Objective: To evaluate the outcomes of an assisted PD peritoneal dialysis program. Worldwide, 10% of patients with End-Stage Renal Disease (ESRD) advanced Chronic Kidney Disease opt for peritoneal dialysis as renal replacement. Assisted peritoneal dialysis is a treatment performed with the help of a family member or health care technician. It enables home dialysis for patients who are ineligible due to social, cognitive, or physical barriers. Materials and methods: We included patients on PD peritoneal dialysis (2022- 2024) who required partial or total assistance to maintain the technique and compared them with a matched similar control group (sex, age, comorbidities). Results: 36 patients were included (18 PD-As assisted/18 control), with 55.5% female sex. No differences were found in sex (p = 0.354), age (p = 0.106), previous treatments (p = 0.153), duration on PD (p = 0.145), PD modality of peritoneal dialysis (p = 0.204) between the groups. Assistance was total in 12(66.6%) and partial in 3(33.3%). The prevalence of complications was similar between the groups: peritonitis (p = 0.104), exit site infection (p = 0.228), catheter dysfunction (p = 0.105), and death (p = 0.243). The cumulative survival in PD peritoneal dialysis was similar between the groups (p = 0.678). Conclusions: PD-As assisted modality is a strategy to maintain PD peritoneal dialysis in patients with limitations. It is not associated with a higher complication rate and ensures a similar duration of PD in the technique compared to those who do not receive assistance.
Allogeneic hematopoietic progenitor cell transplantation is a widely used procedure, and graft-versus-host disease (GVHD) is a common complication. Glomerular involvement due to GVHD is exceptional. OBJECTIVE:to describe the clinical and histopathological presentation of patients with glomerular disease secondary to GVHD. MATERIALS AND METHODS:a retrospective review of the Uruguayan registry of glomerulopathies was conducted to identify renal biopsies from patients with confirmed GVHD. RESULTS:seven patients were identified, four male, with a median age of 41 years (range 8-58). The most common clinical and analytical presentation was nephrotic syndrome. Histopathological findings included membranous glomerulonephritis (4), segmental and focal sclerohyalinosis (1), membranoproliferative glomerulonephritis (1), and thrombotic microangiopathy (1). All patients were treated with corticosteroids in combination with immunosuppressors, most frequently mycophenolate mofetil and rituximab. Five patients achieved complete remission, one had partial remission, and one did not respond. The combination of corticosteroids with rituximab showed a good response rate in those presenting with podocytopathy in the renal biopsy.
Contexto: la alta frecuencia de diabetes mellitus (DM) hace que los pacientes con esta enfermedad puedan tener otras nefropatías. Objetivo: describir la prevalencia y las lesiones histopatológicas en biopsias renales (BR) de pacientes con DM de más de un año de diagnóstico. Metodología: se incluyó a personas con DM seguidas en el programa de salud renal, entre los años 1985-2020, con BR en ese periodo (edad ?15 años, DM ?1 año antes de BR). Resultados: se analizaron 190 BR, de pacientes con DM de 8 (1-24) años de evolución, donde el 2,63?% tuvo retinopatía. El número de BR en personas con DM fue creciente en el tiempo en Uruguay. La indicación de realizar BR fue debido a alteraciones urinarias asintomáticas (AUA) (39?%), síndrome nefrótico (SN) (28,4?%), insuficiencia renal rápidamente progresiva (11,6?%) y enfermedad renal crónica (ERC) (11?%). Además, el 11?% tuvo un patrón de nefropatía diabética (ND) y los patrones principales de nefropatía no-diabética (NND) (89?%) fueron: esclerohialinosis segmentaria y focal (EHSF) (22,63?%), nefropatía membranosa (14,21?%), vasculitis (12,63?%) e IgA (7,36?%). Los pacientes con ND se presentaron con SN (52,4?%) y AUA (19?%). Aquellos con NND y SN tenían EHSF (27,3?%) y nefropatía membranosa (23,6?%), aquellos con AUA tenían EHSF (24,1?%), vasculitis (13,9?%) e IgA (13,9?%) y aquellos con ERC tenían EHSF (20?%) y nefroangioesclerosis (16?%). La distribución de patrones lesionales del grupo con NND fue similar a la población general sin DM. Conclusiones: La BR se indica con frecuencia creciente en pacientes diabéticos, las principales indicaciones en nuestra serie fueron SN y AUA. Un bajo porcentaje tuvo solo lesiones de ND y la distribución de lesiones en pacientes con NND fue similar a la de la población adulta sin DM.
Objective: Peritoneal dialysis (PD) peritonitis is a common complication. Previous studies report an association between the presence of leukocytes in peritoneal fluid (PF) with the use of urine test strips and the cell count in PF; This study seeks to evaluate agreement between the semiquantitative result of leukocytes measured by reagent strip and the quantitative result obtained by LP cytology. Material and methods: 100 samples from 43 PD patients were included; Cytology was studied using an automated counter and urine reactive strips (Multistix 10 SG) were made from the same sample. Results: 26% were women, age 47.7±13.1 years, and 11% were diabetic. To analyze agreement between leukocyte measurement by strip and automated measurement, kappa coefficient (k) and alternative Bland and Altman analysis were used. The result showed good overall correlation. The sensitivity of the strip to make a diagnosis of peritonitis was 95.2% and the specificity was 63.6%. Conclusions: good agreement was observed between leukocyte count by reagent strip and automated cytology. The strip had good sensitivity and a high negative predictive value for the diagnosis of peritonitis. The urine strip is an effective and economical tool for the initial evaluation of peritonitis in patients on PD.
Introducción El trasplante alogénico de progenitores hematopoyéticos es un procedimiento ampliamente utilizado y el desarrollo de enfermedad injerto versus huésped (EIVH) una complicación frecuente. El compromiso glomerular por EIVH es excepcional. Objetivo Describir la presentación clínica e histopatológica de pacientes con enfermedad glomerular secundaria a EIVH. Material y métodos Se revisó retrospectivamente el registro uruguayo de glomerulopatías en busca de biopsias renales de pacientes con EIVH demostrada. Resultados Se identificó a 7 pacientes, 4 de sexo masculino, edad 41 (8-58) años. La presentación clínica-analítica más frecuente fue el síndrome nefrótico. Los hallazgos histopatológicos fueron glomerulonefritis membranosa (4), esclerohialinosis segmentaria y focal (1), glomerulonefritis membranoproliferativa (1) y microangiopatía trombótica (1). La totalidad de pacientes se trataron con corticoides asociados con inmunosupresores, más frecuentemente micofenolato mofetilo y rituximab. Presentaron remisión completa 5 pacientes, remisión parcial 1 y no remisión 1. La combinación de corticoides con rituximab mostró una buena tasa de respuesta en quienes se presentaron con podocitopatía en la biopsia renal.
Nos propusimos evaluar la respuesta al tratamiento con rituximab (RTX) en adultos con síndrome nefro?tico corticodependiente (SNCD) y recaedor frecuente (SNRF). Se analizaron pacientes con diagno?stico de SNCD y SNRF en la infancia que tenían estudio histolo?gico renal y recibieron RTX siendo adolescentes o adultos jo?venes (> 16 años). Analizamos características clínicas y analíticas de los pacientes y su evolucio?n. Se incluyeron 12 pacientes (n = 8 con lesio?n glomerular mínima, n = 4 con glomeruloesclerosis focal y segmentaria). La mediana de exposicio?n a esteroides previa al RTX fue 19 años (rango: 2-31.5 años). El SN fue diagnosticado con una mediana de edad de 3 años (rango: 1.5-6.0 años) y la mediana de edad al recibir la primera dosis de RTX fue 24 años (rango: 16-42 años). Luego del tratamiento con RTX, y al final del seguimiento, todos los pacientes lograron remisio?n sin fármacos inmunosupresores. La tasa de recaídas luego del tratamiento con RTX disminuyo? ocho veces (8.7; IC95%: 2.1-40.5) y la dosis de prednisona promedio se redujo (19.6 ± 17.5 mg vs. 4.6 ± 8.6 mg/dl; p = 0.0027). El rituximab fue eficaz en reducir el número de recaídas y la carga de esteroides en adultos jo?venes con SNCD o SNRF, con un adecuado perfil de seguridad.
T2-DM and chronic kidney disease (CKD) are common pathologies, with a high prevalence in adult population. Diabetic kidney disease (DKD) can present clinically with different phenotypes. The high frequency of T2-DM, CKD and DKD creates difficulty in establishing an accurate diagnosis in patients with T2-DM and manifestations of glomerular disease. Multiple studies report the presence of non-diabetic nephropathy (NDN) in 35-75% of renal biopsies (RB) from patients with T2-DM. The aim of the present study is describe the histopathological findings and their clinical correlation, in patients with T2-DM in whom RB was performed in Uruguay.
Amyloidosis is a group of diseases characterized by the extracellular deposition of amorphous material with a fibrillar structure. Renal involvement represents the most common systemic manifestation in amyloidosis. The widely accepted classification categorizes amyloidosis based on the type of fibril-forming protein including Serum Amyloid A (AA), Immunoglobulin light chain (AL), and transthyretin (ATTR) amyloidosis. Heavy chain amyloidosis (AH) is a rare entity characterized by amyloid fibrils derived exclusively from fragments of the Ig heavy chain.
Hemodialysis is the renal replacement therapy (RRT) most used in the world. Online hemodiafiltration (OL-HDF) and high-flux hemodialysis (HF-HD) techniques demonstrated to be superior in the removal of medium molecular weight (MW) molecules with respect to conventional hemodialysis (CHD). These molecules are linked to an increased risk of cardiovascular events and death. Beta 2 microglobulin (B2M) is a medium PM molecule, in addition to being toxic, it is an example of the concentration of multiple molecules of the same group.
The success of peritoneal dialysis (PD) depends on the timely and appropriate placement of a peritoneal catheter (PC). The lack of access to PC placement and the presence of early complications after placement are barriers to the development of the PD. The nephrologist's percutaneous placement of the PC offers a solution to some of these problems and when performed systematically and with proper patient selection, it is a safe technique. The objective of this study is to describe the experience of percutaneous PC placement by nephrologists at a university peritoneal dialysis center, evaluate complications related to the procedure, and complications (early and late) associated with PC use.
Antecedentes/Objetivo: La hemodiálisis es ineficaz en depurar moléculas de peso molecular intermedio (M-PMI) y su acumulación determina enfermedad y mortalidad cardiovascular. La depuración de ß2-microglobulina es marcador de depuración de M-PMI. Comparamos la eficacia de la hemodiafiltración en línea (HDF-OL) y la hemodiálisis de alto flujo (HD-AF) para depurar ß2-microglobulina. Material y métodos: Estudio prospectivo y observacional. Se incluyeron pacientes ≥ 18 años, tratados con HDF-OL o HD-AF ≥ 6 meses, con diuresis residual ≤ 1,000 cc/día, angioacceso con flujo ≥ 350 ml/min y seguimiento ≥ 24 meses. Se midió la ß2-microglobulina antes y después de cada procedimiento. Resultados: Se evaluaron 41 procedimientos (22 HD-AF/19 HDF-OL) en 22 pacientes. La edad (58.8 ± 21 vs. 53 ± 12 años; p = NS) y permanencia (49.6 ± 32.3 vs. 49.2 ± 22.7 mes; p = NS) en cada técnica fue similar. Las características de los procedimientos (tiempo, Qb, Qd, ultrafiltración) y tolerancia fueron similares en ambos grupos. No hubo diferencia en la depuración de moléculas pequeñas entre grupos y existió mayor tasa de reducción de ß2-microglobulina en el grupo bajo HDF-OL (73.7 ± 10.9 vs. 59.5 ± 15.5 %; p = 0.002). Mayor tiempo bajo HDF-OL (6-36 vs. ≥ 36 meses) se asoció a menores niveles de ß2-microglobulina pre procedimiento y posprocedimiento. Conclusiones: La HDF-OL fue más eficiente que la HD-AF en la depuración de M-PMI. Mayor permanencia en HDF-OL se asoció a menores niveles de ß2-microglobulina circulante.
Autosomal dominant polycystic kidney disease is the most common hereditary kidney disease. It is charac-terized by the progressive appearance of renal cysts that usually lead to extreme chronic kidney disease in adulthood. The approval of the use of tolvaptan (V2 va- sopressin receptor antagonist) has meant a significant change in the treatment of this disease. In recent years, evidence has proved the benefits of initiating treatment with tolvaptan in patients with a rapidly evolving di-sease. A descriptive review of the main clinical studies published in 2012-2022 period is carried out and a use-ful scheme is suggested to select those patients who can benefit from the start of treatment.
Este documento de recomendaciones tiene como objetivo orientar a médicos nefrólogos y no nefrólogos que asisten a pacientes con enfermedad renal crónica (ERC) en todas las etapas de la misma, en el proceso de vacunación contra el SARS-CoV-2. Como consecuencia de la situación epidemiológica y de los tiempos del proceso de elaboración de las vacunas disponibles, no se ha generado evidencia lo suficientemente potente, por lo que las recomendaciones no se acompañan del nivel de evidencia. Se fundamenta la necesidad de priorizar la vacunación en este grupo de pacientes en el mayor riesgo de adquirir la infección por SARS-CoV-2, desarrollar la enfermedad COVID-19 con mayor gravedad y presentar una mortalidad más elevada que la población general. Las recomendaciones se organizan por grupos de pacientes considerando pacientes con ERC no dialítica, diálisis y trasplante renal, y pacientes bajo tratamiento inmunosupresor.
The objective of this document containing recommendations is to provide guidelines for nephrologists and non-nephrologists who assist patients with chronic kidney disease (CKD) at all stages of the disease on the vaccination process against SARS-CoV-2. As a consequence of the current epidemiological situation and the timing of the COVID-19 vaccine development -for available vaccines-there is no solid evidence, and thus, recommendations are not accompanied by the due medical proof. The need to prioritize vaccination in this group of patients is based on the increased risk of acquiring the SARS-CoV-2 infection, developing the COVID-19 disease with greater severity and presenting higher mortality rates than the general population. The recommendations are organized by groups of patients, considering patients with non-dialytic CKD, dialysis and kidney transplantation, and patients under immunosuppressive treatment.
Antecedentes: En fecha reciente se propuso utilizar el ac-PLA2R en el diagnóstico y seguimiento de la nefropatía membranosa idiopática (NMI). Objetivo: Evaluar el rendimiento de los ac-PLA2R durante el inicio, la remisión y la recaída en los primeros casos de NMI estudiados en Uruguay. Metodología: Se analizó en forma retrospectiva (2015-2018) a los adultos con diagnóstico histológico de NMI y determinación de ac-PLA2R por técnica de ELISA. Resultados: Se incluyó a 26 pacientes. La dosificación de ac-PLA2R fue positiva (>14 U/ml) en 13/15 al inicio (mediana: 114 U/ml) y 5/5 durante la recaída (mediana: 37 U/ml). Los pacientes en remisión parcial o completa (n = 6) presentaron títulos de ac-PLA2R normales o bajos (≤20 U/ml). En una segunda determinación de ac-PLA2R (n = 10), los títulos se tornaron negativos (<14 U/ml) en todos los pacientes que alcanzaron remisión parcial o completa (n = 8). El título de ac-PLA2R se relacionó con el grado de proteinuria (r = 0.69) y permitió predecir (R2 = 0.40) un aumento de 400 mg de proteínas por cada 10 U/ml de aumento del ac-PLA2R. Conclusiones: En la NMI, la determinación de ac-PLA2R es positiva al inicio y en las recaídas y se negativiza en la remisión. Los títulos de ac-PLA2R se vinculan positivamente con la proteinuria y permiten predecir el grado de ésta durante el inicio y la recaída.
