Increasing human presence and activities expose wild animals to persistent disturbances. Disturbed populations often become tolerant towards humans, although the underlying mechanisms are poorly understood. We used playback experiments to manipulate perceived level of human disturbance in great tits (Parus major). In the first year, incubating females remained more often on the nest, and parents during brood care exhibited a lower level of vigilance and tended to have shorter return latencies in the human disturbance treatment than in control treatments, demonstrating an increased tolerance. These differences disappeared in the second year. Tolerance was also influenced by distance to roads and the frequency of nest visits by researchers. These results support that behavioural tolerance can quickly emerge by phenotypic plasticity in natural populations, although other mechanisms may also influence behavioural responses after longer exposure to human disturbances, resulting in a complex set of processes involved in the spread of tolerance.
Janus kinase-3 (JAK3) deficiency, first described in 1995, is an autosomal recessive inborn error of immunity that mostly results in variants of severe combined immunodeficiency (SCID). The frequency is estimated to account for 7-14% of heritable SCID, with sporadic cases in the Western world. Neither preferential “hot spots” nor founder effects have yet been documented. Hereby, we aim to describe the global experience of JAK3-related diseases regarding clinical spectrum, genetic landscape, including founder variants and treatment strategies. We extracted clinical, genetic, and immunological data from published cases on patients with СID/SCID phenotype caused by defects in the JAK3 gene. The literature search included unpublished cases from collaborators, reports from meetings of the European Society for Immunodeficiencies (ESID), of the Clinical Immunology Society (CIS), and published data in the biomedical research search engine (PubMed) from 1995 to 2024. Our cohort includes 132 patients with 47 unique genetic defects, including 35 novel variants (18 homozygous, 17 heterozygous). The patients were from 5 continents with majority of Asian ancestry. Country of diagnosis included North America (USA [n = 42]); South America (Brazil [n = 3]); Europe (Turkey [n = 8], Hungary [n = 1], Poland [n = 6], UK [n = 5], Italy [n = 5], Israel [n = 4], Belarus [n = 4], Russian Federation [n = 3], Georgia [n = 3], Spain [n = 1], Germany [n = 1]); Asia (India [n = 16], Iran [n = 6],China [n = 4], Pakistan [n = 1], Japan [n = 1]); and Africa (Egypt [n = 16], Saudi Arabia [n = 1], Sudan [n = 1]). Forty-five (35%) of patients from the cohort were born to consanguineous parents in 31 families from Georgia (n = 3, 1-family), Russian Federation (n = 3/2-family), Sudan (n = 1), Turkey (n = 8, 5-family), Israel (n = 4, 2-family), India (n = 3, 2-family), Egypt(n = 13, 8-family), UK (n = 5, 4-family), China (n = 1), Brazil (n = 1), Spain (n = 1), Italy (n = 1), Pakistan (n = 1), and Saudi (n = 1). 13 novel founder variants are identified. The same founder variant was seen in several countries. The majority of genetic defects were homozygous (68%); 40/132 compound were heterozygous, and 2 patients had germline heterozygous gain-of-function. The variants occurred across the entire JAK3 gene with no hotspots. Only 6 (5%) of 132 cases developed Omenn syndrome. 61 patients were transplanted, 17 died at the age of 9 months (mean age 9-18 months), and 44 are alive. We describe for the first time a global cohort of JAK3 with 132 patients with founder effects in a subgroup. Patients were identified in four continents but are most common in countries with a high rate of consanguinity. Founder effect was identified in 14 regions.
Bevezetés és célkitűzés: Az A-csoportú Streptococcus okozta infekciók száma 2022 óta Európa számos országában jelentősen megemelkedett. A magas esetszámokon túl a progrediáló, súlyos lefolyású, invazív esetek aránynövekedése aggasztó problémaként jelent meg. Kutatásunk célja a hazai Streptococcus pyogenes járvány jellemzőinek bemutatása a gyermek-alapellátásban, egy országos reprezentációjú gyermekpopulációban végzett adatgyűjtés alapján. Módszer: 21 tételből álló, strukturált kérdőívet küldtünk ki a házi gyermekorvosi hálózat platformján 2024 májusában országszerte, gyermekorvosi praxisban dolgozó orvosok számára. A kérdőív retrospektív jelleggel, a 2023. január 1. A páciensek 0,6%-ánál alakult ki peritonsillaris tályog (41 eset), 9 gyermeknél diagnosztizáltak invazív megbetegedést, 2 gyermeknél jelentkezett poststreptococcalis glomerulonephritis, míg reinfekciót átlagosan 15%-ban észleltek. A gyanús esetek közel felénél (47%) történt diagnosztikus mikrobiológiai vizsgálat. A terápia kapcsán a leggyakrabban használt antibiotikum az amoxicillin-klavulánsav volt (41%), azonban a szűkebb spektrumú béta-laktámok (penicillin, amoxicillin, cefprozil) együttes alkalmazása elérte az 50%-ot. Makrolidot 6%-ban, cefiximet 3%-ban alkalmaztak a gyermekorvosok. A növekvő incidenciát azonban hazánkban is észleljük: 2024. július 14-ig 153 jelentett pertussismegbetegedésről számolhatunk be. Orv Hetil. 2024; 165(34): 1307–1318.
147 Background: PolyPEPI1018 is an off-the-shelf, multi-peptide vaccine containing 12 immunogenic epitopes derived from 7 cancer testis antigens (CTAs) frequently expressed in patients with colorectal cancer (CRC). PolyPEPI1018 successfully induced anticancer immunity and triggered recruitment and infiltration of cytotoxic T cells into the tumor of MSS mCRC subjects demonstrating also early evidence of clinical activity, in first-line mCRC. Here we report the initial results of the phase Ib study of PolyPEPI1018 vaccine plus trifluridine/tipiracil (TAS-102) in late-stage mCRC patients. Methods: Patients with MSS mCRC who have progressed on ≤2 lines of prior chemotherapy regimen for mCRC received PolyPEPI1018 subcutaneously on days 1 and 15 and TAS-102 orally twice daily on days 1-5 and 8-12 of a 28-day cycle. Treatment continued for up to 7 cycles until disease progression or unacceptable toxicity. Immunomonitoring was performed at both blood and tumor levels prior to- and on study treatment. The primary endpoint of the study was safety and tolerability. Data on objective response rate (ORR), disease control rate (DCR), progression-free survival (PFS), overall survival (OS) and correlation studies will be also presented. Results: 15 patients (67% male) started treatment. At baseline, median age was 55 years (range 31–71), 73% had liver metastases and the primary tumor site was colon-sigmoid in 40% and rectum in 33%. The combination was well-tolerated; most common side effect related to PolyPEPI1018 was Grade (Gr) 1-2 local skin reactions in 93% of patients. Gr 3 events. There were no Gr 4 or 5 events. The ORR was 0% and the DCR was 50%. The mPFS was 2.5 months (95%CI 2.1-NR). At the data cut-off (September 7, 2022), the mOS has not been reached; median follow-up was 4.0 months (95% CI 2.2-4.4). Post-treatment, vaccine-specific T cell responses were detected ex vivo in the PBMC of 4/5 subjects tested. In addition, one subject who had no detectable T cell response at peripheral level, responded at the tumor level with more than 300% increase of both CD3+ and CD8+ tumor-infiltrating lymphocytes compared to baseline. Patients with increased PFS (≥ 16 weeks) had robust vaccine-specific T cell responses. Conclusions: To our knowledge, this is the first phase Ib study investigating combination of a cancer vaccine with TAS-102 chemotherapy in advanced MSS mCRC. Our results show that PolyPEPI1018 plus TAS-102 was well-tolerated with few grade 3 AEs beyond what is expected with TAS-102 monotherapy. PolyPEPI1018 induced immunological responses at both peripheral and tumor level, albeit no objective tumor responses could be detected. The study is on-going for the collection of overall survival data. Clinical trial information: NCT05130060 .
