Ziele des Rehabilitationsmanagements aus Sicht der Gesetzlichen Unfallversicherung sind: Steigerung der medizinischen Qualität, Verkürzung der Arbeitsunfähigkeitszeit, Überwindung von Schnittstellenproblemen und Verminderung der Rentenzahlungen. Dies soll mittels Steuerung der Rehabilitation durch einen Rehabilitationsmanager anhand eines Rehabilitationsplans erreicht werden. Der Durchgangsarzt (D-Arzt) kann sich sicher mit den Zielen einer Qualitätsverbesserung und Beseitigung von Schnittstellenproblemen identifizieren, könnte sich aber in seiner medizinischen Freiheit durch die Kontrollen des Rehabilitationsmanagers, die Einschaltung eines Rehabilitationsmanagementarztes oder Qualitätssicherungsmaßnahmen des Versicherers eingeschränkt fühlen. Eine Vereinheitlichung des Rehabilitationsmanagementverfahrens für alle gesetzlichen Versicherungsträger sowie Information des behandelnden D-Arztes über für ihn relevante berufliche Aspekte – z. B. das Arbeitsprofil und den Arbeitsvertragsstatus – wären wünschenswert. Ein positiver Aspekt des Rehabilitationsmanagements besteht für den D-Arzt in der Minimierung des Berichtswesens, der Handhabung der Schnittstellenproblematik durch den Rehabilitationsmanager sowie einem besseren Behandlungsergebnis für den Patienten.
Special therapies for posttraumatic gonar-throsis, such as acupuncture and hyaluronicacid injections, are not routinely funded by employers' liability insurance associations. Although the physician is generally free to choose the therapeutic approach, therapy is only funded if the treatment is appropriate and necessary according to good clinical practice. Quality and effectivity have to meet current standards of medical knowledge. The AWMF guideline is hardly more than a list summarising potential therapeutic strategies. The EULAR guideline (2003) gives recommendations based on EBM criteria and expert opinions. We reevaluated scientific evidence relating to nonoperative treatment of osteoarthritis of the knee according to EBM criteria, with a special emphasis on the Cochrane Systematic Reviews. These yielded good evidence to show that hyaluronic acid is an option that must be considered for nonoperative treatment of osteoarthritis of the knee, while the effectivity of other approaches has not so far been documented.
Die Kosten für spezielle Formen der Therapie der posttraumatischen Arthrose wie Akupunktur oder Hyaluronsäureinjektionen werden von der DGUV in der Regel nicht übernommen. Es besteht zwar regelmäßig eine Therapiefreiheit für den Arzt, die Vergütung durch die Träger der GUV erfolgt jedoch nur, wenn die Behandlung nach den Regeln der ärztlichen Kunst erforderlich und zweckmäßig ist. Qualität und Wirksamkeit haben dem allgemein anerkannten Stand der medizinischen Erkenntnis zu entsprechen. Die AWMF-Richtlinien stellen nur summarische Beurteilungen der Therapieoptionen dar. Empfehlungen zur Arthrosetherapie werden in den EULAR-Empfehlungen 2003 für die Kniegelenkarthrose bezüglich der konservativen Behandlung auf EBM-Basis und Expertenmeinung erstellt. Die Studienlage zur konservativen Arthrosetherapie wurde anhand der EBM-Kriterien reevaluiert. Die Literatur und insbesondere die Analysen der Cochrane-Database zeigten, dass Hyaluronsäureinjektionen als wirkungsvolle konservative Therapie der Kniegelenkarthrose erwogen werden müssen, wohingegen die Wirksamkeit vieler anderer Therapiemodalitäten bisher nicht nachgewiesen werden konnte.

OBJECTIVE:Postoperative care for ankle fractures is generally 1 of 2 regimens: 1) functional treatment combined with early weightbearing (EWB), or 2) immobilization in a cast/orthosis for 6 weeks without weightbearing (6WC). The objective of this study was 2-fold: 1) to follow a prospective group treated with EWB as to long-term subjective and objective outcomes, and 2) to compare a subset of this group with a matched group of historic controls treated with 6WC. DESIGN:Prospective, clinical, cohort observation, and retrospective matched pair analysis. SETTING:University hospital, level 1 trauma center. PATIENTS:Forty-three patients (20 males; mean age, 49 +/- 14 years) with operated Weber B/C fractures underwent EWB. For comparison, 23 patients of this group were matched to a same number of historic controls with respect to age, gender, body mass index, and fracture type. INTERVENTION:Open reduction and internal fixation (ORIF) using a 1/3-tubular-fibula-plate for the fibula, and malleolar screws for the medial malleolus fracture (in cases with a bimalleolar ankle fracture) followed by EWB or 6WC. MAIN OUTCOME MEASUREMENTS:Olerud and Tegner scores at follow-up (at least 12 months after surgery), time to full weightbearing, return to work, pain intensity (numerical rating scale (NRS)), and hospital stay. Statistical comparisons were performed by using the Mann-Whitney U test or Fisher exact test (P < 0.05). RESULTS:Patients with EWB were full weightbearing at 7 +/- 3 weeks and returned to work at 8 +/- 5 weeks after surgery. At follow-up (mean, 20 +/- 11 months after surgery), all EWB patients showed good results in the Olerud score (90 +/- 13 points). Matched-pair analysis in 23 patients in each group revealed differences between EWB and 6WC groups for hospital stay (mean, 10.8 +/- 4.7 vs. 13.6 +/- 6 days; P = 0.12), time to full weightbearing (mean, 7.7 +/- 3.1 vs. 13.5 +/- 9.4 weeks; P = 0.01), and time until return to work (mean 9.2 +/- 5.5 vs. 10.8 +/- 7 weeks; P = 0.63). No differences concerning pain intensities were observed (EWB vs. 6WC: NRS = 1.9 vs. 1.7; P = 0.12). At follow-up, Olerud scores were generally considered good for both groups; however, mean values in EWB patients were slightly higher (87 +/- 14 vs. 79 +/- 19 points; P = 0.25). In both groups, the majority of patients reached their preinjury level of activity as demonstrated by Tegner scores. CONCLUSIONS:EWB patients tolerated earlier full weightbearing compared with 6WC patients, and there were no disadvantages with EWB compared with 6WC concerning hospital stay, pain intensities, time until return to work, and Olerud/Tegner Scores. Potential candidates for EWB are patients with a stable osteosynthesis of their fractured ankles as judged by the responsible surgeon, compliance, and high motivation.
Microdialysis is a new approach for monitoring after hemorrhagic shock. The advantage of microdialysis is the local measurement of microcirculation and local changes of mediators. Until now this was mostly used in neuromonitoring. In this experiment we were focussed on the local changes of histamine in the liver after hemorrhagic shock by female pigs. Histamine is a vasoactive mediator which is well described in septic shock. The role of histamine after hemorrhagic shock is rarely known. Especially changes of histamine in the liver after shock is from our knowledge not clear. We could show, that after a short peak by placing the probes in the liver histamine increased in shock and we could further see, that we get two different reactions of histamine in the liver in shock depending on the position of the probes. Central placed probes do not show a relevant increase of histamine during hemorrhagic shock. If probes are placed in periphery regions of the liver microdialysis shows a significant increase of histamine by those animals, which die in an early phase after shock. Surviving animals do not get such an increase of histamine. In our experiment histamine had a prognostic value by peripheral placed probes in the liver after hemorrhagic shock.
Patients' complaints on limitations in activities of daily living (ADL) and sports are possible signs for various injuries of the knee joint. These complaints can be easily assessed by the patient using a questionnaire with visual analogue scale (VAS) responses. A German translation of the English questionnaire concerning knee complaints [4] has been validated and tested for clinical use. It consists of 28 questions. For the statistical analysis, an overall score (VAS score) of the questionnaire was determined. The validation included an expert evaluation on the content of the questionnaire, a test for the reliability, a comparison with subjective knee scoring systems (Cincinnati score, Lysholm score) and a test on VAS score results in patient groups with various knee injuries (discrimination of patients). For the evaluation of operative treatment results, the responsiveness of the questionnaire was tested in patients undergoing arthroscopic meniscus surgery and anterior cruciate ligament reconstruction. The investigation was conducted prior to the operation, 2, 6, 12 and 24 weeks after surgery for both groups and also 36 weeks after surgery for the cruciate ligament patients. Interviews conducted with knee surgeons (so-called knee experts) revealed that 85% judged the questionnaire as being acceptable for clinical use. The reliability of the VAS score for healthy individuals wasr=0.86. The reliability for patients in a postoperative rehabilitation programme wasr=0.92. The correlation coefficient for the Lysholm score wasr=0.88 and for the Cincinnati scorer=0.91. By means of the VAS score we were able to assess the extent of limitations in knee function in various patient groups with meniscus lesions, insufficiency of the anterior and posterior cruciate ligaments and chondromalacia. However, the VAS score was not specific for a single knee disorder. In patients who had undergone partial meniscal resection and anterior cruciate ligament (ACL) reconstruction, alterations in knee function could be adequately evaluated by means of the VAS score. When comparing the VAS score prior to and after the operation, an evaluation of the treatment result was possible. With such a questionnaire using the patient's VAS responses, subjective data can be assessed time-saving for the physician, without observer bias and submitted to statistical analysis. The questionnaire can be used as a simple and valid instrument for measuring the subjective outcome in knee surgery.
We present the case of a 44-year-old man who complained of cervical pain. He was treated with physiotherapy and analgetics. Because of persistent pain, computed tomography (CT) scan and MRI were performed. They revealed an osteolytic destruction of the fourth cervical vertebra. The patient was treated surgically for removal of the tumor and stabilization of his cervical spine. Histology of the osteolytic material led to the diagnosis of an eosinophilic granuloma of the cervical spine. This case report describes the incidence, clinical significance, background and therapy of an eosinophilic granuloma of the spine.
Fragestellung: Die perkutane iliosakrale Verschraubung hat sich in den letzten Jahren zu einem etablierten Verfahren der hinteren Stabilisierung einer iliosakralen Sprengung oder Sakrumfraktur entwickelt. Ungeklärt erscheint jedoch die Notwendigkeit einer späteren Entfernung des Osteosynthesematerials. Ziel der Arbeit war es anhand einer systematischen Literaturanalyse und klinischer Nachuntersuchung zu überprüfen, ob die Entfernung des Osteosynthesematerials indiziert ist.
Die 1999 publizierte S1-Leitlinie Unfallchirurgie [1], empfiehlt zur Nachbehandlung operativ versorgter Sprunggelenksfrakturen:
The effect of arthroscopic lavage and mechanical debridement of osteoarthritic cartilage on the long-term course of gonarthrosis is controversial. Recent controlled studies suggest that a placebo effect has always to be considered if patients report a benefit from surgery. Currently, arthroscopy can be recommended only in case of instabile meniscus flaps, loose bodies or unstable cartilage flaps in addition to osteoarthritic changes of the knee. Symptoms of pain associated with radiographic findings of progressive osteoarthritic changes of the knee alone do not seem to justify an arthroscopic procedure of the joint.
UNLABELLED:The aim of this prospective study was to identify prognostic factors predicting post-operative outcome in patients one year after their hip fracture. METHODS:From October 1996 until September 1997 all patients (n = 98) treated for a hip fracture in our hospital were prospectively included into the study. Follow up was performed at three, six and twelve months after the injury. The main outcome variable was the activity of daily living measured by the Barthel-Index. An univariate analysis of the variables age, gender, ASA-classification, type of fracture and treatment, length of stay and rehabilitation was performed and all these variables were correlated with the Barthel-Index. A multiple logistic regression was performed for identification of prognostic factors predicting outcome. RESULTS:The one year lethality was 33% (n = 31). All patients younger than 75 years reached their preoperative activity level at one year follow up. Patients with age of 75 years or more experienced a reduction of their activities of daily living of 20%. The ADL-differences of both groups at the follow up after 3, 6 and 12 months showed a significant p-value (p < 0.001). The ADL-differences between the group of patients with or without a transfer to a specialized rehabilitation center were not significant. The multivariate analysis showed that the preoperative use of the Barthel-Index was the best prognostic factor for outcome until one year after the injury. CONCLUSION:The preoperative Barthel-Index should be seriously considered for therapeutic decision making in patients with hip fractures.
Zusammenfassung Ziel dieser prospektiven Studie war es, aus-sagekräftige Prognosefaktoren für das Outcome bei Patienten ein Jahr nach hüftgelenksnaher Fraktur zu finden. Methodik. Vom 01.10.1996–30.09.1997 wurden alle hüftgelenksnahen Frakturen (n=98) in unserer Klinik prospektiv erfasst und im Rahmen einer systematischen “Follow-up”-Untersuchung nach 3, 6 und 12 Monaten untersucht. Hauptzielkriterium waren die Aktivitäten des täglichen Lebens gemessen am Barthel-Index. Es wurden die Variablen – Lebensalter, Geschlecht, ASA-Klassifikation, Frakturart, Art der Osteosynthese, Liegedauer und Rehabilitation – univariat untersucht und mit dem Barthel-Index korreliert. Weiterhin wurde in einer schrittweisen logistischen Regressionsanalyse mit multivariatem Ansatz versucht prädiktive Einflussvariablen für das postoperative Outcome zu identifizieren. Ergebnisse. Die Letalität 12 Monate nach dem Unfall betrug 33%. Alle überlebenden Patienten mit einem Alter <75 Jahren hatten nach einem Jahr ihr präoperatives Aktivitätsniveau wieder erreicht. Die Patienten ab 75 Jahre büßten 20% ihres Aktivitätsniveaus ein. Die ADL-Unterschiede beider <<0,001). Die ADL-Durchschnittswerte der Patientengruppe mit einer Rehabilitationsmaßnahme zeigten ein Jahr nach der Operation höhere Werte, als die Gruppe ohne durchgeführte Rehabilitationsmaßnahme (nicht signifikant). Die multivariate Analyse zeigte, dass der präoperativ gemessene Barthel-Index der beste Prognosefaktor für das Aktivitätsniveau bis zu einem Jahr nach dem Unfall war. Schlussfolgerung. Das präoperative Aktivitätsniveau, gemessen mit dem Barthel-Index, sollte stärker bei Therapieentscheidungen bei Patienten mit hüftgelenksnahen Frakturen berücksichtigt werden.
Ziel dieser Patientenfalldarstellung ist es, die verschiedenartigen Ursachen von ‚Verspannungsschmerzen ‘in der Halswirbelsäule mit ausstrahlenden Kribbelparästhesien in die Arme differentialdiagnostisch zu diskutieren und auf die seltene Ursache eines eosinophilen Knochengranuloms der Halswirbelsäule hinzuweisen. Material und Methode: Berichtet wird über einen 44jährigen Patienten, der sich zur Hemicorporektomie und kortikospongiöser Spanauffüllung des HWK IV bei unklarer osteolytischer Raumforderung in unserer Klinik vorstellte. Ergebnisse: Der Patient beklagte Verspannungen in der Halswirbelsäule, die vom ambulanten Orthopäden aufgrund vermuteter degenerativer Veränderungen zunächst physikalisch-konservativ anbehandelt wurden. Bei persistierenden Schmerzen und ausstrahlenden Kribbelparästhesien in beide Arme stellte sich der Patient beim Neurologen vor, weiterführende Diagnostik ergab eine osteolytische Raumforderung des HWK IV. Daraufhin führten wir eine Osteolysenausräumung mit kortikospongiöser Span-und Pallacosauffütterung sowie ventraler Plattenosteosynthese durch. Die intraoperative Schnellschnittdiagnostik erbrachte zunächst den Verdacht auf ein undifferenziertes Magen-carcinom, weitere Präparataufarbeitung ergab histologisch das Vorliegen eines gutartigen eosinophilen Knochengranuloms. Das eosinophile Knochengranulom wurde erstmals 1940 von Lichtenstein und Jaffe [1] beschrieben. Es bildet zusammmen mit der Hand-Schüller-Christian- (chron. Verlaufsform) und der Abt-Letterer-Siwe-Krankheit (maligne dissemi-nierte Verlaufsform) die Trias der sog. Histiocytosis X [2], eines Erkrankungsbildes mit Proliferation von histiozytären und retikulären Gewebestrukturen. Die Ätiologie ist unbekannt [3, 4], bei überwiegend kindlichem Prädilektionsalter (80%) [5]. Die Inzidenz ist selten, bei Kindern < 15 Jahre (1:200000) jedoch häufiger [6] als bei Erwachsenen (1:300000) [7]. Prädilektionsstellen sind Schädelkalotte und Wirbelkörper (7,8%), davon sind 11% in der HWS lokalisiert [8]. Spontanheilungsverläufe nach Monaten sind möglich [5], operative Tumorausräumung, Radiatio oder Chemotherapie bei Rezidiven sind jedoch Therapiemethoden der Wahl [3]. Bei unifokalem, nicht maligne disseminiertem Befall ist die Prognose als gut und kurativ therapierbar zu bezeichnen [9]. In 10% kommt es jedoch zu einem multifokalen Organbefall [3]. Schlußfolgerung: Obwohl bisher 35 Fälle eines eosinophilen Knochengranuloms der HWS beschrieben wurden [5], handelt es sich dabei trotzdem um eine seltene Differentialdiagnose von Schmerzen im HWS-Bereich. Es sollte jedoch gerade bei Kindern, Jugendlichen und jungen Erwachsenen mit dieser Symptomatik als mögliche Differentialdiagnose mitberücksichtigt werden.
In der Literatur werden unterschiedliche Nachbehandlungskonzepte osteosynthetisch versorgter Frakturen des oberen Sprunggelenkes diskutiert. Einheitlich wird dabei jedoch das obere Sprunggelenk für mindestens sechs Wochen ruhiggestellt oder entlastet. Ziel dieser prospektiven Machbarkeitsstudie war es, Patienten mit stabil osteosynthetisch versorgter oberer Sprunggelenksfraktur einer frühfunktionellen, frühbelastenden Nachbehandlung zuzuführen und das postoperative Outcome bis zu 12 Monaten postoperativ zu erfassen. Material und Methode: Von 1/1999–2/2001 wurden 33 Patienten mit uni-, bi- und trimalleolären OSG-Frakturen in unserer Klinik stabil osteosynthetisch versorgt und nach einem frühfunktionellen, frühbelastenden Therapiekonzept nachbehandelt. Ausschlußkri-terien für dieses Nachbehandlungskonzept waren osteosynthetisch instabile, kindliche und offene Frakturen, schlechte Patientencompliance und nicht Vorliegen des Patienteneinverständnisses. Die Patienten wurden nach der dritten postoperativen Woche in einer Sprunggelenksorthese belastet und die Zeit bis zum Erreichen der Vollbelastung und der Wiederaufnahme der täglichen Aktivitäten ermittelt. Das postoperative Outcome der Pat. wurde anhand einer systematischen Follow up Untersuchung nach 3,6, und 12 Monaten u.a. durch die subjektive Syptomevaluierung (‚Olerud Score‘) ermittelt. Der Olerud Score vergibt 100 Pkt. in 9 Kategorien (Schmerzen, Steifigkeit, Schwellung, Treppensteigen, Laufen, Springen, Hocken, Gehhilfen, Arbeiten). Die Summe wird in vier Gruppen eingeteilt: Bis 30 Pkt. ‚schlecht‘, bis 60 Pkt. ‘zufriedenstellend’, bis 90 Pkt. ‘gut’ und > 90 Pkt. ‚exzellent‘. Ergebnisse: Es konnten 17 Pat. mit klinisch instabiler unimalleolärer Weber B-, 12 Pat. mit unimalleolärer Weber C-Fraktur, 2 Pat. mit bimalleolärer Weber B- und 2 Pat. mit trimalleolärer Weber C-Fraktur eingeschlossen werden. Das Patientendurchschnittsalter betrug 48 Jahre, der Body-Mass-Index 27, die durchschnittliche OP-Zeit 64 min und die Krankenhausverweildauer 10,8 Tage. Alle Pat. belasteten spätestens nach der dritten postoperativen Woche teil, nach durchschnittlich 7,2 Wochen voll (2–12 Wochen). Nach 7,4 Wochen (1-18 Wochen) gingen die operativ Behandelten wieder ihren gewohnten privaten und beruflichen Tätigkeiten nach. Bezüglich des Gesamtergebnisses des Olerud Scores zeigte sich bereits nach drei Monaten im Mittel eine ‚gute’ sprunggelenksfunktion (73 Pkt.), die sich nach sechs Monaten auf 81 Pkt. und nach 12 Monaten auf nahezu ‚exzellente ‘Ergebnisse verbesserte (89 Pkt.). Schlußfolgerung: Ein frühfunktionelles, frühbelastendes Nachbehandlungskonzept bei stabil osteosynthetisch versorgter OSG-Fraktur ist möglich, vorrausgesetzt der Patient besitzt eine ausreichende Compliance. Gemessen am Olerud Score erreichen diese Patienten bereits nach drei Monaten ‚gute ‘Ergebnisse und nach zwölf Monaten nahezu exzellente Funktionsergebnisse. Damit scheint sich sowohl die Zeit bis zur Vollbelastung als auch bis zur Erlangung der vollen Funktion deutlich zu verkürzen.
Zusammenfassung Einleitung. Der Constant- und Neer-Score sind 2 weitverbreitete Scores zur Beurteilung der Schulterfunktion nach verschiedenartigen Erkrankungen oder Verletzungen. Ziel der vorliegenden Arbeit ist ein Vergleich von subjektiver und Score-basierter Bewertung der Schulterfunktion unter der Hypothese einer deutlichen Diskrepanz zwischen Scoreergebnis und subjektiver Patienteneinschätzung insbesondere für selektionierte Patientenkollektive. Methodik. Die Schulterfunktion von 51 Patienten wurde, 1–6 Jahre nach operativ versorgter proximaler Humerusfraktur, unter Verwendung des Neer- und Constant-Score analysiert. Gleichzeitig erfolgte eine subjektive Bewertung durch die Patienten in den Kategorien “sehr gut” bis “schlecht”. Ergebnisse. Beide Scoresysteme zeigten eine gute Übereinstimmung mit einer linearen Korrelation von r=97. In der subjektiven Bewertung beurteilten 57% der Patienten ihre Schulterfunktion als “sehr gut” oder “gut”, in der Score-basierten Bewertung erreichen nur 37% (Constant) bzw. 43% (Neer) der Patienten ein entsprechendes Ergebnis. Bezogen auf die Einteilung in Ergebnisgruppen (sehr gut/gut/befriedigend/schlecht) stimmen die Ergebnisse von Neer-Score und subjektiver Einschätzung in 20 Fällen und von Constant-Score und subjektiver Einschätzung in 15 Fällen überein. Die Spearman-Rangkorrelation zwischen der subjektiven und der Score-basierten Einschätzung ist mit 0,50 (Constant) und 0,55 (Neer) nur mäßig. Bei älteren Patienten (>60 Jahre) war die subjektive Beurteilung insgesamt positiver als die Score-Bewertung. Von 20 älteren Patienten kamen 14 (Neer-Score) bzw. 16 (Constant-Score) subjektiv zu einer besseren Einschätzung ihrer Funktion als der Score. Schlussfolgerungen. Die aufgestellte Hypothese einer Diskrepanz zwischen subjektiver und Score-basierter Einschätzung wird durch unsere Ergebnisse insbesondere für ältere Patienten bestätigt. Für die Klinik bedeutet dieses, dass das “Outcome” des Patienten nicht zwangsläufig mit seinem Scoreergebnis korrelieren muss. Therapieempfehlungen und Aussagen wissenschaftlicher Arbeiten, die ausschließlich auf einer Scorebewertung basieren, können deshalb für bestimmte Patientenkollektive nicht uneingeschränkt übernommen werden.
We report on a 29-year-old motorcyclist, who had suffered a traumatic right side arm plexus lesion. The myelo-CT image showed a avulsion of the cervical roots C7/C8. Five days after the accident the patient complained of phantom pain in the right plegic arm and was presented to our acute pain service (APS). The patient complained of lancinating attacks of severe phantom pain in the right arm (visual analogue scale intensity of 80-100 pts.). The initial pain treatment was performed with PCA (piritramide), and because of the lancinating pain character carbamazepine treatment was introduced. The pain intensity increased under carbamazepine (VAS = 100 pts.), and after treatment with five cycles of salmon-calcitonin infusion the pain intensity decreased (VAS = 10 pts). After withdrawal of the infusion therapy with salmon calcitonin the pain intensity increased up to VAS = 70 pts. TENS therapy five times per day showed no analgetic effect. We repeated the calcitonin-infusion therapy and after five i.v. cycles we continued with 200 I.U. salmon calcitonin intranasal per day. The initial phantompain intensity decreased (VAS = 40 pts.), but showed no long term analgesia. The additional psychological treatment with relaxation techniques (Jacobson/Bensen) showed the desired phantom pain relief. An interdisciplinary and multimodal cooperation between anesthesiologists, trauma surgeons, neurosurgeons and psychologists is needed for successful phantom pain treatment after traumatic brachial plexus lesion. Intravenous salmon calcitonin showed only short-term analgetic effect.
The therapy of acromioclavicular dislocations remains controversial. In particular, for injuries classified as Rockwood/Tossy Type III good results have been reported with different operative techniques as well as with conservative treatment. The objective of this study was to obtain data about the current treatment for Rockwood/Tossy III injuries in German trauma departments. In a countrywide anonymous survey 210 German trauma departments were asked about their diagnostic procedures and their treatment strategies for acromioclavicular injuries. 104 questionnaires (49%) were returned and evaluated. In Rockwood/Tossy I/II injuries most clinics recommend conservative treatment (Rockwood/Tossy I/II: 99%/87%). On the other hand, 84% of the clinics would operate on Type III acromioclavicular injuries--especially in athletes or overhead workers. Although 38 percent of the clinics believe that conservative treatment is equal or better than operative treatment, only 13 percent manage Type III injuries conservatively. For more severe acromioclavicular injuries (Rockwood IV to VI) all clinics recommend an operative treatment. The operative techniques of choice for acromioclavicular injuries are K-wire fixation (37%) or a coraco-clavicular cerclage (32%). Of the latter, 73% use a resorbable material, while the remainder use wires.
INTRODUCTION:There is no general agreement on the operative treatment of displaced proximal humeral fractures. While T-plate fixation was the method of choice until the end of the 1980s, minimally invasive techniques have been favoured during the past decade. The indication for primary shoulder prosthesis is controversial. The purpose of this report was to evaluate the scientific evidence of current treatment recommendations.METHODS:Relevant articles were retrieved from "Medline" and "Knowledge-Finder" using the combined search strategy for the keywords "proximal humerus" and "fracture". Retrieved articles were evaluated according to the criteria of evidence-based medicine.RESULTS:The analysis retrieved 3 randomized, 4 prospective, and 26 retrospective studies and a number of review articles relevant to the subject. Limitations of most publications were due to small study populations, differences in patient selection and fracture classification as well as measurements of outcome. According to these studies good functional results can be achieved in dislocated two-part fractures treated with minimal osteosynthesis in the elderly and T-plate fixation in younger patients. For three- and four-part fractures minimally invasive techniques seem to be more favourable in the elderly. However, there is some evidence that alternative therapies such as conservative treatment and plating can be successful in defined populations. A general indication for primary implantation of a prosthesis in four-part fractures in the elderly is not supported by the literature.CONCLUSION:We conclude from our analysis that the scientific evidence for treatment recommendations of displaced proximal humeral fractures is still limited.
