is the treatment of choice. The intra-pancreatic choledochus as well as the ductus pancreaticus major must remain intact. The diagnostic gold standard is 18F-DOPA-PET/CT. Intraoperative sonography is carried out to correctly visualize the focus preoperatively localized by PET/CT in situ during the operation. The enucleation of the focus was carried out 3-20 days after PET/CT in 5 patients at an age of 3.5-14 months. Intraoperative ultrasound was carried out with high-capacity devices of different manufacturers under use of broadband probes (9 - 14 MHz). The localization by intraoperative ultrasound was accurate in all 5 patients with focal CHI, with regard to the intraoperative localization as previously described by PET/CT and histology. D. choledochus and D. pancreaticus major were separated intraoperatively by ultrasound. 3 of 5 patients were cured by complete enucleation of the focus. Nevertheless, the entire intraoperative identification of the segmented focus is still problematic. Characteristic sonographic features of a CHI focus are: hypoechogenicity, variable homogeneous and inhomogenous texture, blurred, irregular limitation without capsule, filiform, lobular processes, and insular dispersal into the surrounding tissue. Intraoperative high-resolution sonography helps the pediatric surgeon to determine size, configuration and topography of a CHI focus.
Einleitung: CHI führt wegen inadäquater Insulinsekretion zu schweren Hypoglykämien, Risiko: irreversible Hirnschäden. Histologie: diffuse/fokale Form. In >95% der fokalen Fälle sind Mutationen im ABCC8/KCNJ11 nachweisbar. Fokusentfernung führt zur Heilung. Lokalisation präoperativ durch L-DOPA-PET/CT. Prä- und intraoperative Sonografie unterstützt die gewebeschonende Fokusenukleation.
Einleitung: CHI ist eine der häufigsten Ursachen für persistierende, rezidivierende Hypoglykämien im Neugeborenenalter, die durch die unregulierte Insulinsekretion ein hohes Risiko neurologischer Komplikationen birgt. Histologisch wird zwischen diffuser/fokaler Form unterschieden. Zur Differenzierung ist die 18F-Dopa-PET-CT der bildgebende Goldstandard. MRT, CT allein und auch US waren bisher ineffektiv. Die fokale Form ist operativ durch Enukleation heilbar, die diffuse nur konservativ behandelbar.
Problemstellung: Der CHI (1:50.000) führt infolge inadäquater Insulinsekretion zu schweren Hypoglykämien bereits bei Neugeborenen. Etwa 2/3 der Säuglinge haben histologisch eine alle Pankreasregionen betroffene diffuse Form, 1/3 einen umschriebenen Herd (fokale Form). Mittels PET-CT (diagnostischer Goldstandard) wird derselbe lokalisiert und durch Fokusentfernung geheilt. Bisher waren Sonografie und MRT diagnostisch ineffizient. Es ist zu prüfen, ob durch prä- und intraoperative Mikrosonografie gemessen an der Histologie dennoch eine dem PET-CT vergleichbare Lokalisation eines Fokus möglich ist.
Der congenitale Hyperinsulinismus (CHI) führt meist bereits im Neugeborenenalter infolge inadäquat hoher Insulinsekretion zu schweren Hypoglykämien. Bei etwa einem Drittel der Betroffenen liegt ursächlich ein kleiner Fokus (<5mm Durchmesser) vor, der durch L-DOPA-PET/CT lokalisiert und durch eine begrenzte Pankreasoperation geheilt werden kann. Bei diffuser Form des CHI wird eine konservative Therapie mittels Diazoxid bzw. Octreotid und adäquater Diät eingeleitet. Um irreversiblen Hirnschäden vorzubeugen, ist im Einzelfall eine subtotale Pankreatektomie indiziert. Mutationen in ABCC8, KCNJ11, Glukokinase, GLUD und UCP2 wurden ursächlich für die inadäquate Insulinsekretion nachgewiesen. Hypoglykämieepisoden im Kindesalter werden unter konservativer Therapie seltener, insbesondere bei milderen Verläufen. Leibowitz et al. berichteten bei >50% der Kinder nach subtotaler Pankreatektomie nach zuvor jahrelanger Euglykämie über eine Diabetesmanifestation (JCEM 80: 386–92, 1995). Um die Entwicklung des Diabetes bei CHI zu untersuchen, analysierten wir die prospektive Langzeitdokumentation aus dem Diabetes-Patienten-Verlaufsdaten (DPV) Register. Zum Zeitpunkt der Datenanalyse (non-interventional audit) waren aus 20 Betreuungszentren im DPV-Register 25 Kinder und Jugendliche (12männlich) mit Diabetes bei CHI im mittleren Alter von 14,9 Jahren (4,67–20,9) erfasst. Im Vergleich wurden 50 645 Patienten (52% männlich) mit Diabetes mellitus Typ 1 (T1DM) im Alter bis 20 Jahre aus 269 Betreuungszentren aus Deutschland und Österreich dokumentiert. Zwischen den Gruppen mit Diabetes mellitus bei CHI und bei T1DM wurden signifikante Unterschiede (nicht parametrischer Kruskal-Wallis-Test) für folgende Parameter nachgewiesen: Alter bei Diabetesmanifestation im Mittel 7,3 (0–17,7) vs. 9,8 Jahre, Diabetesdauer zum Zeitpunkt der Analyse 7,6 (0,92–17,4) vs. 5,5 Jahre, BMI-SDS 0,84 (-1,05/3,5) vs. 0,49 SDS, Körperhöhen-SDS -0,62 (-2,2/1,1) vs. -0,02 SDS. Die mittlere tägliche Insulindosis liegt bei 0,7 (0,1/1,2) vs. 0,79 U/kg Körpergewicht, der HbA1c 7,6 (5,4/12) vs. 7,9%.
Auf Basis von sechs PET/CT-Datensätzen des Pankreas wird eine Computerunterstützung für die Teilresektion der Bauchspeicheldr üse (Pankreas) bei fokalem Hyperinsulinismus von Kindern entwickelt. Ziel ist es, die Lokalisation des krankhaften Fokus im Pankreasgewebe präoperativ dreidimensional zu visualisieren, um so die Sicherheit des Eingriffs zu erhöhen. Die relevanten anatomischen Strukturen werden im CT segmentiert und anschließend dreidimensional visualisiert. Der im PET erkennbare Fokus wird in diese anatomische 3D-Visualisierung eingeblendet. Es zeigt sich eine klare Erkennbarkeit des Fokus in allen sechs Fällen.
