Objective: To analyze the individual, social, and programmatic vulnerabilities and their associations with the history of Sexually Transmitted Infections (STIs) and/or HIV in the Prison Population in Cear & aacute;. Methods: This cross-sectional study was performed in May-September 2022 in Cear & aacute; prison units (20,666 prisoners). Data were collected using a questionnaire with sociodemographic, prison, and health history information. The predictor variables were categorized according to the theoretical framework of vulnerabilities. The diagnosis of any STI/HIV during or after entry into the prison system was the outcome variable. For continuous variables, measures of position, dispersion, and normality analysis were calculated and for the association between vulnerability marker variables and STI history, binary logistic regression was performed. Results: The prevalence of STIs was 445 (2.2%), and Syphilis and HIV were the most prevalent diseases. Most diagnoses (54.4%) were made on admission. The markers of greater vulnerability to STIs were as follows: be 30 years of age or older, have a psychiatric condition or previous chronic illness, low income, have been arrested for a drug crime, have relationships with other prisoners, work in the system, and prison time of up to one year. The markers of lowest vulnerability were the following: having a religion, previous formal employment, having a disability, being LGBTQIA+, receiving condoms in prison, reporting violence by police officers, receiving a visit from a partner, and having been transferred from one unit to another. Conclusion: The prison population of Cear & aacute; presented individual, social, and programmatic vulnerabilities to STIs/HIV, which were active before and during imprisonment. The high admission prevalence reinforces the importance of understanding such vulnerabilities to promote health and life quality actions, considering the pre- and intra-prison context.
Objective: To assess the performance of qSOFA for identifying sepsis in an emergency department. Methods: This retrospective cohort study was conducted at a private hospital in S & atilde;o Paulo, Brazil, between 2018 and 2019. Data were extracted from the medical records of all individuals treated with suspected sepsis and assessed using the qSOFA tool, according to demographic, clinical, and outcome variables. Statistical analysis was performed using Student's t-test, chi-square test, and McNemar's test. Results: A total of 5,017 patients with suspected sepsis were assessed using qSOFA, of which 88% had a negative qSOFA score and 12% a positive qSOFA. Individuals with a positive qSOFA (score>2) had a higher percentage of sepsis diagnosis, with a statistically significant association. Patients with a positive qSOFA were 49% men and 37.3% women, with a mean age of 74.28 years. It was observed that there was a similarity in the percentages between the scored variables of the qSOFA scale (systolic blood pressure (SBP) <= 100 mmHg or MAP < 65 mmHg, respiratory rate (RR) >= 22 rpm and level of consciousness). The most prevalent site of infection was the respiratory (55.7%). In relation to the outcome, 6.8% of sepsis cases progressed to death. Conclusion: In this study, the qSOFA score showed good specificity (94%) and low sensitivity (32%) for detecting sepsis in an emergency department, demonstrating its effectiveness.
