For cancer treatment, diagnostics concerning tumor type and determination of molecular markers in short TAT is critical. The fully automated, real-time PCR-based molecular diagnostic Idylla assays are well established in many laboratories for qualitative detection, short TAT and routine screening of clinically relevant oncogenic mutations. According to the manufacturer, all IVD assays are recommended for use only with FFPE tissue samples of 5–10 µM dissections with at least 10% tumor content. In this study, we tested the performance and accuracy of the IVD assays along with the gene fusion assay (RUO) with different tissue/source materials like isolated DNA/RNA, cryomaterial, etc. The study also included testing archival FFPE tissue sections dating back from 20 years and a performance check for different pan-cancer samples individually. All the assays tested with FFPE sections and gDNA/RNA input showed above 96% accuracy and sensitivity, individually with 100% specificity. The Idylla assays also performed exceptionally well on the archival FFPE tissues, and the use of assays for other solid tumors was also remarkable. The performance test and accuracy of Idylla assays showed high efficiency with certain limitations. For the use of Idylla assays, both qualitative and quantitative applicability of different tumor source materials could produce efficient results in different diagnostic settings within a short TAT.

Juni2021 (16:00–18:00;Virtual Room3) im Rahmen der Virtuellen Pathologietage derDGP statt. Bedingtdurchdas engeZeitfenster der Sitzung konnten leider nicht alle für die AG-Sitzung angemeldeten Beiträge als Vortrag angenommen werden und so wurde ein Teil der Vorträge als ePoster mit Audiodatei vorgestellt, welches ebenfalls wie die Vorträge noch für 3 Monate on demand zur Verfügung gestellt wurden. Die Beiträge wurden auch in diesem Jahr virtuell rege diskutiert. Als Gastredner konnten wir in diesem Jahr Herrn Prof. Daniel Baumhoer, Leiter des Knochentumor-Referenzzentrums, Universitätsklinikum Basel, Schweiz, begrüßen. Herr Baumhoer stellte Aspekte der Knochentumoren aus der neuen und 5. AuflagederWHO-Klassifikation der Knochen-undWeichteiltumorenvor.DieseFassung der WHO erschien im Mai 2020 und man erhält deutlich mehr Informationen zu neuen molekularen Befunden als in der Vorgängerversion aus 2013. Herr Baumhoer gab einen Überblick über Neuerungen der Klassifikationen der Knochentumoren und setzte hierbei einen Schwerpunkt auf relevanten Änderungen in der täglichen Diagnostik, auch im Hinblick auf molekulare Aspekte dieser seltenen Tumoren. Anschließend stellte Herr Benedikt Leinauer aus der Ulmer Arbeitsgruppe von Prof. Barth seine Multicenterstudie zu H3.3 p.G34W-mutierten Riesenzelltumoren (GCTB) des Knochens ohne Riesenzellen als Vorstellung von 3 Fällen vor. GCTBs sind lokal aggressive und selten metastasierende ossäre Neoplasmen und treten zumeist um das Knie herum auf. Bei den vorgestellten Tumoren von 3 Patienten zwischen dem 23. und 33. Lebensjahr handelt es sich um einen sehr seltenen Subtyp dieser Tumorentität. Die molekulare Diagnostik ermöglicht heutzutage die eindeutige Einordnung dieser Tumore ohne Riesenzellen in die Gruppe der GCTB. Fraglich ist, ob bei fehlenden Riesenzellen eine heute in bestimmten Fällen eingeleitete Denusomab-Therapie überhaupt wirken könnte. Der Artikel zu dem Vortrag ist kurz vor der Tagung im Fachjournal Histopathology erschienen [1]. Der dritte Vortag zu einem Fallbericht eines Patienten mit 4 unterschiedlichen Malignomenassoziiertmiteinemgastrointestinalen Stromatumor (GIST) wurde von Frau Stephanie Ellen Weißinger, Institut für Pathologie, Göppingen, vorgestellt. Bei dem 70-jährigen Patienten wurden in den letzten 5 Jahren neben einem kolorektalen Adenokarzinom auch ein maligner peripherer Nervenscheidentumor im Sinne eines TRITON-Tumors und ein Harnblasenkarzinom sowie ein GIST diagnostiziert. In der Literaturwerdendurchausmetachrone Tumore bei GIST-Patienten in etwa 10% der Fälle beschrieben. Zwischenzeitlich konnte humangenetische eine Assoziation mit einem Lynch-Syndrom nachgewiesen werden (. Abb. 1). Benigne notochordale Tumoren (BNCT) und Chordome sind primäre intraossäre Tumoren, die entlang des Achsenskeletts entstehen und ihre höchste Inzidenz im Sakrum, gefolgt vom Clivus und den thorakalen Wirbelkörpern haben. Neben der klassischen Variante (NOS [„not otherwise specified“] mit hepatoider und nierenzellkarzinom-ähnlichen Varianten) sind eine chondroide Differenzierung sowie Chordome mit polymorpher bis anaplastischer Morphologie beschrieben. Besonders ungünstige Formen sind pädiatrische Chordome mit typischem INI1-Verlust. BNCT und Chordome sind charakterisiert durch folgendes immunhistologisches Profil: Vimentin+, Breitbandzytokeratin+/−, epitheliales membranes Antigen (EMA)+/−, S100-Protein+/+, Brachyury+ und lassen sich so immunhistologisch eindeutig definieren und abgrenzen von Chondrosarkomen, chordoiden Meningiomen und Karzinommetastasen.
Die AG Knochen-, Gelenkund Weichgewebspathologie tagte in diesem Jahr in Erlangen. Das Programm zeigte deutlich, dass sich die molekular-pathologische Diagnostik nicht nur im Bereich der Weichgewebstumoren, sondern auch bei den Knochentumoren in der täglichen Routine etabliert hat. Die diesjährige Sitzung war sehr gut besucht, erfreulicherweise auch durch junge Kolleginnen und Kollegen, obwohl die Tagungsräume kurzfristig auf ein benachbartes Hotel verlagert werdenmussten. Die Beiträge wurden rege diskutiert.
Abstract Background Sarcoidosis is a systemic disorder of unknown origin characterized by noncaseating granulomas. Clinical symptoms due to central nervous system (CNS) involvement occur in 5 to 7% of all cases; subclinical involvement is more frequent. Sole CNS involvement is very rare. Case Report A 25-year-old man presented with increasing polyuria and polydipsia over 8 weeks. Magnetic resonance imaging (MRI) revealed a supra- and infra-chiasmatic pre-thalamic mass lesion 1.0 × 1.4 × 1.4cm in diameter. Microsurgical biopsy verified a necrotizing noncaseating epithelioid cell tumor indicative for neurosarcoidosis. All symptoms dissolved within 3 months under stringent corticoid therapy. Conclusion Intracranial mass lesions as the primary and only manifestation of neuronal sarcoidosis are rare. Because conservative treatment is safe and effective, surgery is limited to biopsy and the alleviation of pressure-related symptoms to preserve neurologic function.
Chordomas are rare and slowly growing malignant bone tumors which mostly occur in adults. These bone tumors are characterized by epithelial and mesenchymal aspects. It is suggested that they arise from remnants of the notochord because they are found along the axial skeleton (e.g. clival, spinal and sacrococcygeal locations). It appears that cytogenetic aberrations are not randomly found in this tumor group. Loss of chromosomal material (e.g. 1p, 3p, 10q, 13q and 14q) is more frequently found than gain of material (e.g. 7q, especially 7q33). Several studies demonstrated brachyury expression (T; 6q27) as a possible candidate gene in the oncogenesis of chordomas (e.g. knock down in the chordoma cell line U-CH1). So far therapy consists of complete resection and irradiation, e.g. with carbon ions. Targeting therapy is not yet established in routine protocols but phase II studies with tyrosine kinase inhibitors have shown partial response of tumors and, in some studies stabilization of the disease has been described.
The molecular basis of chordoma is still poorly understood, particularly with respect to differentially expressed genes involved in the primary origin of chordoma. In this study, therefore, we compared the transcriptional expression profile of one sacral chordoma recurrence, two chordoma cell lines (U-CH1 and U-CH2) and one chondrosarcoma cell line (U-CS2) with vertebral disc using a high-density oligonucleotide array. The expression of 65 genes whose mRNA levels differed significantly (p<0.001; ≥6-fold change) between chordoma and control (vertebral disc) was identified. Genes with increased expression in chordoma compared to control and chondrosarcoma were most frequently located on chromosomes 2 (11%), 5 (8%), 1 and 7 (each 6%), whereas interphase cytogenetics of 33 chordomas demonstrated gains of chromosomal material most prevalent on 7q (42%), 12q (21%), 17q (21%), 20q (27%) and 22q (21%). The microarray data were confirmed for selected genes by quantitative polymerase chain reaction analysis. As in other studies, we showed the expression of brachyury. We demonstrate the expression of new potential candidates for chordoma tumorigenesis, such as CD24, ECRG4, RARRES2, IGFBP2, RAP1, HAI2, RAB38, osteopontin, GalNAc-T3, VAMP8 and others. Thus, we identified and validated a set of interesting candidate genes whose differential expression likely plays a role in chordoma.
Chordome sind sehr seltene und langsam wachsende maligne Knochentumoren zumeist des Erwachsenenalters. Dieser Knochentumor ist durch epitheliale und mesenchymale Merkmale gekennzeichnet. Es wird angenommen, dass sich die Chordome aus Resten der Chorda dorsalis entwickeln, da sie sind entlang des Achsenskeletts zu finden sind (Klivus, spinal, sakrokokzygeal). Zytogenetische Veränderungen scheinen nicht zufällig in dieser Tumorgruppe vorzukommen. Die Verluste von chromosomalem Material (1p; 3p; 10q; 13q; 14q) finden sich häufiger als Zugewinne (7q, insbesondere 7q33). In verschiedenen Studien konnte Brachyury (T; 6q27) als mögliches Kandidaten-Gen für die Chordomentstehung bestätigt werden („knocked down“ in der Chordomzelllinie U-CH1). Die Therapie besteht zumeist aus einer möglichst kompletten Resektion und Bestrahlung, z. B. mit Schwerionen. Neue zielgerichtete therapeutische Ansätze sind noch nicht etabliert, wenngleich hierzu erste Phase-II-Studien mit Tyrosinkinaseinhibitoren vorliegen.
BACKGROUND:Multiple activating mutations of the signal- and repair pathway, such as BRAF-, KRAS-mutations and microsatellite instabilities are involved in colorectal cancer pathogenesis. Molecular characterization of specifically locally advanced rectal cancers is scarce. Therefore the retrospective study addresses the intratumoral status of KRAS, BRAF and microsatellites loci with respect to tumor response and patients' antecedent including nicotine abusus, familial history, and health care to further molecularly identify rectal cancer patients.METHODS:The study assesses the molecular status of 50 rectal cancer samples (25 before and 25 after neoadjuvant 5-FU radiochemotherapy). KRAS and BRAF mutations were examined through two independent analytical methods (sequencing and SNaPshot) to ensure efficient mutation detection. The microsatellite analysis was conducted using a fluorescent multiplex PCR-based method.RESULTS:KRAS mutations were found in 9 of 25 (36%) rectal cancer patients and were not significantly associated with the response to therapy (P=0.577), age (P=0.249) or sex of the patient (P=0.566). No link exists between KRAS mutation status and nodal (P=0.371) or metastatic stage (P=0.216). For two patients, KRAS mutation status changed after application of neoadjuvant 5-FU radiochemotherapy. All tumor samples were diagnosed BRAF-negative. Two rectal cancer patients exhibited a MSI-H phenotype and showed no tumor response.CONCLUSIONS:So one can conclude that (I) KRAS mutations status may change after neoadjuvant 5-FU radiochemotherapy relevant for further therapeutic decisions; (II) MSI-H patients do not respond to neoadjuvant 5-FU radiochemotherapy. Further prospective studies are needed to validate these results.
Currently, quantitative real-time PCR (Q-PCR) of archival formalin-fixed, paraffin embedded (FFPE) tissue is a critical tool for research and is not well established in routine diagnostics. Therefore, continuous improvement in mathematics and statistics associated with interpreting final accurate and reproducible results are fundamental. This project describes and discusses specificity and sensitivity with respect to intraand inter-assay variances by use of a commercial Human Reference RNA and individual RNA derived from colorectal cancer patients (n = 25). All patients were treated with 5-fluoruracil (5-FU) and a concomitant pelvic radiotherapy (50.4 Gy). Quality assessment of target tissue samples was evaluated by clinicopathological findings and optical density (OD) measurements. We analyzed the steady state messenger RNA (mRNA) expression level of a small panel of cancer relevant genes, excision repair cross-complementing group 1 (ERCC1), ribonucleoside-diphosphate reductase subunit M1 (RRM1), thymidylate synthase (TYMS) and ß-2microglobulin (ß-2M) as endogenous control. The mRNA of a Human Reference RNA, tumor and non-neoplastic material was reverse transcribed into its complementary DNA (cDNA). cDNA was amplified based on dual-labeled TaqMan real-time fluorescence measurements. The real-time efficiency and therefore the output OPEN ACCESS Appl. Sci. 2012, 2 140 data can be influenced through the kind of calibrator used, the amount and quality of used RNA and by the degree of individual assay variability. Each sample presents an individual amplification curve. Thus, confirmation of primer specificity, one or more invariant endogenous controls, RNA and cDNA quality, as well as real-time PCR amplification efficiencies and linearity calculations from individual slopes or R-values must be included in each study.
