目的:提高对脊柱旁原发性腹膜后肿瘤的CT评估价值.方法:回顾性分析16例经病理证实的脊柱旁原发性腹膜后肿瘤的CT图像,分析其常见类型、CT特点.结果:脊柱旁原发性腹膜后肿瘤以神经源性肿瘤最常见,占75.0%.CT判断肿瘤良恶性的敏感性为87.50%,特异性为85.71%,准确率为81.25%.恶性、良性肿瘤分别有5例、1例表现为不规则形(P=0.035);良性肿瘤均表现为推压改变,而恶性肿瘤多表现为粘连、侵犯(P=0.001).结论:CT扫描对脊柱旁腹膜后肿瘤有较高的评估价值.
Objective: To investigate clinical significance of CT in localization of microeletrode-guidedthalamotomy and posteroventralpallidotomy for Parking' disease,and observe its efficacy. Methods: A series of 120 patients with Parkinson' disease were collected. The target's preoperative localization by CT and postoperative microelectrode-guided localization were compared. After the operation,the diameters of the destroyed lesion were measured by CT. The relationship between the lesioned diameter and the efficacy was investigated. Results: The coincidence rate of CT and microelectrode-guided localization for the target was 45. 8%,and the difference was 1-5mm( average 2. 7mm). It found preoperatively that when the lesion diameter was less than 8mm,the efficacy was not perfect; however,when the diameter was more than 8mm,the operation could provide good effects. Thedifferenceswere statistically significant( P 0. 05). But the operative complications increased with the increase of the lesion diameter. Conclusions: For the patients with Parkinson's disease,CT has an important significance in the preoperative localization of thalamotomy and posteroventralpallidotomy. The efficacy is not related with the lesion diameter,which should be 8-12 mm.
Objective To evaluate CT and MRI findings of portal vein aneurysm(PVA)in order to improve its diagnostic accuracy.Methods CT and MRI findings of 9 patients with PVA proved by pathology and direct angiography were reviewed retrospectively.CT scanning was performed in 7 patients,including plain scan (n=2),both plain and enhanced scan (n=5),CT angiography (CTA)(n=3).Plain and enhanced MRI scan were performed in 3 patients.Results (1 )PVA showed a high predilection for old adults.(2 )Of the 9 tumors,4 located in portal vein trunk,2 located in junction of superior mesenteric vein and portal vein trunk,1 located in intrahepat-ic-extrahepatic portal vein,2 located in intra-hepatic portal vein.(3)8 tumors were characterized as well-defined and quasi-circular mass.1 patient occurred portal hypertension,thrombus as the portal vein trunk was oppressed by the tumor.(4)Plain CT showed the mass was slightly higher than pancreas parenchyma density,and uniform with the density of the liver parenchyma.Enhancement scanning showed 4 tumors represented mild or moderate enhancement in portal venous phase except for 1 patient accompany with portal vein thrombus.CTA showed clearly the relationship mass with portal vein,and classified the type of PVA .The 3 lesions represented hypo-intensity on T1 WI and even hyper-intensity on T2 WI.Enhancement scanning showed the tumor was significantly enhancement in portal venous phase on T1 WI.Conclusion CT and MRI have their own advantages in the diagnosis of portal vein aneurysm.Com-bination of CT and MRI could improve the diagnostic accuracy of portal vein aneurysm.
Objective:To evaluate diagnostic value of MSCT(3D) and multiplanar reconstruction(MPR) for trauma of bones and joints.Methods:122 patients with fracture and /or dislocation undervent MSCT thin slice volume scan.Data collected from scan were reconstructed with 3D or MPR method.Results:Fractures of 122 patients were situated in facial bones(37 patients),ribs(20),limbs(20) vertebral body and appendix(36) and pelvis(9) 3D reconstruction was able to display stereo-anatomy of fractures or dislocation,but was inferior to cross-sectional imaging in showing tinge fractures.MPR was capable of reconstruct images in any direction and with any angle,offering anatomy of fracture and its surrounding with an optimum view angle.Conclusion:MSCT 3D and MPR reconstructions help to improve correct of diagnosis of trauma of bones and joints,offering more information for assessing trauma and conducing surgical treatment.
