Introducción El infarto agudo de miocardio (IAM) es una causa importante de muerte en la población argentina. Actualmente, su tratamiento está dirigido a intentar la reperfusión precoz de la arteria ocluida por un trombo. No conocemos estudios, realizados en nuestro país, que permitan estimar cuántas personas necesitan este tipo de tratamiento en una población determinada. Objetivosa) Describir la tasa anual de hospitalizaciones por infarto agudo de miocardio (IAM) en un distrito de la provincia de Buenos Aires en el período 1995-2005. b) Describir la variación de dicha tasa según edad, sexo y época del año. c) Evaluar la mortalidad intrahospitalaria. Población y Material y métodosEl presente es un estudio descriptivo retrospectivo (1995-2005) realizado en el distrito de Coronel Suárez, provincia de Buenos Aires, a partir de las hospitalizaciones de los únicos hospitales que internan pacientes con IAM. Se consideró a todo paciente que presentara un diagnóstico de IAM con supradesnivel del ST, se correlacionó con el censo 2001 del INDEC y se estimó la tasa anual de hospitalización para toda la población, por sexo, grupo etario y estación del año. Para analizar la significación estadística de la diferencia de tasas en hombres y mujeres y entre estaciones se utilizó el intervalo de confianza para la relación entre dos tasas independientes y la prueba de chi cuadrado. ResultadosA lo largo del estudio se identificaron 367 hospitalizaciones por IAM, 275 en hombres y 92 en mujeres. La tasa anual de hospitalización por IAM (cada 10.000 personas) para toda la población fue de 9,06 [8,13-9,98], en hombres fue de 13,8 y en mujeres de 4,47 (razón: 3,06 [2,37-3,76]). La tasa anual mostró una tendencia creciente con la edad. ConclusionesEn nuestra población, aproximadamente 9 personas cada 10.000 habitantes requerirían internación por IAM por año. La tasa de hospitalización es mayor en hombres y en > 65 años, sin diferencias estacionales significativas. La mortalidad intrahospitalaria hallada en el estudio fue del 9%.
IntroducciónEl ejercicio físico es útil para los pacientes con enfermedad coronaria y es un estímulo para el desarrollo de circulación colateral. Ésta podría estar determinada por un aumento en la producción y circulación de células progenitoras. ObjetivoEvaluar el efecto del ejercicio físico programado sobre la producción y el número circulante de células progenitoras en pacientes coronarios crónicos estables. Material y métodosEstudio prospectivo, controlado, aleatorizado y abierto con la inclusión de 18 pacientes (8 en grupo ejercicio y 10 en grupo control) con enfermedad coronaria estable, < 75 años, que no hubieran participado en grupos de ejercicio programado en los últimos 3 meses. La determinación de las células progenitoras se realizó por citometría de flujo utilizando marcaciones con anticuerpos monoclonales CD45-FITC, CD34-FITC y CD133/1-PE. ResultadosEn el grupo control, el nivel de CD45 no tuvo variación significativa (0,724 ± 0,256 versus 0,765 ± 0,216 [media ± EE cada 100.000 eventos]), mientras que en el grupo ejercicio el nivel de CD45(+)/CD133(+) se incrementó de 0,497 ± 0,161 a 2,265 ± 1,003 luego de un mes de actividad física programada sin alcanzar significación estadística. Al analizar los niveles de CD34 se observó que en el grupo control se incrementaron de 0,196 ± 0,086 a 0,235 ± 0,063 (p = NS). En el grupo ejercicio, la variación fue mucho mayor: 0,220 ± 0,078 a 0,844 ± 0,172 (p = 0,0046; p = 0,0092 versus el grupo control). Conclusión El ejercicio físico programado en pacientes coronarios promueve un incremento de las células progenitoras circulantes. Su estímulo persistente podría ser la base para un mayor desarrollo de circulación colateral.
Corneal neurosensory abnormalities cause pain and discomfort in ocular surface disease, yet their pathophysiology is poorly understood. Here we show that in a mouse dry eye model, the ocular (over)activation of transient receptor potential vanilloid 1 (TRPV1) channels in response to tear deficiency and tissue damage promotes neuroinflammatory gene expression and macrophage reactivity in the trigeminal ganglion, where the cornea-innervating sensory neurons are located. This is accompanied by ocular surface macrophage activation, impaired corneal sensitivity to mechanical and non-TRPV1-mediated chemical stimulation, reduced corneal nerve density and the sensitization of ocular TRPV1 channels, thus establishing a vicious neurosensory cycle. Isolated corneal TRPV1 activation without ocular desiccation recapitulates macrophage reactivity, corneal nerve degeneration and trigeminal neuroinflammation, whereas the ocular substance P blockade reverts most of the TRPV1-driven corneal neurosensory abnormalities. Our study identifies a corneal-trigeminal axis that facilitates corneal neurosensory dysfunction and suggests potential targets for the treatment of ocular surface disease-associated corneal neuropathy.
C3 glomerulopathy is an ultra-rare kidney disease driven by dysregulation of the alternative complement pathway fluid phase C3 convertase. We report the case of a previously healthy 13-year-old girl who presented with concurrent nephrotic and nephritic syndrome and low complement C3. Her initial kidney biopsy surprisingly showed mesangioproliferative glomerulonephritis with dominating IgA deposits resulting in a diagnosis of IgA nephropathy. Despite standard immunosuppressive treatment with prednisone and mycophenolic acid plus angiotensin-converting enzyme inhibitors, she achieved only partial remission and subsequently relapsed, exhibiting severe, persistent C3 consumption (6–8 mg/dl) and sustained C5b9 activation (1059 ng/ml). A repeat biopsy 1.2 years later established the definitive diagnosis of C3 glomerulonephritis (C3GN) with a membranoproliferative pattern. C3NEF antibodies were negative, and no pathological variant was detected in the genetic study. After 1 year and 9 months without response to immunosuppressive treatment, the C3 inhibitor pegcetacoplan was initiated. The patient achieved rapid and complete remission within 3 months, marked by normalization of proteinuria, from 2747 mg/day (urine protein-to-creatinine ratio [UPCR] 2.66 mg/mg) pre-treatment to 19 mg/d (UPCR 0.02 mg/mg) and complement activation markers (from 1162 ng/ml pre-treatment to C5b9 92 ng/ml; reference value 30–150 ng/ml). This case highlights the inherent diagnostic complexity of C3GN, suggesting the critical role of repeat biopsies in treatment-refractory, complement-dysregulated glomerulonephritis. It strongly supports the potential efficacy of C3 inhibition as a targeted therapeutic approach for patients with C3GN.
Immunotherapy has introduced a new era in cancer treatment. The clinical goal of cancer immunotherapy is to prime the host immune system to provide passive or active immunity against malignant tumors. We have previously demonstrated that Brucella abortus ( Ba ) RNA downregulates IFN-γ-induced MHC-I surface expression in human monocytes/macrophages via a TLR8-dependent mechanism. Other bacterial RNAs can mimic this phenomenon. The presence and activity of NK cells in tumors have been correlated with better patient survival, supporting the evidence that these cells are essential in the immune response against tumors. So, we postulated that bacterial RNA (bacRNA) can be used to modulate MHC-I expression in tumors to enhance the NK cell response. Hence, the aim of this study was to investigate the immunomodulatory role of bacRNA in solid tumors. We first stimulated human glioblastomas U251 and LN-229, colorectal adenocarcinoma HT-29, breast cancer MCF-7, and murine melanoma B16-OVA cells with bacRNA in the presence of IFN-γ. Our experiments demonstrated that bacRNA diminished IFN-γ-induced MHC-I surface expression in all tumor cell lines. Moreover, the hTLR8 agonist ORN06/LyoVec mimicked the effect of bacRNA, indicating that MHC-I reduction would be mediated by TLR8. In addition, the decrease in MHC-I mediated by bacRNA correlated with increased NK cytotoxicity. Finally, treatment with either bacRNA or the ORN06 agonist resulted in greater immune cell infiltration and activation within the tumor compared to untreated mice. Furthermore, bacRNA delayed tumor growth in the B16 melanoma model. Overall, our established model of MHC-I downregulation (either by bacRNA or synthetic hTLR8 agonists) could be used as a therapeutic strategy to promote anti-tumor responses.
