Background: Congenital heart diseases (CHD) are responsible for substantial morbidity and mortality in neonates. The preliminary diagnosis often is made by noncardiologists. For this reason, there is a huge demand of training in echocardiography of CHD. This is difficult to achieve due to limited resources of specialized centers. Objective: The goal of this study was to investigate the training effect of the echocardiography simulator EchoCom on trainee's ability to diagnose CHD. Design/Methods: We enrolled 10 residents for simulator-based training in echocardiography of CHD. All participants were instructed on the simulator's basic handling and had one hour to scan the first 9 datasets information (ventricular septal defect, atrial septal defect, atrioventricular septal defect, Tetralogy of Fallot, transposition of great arteries, congenital corrected transposition of great arteries, common arterial trunk, hypoplastic left heart syndrome, normal anatomy) and establish a diagnosis. No help was given except for support regarding simulator related issues. Afterward, 2 rounds of structured simulator based echocardiography training focused on echocardiographic anatomy, spatial orientation, standard views, and echocardiographic anatomy of different CHD followed. All participants completed a standardized questionnaire containing 10 multiple-choice (MC) questions focusing on basic theoretical knowledge in echocardiographic anatomy and common CHD. Results: Almost all of the residents invited from the affiliated children's hospital had little (20%) or no experience (80%) in echocardiography of CHD. Their Pretest and Posttest scores showed significant improvement for both, MC test and performance test, respectively. Conclusions: Our study showed that simulator-based training in echocardiography in CHD could be very effective and may assist with training outside the scope of CHD.

Background. Surgical management of patients with complete atrioventricular septal defect (AVSD) has advanced over the last decades. Definitive early surgical repair for AVSD has become the treatment of choice at many centers. This trend has contributed to the recent decline in postoperative mortality and good long-term results.Methods. We reviewed long-term results of 100 consecutive patients with complete AVSD undergoing definitive early repair with a two-patch technique and complete cleft closure operated on between June 1999 and June 2009. Valve performance, mortality, morbidity, and indications for reoperation were retrospectively studied.Results. Median age at operation was 3.8 months (range, 15 days to 4.7 years); median weight was 5.0 kg (range, 3.0 to 19 kg). Follow-up was 99% complete (mean 58 months; range, 1 to 120 months). Early definitive repair was performed in all patients who initially presented to our institution over the study period. There was no perioperative, in-hospital mortality, or late mortality. At the latest follow up, left atrioventricular valve (LAVV) regurgitation was absent or trace in 80 patients (80%), mild to moderate in 15 patients (15%), and moderate to severe in 4 patients (4%). Right AV valve regurgitation was none or trace in 86 patients (86%), mild to moderate in 11 patients (11%), and moderate to severe in 2 patients (2%). Actuarial freedom from reoperation for LAVV dysfunction at 1, 3, 5, and 10 years was 98%, 95%, 94%, and 94%, respectively. Actuarial freedom from reoperation for left ventricular outflow tract obstruction at 1, 3, 5, and 10 years was 100%, 99%, and remained constant by 99% at 5 and 10 years.Conclusions. Definitive early repair for complete AVSD can be performed with excellent results. The two-patch technique with complete cleft closure is a safe and reproducible surgical method that can achieve very low mortality and stable long-term outcomes, even in neonates. We present the case of a male full-term neonate who presented at 4 h of age with severe cyanosis unresponsive to mechanical ventilation and oxygen supplementation. The diagnosis of rupture of the papillary muscle of the tricuspid valve was made by echocardiography. Preoperative stabilization was achieved by adding nitric oxide and extracorporeal membrane oxygenation therapy could be avoided. Corrective surgery was successfully performed on the 7th day of life. Follow-up examination at 2 months of age revealed an excellent postoperative result. Perinatal rupture of the papillary muscle and/or chordae tendineae of the tricuspid valve is exceedingly rare and usually lethal when untreated or diagnosed late. Therefore, timely diagnosis is mandatory for adequate preoperative management and subsequent surgical treatment.
AIMS:To analyse left ventricular (LV) synchrony and function with respect to the epicardial pacing site in the young.METHODS AND RESULTS:Left ventricular function and synchrony (M-mode, speckle tracking) were evaluated during mid-term follow-up in 32 children with complete non-surgical (n = 15) or surgical (n = 17) atrioventricular block (structural heart disease in 21/32) paced from LV apex (n = 19), right ventricular (RV) apex (n = 7), and RV free wall (n = 6), respectively. Data are in the following order: LV apical, RV apical, and RV free wall pacing. Septal to posterior wall motion delay (SPWMD) = median 0, 69, and 136 ms (P < 0.001), septal to lateral mechanical delay = 54 +/- 29, 73 +/- 24, and 129 +/- 70 ms (P = 0.001), apical to basal mechanical delay = 96 +/- 37, 106 +/- 50, and 79 +/- 18 ms (P NS), and LV ejection fraction (LVEF) = 57 +/- 9, 49 +/- 12, and 33 +/- 10% (P < 0.001), respectively. Left ventricular ejection fraction correlated negatively with SPWMD (R(2) = 0.454, P < 0.001) and septal to lateral mechanical delay (R(2) = 0.320, P < 0.001) but not with apical to basal mechanical delay. Right ventricular free wall pacing (P = 0.014) and SPWMD (P = 0.044) were negative multivariable predictors of LVEF.CONCLUSION:Compared with other sites, LV apical pacing preserves septal to lateral LV synchrony and systolic function and may be the preferred epicardial pacing site in the young.
Zusammenfassung Unter den wichtigen technischen Neuerungen der pädiatrischen Echokardiografie in den letzten 5 Jahren drängt sich als Quanten-sprung das Verfahren der dreidimensionalen Echtzeit-Echokardiografie (real-time three-dimensional echocardiography [RT3DE]) in den Vordergrund. Sie ermöglicht eine realitätsnahe Darstellung von Morphologie und Bewegung des Herzens mit nahezu beliebigen Betrachtungsvarianten. Hinzu kommt eine verbesserte Funktionsbeurteilung der Ventrikel, insbesondere des geometrisch schwer fassbaren rechten Ventrikels. Die Optionen für die Nachbearbeitung haben sich erweitert. Nach 2½-jährigen eigenen Erfahrungen mit 450 untersuchten Patienten im klinischen Routinebetrieb zeichnen sich wichtige Vorteile für Operationsplanung und Indikationsstel-lung für Operation oder Katheterintervention ab. Anhand von typischen Einzelfällen bzw. Fallgruppen werden sie erläutert. Die RT3DE hat auch die Entwicklung von Trainingssimulatoren für die Echokardiografie ermöglicht. Eine neue Methode der Funktionsdiagnostik mit erheblichem Entwicklungspotenzial ist das Speckle-Tracking-Verfahren, das die Beurteilung des segmentalen Deformations-ablaufs durch die Berechnung von zweidimensionalem Strain (2DS) ermöglicht und von uns vor allem für die Analyse mechanischer Dyssynchronie im Rahmen der kardialen Resynchronisationstherapie eingesetzt wird.
Hintergrund: Die kritische TI beim Neugeborenen ist eine Rarität. Die in der Literatur (<10 Fälle) berichtete Mortalität ist hoch. Fallbericht: Nach unauffälligem Schwangerschaftsverlauf (inkl. Feindiagnostik) und Geburt fiel ein Neugeborenes 4 Stunden postnatal durch eine schwere Zyanose auf. Unter maschineller Beatmung (FiO2 100%) lag die Sauerstoffsättigung (SpO2) bei 60%. Echokardiographisch zeigten sich ein verminderter Fluss in der Pulmonalarterie (Qp), eine hochgradige TI, ein normaler Druck im kleinen Kreislauf und ein Rechts-Links-Shunt (RLS) über ein großes Foramen ovale (PFO). Aufgrund einer echoreichen Struktur an der Trikuspidalklappe (TK), die als Rest eines Sehnenfadens interpretiert wurde, konnte die Verdachtsdiagnose einer RPM gestellt werden. Unter Therapie mit inhalativem Stickstoffmonoxid kam es zu einer Zunahme des Qp und einem Anstieg der SpO2 auf 80%. Nach Rekompensation erfolgte am 7. Lebenstag die Operation–bei der die Verdachtsdiagnose der RPM bestätigt wurde – mit Rekonstruktion der TK (Implantation von GoreTex-Schleifen zwischen dem Stumpf des PM und der TK, DeVega Anuloplastik) und PFO-Verschluss. Intra- und postoperativer Verlauf waren komplikationslos. Auch bei der Nachuntersuchung im Alter von 2 Monaten zeigte sich ein sehr gutes OP-Ergebnis mit nur geringer TI. Diskussion: Die RPM ist eine sehr seltene Ursache für eine kritische TI beim Neugeborenen. Der RLS Shunt über das PFO führt zu einer schwerer Zyanose, trägt aber zur Aufrechterhaltung des Systemkreislaufs bei. Frühzeitige Diagnosestellung sowie adäquate chirurgische Strategie und perioperatives Management sind unerlässlich, um bei dieser schweren Erkrankung eine günstige Prognose zu erzielen.

Aims Wolff-Parkinson-White syndrome with right septal. or posteroseptal accessory pathways causes eccentric septal mechanical activation and may provoke left ventricular (W) dyssynchrony and dysfunction. The aim of the study was to evaluate the effect of radiofrequency catheter ablation (RFA) of the accessory pathways on LV function.Methods and results Retrospectively, transthoracic echocardiography and electrocardiogram recordings were analysed in 34 patients (age: 14.2 +/- 2.5 years) with right septal. or posteroseptal accessory pathways prior and after (median: 1 day) successful RFA. Results prior to RFA, LV ejection fraction was decreased (<55%) in 19/34 patients (56%). After RFA, QRS duration was normalized (129 +/- 23 vs. 90 +/- 11, P < 0.0001), LV function improved (ejection fraction: 50 +/- 10 vs. 56 +/- 4%, P = 0.0005) and septal-to-posterior watt motion delay as a global measure for LV dyssynchrony decreased (110 +/- 94 vs. 66 +/- 53, P = 0.012). Longitudinal two-dimensional strain evaluated in five patients demonstrated a decrease of left intraventricular mechanical delay from 292 +/- 125 to 118 +/- 37 ms after RFA.Conclusion Wolff-Parkinson-White syndrome with right septal or posteroseptal accessory pathways may cause LV dyssynchrony and jeopardize global LV function. Radiofrequency catheter ablation resulted in normalized QRS duration, mechanical resynchronization, and improved LV function. Even in the absence of arrhythmias, RFA of right septal or posteroseptal pathways may be considered in patients with significantly decreased LV function.
Einleitung: Der katheterinterventionelle Verschluss von Vorhofseptum-Sekundum-Defekten durch Implantation eines Device (D-ASD) ist bei – meist asymptomatischen – älteren Kindern ein Routineverfahren. Über Sicherheit und Ergebnisse der Methode bei kleineren Kindern ist wenig bekannt. Wir untersuchten daher die Ergebnisse des D-ASD in dieser Patientengruppe. Methodik: Retrospektive Analyse der Krankenakten von 2001 bis 2007. Eingeschlossen wurden alle Kinder mit einem Körpergewicht von unter 10kg und/oder einem Alter von unter 2 Jahren, bei denen die Indikation zum D-ASD gestellt wurde. Ergebnisse: Die Indikation zum D-ASD wurde bei 16 Kindern gestellt. Der Median des Alters, des Gewichts und der Körpergröße lag bei 11 (3–28) Monaten, 7 (3,5–12,0)kg bzw. 67 (48–87)cm. Die Indikationen waren Gedeihstörung (5), Atemnot (6), Pulmonale Hypertension (3) und Zyanose (2). Weitere Diagnosen waren restriktiver Ventrikelseptumdefekt (3), Ductus arteriosus Botalli (PDA; 2), Kardiomyopathie (1), palliierte Pulmonalatresie mit intaktem Ventrikelseptum (1), Scimitar-Syndrom mit funktionellem Fehlen einer Lunge durch pulmonale Sequestration (1), Bronchopulmonale Dysplasie (3), Bronchusstenose (1), Tracheomalazie nach Korrektur einer Ösophagotrachealen Fistel (1), Omphalozele (1), Hydronephrose (1), Zerebrale Malformation (1), Analatresie (1), Trisomie 21 (1) und ein nicht klassifiziertes Syndrom (1). Die Defektgröße betrug 5–19 (Median 8)mm. Die Intervention wurde unter Durchleuchung und echokardiographischer Kontrolle durchgeführt. Bei allen Patienten gelang die erfolgreiche Implantation eines Amplatzer Septal Occluders® (Größe 5–20, Median 9mm). Bei 4 Patienten wurden weitere Interventionen durchgeführt: PDA-Verschluss (2), Verschluss eines chirurgisch angelegten Shunts (1), Verschluss von einen Lungensequester versorgenden Arterien (1). Die Dauer der Eingriffe und die Durchleuchtungszeit lagen im Median bei 114 (58–215)min. bzw. 19,9 (0,8–29,2)min.. Bei 3 Patienten traten frühe Komplikationen auf. Dabei handelte es sich um einen Verschluss der V. iliaca (1) und um eine – unter verlängerter Heparintherapie regrediente – Obstruktion der A. femoralis (2). Bei Entlassung waren bei 3 Patienten (19%) kleine Restdefekte nachweisbar. Die Nachbeobachtungszeit betrug im Median 46 (1–85) Monate. Bei der Nachuntersuchung nach einem Jahr waren keine Restdefekte nachweisbar (0/13). Späte Komplikationen traten nicht auf. Alle Kinder, die wegen einer Gedeihstörung oder Atemnot behandelt worden waren, zeigten eine deutliche Verbesserung der Symptome. Bei zwei Patienten wird noch eine pulmonale Hypertension medikamentös behandelt. Schlussfolgerung: Sowohl das Spektrum der Indikationen zum D-ASD bei Kindern unter 10kg und/oder unter 2 Jahren wie auch das der Begleiterkrankungen ist ausgesprochen vielfältig. Der D-ASD stellt bei ausgewählten Patienten dieser Gewichts- bzw. Altersgruppe eine sichere und effektive Alternative zum chirurgischen ASD-Verschluss dar.
Background: Echocardiography is one of the most important diagnostic imaging modalities in paediatric cardiology. Owing to the large number of lesions, achieving expertise often requires years of training. Echocardiography is still taught using the apprenticeship model, which is time- and personnel consuming. Objectives: To extend the echocardiography simulator EchoCom to enable simulation of congenital heart lesions and validate it for training in paediatric echocardiography. Methods: The simulator consists of a life-size manikin, a dummy transducer with attached three-dimensional (3D) tracking system and a computer application. Transthoracic real-time (RT) 3D echocardiographic datasets were collected and embedded into the simulator. Two-dimensional images were calculated and resliced from these datasets according to the position of the tracking sensor. Ten RT 3D datasets of congenital heart lesions were selected for validation. Datasets were blinded and without additional information presented to 43 participants who were stratified according to their expertise (12 experts, 16 intermediates, 15 beginners). Participants were asked to list the relevant findings and make a diagnosis. Construct validation was tested comparing diagnostic performance for each group. Face and content validation were tested using a standardised questionnaire. Results: Participants judged the simulator as realistic and useful. The main drawback was the adult size of the manikin. The diagnostic performance of each group differed significantly proving construct validity. Conclusions: According to this validation the prototype simulator could make a significant contribution to training in the use of echocardiography in congenital heart disease.
We are describing a most unusual variant of aortic arch interruption. The ascending aorta was atretic above the sinutubular junction forming a blind-ending pouch. The coronary arteries were the only vessels arising from the aortic trunk. All the brachiocephalic vessels were perfused in retrograde fashion through the arterial duct. The aortic valve itself was dysplastic, stenotic, and permitted severe regurgitation. There was no subaortic obstruction. A modified first stage Norwood palliation was performed. Due to myocardial insufficiency transthoracic extracorporeal membrane oxygenation had to be commenced at the end of surgery. Unfortunately the child died due to severe intracranial hemorrhage.
A 37-year-old man presented with Eisenmenger's reaction. He had progressive cyanosis and signs of right-sided heart failure. In early childhood he was palliated with a Waterston shunt for cyanotic congenital heart disease. Two-dimensional echocardiography provided only a rudimentary view of his heart's anatomy and hemodynamics. Real-time three-dimensional echocardiography (RT3DE) was superior in visualizing both the size and the shape of the large shunt. RT3DE easily identifies and quantifies a key pathogenic factor for pulmonary hypertension with irreversible vascular damage in such patients. If applied early, RT3DE may prove to be a valuable follow-up tool.
