BACKGROUND AND OBJECTIVE:LPAC (low phospholipid-associated cholelithiasis) syndrome is a rare genetic form of cholelithiasis. ERCP (endoscopic retrograde cholangiopancreatography) is often used to remove gallstones in the bile duct. No published data is available on the role of ERCP in LPAC syndrome. PATIENTS AND METHODS:In this retrospective cohort study, we included patients diagnosed with LPAC syndrome in a single tertiary referral center between 2009 and 2021. Our aim was to assess the frequency, indications, modalities, results, and complications of ERCP, as well as predictive factors for ERCP, in LPAC syndrome. Independent factors associated with ERCP occurrence were identified using a multivariable Cox regression analysis. RESULTS:ERCP was required in 31.2 % of the 269 patients included for analysis. Among patients who required ERCPs, 78.6 % had the procedure before diagnosis (i.e., starting UDCA). Most common indications were choledocholithiasis (53.6 %) and acute cholangitis (29.5 %). Post ERCP pancreatitis, perforation and bleeding rates were 7.2 %, 2.6 %, and 1.3 %, respectively. Age and history of cholelithiasis in first-degree relatives were associated with a higher risk of ERCP (Hazard-ratio [HR]=1.30 [95 %confidence-interval [CI] 1.04-1.62] and HR=1.88 [95 %CI 1.15-3.07] respectively). Female gender and UDCA intake ≥ 1 year were associated with a lower risk of ERCP (HR=0.49 [95 %CI 0.29-0.82] and HR=0.44 [95 %CI 0.22-0.90] respectively). Median follow-up was 10.8 years. CONCLUSION:One-third of patients with LPAC syndrome undergo sphincterotomy. However, most procedures are performed before diagnosis and UDCA is associated with a lower risk of endoscopic procedure. Earlier diagnosis and treatment with UDCA may further reduce the need for ERCP in patients with LPAC syndrome.
Aims Contaminated duodenoscopes have resulted in multiple outbreaks and deaths involving multi drug-resistant organisms. In France the DGOS (health ministry) published an instruction in 2018 to supervise duodenoscopes disinfection and control. The aim of our study was to determine whether the 2018 guideline have decreased duodenoscopes contamination.
Aims EUS-guided gallbladder drainage has become a validated alternative treatment to cholecystitis in patients not requiring surgery.This French multicenter observatory carried out within the GRAPHE lists the cases of gallbladder drainage carried out with the placement of an HOT-AXIOS device (Boston Scientific) under endoscopic ultrasound (EUS) from 2016 to 2022.

The use of luminal apposing metal stent (LAMS) to facilitate the procedure is prospectively poorly established. Aims In case of malignant obstruction of the distal biliary duct, electrocautery-enhanced lumen apposing metal stent (EC-LAMS) allows either endoscopic ultrasound-guided hepaticogastrostomy (EUS-HGS) or choledochoduodenostomy (EUS-CDS) if endoscopic retrograde cholangiopancreatography (ERCP) is not feasible. EUS-CDS is associated with less adverse events than EUS-HGS but can be challenging. Thus, a simple alternative would be welcomed. We aim to assess the efficacy and safety of EUS-guided transmural gallbladder drainage using an EC-LAMS in malignant distal biliary obstruction.
Aims Esophagectomy remains the standard care for a vast proportion of esophageal or esophagogastric lesions.This procedure is associated in 10% of cases with anastomotic fistulas. Endoscopic management of post-operative anastomotic complications has been mostly reported with esophageal stents.Recently, internal drainage by double pigtail stents has been introduced for managing these complications.Our aim was to assess the overall efficacy of the endoscopic treatment for anastomotic leaks after esophagogastric resection for cancer.
La reconnaissance et l'interprétation en temps réel de critères bien définis de lésions détectées en endoscopie est censée guider leur prise en charge. La classification de Paris est l'une des plus simples et doit être utilisée pour décrire chaque lésion. Cependant, des publications récentes ont mis en évidence un corrélation inter-observateur moyenne pour les lésions coliques. L'objectif de cette étude était d'évaluer la performance d'un groupe d'endoscopistes français pour décrire une lésion oesophagienne au sein d'un endobrachyoesophage (EBO) selon la classification de Paris.
Le drainage biliaire sous écho-endoscopie est devenu l'alternative numéro 1 en cas d'échec ou d'impossibilité de CPRE pour les sténoses biliaires tumorales distales. Récemment nous avons évalué l'efficacité et la sécurité du système hot-AXIOS dans une étude multicentrique pilote incluant tous les centres ayant effectué au moins une pose l'année suivant sa commercialisation. Dans cette étude 11,5% des procédures étaient en échec. Le non suivi de la procédure recommandé (ponction directe de la voie biliaire avec le cystotome intégré + utilisation d'une prothèse dédiée (6 mm) + utilisation d'un courant de section pure) était le principal facteur de risque d'échec technique.
Background: The aim of this study was to assess whether magnetic resonance cholangiography (MRC), clinical and biochemical criteria are able to predict improvement after endoscopic treatment (ET) for dominant stenosis (DS) in patient with primary sclerosing cholangitis (PSC).
La cholédocoduodénostomie sous échoendoscopie (CDE) est une alternative au drainage radiologique percutané en cas de tumeur duodénopancréatique obstruant la voie biliaire (VBP) avec échec de cathétérisme biliaire (CPRE). Le geste est techniquement difficile avec un taux de succès de 94% pour un taux de complication de 19% (1). Un nouveau modèle de prothèse d'apposition entièrement couverte couplée à un cystotome (Hot Axios Boston Scientific) simplifie et sécurise la procédure. Les résultats de la première étude rétrospective internationale sont prometteurs (2). Le but de notre étude est d'évaluer rétrospectivement les résultats de l'ensemble des procédures effectuées en France depuis la commercialisation du produit en avril 2016.
Le risque d'échec d'hémostase endoscopique par méthodes classiques (sclérose au sérum adrénaliné, clips) est d'environ 10%. En particulier, certaines conditions techniques, l'existence de troubles de l'hémostase ou le type de lésion hémorragique peuvent rendre l'hémostase endoscopique difficile. Récemment, l'utilisation d'une poudre hémostatique a permis de traiter des hémorragies digestives avec des résultats prometteurs chez l'homme dans les études pilotes. Les buts de ce travail prospectif multicentrique étaient de déterminer 1) la faisabilité de l'application de la poudre hémostatique en pratique clinique de routine, et 2) son efficacité à court et moyen terme dans différentes situations cliniques.
Endoscopy has an ever-increasing role in the treatment of complications in digestive surgery. Endoscopic treatment is essentially used for (i) fistula or intra-abdominal collection secondary to anastomotic dehiscence and (ii) anastomotic stricture, especially esophagogastric, but also sometimes after colorectal surgery. First intention treatment of fistula following esophagogastric surgery is the insertion of an extractable self-expandable metallic stent (partially or entirely covered); this is supported by a low level of scientific evidence, but clinical experience has been satisfactory. Other techniques for treatment of anastomotic leak have also been reported anecdotally (clip placement, sealing with glue). There are few data available in the literature on endoscopic management (stents essentially) of postoperative colonic fistula. Whatever the approach chosen to treat a postoperative digestive tract fistula, management is medico-surgical and cannot be limited to simple closure of the digestive tube wall defect. Drainage of any collections by endoscopic, radiologic or surgical approach, systemic treatment of infection and nutritional support are essential adjuvant treatment modalities. Treatment of postoperative esophageal or colonic strictures is essentially endoscopic and is based on initial dilatation (endoscopic with hydrostatic balloon or bougie), and placement of extractable metallic stents (partially or entirely covered) in case of refractory outcome.
L’endoscopie occupe une place croissante dans le traitement des complications de la chirurgie digestive. Le traitement endoscopique s’envisage essentiellement : en cas de fistule ou collection intra-abdominale secondaire à un lâchage d’anastomose, et en cas de sténose anastomotique, surtout à l’étage œsogastrique, mais parfois dans les suites d’une chirurgie rectocolique. Les fistules de la chirurgie œsogastrique sont en première intention traitées par pose de prothèse auto-expansible métallique extractible (partiellement ou totalement couverte), avec un niveau de preuve scientifique faible, mais une expérience clinique satisfaisante. D’autres techniques ont également été rapportées de façon anecdotique (pose de clips, encollage). Peu de données sont disponibles dans la littérature sur la prise en charge endoscopique (prothèses essentiellement) des fistules coliques postopératoires. Quelle que soit l’approche choisie pour traiter une fistule digestive postopératoire, la prise en charge doit rester médicochirurgicale, et ne saurait se limiter à la seule fermeture de l’effraction de la paroi digestive. Le drainage d’éventuelles collections (endoscopique, radiologique ou chirurgical), le traitement systémique de l’infection et l’assistance nutritive sont des adjuvants incontournables. Le traitement des sténoses œsophagiennes ou coliques postopératoires est essentiellement endoscopique et repose sur les dilatations (endoscopique par ballon hydrostatique ou par bougie) en première intention, et la pose de prothèses métalliques extractibles dans les situations réfractaires (partiellement ou totalement couvertes).
Introduction: Le diagnostic étiologique des sténoses biliaires reste difficile avec une rentabilité diagnostique de l'ordre de 50% des techniques usuelles (aspiration, biopsies, brossage biliaire). De nouvelles techniques, la cholangioscopie et l'endomicroscopie confocale, ont montré, de manière isolée, un apport diagnostique dans la caractérisation des sténoses biliaires. Le but de cette étude était d'évaluer la combinaison de la cholangioscopie selon le système Spyglass® et de l'endomicroscopie confocale par sonde Cellvizio® dans le diagnostic des Sténoses Biliaires d'origine Indéterminée (SBI).
Introduction: Le traitement endoscopique classique des sténoses biliaires bénignes consiste à mettre en place une ou plusieurs prothèses biliaires plastiques après sphinctérotomie endoscopique. Ce traitement est inconstamment efficace et s'accompagne d'une morbidité et d'un inconfort liés aux fréquents gestes endoscopiques nécessaires. L'objectif de ce travail était d'évaluer prospectivement la faisabilité de la mise en place et de l'extraction endoscopique de prothèses métalliques expansives entièrement couvertes (PMEC) dans cette indication. Objectives - In 2004; the French health authorities published guidelines on the indications for colonoscopy However, no study has evaluated the awareness of healthcare practitioners of these guidelines The aim of this study was to determine the level of awareness of the Anaes guidelines among French gastroenterologistsPatients and methods - A questionnaire comprising 20 multiple choice questions (MCQ) was presented to a group of 79 gasi roenterologists between February and June in 2008 The questions covered screening tests 1 or colon cancer (one question), endoscopic mucosa resection (two questions) and the Anaes guidelines (17 questions) According to the number of colonoscopies performed per year (less than 100, 100-500, more than 500), the answers to these questions were analyzed separatelyResults - Among the practitioners carrying out less than 100, 100-500 and more than 500 colonoscopies per year, the guidelines for colon cancer screening were known by 33, 50 and 56%, respectively, the quality criteria for endoscopic mucosal resection by 0, 0 and 3 7%, respectively, and the Anaes guideline indications for colonoscopy by 34 3, 51 2 and 48 9%, respectively (P < 0 001) The Arises guidelines were significantly better known by practitioners who were performing more than 100 colonoscopies per year, while the indications for con trot colonoscopy were less often correctly anticipated No differences were found concerning postponed indicationsConclusion - The Anaes guidelines consists of the following elements (1) awareness of the Anaes guidelines is poor, with control colonoscopy being correctly anticipated in just over a third of the gastroenterologists, (2) performing more than 100 colonoscopies per year improves knowledge of the Anaes guidelines, and (3) the Anaes guidelines need to be simplified and should be covered by continuing medical education (c) 2010 Elsevier Masson SAS All rights reserved

La technique FICE utilise un algorithme qui sélectionne des longueurs d'ondes précises et reconstitue une image de façon instantanée Le but de ce travail a été de décrire la sémiologie endoscopique du système FICE après application d'acide acétique chez des patients porteurs d'un EBO avec et sansdysplasie de haut grade (DHG) ou des carcinomes intra muqueux sur oesophage de Barrett et ainsi d'évaluer l'apport diagnostique de cette technique.
neumatosis cystoides intestinalis is characterized by the resence of gas-filled cysts within the intestinal wall below ither the mucosal or the serosal membrane. This rare and enign disease, first described in 1730 by Du Vernois [1], ccurs predominantly between 40 and 50 years of age in oth men and women [2]. Cysts are located along the entire astrointestinal tract from the esophagus to the rectum 3] but are usually predominant in the small as well as he large bowel [2,4]. The pathophysiology of the disease emains unknown. The diagnosis is made during colonoscopy ith the visualization of cysts appearing as round heightenngs in the colonic wall membrane, although a laparotomy s the only way to diagnose cysts located below the erosa [5]. In most cases, cysts are totally asymptomatic. When ymptoms are present, they are not specific, i.e., abdominal ain, intermittent rectal bleeding, diarrhea or constipation. e report a case where constipation was the predominant ymptom. In this patient, the symptomatic evolution after yst treatment suggested that cysts were in fact responsible or functional distal constipation.
