Die präoperative Nüchternheit für klare Flüssigkeiten ist um ein Vielfaches länger, als von den Fachgesellschaften empfohlen. Ziel der Arbeit ist, ein liberales Flüssigkeitsregime zur präoperativen Karenz ausgewählter klarer Flüssigkeiten mittels standardisierter Nüchternheitskarten zu implementieren sowie erste Rückmeldungen zu dessen Umsetzung zu erheben. Ein liberalisiertes Flüssigkeitsregime, welches das Trinken von Wasser, Apfelsaft, Tee und Kaffee bis zum Abruf in den OP erlaubt, wurde mittels Nüchternheitskarten implementiert. Acht Monate nach der Einführung erfolgte eine repräsentative Befragung der Leitungen chirurgischer Stationen zum Konzept. Alle Stationsleitungen befürworten das Trinken klarer Flüssigkeiten bis zum Abruf in den OP, fast alle die Umsetzung dieses Konzeptes mittels Nüchternheitskarten. Neun von 11 Stationen empfinden die auf ihre Operation wartenden Patienten entspannter; eine i.v.-Flüssigkeitssubstitution wird seltener angefragt. Ausnahmslos alle Stationsleitungen möchten im Falle einer eigenen Operation nach dem neuen Nüchternheitskonzept behandelt werden. Patienten sollte das Trinken von hypotonischen klaren Flüssigkeiten bis kurz vor der Operation erlaubt werden, um Komplikationen einer zu langen Nüchternheit zu vermeiden. Dabei sollte das Konzept klar strukturiert, für alle transparent, schriftlich fixiert und den Patienten ohne Sprachbarriere zur Kenntnis gebracht werden.
Einleitung Erkrankungsschwere bei CED lässt sich aufgrund des sehr variablen meist schubweisen Verlaufes unzureichend über die aktuelle Entzündungsaktivität und Lebensqualität beschreiben. Für eine umfassendere Beschreibung steht ein arztbasierter Schwere-Index zur Verfügung (DSI. Siegel, 2018).
20% der Bevölkerung leiden an unangenehmen oder schmerzhaften Blähungen. Sie können durch eine Vielzahl abdominaler Erkrankungen verursacht werden. Darüber hinaus sind Blähungen eines der Kardinalsymptome des Reizdarmsyndroms.
Frage 1: Was ist die beste Methode für die Beobachtung von Patienten mit chronisch-entzündlicher Darmerkrankung (CED)? Sonografie, Endoskopie, Labor oder einfach klinisch? Böcker: Das beste Mittel für die Tumor-Surveillance ist die Endoskopie, für die Überwachung auf Strikturen sind es die klinischen Symptome, für die Beobachtung des (distalen) Entzündungsgeschehens ist es das Calprotectin im Stuhl. Ehehalt: Der Goldstandard ist wohl die Endoskopie, aus praktischen Gründen liegt aber die klinische Untersuchung in Verbindung mit Laborwerten vorn. Götz: Tumor-Surveillance: Endoskopie; Ansprechen auf eine Behandlung: klinisch; Labor: häufig; Sonografie: wird geprüft, wahrscheinlich gut (Darmwanddicke, Limberg-Score); Endoskopie: optimal, aber invasiv und besonderen Situationen vorbehalten. Hasselblatt: Der beste Ansatz zur routinemäßigen Beobachtung von Patienten mit CED ist klinisch in Kombination mit Calprotectin (Wert sollte <250 sein). Bei Symptomen nehmen wir Ultraschall hinzu, Endoskopie wenn nötig (oder indiziert; z.B. zur Tumor-Surveillance und wenn der Befund die Therapieentscheidung beeinflusst), gegebenenfalls auch MRT. Kreis: Jede davon in angemessenen Intervallen. Kroesen: Das hängt vom Zustand des jeweiligen Patienten ab. Im Allgemeinen sind Sonografie und klinische Untersuchung ausreichend. Wenn der Patient plötzlich schlechter wird oder zunehmend stenotische Beschwerden hat, kann eine Endoskopie erforderlich werden, vor allem wenn eine Operation oder endoskopische Dilatation als Behandlungsoption näherrückt.
Hintergrund/Einleitung: Aldehyde haben potentielle Kofaktorfunktion in der Pathogenese inflammatorischer und neoplastischer intestinaler Erkrankungen.
Hintergrund: Frühere Untersuchungen zeigten einen möglichen Einfluss der Blutgruppe auf die Häufigkeit und den Verlauf der Hepatitis C Infektion. In dieser Studie wollten wir einen möglichen Einfluss des AB0-Blutgruppenssystem auf den Fibrosegrad und auf das Ansprechen auf eine antivirale Therapie untersuchen.
Hintergrund: Akut infektiöse Gastroenteritiden und Enterocolitiden sind ein häufiges Krankheitsbild. Die Erregerdiagnostik ist zeitaufwendig und nicht immer erfolgreich. Ziel dieser Untersuchung war es, potentiell geeignete Parameter für die Unterscheidung zwischen bakteriellen und viralen Erregern zu identifizieren.
Hintergrund: In den letzten Jahren ist die Häufigkeit Clostridium difficile-assoziierter Erkrankungen angestiegen, verbunden mit den entsprechenden medizinischen und ökonomischen Folgen. Entscheidend für eine erfolgreiche Therapie ist nicht nur die frühe Erkennung, sondern auch eine individualisierte, im Bedarfsfall eskalierte Therapie, die sich an prädiktiven Markern orientiert.
PURPOSE:Models of colon cancer in small rodents are of particular interest as they most closely simulate the development and growth of colonic cancer in humans. Micro-computed tomography has been used for detection of polyps in murine models of colon cancer. The study was performed to evaluate whether a novel high-speed continuous-rotation, single-breath-hold scanning protocol in combination with double-contrasting of the colon can be successfully applied for colonoscopy of live mice at acquisition times of 40 s.METHODS:C57BL/6JApcMin/+ mice were intubated and ventilated. After double-contrasting the colon with barium and air, mice underwent continuous rotation micro-CT (mean resolution 41 × 41 × 53 μm) during a single-breath-hold period of 40 s. Sensitivity to detect colon polyps by four blinded radiologists was analysed. Number and location of polyps were verified in the excised colon. Radiation dose was measured using a thermoluminescence dosimeter placed within the distal colon.RESULTS:In six of seven mice, a total of 12 polyps were detected in the explanted colon (one mouse without polyps). One tumor (8.3%) was located in the proximal third, seven tumors (58.1%) and four tumors (33.2%) were located in the middle and in the distal third of the colon, respectively. Mean tumor volume was 6.5 ± 3.6 mm(3). Sensitivity to detect colon polyps was 0.85 ± 0.1. Mean radiation dose was 0.241 ± 0.002 Gy.CONCLUSION:Using a high-speed continuous rotation micro-CT protocol, double-contrast single-breath-hold colonoscopy in mice is feasible and yields sufficient contrast to visualize the proximal colonic folds and to detect colonic polyps in vivo.
In September 2009, the German "Standing Committee for Vaccination" (STIKO) recommended the H1N1 influenza vaccination to all patients with chronic diseases. We investigated the adherence to this recommendation in patients with the inflammatory bowel diseases (IBD) Crohn's disease or ulcerative colitis. Special attention was paid to arguments for vaccination refusal.In an explorative multicenter study we asked adult patients to answer a questionnaire about their participation in the H1N1 vaccination campaign, their arguments for and against this vaccination and disease specific parameters.Out of 1389 participating patients, 226 (16 %) received the H1N1 vaccination. Among patients who were vaccinated against the seasonal flu as well as patients who were treated with anti-TNF-treatment and members of the German Crohn's and Colitis Association, the participation rates were significantly higher (32 %, 26 %, 25 %, respectively). The main argument against the H1N1 vaccination was fear of side effects (59 %). However, 77 % of all vaccinated patients judged the vaccine as very well tolerated. The non-adherence to general vaccination recommendations against tetanus and seasonal flu was also high (25 % and 66 %, respectively).Only a minority of patients with Crohn's disease or ulcerative colitis had adhered to the official recommendation concerning vaccination against H1N1. In order to reach higher acceptance, further vaccination campaigns must focus on the safety of the recommended vaccine.
In a survey comprising 1,176 patients with inflammatory bowel disease (IBD) we recently showed that azathioprine (AZA) beyond 4 years is beneficial in ulcerative colitis (UC) patients and in a subset of Crohn's disease (CD) patients. Here, we show for the first time that azathioprine responsiveness depends on body mass index (BMI). The relationship is reciprocal in UC and CD, with a better outcome in UC patients with a BMI<25 and in CD patients with a BMI>25. These observations are particularly interesting considering the evolving concept of a relationship between fatty metabolism and immune regulation. Additionally, we show that CD patients, but not UC patients, respond better to AZA when it is started in clinical remission. This observation may support data favouring a "hit hard and early" regime in CD. Finally, we were able to demonstrate a decrease in the incidence of CD-related complications requiring surgery through treatment with AZA.
BACKGROUND:Liver transplantation is the only definitive treatment for end stage liver disease. Donor organ scarcity raises a growing interest in new therapeutic options. Recently, we have shown that injection of monocyte-derived NeoHepatocytes can increase survival in rats with extended liver resection. In order to apply this technology in humans with chronic liver diseases in an autologous setting, we generated NeoHepatocytes from patients with alcoholic liver disease and healthy controls and compared those to human hepatocytes.METHODS:We generated NeoHepatocytes from alcoholics with Child A and B cirrhosis and healthy controls. Hepatocytes marker expression and transforming growth factor (TGF)-beta signaling was investigated by RT-PCR, Western blot, immunofluorescent staining, and adenoviral reporter assays. Glucose and urea was measured photometrically. Phase I and II enzyme activities were measured using fluorogenic substrates. Neutral lipids were visualized by Oil Red O staining.RESULTS:There was no significant difference in generation and yield of NeoHepatocytes from alcoholics and controls. Hepatocyte markers, e.g., cytokeratin18 and alcohol dehydrogenase 1, increased significantly throughout differentiation. Glucose and urea production did not differ between alcoholics and controls and was comparable to human hepatocytes. During differentiation, phase I and II enzyme activities increased, however remained significantly lower than in human hepatocytes. Fat accumulation was induced by treatment with insulin, TGF-beta and ethanol only in differentiated cells and hepatocytes. TGF-beta signaling, via Smad transcription factors, critically required for progression of chronic liver disease, was comparable among the investigated cell types, merely expression of Smad1 and -3 was reduced (approximately 30 and approximately 60%) in monocytes, programmable cells of monocytic origin, and NeoHepatocytes. Subsequently, expression of TGF-beta regulated pro-fibrogenic genes, e.g., connective tissue growth factor and fibronectin was reduced.CONCLUSIONS:Generation of NeoHepatocytes from alcoholics, displaying several features of human hepatocytes, offers new perspectives for cell therapeutic approaches, as cells can be obtained repeatedly in a noninvasive manner. Furthermore, the autologous setting reduces the need for immunosuppressants, which may support recovery of patients which are declined for liver transplantation.
Hintergrund & Ziel: Patienten mit chronisch entzündlichen Darmerkrankungen können hepatobiliäre Pathologien in Form extraintestinaler Manifestationen oder als Ausdruck von Krankheits- und Therapiefolgen entwickeln. Invasive Untersuchungsmethoden sind nur begrenzt einsetzbar. Ziel der Studie war daher die Erfassung von Krankheitsdaten und die Bestimmung nicht-invasiver Leberschädigungsmarker in einem Kollektiv von ambulanten und stationären Patienten mit M. Crohn.
Fragestellung: Die IL-1R-assoziierte Kinase ist ein kritischer Mediator der Toll-like Rezeptor/Interleukin-1 Rezeptor-induzierten Aktivierung des Transkriptionsfaktors NF-κB, der die Zytokinexpression bei chronischen entzündlichen Darmerkrankungen (CED) moduliert. Kürzlich wurde gezeigt, dass ein funktionaler IRAK-1 single nucleotide polymorphism (rs1059703) – lokalisiert auf dem X-Chromosom – mit einer erhöhten NF-κB Transkriptionsaktivität und Expression NF-κB-abhängiger Zytokine assoziiert ist. Ziel der Arbeit war es zu prüfen, ob der single nucleotide polymorphism rs1059703 für chronisch entzündliche Darmerkrankungen disponiert.
Recently, two functional IL18 promoter variants, −607C>A (rs1946518) and −137G>C (rs187238), were associated with viral clearance in patients with hepatitis C. The present study focused on their relevance for treatment response.
