Percutaneous endoscopic gastrostomy (PEG) provides long-term enteral nutritional access. This study aims to evaluate the feasibility, indications, success rates and complications associated with PEG. A retrospective descriptive study was conducted on 93 patients between 2020 and 2023 at Sheikh Khalifa University Hospital. Epidemiological, clinical and biological data were analysed. Multivariate logistic regression was performed to identify factors associated with complications. The mean age was 70 ± 14 years; 72% were men. The main indications were neurological (76%) and ENT (24%). The success rate was 100%. Minor complications were observed in 7.5%: peristomal infection (2.1%), obstruction (3.2%), leakage (2.1%). Significant predictive factors were intensive care unit admission (p < 0.0001), hypoalbuminaemia < 30 g/L (ORa: 15.5; 95% CI: 1.77-135.6; p = 0.0042) and neurological indication (ORa: 6.1; 95% CI: 1.11-33.55; p = 0.0335). PEG is a safe and effective technique for prolonged nutritional access, with a low complication rate when preventive measures are followed.
Introduction Les plasmocytomes solitaires peuvent être osseux ou extramédullaires et doivent être distingués du myélome multiple, maladie systémique caractérisée par une infiltration médullaire diffuse et des atteintes d’organes (CRAB). Cette distinction guide la stratégie thérapeutique : radiothérapie locale pour les plasmocytomes versus traitement systémique pour le myélome multiple. La TEP/TDM au 18F-FDG joue un rôle majeur dans cette différenciation. L’objectif de notre étude est de souligner l’apport de la TEP/TDM au 18F-FDG dans le diagnostic, l’évaluation thérapeutique et le suivi évolutif des proliférations plasmocytaires. Matériels et méthodes Notre étude inclut 7 patients d’âge moyen de 53 ans, avec un sex-ratio de 1,3 (H/F), ayant bénéficié, entre 2023 et 2025, d’une TEP/TDM à l’aide d’une gamma-caméra hybride GE Discovery (GE Healthcare, 2014) pour suspicion de plasmocytome ou évaluation thérapeutique. Les données analysées incluaient : nombre de lésions hypermétaboliques, atteinte extra-osseuse, SUVmax, bilan biologique (EPP, chaînes légères libres, calcémie, créatininémie), évaluation médullaire et imagerie morphologique (IRM/Scanner). Résultats Parmi les cinq patients adressés pour suspicion de plasmocytome, la TEP/TDM a permis de redresser le diagnostic vers un myélome multiple chez trois patients. Deux patients présentaient un plasmocytome osseux solitaire. Deux patients ont été adressés pour bilan d’évaluation d’un plasmocytome en rechute dans le cadre d’un myélome multiple. Trois patients atteints de myélome multiple ont présenté une rechute extramédullaire, visualisée en TEP/TDM. Conclusion La TEP/TDM au 18F-FDG se montre supérieure à l’IRM pour l’identification des foyers occultes grâce à son champ d’étude élargi (corps entier), la détection des localisations extramédullaires et l’identification de foyers non visibles en imagerie morphologique. Cependant, l’IRM reste plus sensible pour l’infiltration médullaire diffuse. Leur utilisation conjointe est complémentaire, conformément aux études de la littérature et aux recommandations IMWG. La TEP/TDM constitue ainsi un outil essentiel pour différencier formes localisées et formes systémiques et pour évaluer précocement la réponse thérapeutique. Les limites de notre étude sont l’effectif restreint et un suivi pas toujours effectué par TEP/TDM. Des études prospectives multicentriques sont nécessaires pour préciser sa place dans le parcours de prise en charge.

Les données épidémiologiques, cliniques et biologiques ont été analysées. Une régression logistique multivariée a été réalisée pour identifier les facteurs associés aux complications. L'âge moyen était de 70 ± 14 ans; 72% d'hommes. Les principales indications étaient neurologiques (76%) et l'oto-rhino-laryngologie (ORL) (24%). Le taux de succès était de 100%. Des complications mineures ont été observées chez 7,5%: infection péristomiale (2,1%), obstruction (3,2%), fuite (2,1%). Les facteurs prédictifs significatifs étaient l'hospitalisation en soins intensifs (p < 0,0001), l'hypoalbuminémie < 30 g/L (ORa: 15,5; IC 95%: 1,77-135,6; p = 0,0042) et l'indication neurologique (ORa: 6,1; IC 95%: 1,11-33,55; p = 0,0335). La GPE est une technique sûre et efficace pour l'accès nutritionnel prolongé, avec un faible taux de complications lorsque les mesures préventives sont respectées.
Vertebral artery dissection (VAD) can cause posterior-circulation ischemia after cervical manipulation; postpartum physiological changes may increase vascular vulnerability. A 30-year-old woman, nine months postpartum, developed an abrupt occipital headache, vertigo, nausea, and gait imbalance during a commercial neck massage. On examination, she had truncal ataxia and gait instability without focal limb weakness. We obtained 1.5-T MRI (diffusion-weighted imaging (DWI)/apparent diffusion coefficient (ADC), T1-weighted imaging (T1), T2-weighted imaging (T2), fluid-attenuated inversion recovery (FLAIR), T2*/susceptibility-weighted imaging (SWI), and postcontrast 3D T1), time-of-flight magnetic resonance angiography (TOF-MRA), and computed tomography angiography (CTA). Imaging showed a right VAD with an intramural hematoma and luminal irregularity, right posterior inferior cerebellar artery (PICA) occlusion, and right-predominant cerebellar infarcts. A posterior fossa developmental venous anomaly (DVA) drained adjacent to the outlets of the fourth ventricle. Cerebellar swelling produced triventricular hydrocephalus with transependymal flow and early tonsillar descent. After multidisciplinary discussion, we initiated antithrombotic therapy, provided supportive care and a short course of corticosteroids for vasogenic edema, and preserved the DVA drainage. Neurosurgery placed an external ventricular drain for obstructive hydrocephalus; serial imaging documented regression of the edema and normalization of the ventricles. The patient improved but retained mild truncal ataxia at follow-up. Prompt vascular imaging should be obtained for acute posterior head or neck pain with neurologic signs after cervical manipulation. CSF diversion and edema control should be prioritized when posterior fossa mass effect arises, and DVA drainage should be preserved to avoid venous infarction.
Introduction: The objective of this work is to study a certain number of criteria to meet the expectations of healthcare services seeking to improve the availability of pharmaceutical products. Methods: Regarding the data collection method, we conducted a survey. The latter was intended for healthcare services. The questionnaire was validated by a team of hospital pharmacists and managers. To develop our questionnaire, we used the Sphinx tool, and for the quantitative data, we processed it on a computer using SPSS software. Results: The internal pharmacy management models of the two hospitals represent several differences but also several similarities. Two major differences can be noted, the first one is the supply model that is based on annual markets for AMSS hospitals versus negotiated and just-in-time purchases for private hospitals. The second one is the hospital central pharmacy healthcare service procurement model. Conclusion: Following the multiple changes currently affecting the health care sector, hospital structures must optimize their management to reduce expenses while guaranteeing the safety and quality of care administered to patients.
