Die patellofemorale Dysbalance manifestiert sich meist in zwei Symptomen: vorderer Knieschmerz und Patellaluxationen. Die Ursachen sind vielfältig. Oft übersehene Ursache können Torsionspathologien des Femur (Innentorsion) bzw. der Tibia (Außentorsion) sein. Hierunter leidet die Sportfähigkeit der Patienten massiv. Therapie der Wahl ist die kniegelenknahe Torsionskorrekturosteotomie. Bisher gibt es noch keine Publikationen über klinische Ergebnisse nach derartigen Operationen. Bei 24 Patienten (18 weiblich, 6 männlich; Alter 15 bis 47 (Median 30,1) Jahre) wurden acht beidseits operiert, damit erfolgten 32 Torsionsosteotomien gesamt. Bei jedem Patienten wurde neben der klinischen und radiographischen Untersuchung inklusive Ganzbeinaufnahme ein Torsionswinkel-CT durchgeführt. Der Nachuntersuchungszeitraum betrug 38 (4–90) Monate. Neben der klinischen Nachuntersuchung wurden VAS, Japanese Knee Society Score, Tegner-Aktivitätsscore und Lysholm Score ermittelt. In 28 Fällen bestand präoperativ ein vorderer Knieschmerz, elf Patienten hatten anamnestisch Patellaluxationen erlitten, acht davon fast täglich. 12 mal wurde eine supracondyläre Femur-Außentorsionsosteotomie (11 ° (5–20)) durchgeführt, 18 mal eine supratuberositäre Tibia-Innentorsionsosteotomie (9,3 ° (5–15)), zweimal wurde bifokal umgestellt. Bei 19 Varus-Deformitäten wurde eine Valgisation, bei zwei Valgus-Deformitäten zwei Varisationen durchgeführt, zweimal erfolgte zusätzlich eine Tuberositas Medialisierung. Neun Patienten konnten aufgrund der Beschwerden bisher nie Sport treiben. 15 Patienten nannten an Sportarten Jogging, Walking, Inlineskaten, Radfahren, Tennis, Tischtennis, Fußball, Volleyball, Skifahren und Karate, alle mussten präoperativ die Sportausübung reduzieren oder komplett einstellen. Postoperativ erreichten sieben Patienten ihr präoperatives Maximalniveau wieder, zwei Patienten konnten ihr Niveau steigern. Der Tegner Aktivitätsscore lag präop bei 3,8, zum Nachbefragungszeitpunkt bei 4,4. Der Lysholm Score verbesserte sich von 57,1 auf 82,6, der Japanese Knee Society Score von 66,3 auf 86,6 Punkte. Der VAS konnte von 7,6 präoperativ auf 2,6 nach OP gesenkt werden. Bei den elf Patienten mit Patellaluxationen wurde nach OP keine einzige Reluxation beobachtet. 86% der Patienten gaben an, sie würden sich nochmals für die OP entscheiden. Torsionspathologien der unteren Extremität induzieren patellofemorale Dysbalancen. Die Pathologie muss erkannt und genau analysiert werden. Therapie der Wahl ist die individuell maßgeschneiderte kniegelenknahe Torsionsosteotomie. Bemerkenswert ist die Reluxationsrate von 0% bei zuvor oft täglichen Patellaluxationen. Deutlich ist die postoperative Schmerzreduktion, die Reduktion der VAS beträgt 5 Punkte. Die Aktivität der Patienten im Alltag und Sport hat sich verbessert (Tegner Aktivitätsscore). Die ebenfalls deutlichen Steigerungen im Japanese Knee Society Score und Lysholm Score dokumentieren den Therapieerfolg.
El sistema de cierre asistido con vacío (CAV) comprende una espuma, un sistema de drenaje, una película adhesiva y un sistema de vacío. La presión negativa en la interfase espuma/herida reduce el edema intersticial, mejora la perfusión tisular y, de ese modo, estimula la formación del tejido de granulación, con un efecto de retracción de la herida. También reduce la carga bacteriana e inflamatoria en las heridas infectadas. Se utiliza en las siguientes indicaciones: traumatología (síndrome compartimental, fractura abierta, pérdida de partes blandas, fijación de un injerto cutáneo, quemaduras), heridas infectadas agudas/crónicas (por ejemplo, aflojamiento de prótesis infectadas, celulitis, fascitis necrosante, infección postesternotomía), escaras sacras y trocantéreas, úlceras venosas, mixtas y diabéticas. Está contraindicado en las superficies deslizantes (cartílago, serosa peritoneal) y no debe utilizarse en los siguientes casos: hemorragia excesiva, presencia de pus en la espuma o en el sistema de drenaje, dolor excesivo o falta de respuesta tisular. El CAV es una técnica de eficacia comprobada para el tratamiento de las heridas complejas.
OBJECTIVEThe purpose of this retrospective study was to evaluate arthroscopy in upper ankle osteoarthritis.METHODSIn the period from 1988 to 1997, 358 arthroscopies of the upper ankle joint were performed in 178 cases due to arthritis. Of 132 accessible patients, 124 replied to our questionnaire: 90 (73%) of the responders underwent clinical examination 94 (37-152) months following the initial arthroscopy and were graded according to the score of Evanski and Waugh. All arthroscopies were performed under fluid filling using anterolateral and anteromedial approaches. In only five cases was the posterior compartment additionally investigated. Generally, neither tourniquet nor mechanical joint distraction were applied.RESULTSOf a total of 12 (6.7%) complications, only 3 (1.7%) hematomas needed surgical revision. One hematoma was due to an arteriovenous fistula treated by double ligation. All complications including five hypesthesias were temporary and subsided spontaneously. The 124 patients of the questionnaire group assessed the result of arthroscopic surgery as excellent in 11%, good in 46%, fair in 22%, and poor in 21%. Of all patients, 22% required further surgery of the upper ankle: in 9% further arthroscopy, in 3% arthrodesis, and in 10% microsurgical denervation. The total range of motion increased from 10/0/40 degrees (extension/flexion) preoperatively to 15/0/44 degrees at the follow-up examination. The Evanski score improved significantly (p<0.001) from 41 to 76 points.CONCLUSIONDue to minimal invasiveness and low risk of complications, arthroscopy is recommended for the following indications of upper ankle osteoarthritis: focal arthrosis, limited range of motion caused by osteophytes, soft tissue impingement, corpora libera, and synovitis. Severity and extent of upper ankle arthritis, range of motion, pain, local bone and soft tissue quality as well as the age, physical activity and compliance of the patient concerned are decisive for the individual therapeutic protocol. Alternative surgical techniques in upper ankle osteoarthritis are assessed such as denervation, distraction arthroplasty, correction osteotomy, ankle arthrodesis and total ankle replacement.
Background: Estimation of trauma severity currently relies on clinical diagnoses and scoring systems. However, the early estimation of the severity of chest trauma and overall soft tissue trauma (STT) remains insufficient. Traditional trauma scoring systems fail to reflect the individual trauma pattern and severity, neglecting the different outcomes after injuries in different body regions. Therefore, the aim of this prospective study was to detect laboratory markers that may reflect the pattern and extent of individual trauma in the very early phase after injury. Patients and methods: In 107 non-selected trauma patients, blood samples were collected almost immediately and then at short intervals after the trauma. In addition to the biochemical analysis of 20 different mediators viewed as potential trauma markers, the following data were correlated with the laboratory results: injury severity score (ISS), polytrauma score (PTS), Ulmer score HTAPE (trauma pattern specific: head (H), thorax (T), abdomen (A), pelvis (P), extremities (E); 0–3° each), multiple organ failure score (MOF), overall, primary and secondary lethality. Results: ISS and the severity of head injury were clearly higher in non-survivors (n=17) than in survivors (n=90) (median ISS: 35 versus 18; median severity of head injury (H): 3 versus 1). Whereas head injury was correlated with early death (≤3 days: r=0.45), late death (>3 days post-trauma) was influenced by thoracic trauma (r=0.15) as well as by soft tissue trauma (STT, r=0.12). Of all investigated mediators, interleukin-6 (IL-6) displayed the highest correlations (r=0.66, P<0.00001) with the extent of chest trauma, followed by correlations with PTS, STT, fracture trauma (FT) and ISS during the first hour after trauma. There was no correlation between IL-6 and head injury. The extent of STT was correlated best to IL-8 (r=0.75), IL-6 (r=0.54), and creatin kinase (CK, r=0.49) plasma concentrations. Conclusion: In the very early stage after an accident the severity of chest trauma is strongly correlated with the plasma concentration of IL-6, and the extent of overall soft tissue trauma (STT) to plasma concentrations of IL-8, IL-6, and CK.
Zusammenfassung Fehlheilungen am diaphysären Unterschenkel lassen sich im Einzelnen folgendermaßen charakterisieren: · Infekt, oberflächlich/tief, · Pseudarthrose, · Defekt, · Deformität. Auf die einzelnen Pathologien, deren Genese und auf entsprechende Therapieoptionen wird exemplarisch eingegangen.Probleme und Lösungsmöglichkeiten bei der Korrektur diaphysärer Unterschenkeldeformitäten werden anhand eigener Ergebnisse aufgezeigt. Hierzu wurden 50 Korrekturosteotomien prospektiv analysiert. Infekte traten nicht auf, 2 Korrekturosteotomien waren unbefriedigend bzgl. der Achsausgradung und in 6 Fällen erforderten Pseudarthrosen eine erneute operative Revision mit autogener Spongiosaplastik. Auf Probleme der intraoperativen Korrekturkontrolle wird ebenso eingegangen wie auf verschiedene Techniken der Knochendurchtrennung und die Bedeutung einer stabilen Osteosynthese.Typische Fallstricke und Fehler, die derartige Korrekturtechniken belasten können,werden beschrieben.Besondere Aufmerksamkeit verlangen hierbei Verfahrenswechsel von einer ursprünglich externen auf eine interne Osteosynthese, infratuberositäre Osteotomien,Varisations- und Innentorsionskorrekturen am proximalen Unterschenkel, instabile Implantate sowie inadäquates Material.Von besonderer Bedeutung sind darüber hinaus die individuelle Compliance und Kooperationsbereitschaft des betroffenen Patienten.Sonderfälle wie bilaterale Unterschenkeldeformitäten, die Kombination von Unterschenkeldeformitäten und Arthrosen der Nachbargelenke sowie ipsilaterale Oberschenkel- und Unterschenkeldeformitäten werden dargestellt.
Die Indikation zur Physiotherapie bei Bewegungseinschränkungen im Ellenbogengelenk nach kindlichen suprakondylären Humerusfrakturen wird in der Literatur sehr unterschiedlich gestellt. Um die Effektivität der Physiotherapie nachzuweisen, wurde eine prospektiv randomisierte Studie durchgeführt. Die beiden Gruppen bestanden aus 21 beziehungsweise 22 Kindern mit einer suprakondylären Humerusfraktur Typ Felsenreich II oder III ohne neurovaskuläre Schädigungen. Alle Frakturen wurden offen reponiert und durch eine Kirschner-Draht-Adaptationsosteosynthese von radial stabilisiert. Bei den Kontrollen 12 und 18 Wochen nach dem Unfallereignis konnte eine signifikant bessere aktive Beweglichkeit der Streckung und Beugung in der Gruppe mit wöchentlich 2 - 3-mal Physiotherapie nachgewiesen werden. Dieser Unterschied war nach einem Jahr nicht mehr nachweisbar. In beiden Gruppen befand sich je ein Patient mit einer Bewegungseinschränkung von 15° beziehungsweise 20°. Physiotherapeutische Maßnahmen sind bei Kindern mit einer suprakondylären Humerusfraktur ohne neurovaskuläre Schädigungen nicht effektiv.
Zusammenfassung Die operative Korrektur bei Asymmetrien der unteren Extremitäten ist sehr komplex. Trotz modernster diagnostischer Technik sind die sorgfältige Anamneseerhebung und die körperliche Untersuchung des Patienten nach wie vor der erste Schritt zur Festlegung der weiteren Diagnostik. Die Kenntnis der methodenabhängigen Normwerte, ihrer physiologischen Schwankungen und der intraindividuellen Differenzen sind Grundvoraussetzung. Bei der posttraumatischen Deformität kann man sich an der gesunden Gegenseite orientieren. Ab der 3fachen intraindividuellen Standardabweichung besteht in der Regel die Indikation zur Operation. Die Indikationsstellung darf sich aber nicht nur auf einzelne Messwerte stützen, die Beschwerden und Bedürfnisse des Patienten sowie die Kompensationsmöglichkeiten müssen ebenfalls berücksichtigt werden. Aus der Vielzahl der möglichen Operationstechniken muss die für den Patienten geeignetste gewählt werden. Vor allem bei kniegelenksnahen Umstellungsosteotomien zur Behandlung der medialen Gonarthrose sollte der Patient realistisch über die Erfolgsaussichten und endoprothetische Alternativen aufgeklärt werden. Bei Kindern zwischen 10 und 14 Jahren ist heute die perkutane Epiphysiodese eine zuverlässige minimal invasive Methode zur Korrektur von Längen- und Achsdeformitäten im Bereich der unteren Extremität.
Die Versorgung von Wunden, Frakturen und Luxationen gehört zu den grundlegenden Fertigkeiten der Notfallmedizin. Schon die Kenntnis des Unfallmechanismus erlaubt häufig Rückschlüsse auf das vorliegende Verletzungsmuster.Diese wird durch die rasche Inspektion des Verletzten und der Unfallstelle zur Bewertung von Unfallmechanismus und einwirkenden Kräften ergänzt. Nach den äußerlich sichtbaren Fehlstellungen und Verletzungen werden Frakturen und Luxationen nach Art und Schwere der geschlossenen und offenen Weichteilschäden bewertet. Ziel der präklinischen Therapie ist nach der Stillung starker Blutungen die Wiederherstellung der Extremitätendurchblutung durch Reposition und Lagerung. Schienung und adäquate Lagerung sichern das Repositionsergebnis und ersparen dem Patienten während des Transportes vermeidbare Schmerzen. Darüber hinaus trägt der erstbehandelnde Arzt Verantwortung für die Abschätzung der Dringlichkeit der Reposition von Luxationen und die Abschätzung der Replantationsindikation von Amputationsverletzungen.

Bei keinem Patienten zeigte sich eine eingeschränkte Beweglichkeit im Bereich des Hüft- oder Kniegelenkes, verglichen mit der gesunden Gegenseite, insbesondere fand sich auch kein Streckdefizit im Kniegelenk. 5 der 13 Patienten (38%) wiesen keine Deformitäten auf. Bei den anderen 8 Patienten (62%) fanden sich folgende ein- oder mehrdimensionale Deformitäten (verglichen zum kontralateralen, gesunden Femur): 2 Patienten hatten eine Varus- und 5 Patienten eine Valgusdeformität in einer Größenordnung von bis zu 10 Grad. Sagittale Deformitäten traten nicht auf. Bei 5 Patienten entwickelte sich posttraumatisch eine Verkürzung am Femur (8–19 mm), bei 2 Patienten fand sich eine Femurverlängerung von 10 bzw. 15 mm. Zusammenfassungg: Bei allen Patienten mit Epiphysenfrakturen am distalen Femur empfehlen wir nach Wachstumsabschluß eine klinische und ggf. auch radiologische und/oder sonographische Kontrolle zum Ausschluß bzw. Nachweis eines Fehlwachstums.
Trauma may cause a relevant reduction in antithrombin (AT) III activity, which is associated with adverse events. The very early changes in AT III activity after accident trauma are still unclear and possible relations with Interleukin (IL)-6, which is known to interact with AT III, have not been investigated so far. Upon approval of the IRB/IEC, 30 patients were enrolled with multiple injuries (ISS 9-75). Groups were performed according to injury severity, IL-6 concentration, and survivors versus non-survivors. Blood samples were collected at the scene of accident then at 2, 4, 6, 12, and 24 h and at day 3, 5, 10 and 15. No patient received AT III concentrates. In all groups a reduction in AT III activity occurred, which was most pronounced in very severe injuries. The activity re-increased spontaneously and steadily in all groups regardless of the IL-6 concentration. There was no clear impact of the AT III activity on survival.
The increase of intramedullary femoral pressure can lead to the intravasation of bone marrow and fat cells into the blood stream of the femoral vein and consequently into the pulmonary circulation. This effect is the same in intramedullary nailing and in the implantation of femoral stem prostheses.In a prospective study we evaluated the intraoperative, intramedullary pressure in the distal femur during the implantation of femoral stem prostheses with two different designs. In eight patients we implanted Muller straight stems and in another eight we implanted stem type Option 3000, Intramedullary pressure was recorded continuously by the implantation of a microtip pressure probe (piezoresistive principle, 50 Hz) in the distal femur. We found markedly higher pressure in Muller straight stem prostheses: range: 590-2,570 mmHg (median=1,293, SD=627 mmHg). Intramedullary pressure in stem prosthesis type Option 3000 was much lower: range: 59-574 mmHg (median=289, SD=219 mmHg). The differences were statistically significant (P=0.0008).By changing the designs of femoral stem prostheses, the intramedullary pressure can be markedly reduced. In the case of elderly patients or those with pulmonary illness we recommend femoral stem prosthesis designs,which induce little increase in the intramedullary pressure, in order to reduce cardiopulmonary complications.

External fixation systems proved to decrease the osteomyelitis rate in patients in the tropics compared with internal stabilization. This study was designed to show how external fixators being used for treatment of patients in industrial countries compare with cheap alternatives regarding their suitability for the application in tropical countries. Eleven external fixation systems were compared for stability, cost, weight, variability, handling, and capability of being produced locally. Stiffness, slipping moment, and irreversible deformation were determined in material testing machines. The technically best fixators are expensive and cannot be manufactured locally. The inexpensive constructs lack variability and stability. When cost is not a problem, the Synthes model is recommended. With some restrictions in mechanical stability and variability, the Pfeifer Fixator II and a wooden model offer inexpensive and locally producible alternatives. These results may help to select an external fixation device to meet local needs and possibilities in tropical countries.
Deformities of the distal femur are usually corrected by supracondylar osteotomy. In the "classical" procedure the bone cut is performed with an oscillating saw, and internally fixed using a plate. This technique is hampered first by an invasive approach and second by limited corrective options in case of complex deformities. A supracondylar bone cut by focal dome osteotomy or drill osteoclasis in combination with internal fixation by retrograde intramedullary nailing (RN) might be a promising alternative procedure. 12 patients with multidimensional post-traumatic deformities of the distal femur were prospectively enrolled in a study to investigate this new minimal-invasive technique. In all patients a meticulous analysis of leg geometry was done pre- and postoperatively. Details of operative planning, osteotomy and fixation procedure are given as well as the postoperative treatment. 7 corrective osteotomies were one-step procedures, in 5 patients additional lengthening over the RN was performed using unilateral external fixation. The mean follow-up was 15 (range 7-27) months. All of the osteotomies healed in a normal expected time frame. All patients had important functional benefits. In 11 patients the goal of deformity correction was achieved. In one patient the correction in the frontal plane remained insufficient. 6 months after the completion of femoral lengthening osteomyelitis developed in one patient, probably due to a pin-track infection. The infection subsided after early removal of the RN. No further complications were observed. The presented technique is demanding concerning pre-operative planning and surgical realization but it offers a minimal-invasive and promising approach for the correction of multidimensional femoral deformities.

During a period of 12 years (1983-1994) we operated 13 patients with fractures involving the distal epiphyseal cartilage of the femur. All patients could be followed up in clinical and radiological examination following the completion of growth. By examination no patients had any complaints. The clinical examination revealed no limitation in range of motion of the hip or knee joint compared to the other side in any of the patients. 5 patients out of 13 (38%) were found to have a correct axial alignment with no axis deviation, shortening or lengthening of the leg. The other 8 patients (62%) presented the following one- or multidimensional deformities (compared to the contralateral, uninjured femur): 2 patients had varus deformities, 5 patients had valgus deformities, the axial malalignment in the frontal plane was always less than 10 degrees in all cases. 5 patients were found to have a shortened femur (between 8 and 19 mm), while an elongation of the primarily injured femur (10 and 15 mm) was established in 2 patients. On the basis of this study, we recommend that clinical and, if necessary, radiological and/or sonographic examinations are performed following the completion of growth in all patients with injuries involving the epiphyseal plate of the distal femur.
Zusammenfassung Intramedulläre Drucksteigerungen im Femur können zur Einschwemmung von Knochenmark- und Fettzellen in das femoralvenöse Blut und konsekutiv die pulmonale Strombahn führen. Dies gilt gleichermaßen für intramedulläre Osteosyntheseverfahren wie auch für die Schafteinbringung bei der Hüftendoprothetik. Im Rahmen einer prospektiven Untersuchung wurden an unserer Klinik intramedulläre Druckänderungen im distalen Femur bei der Implantation von 2 Femurschaftprothesen unterschiedlichen Prothesendesigns und unterschiedlicher Zementiertechnik (8 Müller-Geradschaftprothesen vs. 8 zementierte Femurschaftkomponenten des Typs Option 3000 ® ) gemessen. Hierzu wurde eine Drucksonde (auf piezoresistiver Grundlage) in die distale Femurdiaphyse implantiert und der Druck während der Operation kontinuierlich gemessen (50 Hz). Deutlich erhöhte Werte (590–2570 mmHg) fanden sich bei der Implantation von Müller-Geradschaftprothesen (Median: 1293±627 mmHg). Verglichen hierzu wurden beim Prothesentyp Option 3000 ® lediglich Druckanstiege zwischen 59 und 574 mmHg (Median: 289±219 mmHg) gemessen. Die Unterschiede waren statistisch signifikant ( p =0,0008). Durch die Veränderung des Prothesendesigns bei den Femurprothesen und der Zementiertechnik lassen sich intramedulläre Druckanstiege deutlich reduzieren. Insbesondere bei pulmonal belasteten und multimorbiden Patienten empfiehlt sich in der Hüftendoprothetik die Verwendung von Prothesen mit möglichst geringer intramedullärer Druckentwicklung zur Reduktion kardiopulmonaler Komplikationen.
