SummaryBackgroundGlecaprevir/pibrentasvir is a pangenotypic direct‐acting antiviral regimen approved for treating adults chronically infected with hepatitis C virus (HCV). There are limited real‐world data on glecaprevir/pibrentasvir to date.AimTo evaluate the effectiveness and safety of glecaprevir/pibrentasvir under real‐world conditions in the German Hepatitis C‐Registry (DHC‐R).MethodsThe DHC‐R is an ongoing, non‐interventional, multicentre, prospective, observational cohort study that monitors patients with chronic HCV infection. Data were collected from patients who initiated glecaprevir/pibrentasvir and completed a screening visit on or after 2 August 2017. The primary effectiveness endpoint was sustained virological response at post‐treatment Week 12 (SVR12). Safety and tolerability were also assessed.ResultsAs of 15 July 2018, 586 patients received glecaprevir/pibrentasvir and had documented SVR12 data, treatment discontinuation, loss to follow‐up or HCV reinfection. Five hundred and fifty‐two patients (94%) received on‐label treatment. At baseline, most on‐label patients were infected with HCV genotype 1 (53%) or 3 (33%), HCV treatment‐naïve (90%), without cirrhosis (94%), and treated for 8 weeks (93%). Five hundred and thirty‐four patients (96.7%) achieved SVR12 (intention‐to‐treat [ITT] analysis). By modified ITT analysis (excluding patients who discontinued and did not achieve SVR12 or patients lost to follow‐up), the SVR12 rate was 99.4% (n/N = 534/537). There was one documented virological failure (relapse) and two documented HCV reinfections. One hundred and forty‐two (26%) adverse events (AEs) and 9 (2%) serious AEs occurred; 2 (<1%) AEs led to treatment discontinuation. All patients treated off‐label (N = 34) achieved SVR12.ConclusionGlecaprevir/pibrentasvir was highly effective and well tolerated under real‐world conditions. Die koformulierten direkt antivial wirksamen Substanzen Glecaprevir (entdeckt von AbbVie und Enanta) und Pibrentasvir (G/P) sind zur Behandlung der HCV-Infektion zugelassen. Aktuelle Real-Life-Daten zeigen hohe Wirksamkeit und Sicherheit von G/P; Lebensqualitätsdaten (PROs) sind jedoch nur begrenzt verfügbar – insbesondere für wichtige Subgruppen für die HCV-Elimination. Wir berichten Real-Life-Daten aus dem Deutschen Hepatitis C-Register (DHC-R) zur Sicherheit und Wirksamkeit von G/P und PROs in Patienten mit psychiatrischen Erkrankungen, Alkohol Abusus, aktivem Drogengebrauch, mit Opioid Substitutionstherapie (OST) und/oder HIV-Koinfektion. Das DHC-R ist eine nicht-interventionelle, multizentrische, prospektive Kohortenstudie. Die Dokumentation erfolgte 135 deutschen Zentren. Einschluss war möglich für Erwachsene GT1 – 6-Patienten mit einer Behandlung laut Zulassung. Der primäre Endpunkt war SVR12, welcher bei Patienten mit mindestens einer G/P-Dosis erhoben wurde. Sicherheit, Verträglichkeit und PROs wurden auch erfasst.

Background and Aims: Tenofovir DF (TDF) represents in clinical studies a very effective and tolerable CHB treatment and is a recommended first-line therapy.Knowledge on long-term efficacy and safety in daily practice is yet still limited.Methods: 400 monoinfected TDF-naïve CHB patients were prospectively enrolled into this observational study. 2 year data are reported here.Results: At BL, mean age was 45 years, 69% male, 76% Caucasian, 11% cirrhotic, 69% HBeAg-, 46% treatment-naïve.92% received TDF monotherapy, 14% had liver biopsies; comorbidities: diabetes (16%), arterial hypertension (14%), cardiovascular (11%).Median HBV-DNA at baseline was 6.9exp4 in naïve, 2.6exp2 in pretreated patients.Virologic response at 24 months (HBV-DNA <169IU/ml): HBeAg+ patients (naïve/pretreated) 88%/94% (BL: 12%/41%), HBeAgpatients 94%/97% (BL: 10%/52%), in ADV+LAM (n = 57) and in ETV (n = 41) pretreated patients 100%/96% (BL: 61%/46%), respectively.20% with HBeAg seroconversion, HBsAg-loss in 5.1% of HBeAg+.No virologic breakthrough (confirmed >1log increase from nadir) and no resistance reported to date. 4 patients added another drug (naïves added LAM, LAM, ETV, 1 LAM/ETV-experienced added ETV), 7 patients switched to another drug (6 ETV, 1 LAM) all due to adverse reactions (fatigue, headache, nausea).TDF retention rate was 92%, >7 day TDF pause was documented in 24 patients, primarily for non-compliance (n = 7), stay abroad/vacation (n = 6).Mean±SD ALT [U/L] reduction BL-month 24: 78±121-36±22; ALT elevations in HBV-DNA-suppressed patients are more likely in obese (BMI≥30 kg/m 2 ).Serum CrCl (107±32-102±31 mL/min) and phosphorous (1.11±0.2-1.07±0.2mmol/L) remained stable.No frequent AR reported.4 renal events were detected, all in patients with prior long-term LAM +/-ADV therapy, and comorbidities: diabetes (2x), renal insufficiency (2x), cirrhosis (1x).Prior to the occurrence of these 4 AR, TDF had not been dosed according to recommendation.5 pregnancies during TDF were reported, in 4 TDF was given in all trimesters and in 1 in 2 nd /3 rd , all newborns were HBsAg-and healthy.Conclusions: After 2 years in routine practice TDF poses a very effective treatment for CHB.TDF profoundly suppressed HBV replication in the majority of naïve and pretreated patients, the safety profile was favorable.TDF should be dosed according to SmPC.
Hintergrund und Ziele: TDF stellt in klinischen Studien eine sehr wirksame Therapie der CHB dar. Kenntnisse über Langzeitwirksamkeit und Verträglichkeit in der Praxis sind jedoch limitiert.
Hintergrund: Hepatitis C Patienten mit Zirrhose gehören zu den am schwierigsten behandelbaren Patienten. Welche Prädiktoren geben Aufschluss über einen möglichen Therapieerfolg einer interferon-basierten Therapie?
Einleitung: Tenofovir (TDF) stellt eine effektive und sehr gut verträgliche Behandlungsoption bei CHB dar, belegt aus Zulassungsstudiendaten über 5 Jahre. Das Wissen um die Langzeitwirksamkeit und -sicherheit im klinischen Alltag ist jedoch begrenzt.
AIM: Pegylated interferon-alpha (PEG) and ribavirin (RBV) is the mostly used treatment for patients with chronic hepatitis C (CHC) leading to a strong and in many patients sustained viral response (SVR). In this study we analyzed the value of weight loss under PEG/RBV treatment as an easy to recognize on-treatment predictive factor besides viral response. Methods: We conducted a Phase IV, multi-center, open-label, non-interventional study with treatment of Peginterferon alfa-2a and ribavirin. 1100 patients treated with a complete course of CHC therapy according to consensus recommendations (GT-1/4/5/6 for 48±4wks, GT-2/3 for 24±2wks). For these patients descriptive, univariate and multivariate analyses were performed to determine on-treatment predictive factors associated with SVR. Results: In 737 genotype 1/4/5/6 infected patients the SVR rate was 39.3%. However, patients with a weight loss ≥2 kg under therapy achieved an (P=0.000, OR=2.361, 95%-CI: 1.639 – 3.403) statistically significant increased SVR of up to 44.4% whereas patients with a lower weight loss <2 kg showed only a SVR of 25.3%. In 363 patients with genotype 2/3 the overall SVR rate was 64.5% and in case of weight loss of more than 2 kg they achieved a SVR of 68.8% whereas patients who lost less than 2 kg showed a SVR of only 55.2%. A multivariate regression analysis revealed that EVR, RVR, age and weight loss under therapy but not genotypes have been positive and significant on-treatment predictors for SVR. Conclusion: The present study demonstrates that weight loss is a reliable on-treatment predictive factor for estimating treatment outcome.
Studies showed that SVR in patients with low viral load (LVL) is high and treatment duration can be reduced while maintaining high SVR rates. But is this true under real life conditions? Methods: Between 2008 and 2011 data of 3810 patients completing a treatment period of 6 months for GT 2/3 or 12 months for GT 1 plus 6 months follow-up were recorded in a noninterventional study. This analysis included 745 naïve patients of G–1/2/3 with RVR and achievement of planned end of therapy, 165 patients of them got shortened treatment: GT 1 20–28 wks (n=110) and GT 2/3 (n=55) 14–18 wks in comparison to standard duration of 40–52 wks in GT 1 and 22–26 wks in GT 2/3. Results: 64.8% male, 38.7 yrs old, BMI 24.8kg/m², mean duration of infection 10.5 yrs. There were no major differences between genotypes or patient demographics with shorter vs. standard therapy. Standard treatment duration were 47.8 and 24.0 wks in GT 1 and GT 2/3 patients whereas shortened therapies were 24.4 and 15.9 wks, respectively.110/278 patients (39.6%) with GT1 and 55/467 patients with GT 2/3 (11.8%) got a shorter treatment. 91.7% of GT 1 and 96.4% of GT 2/3 patients with shortened treatment had LVL ( 800,000 IU/ml). Patients with standard treatment duration and RVR had low viral load in 59.5% of GT 1 and 61.1% of GT 2/3 patients. Rates of AEs and SAEs were 68.2%, 1.8% in GT 1 and 58.2%, 1.8% in GT 2/3 with shorter therapy and 70.2%, 3.6% in GT 1 and 62.1%, 0.7% in GT 2/3 with standard therapy. Conclusion: In real life patients with RVR were considered eligible for shorter therapy only with LVL in the majority of cases. These patients achieved high rates of SVR, similar to standard therapy in patients with RVR. Shorter treatment duration led to better tolerability especially in GT 1 patients. This analysis with very high SVR rates particular in GT 1 patients argues for a 4 week lead-in PEG/RBV period to identify RVR patients before starting triple therapy with protease inhibitors.
Hintergrund: TDF ist eine Erstlinientherapie zur Behandlung der CHB. GEMINIS soll die Patientencharakteristika unter TDF in der Behandlungspraxis beschreiben sowie das Profil der Langzeitwirksamkeit und Sicherheit schärfen.
Eine RVR nach 4 Behandlungswochen und das Nicht-Erreichen einer pEVR nach 12 Wo. sind etablierte prädiktive Faktoren. Kürzlich wurde der Rückgang der Viruslast nach 8 Wo. als prädiktiver Faktor untersucht. Der BV Niedergelassener Gastroenterologen Deutschlands e.V. (bng) führt zusammen mit Roche eine Beobachtungsstudie zur Untersuchung der Qualität der CHC-Behandlung unter Alltagsbedingungen durch. Bis April 2010 wurden Daten von 6005 GT 1-Patienten gesammelt, die mit PEG und RBV behandelt wurden. Bei 1399 der 6005 Patienten lagen zusätzliche HCV RNA-Ergebnisse nach 8 Wochen vor. Mit einer ROC-Analyse wurden die besten Cut-offs und log-Rückgänge der Viruslast nach 8 Wochen bestimmt. Zusätzlich wurde mit einer MLR-Analyse ermittelt, welche HCV RNA-Veränderung nach 8 Wochen (complete oder partial Response) die beste Vorhersage erlaubte. Mit einer zweiten MLR wurde der prädiktive Wert des virol. Ansprechens nach 8 Wo. im Hinblick auf eine SVR mit den etablierten Kriterien wie RVR, pEVR und cEVR verglichen. Die mit der ROC-Analyse ermittelten besten Cut-offs (Sensitivität u. Spezifität) nach 8 Wo. waren: Rückgang der HCVRNA -1.99 log, absolute HCV RNA-Schwelle 11,500 IU/ml. In der ersten MLR war pVR8 (p<0.001, OR 3.078 (95%CI 2.166–4.374) der beste Prädiktor der SVR. In der zweiten MLR (N=491 Pat. mit vollstg. Daten zu allen Zeitpunkten) waren cEVR (p<0.001, OR 6.862, 95% CI 4.358–10.803) und pEVR (p=0.031 OR 1.902, 95%CI 1.060–3.412) die besten Prädiktoren der SVR vor RVR, pVR8 und cVR8. Das virol. Ansprechen und die dazugehörigen PPV/NPV für jeden Parameter sind in der Tabelle 1 dargestellt.
Hintergrund: Seit der Erstpublikation des IL28B-Wirtspolymorhismus und der Korrelation des Genotyps mit der SVR durch Ge et al. im August 2009 sind zahlreiche Untersuchungen und Auswertungen zu möglichen Einsatzformen des IL28B veröffentlicht worden. Obgleich in Deutschland die Messung des IL28B zurzeit keine kassenärztliche Leistung darstellt, wird der Test mittlerweile vielfach eingesetzt.
Eine Analyse (Sulkowski et al) zeigte, dass der Hb-Abfall unter PEG/RBV zu Nebenwirkungen und RBV Reduktion führt, aber gleichzeitig mit erhöhter Wahrscheinlichkeit für eine SVR assoziiert ist. Da Frauen einen niedrigeren Hb-Ausgangswert als Männer haben und weit häufiger anämisch werden, sollte die beschriebene Assoziation in dieser Population noch ausgeprägter zu finden sein.
Hintergrund: Studien zeigten unterschiedliche Ergebnisse in Bezug darauf, ob ein größerer Hb-Abfall unter Therapie mit Peginterferon/Ribavirin mit einer höheren SVR assoziiert ist.
05g: VIRAL HEPATITIS -g) HEPATITIS C -CLINICAL (THERAPY) S239 and higher baseline HCV-RNA levels (0.25 [0.13-0.51]<0.001) were the only independent predictors of lack of SVR.Conclusions: SVR is achieved at similar rates using pegIFNa-2a and pegIFNa-2b, when given along with weight-based RBV dosing.PegIFNa-2a seems to exert greater antiviral potency during therapy than pegIFNa-2b, counterbalanced with more frequent severe toxicity and relapse rate.
