Purpose Induction chemotherapy is a rather new approach for therapy of Pancreatic adenocarcinoma (PDAC), enabling secondary resectability and an R0-resection as only curative approach to this highly lethal disease. However, chemotherapy-response varies greatly between individuals and is difficult to evaluate by conventional imaging. Molecules within PDACs extensive stroma, such as Fibroblast activation protein (FAP), offer the possibility of molecular markers for evaluation of chemotherapy response. Methods All patients enrolled in our retrospective study underwent surgical tumor resection with one cohort undergoing induction chemotherapy (ICTx) and a matched cohort undergoing upfront resection (US). Survival data derived from the surgical database of Heidelberg University Hospital were compared with histopathological data and FAP immunohistochemistry of tumor tissue derived during resection. Results The ICTx-cohort showed a lower FAP-Expression than the US-cohort and our results indicated a higher stromal component within the ICTx-cohort. Whilst within US no correlation between FAP-expression and stromal component could be made, within ICTx higher FAP levels correlated with a smaller stromal component. FAP-dependent survival also differed between the cohorts: Within US higher FAP levels were associated with shorter survival, while ICTx showed higher FAP levels correlating with improved survival. Conclusion Our results indicate that FAP may be a relevant marker for response to induction chemotherapy in PDAC. It may give significant information on patient survival, depending on clinical context. These preliminary findings need to be confirmed in larger studies.
Background:Climate change increasingly affects public health and presents new challenges for outpatient care. Physicians require specific training opportunities to address these developments. As part of the project AdaptNet ("Adapting primary and specialised outpatient care to the health impacts of climate change"), an online training course on climate change and health was developed to provide essential foundational knowledge on climate-related health risks and adaptation strategies for medical practice. The aim of this study was to evaluate the training in terms of comprehensibility, applicability, and its relevance to medical practice. Methods:The training was provided as an online learning format. The evaluation followed a mixed-methods approach involving 23 general practitioners and specialised physicians from the Northern Bavaria region (Germany). Data collection included a standardised online questionnaire as well as structured group discussions. The analysis was guided by the Consolidated Framework for Implementation Research, with data being evaluated using qualitative content analysis and quantitative descriptive methods. Results:The quantitative analysis showed that the training was perceived as comprehensible, practically relevant, and well-structured. Participants considered the scope and duration of the training to be appropriate. The multimedia design was particularly positively highlighted. The qualitative results indicated increased awareness of climate-related health risks and a high perceived added value for medical practice. The training generated great interest among participants and was considered relevant for outpatient care. The flexible integration of the online format into daily work routines was seen as advantageous. Critical feedback referred to the lack of opportunities for interactive exchange and a desire for additional regional adaptation of the training content. Conclusion:Climate change and adaptation are recognised as relevant topics in medical practice and can be effectively and practically communicated in the outpatient sector through the online training developed specially for this purpose. Regional customisation of the training content appears sensible in order to address climate and health challenges more specifically. Online formats may prove to be suitable tools for effective knowledge transfer.
Ausgedehnte Thoraxwanddefekte können durch die Resektion maligner Tumoren oder durch eine Sternumosteomyelitis nach kardiochirurgischen Eingriffen entstehen. Das Ziel dieser Studie ist es, die Durchführbarkeit der Rekonstruktion mit freien Lappenplastiken unter Berücksichtigung der Anschlussgefäßsituation, operative Komplikationen und das rekonstruktive Ergebnis zu analysieren. In der bizentrischen retrospektiven klinischen Studie wurden alle Patienten eingeschlossen, bei denen im Zeitraum Januar 2013 bis September 2024 eine freie Lappenplastik zur Rekonstruktion eines ausgedehnten Thoraxwanddefektes transplantiert wurde. Das Ziel war es, operative Details und Ergebnisse sowie Komplikationsraten auszuwerten. In einer Subgruppenanalyse wurden außerdem Komplikationen und Ergebnisse von Lappenplastiken, bei denen zusätzlich eine arteriovenöser Gefäßschleife (AVL) als Anschlussgefäß notwendig war, mit denen ohne zusätzliche gefäßchirurgische Maßnahmen verglichen. Insgesamt wurde bei 107 Patienten (n = 66, 62% nach herzchirurgischer Operation; n = 41, 38% nach onkologischer Operation) eine Thoraxwandrekonstruktion mit einer freien myokutanen Tensor-fasciae-latae- (TFL, n = 47, 44%), myokutanen Vastus-lateralis-Muskellappenplastik (VL, n = 37, 35%), kombinierten Vastus-lateralis-Anterior-lateral-Thigh- (kVL-ALT, n = 17, 16%), kombinierten Vastus-lateralis-Tensor-fasciae-latae-Muskellappenplastik (kVL-TFL, n = 2, 2%), einer Anterior-lateral-Thigh- (ALT, n = 2, 2%) oder einer transversen Rectus-abdominis-Muskellappenplastik (TRAM, n = 2, 2%) durchgeführt. Bei 39 Rekonstruktion (36%) war simultan eine AVL-Anlage notwendig. Insgesamt traten postoperativ 6 Thrombosen der Lappenstielgefäße auf (6%). Vollständige Lappennekrosen traten in 3 Fällen (3%; VL, n = 2; kVL-ALTL, n = 1) und partielle Lappennekrosen traten bei 8 Lappenplastiken auf (8%; TFL, n = 3; kVL-ALT, n = 2; VL, n = 2; TRAM, n = 1). Der Vergleich postoperativer chirurgischer Komplikationen zwischen Rekonstruktionen mit und ohne simultane AVL-Anlage zeigte keine signifikanten Unterschiede hinsichtlich venöser Thrombosen (n = 0 vs. n = 3), arterieller Thrombosen (n = 2 vs. n = 1), partieller Lappennekrose (n = 3 vs. n = 5) sowie totaler Lappennekrose (n = 0 vs. n = 3). Diese Studie zeigt, dass die Rekonstruktion ausgedehnter Thoraxwanddefekte mit freien VL- oder TFL-Lappenplastiken mit nur geringen Komplikationsraten durchführbar ist, auch wenn dies simultan eine AVL-Anlage erfordert.
Künstliche Intelligenz (KI) hat sich in den letzten Jahren in unserem Alltag etabliert, z. B. über Sprachassistenten im Handy oder als Chatbot bei einer Terminreservierung. Auch in der kardiovaskulären Medizin überschlagen sich die Meldungen zur Anwendung von künstlicher Intelligenz und Machine Learning (ML). Allein für das Jahr 2024 gibt es bei PubMed über 2000 Ergebnisse unter dem Stichwort „AI in cardiovascular Medicine“. In diesem Artikel sollen einige der neuesten Entwicklungen und Anwendungen von künstlicher Intelligenz in der Kardiologie aus den Bereichen Elektrophysiologie, Bildgebung und Risikostratifikation beleuchtet werden.
