A térd osteoarthritisének kezelési stratégiájában új, innovatív lehetőség a femoralis artériából eredő leszálló genicularis artéria (a. genus descendens) embolisatiója. TP53 aberrations predict chemoresistance and represent a contraindication for the use of standard chemoimmunotherapy in chronic lymphocytic leukaemia (CLL). Recent next-generation sequencing (NGS)-based studies have identified frequent low-burden TP53 mutations with variant allele frequencies below 10%, but the clinical impact of these low-burden TP53 mutations is still a matter of debate. In this study, we aimed to scrutinise the subclonal architecture and clinical impact of TP53 mutations using a sensitive, NGS-based mutation analysis in a 'real-world' cohort of 901 patients with CLL. In total, 225 TP53 mutations were identified in 17.5% (158/901) of the patients; 48% of these alterations represented high-burden mutations, while 52% were low-burden TP53 mutations. Low-burden mutations as sole alterations were identified in 39% (62/158) of all mutated cases with 82% (51/62) of these being represented by a single low-burden TP53 mutation. Patients harbouring low-burden TP53 mutations had significantly lower time to first treatment compared to patients with wild-type TP53. Our study has expanded the knowledge on the frequency, clonal architecture, and clinical impact of low-burden TP53 mutations. By demonstrating that patients with sole low-burden TP53 variants represent more than one-third of patients with TP53 mutations and have an increased risk for treatment initiation, our findings strengthen the need to redefine the threshold of TP53 variant reporting to below 10% in the routine diagnostic setting.
Egy 13 éves fiú lázzal, köhögéssel, szaglásvesztéssel és kb. 2 × 3 cm átmérőjű, jobb oldali nyaki elváltozással jelentkezett hozzátartozójának kíséretében kórházunk gyermekgyógyászati szakambulanciáján. A vizsgálat előtt 24 nappal a dokumentált nasopharyngealis COVID–19-gyorstesztje pozitivitást mutatott. Idővel a Bartonella henselae szerológiai vizsgálat pozitivitást mutatott. A nyaki képlet műtéti eltávolítása mellett döntöttünk. The authors of this publication have developed a new screening method to evaluate the physical development of infants, which identifies growth and developmental restrictions based on their gestational age, gender, weight and length standard positions and nourishment status. This so-called MDN-method (MDN = Maturity, Development, Nourishment) offers several benefits: 1. It enables the expansion of the scope and definition of intrauterine growth restriction (IUGR) and the establishment of five separate condition subtypes. 2. It facilitates the study of the risk of mortality of infants based on their development and nourishment status. 3. The method is suitable for the screening of high-risk IUGR infants and neonates. 4. The MDN method allowed for the development of a new ultrasound screening methodology for high-risk IUGR infants and neonates, based on abdominal circumference (AC) and femur length (FL) standards, which correlate the most with the weight and length of the fetus. The data of 1,244,918 Hungarian live and stillbirths recorded during 2000-2012 were used for these studies. It is believed that these new screening methods may have a significant impact on reducing the frequency of IUGR-related stillbirth and infant mortality cases.
The twentieth century saw rapid environmental degradation due to changes that contributed to increased net GHG emissions, loss of natural ecosystems, and declining biodiversity. Deterioration of unprotected landscapes during swift industrialization, urbanization, increasing monocultures in agriculture, expansion of commercial production significantly contributed to these negative consequences. However, a cultural shift occurred during the last two decades in favour of landscape conservation. In response to widespread landscape degradation and loss of ecosystem services, the Council of Europe saw the need to protect, manage, and develop the landscapes, and thus signed the European Landscape Convention (ELC) in 2000. This was the world's first international agreement that described all aspects of landscape management in detail. The European Landscape Convention fully meets the challenges through its goal of correcting a lack of understanding of landscapes as a unique system embracing natural, economic, and social features throughout Europe. It goes beyond simply protecting landscapes and addresses landscape management and development, as well as raising public and government awareness of the importance of paying attention to all types of landscapes, whether exceptional or spoiled. Landscape observatories, multifunctional platforms and knowledge centres for researchers, technicians, administrators, and citizens, are one of the Council of Europe's instruments for implementing the European Landscape Convention (ELC). They can be established on a variety of scales and can serve as a vital link between administrations, civil society, researchers, and the economic sector. This article discusses the emergence of landscape observatories and the role they can play as decision support instruments in promoting sustainable landscape development through a regenerative approach. Additionally, the paper discusses the implementation of ELC in Västra Götaland in Sweden through the establishment of Landscape Observatory Västra Götaland, and its impacts and challenges associated with landscape development. Furthermore, we propose a comprehensive and holistic, to any landscape type adaptable landscape observatory concept, based on multifunctionality of these institutions, emphasizing their decision support roles, social and economic importance.
