通过对清代著名医家林珮琴《类证治裁》与唐宗海《血证论》书中鼻衄篇章的深入研读,探讨二位医家对于本病论治思想的异同.林氏以三因立论,治衄尤擅调气,疗鼻衄顽症颇有心得;唐氏以太阳阳明人手,主治肺肝,外治法别具一格.两位医家均善用虫类药,林氏以“清”为重,唐氏以“通”为要.

目的:探讨清代医家林珮琴的血证论治规律.方法:收集整理林珮琴《类证治裁》血证部分涉及方药,建立数据库,运用频数分析及聚类分析方法行统计.结果:共纳入方剂152首,中药236味,总用药频次为958次;由频数分析得出林氏治疗血证的高频用药有当归(55),人参(48),天门冬(48),甘草(46),白术(41),熟地黄(38),黄芪(31),茯苓(28),川芎(25),生地黄(24)等;由聚类分析得出血证治疗常用药对8组,角药5组.结论:林氏论治血证,善于运用甘味药;熟谙气血关系,重视升降同调;突出从肾论治的同时,注重诸脏协同配合.

Objective:To summarize the ideas and formulas in treatment of pain syndrome of LIN Pei-qin in the Qing Dynasty,and to explore Lin's academic thought and treatment characteristics.Methods:By literature research methods,carefully read chapters in Categorized Patterns with Clear-cut Treatments:Syndrome Differentiation and Treatment on Headache,on Pectoral Stuffiness Pain,on Heartache,on Epigastric Pain,on Hypochondriac Pain,on Abdominal Pain,on Shoulder,Back and Upper Limbs Pain,on Lumbar Vertebrae and Lower Limbs Pain,and on Body Pain,so as to elaborate Lin's academic thoughts,ideas of syndrome differentiation and treatment,and characteristics of medication.Results:LIN Pei-qin followed the classic,absorbed advantages of different sides,and formed his own ideas.In the treatment of pain syndrome,he learned widely from others,but did not repeat others;he examined care-fully the causes,detailed the differentiation,and identified the order of priority;he regulated the five zang organs,focused on treating the liver,and was proficient in the various methods of clearing,dredging,purging and moistening;he advocated warming and dredging methods,and comprehensive treatment,in order to remove cold stasis and deficiency stagnation;he preferred mineral medicine,purging and clearing methods in order to get the concentrated and forceful effect.Conclusion:Lin collected all schools of thoughts,and compiled the strengths of many skilled doctors,combining the sincere inheritance and the great originality.This paper started from the treatment of pain syndrome to appreciate deeply the master's style so as to absorb the essence for the better treatment of the disease in clinical practice.
