Background and Aims Patients with liver cirrhosis (LC) and acute-on-chronic liver failure (ACLF) show distinct changes regarding the cellular structure of the liver and the immune system. Cell-cell communication and interactions of different liver cell subtypes and immune cells play an important role during disease progression and complications (e.g. infections). Within the last years, the role of extracellular vesicles (EVs) as communicators between cells is raising more and more attention. We therefore aimed to characterize phenotype and function of EVs derived from the blood of LC and ACLF patients.
Nonalcoholic fatty liver disease (NAFLD) is quickly becoming the most common liver disease worldwide. Within the NAFLD spectrum, patients with nonalcoholic steatohepatitis (NASH) are at the highest risk of developing cirrhosis and disease progression to hepatocellular carcinoma. To date, therapeutic options for NASH patients have been ineffective, and therefore, new options are urgently needed. Hence, a model system to develop new therapeutic interventions is needed. Here, we introduce two new in vitro models of steatosis induction in HepG2 cells and primary murine hepatocytes. We used a recently discovered novel class of bioactive anti-inflammatory lipids called branched fatty acid esters of hydroxyl fatty acids. Among these bioactive lipids, palmitic-acid-9-hydroxy-stearic-acid (9-PAHSA) is the most promising as a representative nondrug therapy based on dietary supplements or nutritional modifications. In this study, we show a therapeutic effect of 9-PAHSA on lipotoxicity in steatotic primary hepatocytes and HepG2 cells. This could be shown be increased viability and decreased steatosis. Furthermore, we could demonstrate a preventive effect in HepG2 cells. The outcome of 9-PAHSA administration is both preventative and therapeutically effective for hepatocytes with limited damage. In conclusion, bioactive lipids like 9-PAHSA offer new hope for prevention or treatment in patients with fatty liver and steatosis.
Einleitung Alkoholismus ist eine weltweit kontinuierlich zunehmende Ursache Leber-assoziierter Morbidität und Mortalität. Die alkoholische Hepatitis (AH) ist ein schwerwiegendes Krankheitsbild mit limitierten Therapieoptionen. Inflammation ist ein bekannter Treiber in der Pathogenese der AH. Lipoxin A4 (LXA4), ein 15-Lipoxygenase (Alox15) abhängiger Lipidmediator, ist involviert in den Mechanismus der Entzündungsauflösung und zeigte bereits vielversprechende präklinische Ergebnisse in der Therapie inflammatorischer Erkrankungen.
Einleitung Eine chinolon-basierte antibiotische Prophylaxe wird bei Patienten mit fortgeschrittener Leberzirrhose und hohem Risiko für eine spontan bakterielle Peritonitis (SPB) empfohlen. Bisher ist die Auswirkung einer Langzeitgabe von Antibiotika bei diesem Patientenkollektiv nur unzureichend untersucht.
Alcoholism is one of the leading and increasingly prevalent reasons of liver associated morbidity and mortality worldwide. Alcoholic hepatitis (AH) constitutes a severe disease with currently no satisfying treatment options. Lipoxin A4 (LXA4), a 15-lipoxygenase (ALOX15)-dependent lipid mediator involved in resolution of inflammation, showed promising pre-clinical results in the therapy of several inflammatory diseases. Since inflammation is a main driver of disease progression in alcoholic hepatitis, we investigated the impact of endogenous ALOX15-dependent lipid mediators and exogenously applied LXA4 on AH development. A mouse model for alcoholic steatohepatitis (NIAAA model) was tested in Alox12/15+/+ and Alox12/15-/- mice, with or without supplementation of LXA4. Absence of Alox12/15 aggravated parameters of liver disease, increased hepatic immune cell infiltration in AH and elevated systemic neutrophils as a marker for systemic inflammation. Interestingly, i.p. injections of LXA4 significantly lowered transaminase levels only in Alox12/15-/- mice and reduced hepatic immune cell infiltration as well as systemic inflammatory cytokine expression in both genotypes, even though steatosis progressed. Thus, while LXA4 injection attenuated selected parameters of disease progression in Alox12/15-/- mice, its beneficial impact on immunity was also apparent in Alox12/15+/+ mice. In conclusion, pro-resolving lipid mediators may be beneficial to reduce inflammation in alcoholic hepatitis.
Sphingosin-1-phosphat (S1P) ist ein Signalsphingolipid mit wichtigen Funktionen bei der Zytokinausschüttung, endothelialen Permeabilität und Immunantwort bei systemischen Inflammationen. Seine Rolle im akut-auf-chronischen-Leberversagen (ACLF) oder in einer akuten Dekompensation (AD) bei Patienten mit Leberzirrhose ist bisher wenig untersucht. AD und ACLF sind mit einer mittel- bzw. hochgradigen Entzündungsantwort assoziiert, die beim ACLF als ursächlich für die hohe Frühmortalität angesehen wird.
Prostanoide sind wichtige Regulatoren der Thrombozytenaggregation und involviert in arterielle thrombotische Erkrankungen. Ihre Rolle im portalvenösen Kompartiment bei dekompensierter Leberzirrhose ist nicht bekannt.
Das akut-auf-chronische Leberversagen (ACLV) ist ein hochrelevantes Krankheitsbild, an dessen Entstehung fehlregulierte hepatische und systemische Entzündungsreaktionen beteiligt sind. Klinisch zeigt sich eine akute schwerwiegende Verschlechterung der Leberfunktion mit begleitendem Versagen extrahepatischer Organe. Bisher sind keine zielgerichteten Therapieoptionen verfügbar.

The aim of the presented prosprective study was to investigate whether differences in serum Vitamin D levels in patients with liver cirrhosis have an impact on the risk of hepatic decompensation.

Hepatitis C virus (HCV) infection is associated with alterations in host lipid and insulin signaling cascades, which are partially explained by a dependence of the HCV life cycle on key molecules in these metabolic pathways. Yet, little is known on the role in the HCV life cycle of glycogen synthase kinase 3 (GSK3), one of the most important kinases in cellular metabolism. Therefore, the impact of GSK3 on the HCV life cycle was assessed in human hepatoma cell lines harboring subgenomic genotype 1b and 2a replicons or producing cell culture-derived HCV genotype 2a by exposure to synthetic GSK3 inhibitors, GSK3 gene silencing, overexpression of GSK3 constructs and immunofluorescence analyses. In addition, the role of GSK3 in hepatitis E virus (HEV) replication was investigated to assess virus specificity of the observed findings. We found that both inhibition of GSK3 function by synthetic inhibitors as well as silencing of GSK3β gene expression resulted in a decrease of HCV replication and infectious particle production, whereas silencing of the GSK3α isoform had no relevant effect on the HCV life cycle. Conversely, overexpression of GSK3β resulted in enhanced HCV replication. In contrast, GSK3β had no effect on replication of subgenomic HEV replicon. The pro-viral effect of GSK3β on HCV replication was mediated by supporting expression of microRNA-122 (miR-122), a micro-RNA which is mandatory for wild-type HCV replication, as GSK3 inhibitors suppressed miR-122 levels and as inhibitors of GSK3 had no antiviral effect on a miR-122-independent HCV mutant. In conclusion, we have identified GSK3β is a novel host factor supporting HCV replication by maintaining high levels of hepatic miR-122 expression.
IL-7 übt als Zytokin wichtige überlebensfördernde Signale vor allem auf Lymphozyten in allen Entwicklungsstadien aus. Dagegen ist die Bedeutung von IL-7 für das angeborene Immunsystem nur unzureichend verstanden. Die akute Dekompensation bei Leberzirrhose sowie das akut-auf-chronische-Leberversagen (ACLV) sind mit deregulierten Entzündungsreaktionen assoziiert, wobei die Rolle von IL-7 bisher nicht charakterisiert ist. Daher untersucht diese Arbeit den Einfluss von IL-7 auf die Funktion von Makrophagen und dessen Rolle bei Patienten mit Leberzirrhose und ACLV.
In Tiermodellen spielen Thrombozyten eine Rolle bei der Entwicklung der Leberfibrose. Beim Menschen ist jedoch wenig über den Nutzen von Thrombozytenaggregationshemmern bei der Prävention der Leberfibrose bekannt.
Das PNPLA3-Gen codiert für das Enzym Adiponutrin, das an der Hydrolyse von Triacylglyceriden beteiligt ist. Ein bekannter „Single-nucleotide-polymorphism“ (SNP) von PNPLA3, namentlich rs738409 C>G, führt zum Verlust der Hydrolaseaktivität. Während Assoziationen des PNPLA3-Risikoalleles mit dem Risiko einer Fettleber, einer Leberfibrose oder einem hepatozellulären Karzinom bekannt sind, liegen nur unzureichende Daten zu einer möglichen Assoziation zwischen der PNPLA3-Variante und Determinanten des metabolischen Syndroms vor. In der vorliegenden Studie untersuchten wir diesen Zusammenhang bei Hochrisiko-Patienten für kardiovaskuläre Erkrankungen.
Einleitung: Interleukin-7 (IL-7) ist ein nicht-redundanter Mediator des adaptiven Immunsystems, der bei der T-Zellhomöostase und damit bei der Bekämpfung viraler Infektionen eine zentrale Rolle spielt. Im Tiermodell wurde gezeigt, dass Hepatozyten zu einer Interferon-(IFN)-α-abhängigen lokalen Sekretion von IL-7 fähig sind. In der vorliegenden Arbeit untersuchten wir die Mechanismen und Konsequenzen einer hepatischen IL-7-Produktion im humanen System.
