PurposeThis study explored the therapeutic potential of puerarin in diabetic atherosclerosis (DA) by targeting endothelial dysfunction and lipid metabolism in apolipoprotein E (APOE)-/- mice.MethodsIn vitro, human aortic endothelial immortalized cells cultured under high glucose conditions were treated with puerarin. Cell viability was quantified using cell counting kit-8 (CCK-8) assay. Apoptosis rates were measured via Annexin V/PI flow cytometry. Lipid accumulation was assessed through Oil Red O staining. iNOS levels were detected by ELISA. In vivo, diabetic APOE-/- mice fed a high-fat diet received daily puerarin administration. Aortic collagen deposition was evaluated using Masson trichrome staining. Plaque burden was analyzed via hematoxylin-eosin staining. Serum lipid profiles, including low-density lipoprotein cholesterol and high-density lipoprotein cholesterol, were determined by enzymatic assays. Follistatin-like 1 (Fstl1) protein expression and downstream inflammatory mediators were examined through Western blot and immunofluorescence.ResultsPuerarin significantly improved endothelial cell survival and reduced apoptosis under high glucose. Lipid droplet formation decreased alongside iNOS suppression. In diabetic mice, puerarin attenuated aortic plaque area and collagen content while improving dyslipidemia. Fstl1 expression and associated inflammatory markers were downregulated.ConclusionPuerarin alleviates DA progression through dual modulation of endothelial protection and Fstl1-mediated inflammatory pathways.
Background: The incidence of endometrial cancer is steadily rising worldwide. Although surgical interventions, particularly comprehensive lymphadenectomy, have improved survival rates, postoperative lymphoceles formation remains a significant clinical complication adversely affecting patient recovery and quality of life. In recent years, the estimation of physiologic ability and surgical stress (E-PASS) scoring system has emerged as a valuable tool in perioperative risk assessment. This system provides a comprehensive evaluation by quantifying patients’ physiological reserves and surgical stress. Although this scoring method has demonstrated value across various surgical disciplines, its potential utility in gynecological oncology remains to be thoroughly explored. Methods: In this retrospective cohort study conducted at Dongyang Hospital affiliated with Wenzhou Medical University, China, we analyzed 180 patients with endometrial cancer who underwent radical surgery between 2012 and 2023. We evaluated the predictive performance of E-PASS components: preoperative risk score (PRS), surgical stress score (SSS), and comprehensive risk score (CRS). Results: Lymphocele developed in 62 patients (34.4%). The CRS demonstrated superior predictive performance (area under the curve [AUC] = 0.930; 95% confidence interval [CI]: 0.893–0.966) with 0.806 sensitivity and 0.915 specificity. Multivariate analysis identified CRS (odds ratio [OR] = 1.161; 95% CI: 1.110–1.214) and advanced International Federation of Gynecology and Obstetrics stage (OR = 3.211, 95% CI: 1.262–8.172) as independent risk factors. Conclusions: The E-PASS scoring system, particularly the CRS, effectively predicts postoperative lymphocele formation in patients with endometrial cancer undergoing radical surgery, potentially facilitating early risk assessment and guiding preventive interventions.
Background: Endometrial cancer (EC) is a common gynecologic malignancy marked by ever-increasing incidence. This study primarily aimed to evaluate the association between preoperative neutrophil-to-lymphocyte ratio (NLR) and lymphovascular space invasion (LVSI), with the secondary objective to assess NLR's prognostic significance for recurrence-free survival (RFS) in endometrial cancer. Methods: This single-center retrospective cohort study included 335 endometrial cancer patients at Dongyang Hospital of Wenzhou Medical University from January 2012 to January 2024. Two distinct analyses were conducted: (1) Cross-sectional association analysis using ordinal logistic regression and restricted cubic spline (RCS) modeling to evaluate the relationship between preoperative NLR and LVSI status; (2) Survival analysis using Cox proportional hazards (CPH) modeling to assess NLR's prognostic significance for RFS and examine prognostic interactions between NLR and LVSI status. Results: Multivariate logistic regression revealed no significant association between NLR and LVSI after comprehensive adjustment (odds ratio [OR] = 1.09, 95% confidence interval [CI]: 0.77-1.53, p = 0.636). Though exploratory spline modeling showed stable risk estimates across the NLR spectrum, formal statistical testing revealed no significant non-linear relationship (p = 0.465). Survival analysis showed borderline significance in patients with superficial invasion (hazard ratio [HR] = 1.56, 95% CI: 1.00-2.43) for RFS, with consistent prognostic patterns across clinicopathological subgroups (all interactionp > 0.05). Conclusion: This study revealed no significant association with NLR and LVSI, indicating no independent predictive value of NLR in LVSI risk stratification. However, patients with superficial myometrial invasion may present borderline risk for recurrence LVSI. Multi-center validation studies are needed to verify the prognostic value of NLR for LVSI risk stratification in clinical settings.
The significant global burden of endometrial cancer (EC) and the challenges associated with predicting EC recurrence indicate the need for a dynamic prediction model. This study aimed to propose nomograms based on clinicopathological variables to predict recurrence-free survival (RFS) and overall survival (OS) after surgical resection for EC. This single-institution retrospective cohort study included patients who underwent surgical resection for EC. Web-based nomograms were developed to predict RFS and OS following resection for EC, and their discriminative and calibration abilities were assessed. 当前中药炮制行业人才依旧缺乏,饮片质量良莠不齐,学校教育要结合产业化需要,学指导产,产检验学.以此为基础,才能促进炮制理论和内涵的深入研究,培养适应社会和学科发展的行业人才.教学团队以产学研结合为导向,在进一步加强课堂教学的基础上,结合企业短期实训和开放实验,对中药炮制实验的教学模式进行了改革和探索.
目的:探讨升阳排毒汤对慢性肾脏病3~4期患者肾功能和微炎症状态的影响.方法:将CKD3~4期脾肾气虚型患者随机分为对照组40例、治疗组40例,均给予西医常规治疗,治疗组在对照组的基础上给予升阳排毒汤治疗,两组均治疗12周.比较两组治疗前后的中医症候积分、肾功能、微炎症指标.结果:治疗后,两组中医证候评分、血肌酐、尿素氮、超敏C反应蛋白、白介素-6、肿瘤坏死因子-α均降低(P<0.05),且治疗组各指标水平低于对照组(P<0.05);两组肾小球滤过率均升高,且治疗组指标水平高于对照组(P<0.05);治疗组总有效率明显高于对照组(P<0.05).结论:升阳排毒汤联合西医常规治疗慢性肾脏病3~4期患者疗效优于单纯西医治疗,可改善临床症状和肾功能,减轻微炎症反应.
[目的]探讨黄芪多糖(Astragalus polysaccharides,APS)对糖尿病动脉粥样硬化大鼠糖脂代谢的影响及一氧化氮(nitric oxide,NO)相关的血管内皮保护机制.[方法]选择无特定病原体(specific pathogen free,SPF)级SD健康大鼠,采用高脂饮食+链脲佐菌素(streptozotocin,STZ)腹腔注射联合维生素D3(vitamin D3,VitD3)灌胃+免疫损伤法建立糖尿病动脉粥样硬化大鼠模型.将未造模的大鼠随机分为空白对照组、APS对照组,造模大鼠分为模型组、治疗组.模型组与治疗组以高脂饲料喂养,空白对照组与APS对照组以正常饲料喂养,模型组和空白对照组给予0.9%氯化钠溶液灌胃,APS对照组和治疗组给予APS 700mg·kg-1灌胃,共灌胃8周.观察记录大鼠健康状态,检测血清空腹血糖(fasting blood glucose,FBG)、 空腹胰岛素(fasting serum insulin,FINS)、总胆固醇(total cholesterol,TC)、甘油三酯(triglyceride,TG)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol,LDL-C)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol,HDL-C)水平,并计算胰岛素抵抗指数(homeostatic model assessment of insulin resistance,HOMA-IR);采用苏木素-伊红(hematoxylin-eosin,HE)染色观察胸主动脉组织病理变化;实时荧光定量PCR(Real-time quantitative polymerase chain reaction,Real-time qPCR)检测胸主动脉组织内皮型一氧化氮合酶(endothelial nitric oxide synthase,eNOS)、 烟酰胺腺嘌呤二核苷酸磷酸氧化酶4(nicotinamide adenine dinucleotide phosphate oxidase 4,NOX4)、 磷酸腺苷活化蛋白激酶(adenosine monophosphate-activated protein kinase,AMPK)的mRNA表达水平.[结果]与空白对照组比较,模型组大鼠一般情况欠佳,出现多食、多饮、多尿症状以及体重减少等现象,FBG、FINS水平及HOMA-IR升高(P<0.05,P<0.01),TC、TG、LDL-C水平增高,HDL-C水平降低(P<0.01).HE染色显示胸主动脉内膜结构受损、钙盐沉积,伴泡沫细胞、炎性细胞浸润,并有斑块形成.Real-time qPCR提示胸主动脉中eNOS、AMPK mRNA表达下降(P<0.01),NOX4 mRNA表达增加(P<0.01).APS对照组FBG、FINS、TC、TG、LDL-C、HDL-C、eNOS水平和HOMA-TR,AMPK mRNA、NOX4 mRNA差异不明显,均无统计学意义(P<0.05).与模型组比较,治疗组大鼠一般情况改善,FBG、FINS水平及HOMA-IR降低(P<0.05),TC、TG、LDL-C降低(P<0.05),HDL-C水平差异无统计学意义(P<0.05).HE染色显示胸主动脉内膜病变改善.胸主动脉组织中eNOS mRNA表达增高(P<0.05),NOX4 mRNA表达降低(P<0.05),AMPK mRNA表达增高(P<0.05).[结论]APS能有效降低糖尿病动脉粥样硬化大鼠血糖、血脂水平,抑制糖尿病动脉粥样硬化形成,其机制可能与激活AMPK信号通路,活化eNOS以及抑制NOX4的表达有关,且APS对健康大鼠无明显影响.
胡臻主任中医师系浙江省名中医、浙江省国医名师,从事临床工作30余年,深入研究中医升阳学说,实践并总结出一套中医气化疗法,主张临床辨证疾病的阳化、阴化太过与不及,并结合气机的升降出入逆乱,治疗多种内科疾病.现将其运用气化疗法治疗下焦病证的经验介绍如下.
During the 2019 coronavirus disease (COVID-19) outbreak, obesity may contribute to COVID-19 transmission and deterioration. In addition, many patients with COVID-19 infection have suffered liver damage which might contribute to a worse prognosis. We conducted a clinical epidemiological analysis to investigate the association of overweight/obesity and abnormal liver function (ALF) with hospitalized duration in patients infected with COVID-19. Fifty-eight patients with diagnosed COVID-19 (22 women & 36 men; average age: 49.2 ± 13.1 yr) were included, and their clinical data were collected at The Second Affiliated and Yuying Children's Hospital of Wenzhou Medical University, Zhejiang. Overweight/obesity was determined as body mass index (BMI) ≥24 kg/m2, ALF was determined as alanine aminotransferase >40 U/L, and prolonged hospitalization was lasting more than the median value of the hospitalized days (19 days) in this population. The proportions of prolonged hospitalization were elevated in patients with overweight/obesity and ALF compared with those without overweight/obesity (62.1% versus 26.1%, P = 0.010) and those without ALF (70.6% versus 41.5%, P = 0.043). Kaplan–Meier analysis showed that the hospitalized duration was increased from the patients with neither overweight/obesity nor ALF to those with either overweight/obesity or ALF, and to those with both of overweight/obesity and ALF (mean with 95% confidence interval: 16.4 [14.5–18.3] versus 25.3 [21.6–29.1] versus 28.3 [24.6–32.0], P for trend = 0.001). Being discharged from hospital in time was inversely and independently associated with BMI (hazard ratio [HR] = 0.75, 95% CI: 0.63–0.90, P for trend = 0.002) and ALT (HR = 0.95, 95% CI: 0.92–0.99, P for trend = 0.007). Present findings suggested that overweight/obesity and/or ALF contributed to predicting a probability of prolonged hospitalization in patients with COVID-19 infection, to whom extra attentions and precautions should be paid during clinical treatments.
目的:分析新型冠状病毒肺炎普通型患者一般情况及中医临床特征.方法:选取2020年2月5日至2月20日温州医科大学附属第二医院收治的新型冠状病毒肺炎普通型患者64例,于入院第1天采集流行病学资料及血常规、C反应蛋白、首发症状、舌象等信息,依据《新型冠状病毒肺炎中医诊疗手册》进行辨证.结果:新型冠状病毒肺炎普通型患者年龄(48.8±12.9)岁,男女比为1.56:1;14例(占21.88%)有武汉/湖北旅居史,38例(占59.38%)有确诊患者密切接触史;15例(占23.44%)患者白细胞计数下降,17例(占26.56%)患者淋巴细胞计数下降,35例(占54.69%)患者C反应蛋白升高;首发症状以发热(42例,占65.63%)、咳嗽(9例,占14.06%)多见,6例(占9.38%)患者无症状;舌象以舌质红黄腻苔(14例,占21.88%)、舌质淡红薄白腻苔(8例,占12.50%)、舌质红白腻苔(8例,占12.50%)多见;中医证型分为寒湿疫毒袭肺证6例(占9.38%)、湿热疫毒蕴肺证58例(占90.62%),湿热疫毒蕴肺证中热重于湿型20例(占34.48%)、湿重于热型38例(占65.52%).结论:新型冠状病毒肺炎普通型的证型以湿热疫毒蕴肺证为主,涉及病理因素包括寒、热、湿、毒、虚,湿毒是核心病机.
Yang sheng therapy is a commonly used treatment for ophthalmic diseases, which can be flexibly applied by reinforcing qi, invigorating the spleen, dispersing the liver, promoting blood circulation, diuresis, heat clearing, toning the kidney, etc. With clinical medications that include shengma, gegen, qianghuo, fangfeng, chuanxiong, chaihu etc. The curative effect has been obtained in the diagnosis and treatment of dry eye combined with blepharospasm, central serous chorioretinopathy, uveitis, and oculomotor nerve palsy. The experience of Professor Hu is summarized in treating ophthalmic diseases with clinical medication characteristics using Yang sheng therapy. It shows that Yang sheng therapy is widely used in traditional Chinese medicine in the treatment of ophthalmic diseases, and its effect is significant.
同行评审作为国际上医学期刊惯用的一种审稿机制,是保障医学期刊质量的关键环节.在“互联网+”时代,如何巧妙地借助互联网的力量,来提高医学期刊同行评审质量,值得深思.笔者在分析医学期刊同行评审问题的基础上,提出了利用互联网信息优势对评审者进行信息审查和身份识别,建立同行评审评价体系和同行评审专家数据库动态化管理机制,并创新同行评审方式等质量控制方法,以期提高医学期刊的同行评审质量,促进医学期刊的可持续发展.
组稿是期刊编辑工作的重要内容,是获取优质稿件、提升学术期刊影响力的重要途径.《中华眼视光学与视觉科学杂志》采用聘请学组专家负责组稿工作的模式进行组稿.编辑通过在选题、稿件组织、审理等过程中发挥主观能动性,协助组稿专家做好组稿工作,组稿结束后做好回报工作等措施,对期刊发展产生较好成效,极大地扩充了期刊的优质稿源.
分析《温州医科大学学报》2009—2018年高被引论文特征,为挖掘优质稿源,组织高质量稿件,提升期刊学术影响力和期刊质量提供参考.通过检索中国知网数据库,《温州医科大学学报》2009—2018年共发表被引频次≥10的高被引论文162篇,集中发表于2009—2015年,论著栏目的高被引论文数最多,共76篇(占46.9%).另外,综述(占18.6%)、Meta分析(占33.3%)、护理研究(占11.6%)、技术与方法(占10.9%)占相应栏目或论文类型载文量的比率较高.编辑可通过优先报道研究热点和学术前沿,适当组织高质量综述和Meta分析,合理设置栏目和加强优势学科组稿,客观看待基金论文,着重关注论文实际学术质量来提高高被引论文数,进而提升期刊的学术影响力和期刊质量.
探究医药大学学报微信公众号的开通情况和运营状况,将2018年版《中国科技期刊引证报告(核心版):自然科学卷》中的医药大学学报56例和中医药大学学报13例纳入本研究,并将已开通微信公众号的学报导入清博大数据,通过分析各项指标发现各微信公众号存在功能建设不完善,推送内容形式单一,消息推送数量少,推送时间不合理等问题.笔者就此提出转变思维模式,优化功能建设,丰富内容形式,优化推送时间,实现多渠道推广等策略,为医药大学学报微信公众平台建设提供思路.
诚信是科研之本,医学研究生作为一个特殊的科研工作群体,其学术诚信建设更要引起社会的广泛重视,而学术诚信建设是学术期刊应承担的社会职责.文章在分析医学研究生学术失信表现及原因的基础上,从期刊视角提出了事前的预防与引导机制、审稿完善机制、责任编辑学术诚信负责制、导师学术诚信负责制、学术诚信控制机制等医学研究生的学术诚信建设机制,以期全方位预防和遏制医学研究生的学术失信行为,净化学术环境,促进期刊的可持续发展.
目的:探讨栀子苷改善糖尿病肾病的作用效果.方法:利用链脲佐菌素(STZ)建立1型糖尿病小鼠模型,随机分为模型组、栀子苷低剂量组(100 mg/kg)、栀子苷高剂量组(200 mg/kg)和正常组.模型组和正常组给予0.9%氯化钠溶液,栀子苷剂量组给予相应剂量栀子苷,连续灌胃8周,观察记录小鼠生活状态,检测体质量和血糖,HE染色观察肾组织病理变化,RT-qPCR法检测TNF-α、IL-6和IL-1βmRNA表达情况.结果:与正常组比较,模型组小鼠生活质量降低,体质量增长缓慢(P<0.01),血糖增高(P<0.01),病理染色显示肾小球结构明显受损,肾间质炎性细胞大量浸润,RT-qPCR法检测结果显示肾组织中TNF-α、IL-6和IL-1βmRNA表达量上升(P<0.01).相较于模型组,栀子苷低、高剂量组小鼠生活状态尚可,体质量增长较快(P<0.05),血糖明显下降(P<0.01),肾组织病理形态明显改善,肾组织中TNF-α、IL-6和IL-1βmRNA表达量下降(P<0.01).结论:栀子苷能有效改善小鼠的糖尿病肾病状态,降低血糖,保护肾组织,其调控机制可能与其能改善糖尿病小鼠炎症状态有关.
目的:了解2017年度国际品牌期刊中医药卫生科技期刊现状.方法:对2017年中国国际影响力品牌期刊中的医药卫生科技期刊进行分析,探讨入选期刊的SCI收录情况、文种、PubMed收录情况等,分析以上因素对期刊发展的影响.结果:2017年国际品牌期刊中医药卫生科技期刊47种,分属18个学科.28种处于TOP 5%,19种处于TOP 5%~10%.SCI期刊19种,均为英文期刊,且分布在TOP 5%内.除2种期刊外,45种期刊被PubMed收录.期刊影响力指数为32.655~534.817(中位数97.242);国际他引频次138~7587(中位数732);国际他引影响因子0.076~9.116(中位数0.858).结论:入选2017年度国际品牌期刊的医药卫生期刊学科分布较均衡,影响力数据各刊差距较大.加入PubMed是中文医药卫生科技期刊提高国际显示度的一个重要途径.
分析《温州医科大学学报》零被引论文的特征,以期降低零被引率,提高学报的整体质量.2011-2012年发表的390篇论文中,有72篇论文的被引次数为零,论文下载频次、是否有基金项目与文章的零被引率关系不大,研究生论文零被引率最高,文章篇幅及学科分布与零被引没有相关性.编辑可通过加强组稿,提高审稿质量和编校质量、加强研究生论文的审核力度、谨慎对待基金论文等措施来降低零被引论文的刊发.
