BackgroundXiaoYao San (XYS), a classical Traditional Chinese Medicine (TCM), has demonstrated efficacy in alleviating stress-related neuropsychiatric disorders. However, its therapeutic potential against maternal immune activation (MIA)-induced neurobehavioral impairments remains unexplored. This study aims to investigate the neuroprotective effects of XYS on MIA-related behavioral dysfunctions and elucidate its underlying mechanisms.ResultsUsing a poly (I:C)-induced MIA mouse model, we demonstrated that XYS effectively ameliorates autism spectrum disorder (ASD) related behavioral phenotypes. Mechanistic investigations revealed that XYS exerts its therapeutic effects through: (1) Attenuation of core behavioral deficits including enhanced social interaction and reduced repetitive behaviors; (2) Downregulation of intestinal amino acid transporters; (3) Restoration of cerebral glutamate-GABA balance via modulation of glutamine pathway; (4) Structural remodeling of gut microbiota with specific enrichment of Bacteroides spp. Notably, B. uniformis was identified as a key microbial mediator capable of recapitulating XYS-mediated neurophysiological improvements through metabolic regulation.ConclusionThis study elucidates XYS as a multi-target therapeutic agent that coordinately modulates gut microbial ecosystems, amino acid homeostasis, and neurotransmitter homeostasis. The findings provide novel insights into the gut-brain axis mechanisms of TCM formulations, offering a scientific foundation for developing microbiota-based intervention strategies for neurodevelopmental disorders.
Cancer-related fatigue is one of the common concomitant symptoms of patients with malignant tumor, which belongs to the category of “fatigue” in traditional Chinese medicine, and can have a serious impact on the follow-up treatment, rehabilitation and quality of life of cancer patients. Based on the theory of “Sadness Dissipating Qi” in Huangdi Neijing, this paper explores the relationship between negative emotions such as sadness and cancer-related fatigue, and concludes that the pathological basis of cancer-related fatigue is “Qi elimination”, and the key to the pathogenesis is sorrow and depression for a long time and damage to the physiological function of the lungs. in addition, liver wood insults gold, viscera disharmony, deficiency of qi and blood yin and yang. The treatment always focuses on tonifying qi and unblocking emotion, soothing the liver and invigorating the spleen and kidney, and putting forward the treatment methods of tonifying the lung and nourishing qi, soothing the liver and regulating qi, invigorating the spleen and kidney, regulating qi with emotion and so on. The purpose of this study is to enrich the traditional Chinese medicine theories and methods of traditional Chinese medicine in the treatment of cancer related fatigue, in order to open up some new clinical ideas.
OBJECTIVE:The aim of this study was to investigate the relationship between miR-767-3p and hepatocellular carcinoma (HCC).METHOD:We examined the expression of miR-767-3p in both HCC tissues and HCC cell lines by qRT-PCR and western blot assay. We also investigated the influence of miR-767-3p on HCC by transfecting HCC cells with either miR-767-3p mimics or inhibitors.RESULT:MiR-767-3p expression was increased in HCCs and cell lines. Functional analyses demonstrated that miR-767-3p increased HCC cell proliferation and prevented apoptosis both in vitro and in vivo, whereas miR-767-3p inhibition had the opposite effect. Caspase-3 and caspase-9 were found to be direct targets of miR-767-3p in HCC cell lines, and overexpression of miR-767-3p suppressed caspase-3/-9 production. Caspase-3 and caspase-9 siRNA knockdown revealed similar promoting of cell proliferation and inhibiting of cell apoptosis produced by miR-767-3p overexpression, whereas caspase-3/-9 siRNAs inhibited miR-767-3p knockdown-induced inhibition of cell proliferation and promotion of cell apoptosis.CONCLUSION:MiR-767-3p promoted proliferation and prevented apoptosis in human hepatocellular carcinoma (HCC) through inhibiting the caspase-3/caspase-9 pathway.
BackgroundMalignant tumors, mainly solid tumors, are a significant obstacle to the improvement of life expectancy at present. Epithelial cell adhesion molecule (EpCAM), a cancer stem cell biomarker, showed widespread expression in most normal epithelial cells and most cancers. Although the clinical significance of EpCAM in various malignant solid tumors has been studied extensively, the latent relationships between EpCAM and pathological and clinical characteristics in solid tumors and differences in the roles of EpCAM among tumors have not been clearly determined. The destination point of this study was to analyze the value of EpCAM in solid tumors in clinicopathological and prognostic dimension using a meta-analysis approach.Method and materialsA comprehensive and systematic search of the researches published up to March 7th, 2022, in PubMed, EMBASE, Web of Science, Cochrane library and PMC databases was performed. The relationships between EpCAM overexpression, clinicopathological characteristics, and survival outcomes were analyzed. Pooled hazard ratios (HRs) with 95% confidence intervals (CIs) and odds ratios (ORs) were estimated as indicators of the degree of correlation. This research was registered on PROSPERO (International prospective register of systematic reviews), ID: CRD42022315070.ResultsIn total, 57 articles and 14184 cases were included in this study. High EpCAM expression had a significant coherence with a poorer overall survival (OS) (HR: 1.30, 95% CI: 1.08–1.58, P < 0.01) and a worse disease-free survival (DFS) (HR: 1.58, 95% CI: 1.28–1.95, P < 0.01), especially of gastrointestinal tumors’ OS (HR: 1.50, 95% CI: 1.15–1.95, P < 0.01), and DFS (HR: 1.84, 95% CI: 1.52–2.33, P < 0.01). The DFS of head and neck tumors (HR: 2.33, 95% CI: 1.51–3.61, P < 0.01) was also associated with the overexpression of EpCAM. There were no positive relationships between the overexpression of EpCAM and sex (RR: 1.03, 95% CI: 0.99–1.07, P = 0.141), T classification (RR: 0.93, 95% CI: 0.82–1.06, P = 0.293), lymph node metastasis (RR: 0.85, 95% CI: 0.54–1.32, P = 0.461), distant metastasis (RR: 0.97, 95% CI: 0.84–1.10, P = 0.606), vascular infiltration (RR: 1.05, 95% CI: 0.85–1.29, P = 0.611), and TNM stage (RR: 0.93, 95% CI: 0.83–1.04, P = 0.187). However, the overexpression of EpCAM exhibited a significant association with the histological grades (RR: 0.88, 95% CI: 0.80–0.97, P < 0.01).ConclusionBased on pooled HRs, the positive expression of EpCAM was totally correlated to a worse OS and DFS in solid tumors. The expression of EpCAM was related to a worse OS in gastrointestinal tumors and a worse DFS in gastrointestinal tumors and head and neck tumors. Moreover, EpCAM expression was correlated with the histological grade. Ethnopharmacological relevance: As a typical heat-clearing prescription, Shaoyao decoction (SYD) has a robust function of clearing viscera heat for the treatment of several intestinal discomfort symptoms. Clinical evidence indicated that it had the potential to cure radiation enteritis. However, its underlying mechanisms remain unclear. Aim of the study: The present study was designed to probe the protective effects and the involved mechanisms of SYD on X-ray radiation-induced enteritis of C57BL/6 mice. Materials and methods: X-ray irradiation were used to establish the radiation enteritis model. Forty-eight male C57BL/6 mice (20 & PLUSMN; 2 g) were randomly divided into six groups: the control group, model group, dexamethasone group (DEX, 0.12 mg/kg) and SYD groups (0.12, 0.24 and 0.36 g/mL), respectively. All mice (except the control group) were intragastrically administrated for a continuous 7 days. H & E and Masson staining were employed to evaluate the morphological and collagen fibers changes of the colon. ELISA was performed to assess the levels of MDA, SOD, COX, LPS, IL-6, IL-1 beta and TNF-alpha in serum. Moreover, TUNEL fluorescence, western blot and qRT-PCR were used to detect the levels of apoptosis-related proteins and genes of Dclk-1, ATM, MRE-11, Bcl2, Bax, Caspase-3, and Cyto-c. Furthermore, immunofluorescent staining was applied to detect the protein levels of p53 and Claudin-1 in colon. Results: Treatment with SYD decreased the exfoliated and necrotic epithelial cells and prevent the proliferate from damaged fibrous tissue in the crypt layer of mucos. The levels of serum peroxidation and pro-inflammatory cytokines (MDA, COX, LPS, IL-6, IL-1 beta and TNF-alpha) were obviously inhibited, while SOD sharply increased in serum after administration. Moreover, SYD can significantly ameliorate the apoptosis of colon cells, evidenced by the reduced positive expression of TUNEL staining. Meanwhile, the results of qRT-PCR and western blot demonstrated that SYD can dramatically stimulate the expression of genes and proteins Dclk-1, ATM and MRE11, thus promoting the expression of mitochondrial pro-apoptotic proteins Bax, Caspase-3 and Cyto-c, while increasing the level of anti-apoptotic protein Bcl-2. Furthermore, immunofluorescence revealed that SYD can notably decreased the protein level of p53 while reverse the reduction of Claudin-1. Conclusions: These results indicated that radiation enteritis in C57BL/6 mice can be ameliorated by treatment with SYD. The potential protection mechanisms may be involved in ameliorating tissue fibrosis by decreasing inflammatory and apoptotic events.
近年来,靶向药物逐渐成为非小细胞肺癌治疗的主力军.但其带来的不良反应如皮疹等也是不容忽视的,李世杰教授认为此类皮疹的发病关键为"风湿热邪"入侵,且不离气血虚弱之本,多从"疏风除湿,养血清热"论治,临床中常以消风散加减治疗,收效甚佳.
明代汪绮石所著的《理虚元鉴》,对虚劳疾病论述颇精.不论在虚劳的病因、病机,还是在治疗和预后等方面,《理虚元鉴》都有其较为独特的观点.用这些理论来论治属于虚劳范畴中的肺癌咯血,我们仍能从其中得到较多的启示.
目的:观察甲地孕酮联合针刺四缝穴治疗癌性厌食的疗效.方法::随机选取成都中医药大学附属医院肿瘤一科2019年12月-2020年12月住院病人中具有厌食症状的Ⅲ、Ⅳ期恶性肿瘤患者90例,随机分为对照组(46例)及观察组(44例).对照组予醋酸甲地孕酮分散片,每日1次,每次4片(160mg),口服,连续4周;观察组在对照组基础上联合针刺四缝穴治疗,每周3次,治疗2周为1疗程,连续治疗2疗程.观察治疗前后两组患者体质量、食欲、营养指标等方面的变化.结果:对照组与观察组食欲改善分别为22例(47.8%)、31例(70.4%),体质量有改善分别为22例(47.8%)、33例(75%),观察组体质量改善率及食欲改善率均高于对照组(P均<0.05).观察组治疗后前白蛋白水平均明显高于治疗前及对照组(P<0.05).结论:针刺四缝穴联合甲地孕酮治疗方案可明显改善中晚期恶性肿瘤患者的癌性厌食症状,增加患者体质量,改善其营养状况.
现代医学中,早期肺癌患者术后被咳嗽、乏力、疼痛、失眠、焦虑等损伤症状困扰,且存在复发、转移的可能性.李世杰教授根据"阳化气,阴成形"理论,通过数十年临床经验总结提出,早期肺癌术后患者病理因素有以下5种:气虚、血瘀、寒凝、痰湿、热毒;并以气虚为本.其病机特点概括为正虚邪伏,使用中药治疗早期肺癌术后患者,疗效良好.
Cancer-related depression,as a common pathological depression during the diagnosis and treatment of malignant tumors,is one of the important factors affecting the progression and treatment of tumor diseases.At present,the treatment of cancer-related depression is mainly based on simple antidepressants,and there are no specific antidepressants for tumor patients.The patients had poor compliance to the drug,and long-term use of antidepressants is prone to cause drug-resistance and adverse reaction,which result into unsatisfactory efficacy.Traditional Chinese medicine(TCM)characteristic therapies(acupuncture,acupoint application,five-element music therapy,emotional therapy,traditional health-care exercises,massage and scraping therapy,etc.)belong to the non-drug treatment under the guidance of the TCM theory,which have the advantages of good curative effect,low cost,high safety and high patients’compliance,and can play an important role in the treatment of cancer-related depression.The purpose of this paper is to summarize the progress of the application of characteristic TCM therapy in the treatment of cancer-related depression,and to provide some reference for its clinical treatment and research.
IntroductionCisplatin, a chemotherapeutic drug, is widely used for the treatment of various malignant tumors with good effects. However, cisplatin-induced nephrotoxicity is a major dose-limiting factor and a significant adverse event. Mannitol is used to reduce cisplatin-induced nephrotoxicity, which is controversial. This study aimed to evaluate the efficacy and safety of a hydration regimen containing mannitol against cisplatin-induced nephrotoxicity through a meta-analysis.MethodsPotential records from PubMed, EMBASE, Cochrane Library, and ClinicalTrials that met the inclusion criteria were included from inception to May 2021. Cochrane Collaboration tools were used to assess the risk of bias in the included studies. Jadad’s and NOS scores were applied to assess the quality of randomized controlled trials (RCTs) and case-control studies. A random-effects model or fixed-effects model was used depending on the heterogeneity. Subgroup analyses were performed to evaluate the potential study characteristics. The pooled odds ratios (ORs) and 95% confidence intervals (CIs) were evaluated.ResultsFour RCTs and seven case-control studies involving 4168 patients were included. Pooled results showed that mannitol use could reduce the incidence of cisplatin-induced nephrotoxicity (OR = 0.66, 95% CI [0.45–0.97], p = 0.03), especially reducing grade 3 nephrotoxicity events according to CTCAE 4.0 (OR = 0.37,95% CI [0.16–0.84]). Moreover, mannitol use was not significantly associated with creatinine clearance, serum creatine, and electrolyte disturbance (p > 0.05). Gastrointestinal cancer (OR = 0.36, 95% CI [0.15–0.83], p = 0.02) and urinary tract cancer (OR = 0.32,95% CI [0.14–0.73], p = 0.007) may be more sensitive to mannitol, although the test for overall effect was significantly different (OR = 0.66, 95% CI [0.49–0.89], p = 0.007). For patients with diabetes and hypertension, mannitol may worsen renal function (OR = 1.80, 95% CI [1.18–2.72], p = 0.006; OR = 2.19, 95% CI [1.50, 3.19], p < 0.0001, respectively). Mannitol may have a better protective effect when doses of mannitol were ≥ 25 g (OR = 0.58, 95% CI [0.39–0.88], p = 0.01) and doses of cisplatin < 75 mg/m2 (OR = 0.59, 95% CI [0.36–0.94], p = 0.03). It revealed that mannitol use was likely to cause nausea or vomiting (OR = 1.86, 95% CI [1.20–2.89], p = 0.006).ConclusionCurrent evidence revealed that mannitol was an effective and safe drug to reduce cisplatin-induced nephrotoxicity events, especially Grade 3 events. However, it may cause more nausea/vomiting events and deteriorate renal function in patients with diabetes or hypertension. We also found that mannitol had the best effect when mannitol was ≥ 25 g in total or cisplatin was < 75 mg/m2. 肺结节可为炎性结节、特殊感染结节等良性结节,也可进一步演变为肺恶性肿瘤.中医在此病的治疗上有独特优势,李世杰教授认为肺结节的主要病机为"气虚血瘀,痰热郁结",以"补气活血,清热化痰"为主要治则,临床观察以自拟方治疗肺结节,疗效甚佳.
This article explores the effects and mechanisms of magnolol on the proliferation of gastric cancer cells as well as the apoptosis. First, 0 (control group), 20, 40, and 80 /x mol/L magnolol were observed on SGC-7901 cells for 24, 48, and 72 h. We use MTT method to measure the cell viability, and apoptosis and cells were detected by flow cytometry. Cell proliferation inhibition rate, apoptosis and cell cycle experiments showed that P-value < 0.05 means the difference is statistically significant. And the results which compare the control group, the 20, 40, and 80 /x mol/L show that honokiol had lower cell viability (P < 0.01), increased apoptotic rate (P < 0.01), and cell cycle stay in the G1 phase (P < 0.01), so we found that honokiol may suppress the proliferation of SGC-7901 cells and stimulate apoptosis by regulating cyclin and apoptosis-related proteins. With the development of nanomaterials synthesis technology and application in biomedicine, gold magnetic composite nanomaterials have unique properties, so they have been widely concerned in many applications. We combine gold and magnetic nanomaterials through other nanostructures to achieve the integration of diagnosis and treatment of tumors. We have synthesized two kinds of gold magnetic nanocomposites, GNR-PPy-FA nanocomposites. With the role of chemotherapy and heat and light therapy, GNR-PPy-FA nanocomposites have high light-to-heat conversion efficiency. Cell experiments verify the effect of chemotherapy and photothermal treatment of composite nanomaterials. After incubation with gold magnetic composite nanomaterials, the cell survival rate of tumor cells decreased to about 15%. In addition, both types of gold magnetic nanocomposites have the ability to dually target cancer cells, and the modification of folic acid and cancer cell membranes makes the material more biocompatible.
肺癌术后咳嗽为术后创伤致气道受损,其病因是虚、痰、瘀、癌毒聚集,总病机属本虚标实,以气血亏虚为本,瘀血阻络、痰湿聚集、邪毒侵袭为标.常规药物疗效欠佳,且停药后易反复发作.李世杰认为,肺癌术后咳嗽常见肺热阴虚型、气虚痰湿型,分别以玄麦甘桔汤加减滋阴清热,以参苏饮加减益气化痰,收到较好疗效,并附临床验案两例.
胃肠道毒性是化疗药物最常见的不良反应之一,主要包括恶心、呕吐、口干、纳差、反酸、腹泻、便秘等胃肠功能调节紊乱症状,发生率达65%~85%.从临床表现和特点来看,化疗引起的胃肠道毒性归属于中医上的"呕吐""痞证""泄泻""腹痛"等范畴.
信迪利单抗是一种新型的全人源化IgG4抗PD-1单克隆抗体,是具有国际品质的中国创新药,在临床前模型中显示了其强大的抗肿瘤效果.在我国批准用于至少经过二线系统化疗的复发或难治性经典霍奇金淋巴瘤的治疗.2019年11月,信迪利单抗注射液成为唯一列名国家医保目录的PD-1单抗.信迪利单抗正在进行用于各种实体肿瘤的I、II、III临床试验,目前在多项针对非小细胞肺癌的临床研究中获得了令人鼓舞的数据,将有望为这一领域带来新的希望,成为非小细胞肺癌治疗的新解决方案.故本文对该药目前用于非小细胞肺癌的部分关键临床试验进行总结.
为保证川乌与草乌的合理用药,本文对川乌与草乌的鉴别特征进行总结分析,并对两者的研究进展展开综述.川乌与草乌源于同一原植物的不同部位,两者外观性状相似,可以通过显微鉴别以及理化鉴别进行明确区分.川乌与草乌均具有抵抗寒冷、抗炎、止痛、改善血液循环、保护心肌、强心、抗休克、抗肿瘤等药理作用,在药效作用强度上,草乌性热于川乌,在除痹止痛、祛风寒湿等药效上强于川乌;在临床上,草乌属于一线麻醉药,川乌则不是;在抗炎作用上,川乌抗炎作用更明显.在临床研究以及科研工作中,应充分挖掘这类毒性中药的药学知识,以期充分发挥有毒中药的药用价值.
目的 基于TLR4/NF-κB信号通路探讨托里消毒散治疗放射性直肠炎的作用机制.方法 48只SD大鼠随机分为6组,除空白组外,其余各组均诱导造放射性直肠炎大鼠模型,造模成功后大鼠正常喂养3d后开始灌肠,空白组、模型组以2ml的0.9%生理盐水灌肠,阳性对照组以蒙脱石散(0.2 g/ml)联合地塞米松(1 mg/ml)2 ml灌肠,托里消毒散低(1 g/ml)、中(2g/ml)、高剂量(4g/ml)分别2ml灌肠,每天1次,连续10 d.处死大鼠,测直肠直径,采用HE染色考察直肠组织病理学变化;ELISA检测血清干扰素-γ(IFN-γ)、肿瘤坏死因子-o(TNF-α)、白细胞介素4(IL-4)、IL-10、表皮生长因子(EGF)水平及血清免疫球蛋白IgA、IgM、IgG含量;Western blot检测直肠TLR4、Myd88、NF-κBp65、IκBα表达;qRT-PCR检测直肠TLR4、Myd88、NF-κBp65、IL-6、IL-10 mRNA表达.结果 托里消毒散高剂量可改善直肠组织病理变化,降低直肠炎症评分(P<0.05);托里消毒散高剂量较模型组可升高大鼠血清中EGF、IL-4、IL-10、IgA、IgM、IgG水平(P<0.05),降低大鼠直肠组织IFN-γ、TNF-α、TLR4、Myd88、NF-κB p65、IκBα表达及TLR4、Myd88、NF-κBp65、IL-6 mRNA表达(P<0.01).结论 托里消毒散高剂量可改善放射性直肠炎大鼠的直肠组织病理学改变,对TLR4/NF-κB信号通路相关因子及炎性细胞因子的表达均具有一定作用.
单纯性肥胖是一种临床常见的由多种因素引起的慢性代谢性疾病.脾阴学说是脾胃学说的一个分支,在指导中医"脾"病的诊疗过程中发挥着重要的作用.基于"脾阴学说"探讨单纯性肥胖发病的病机及治疗原则.认为"脾阴亏损,健运失司,痰浊内生"是单纯性肥胖不容忽视的病机,临证则以滋脾助运,化浊祛湿为治疗原则,从"脾阴"的角度为该病的临床干预提供一条新的思路.
Based on clinical observation and literature review, I found that the occurrence and development of colorectal cancer related to the damage of YangQI due to the stasis of dampness, heat,blood stasis as well as poison. Furthermore, it is pointed out from the perspective of integrating TCM and western medicine,that the occurrence of tumor disease is the accumulation of the body injury reaction, which is the long-term consolidation of the pathological factors such as dampness, heat, phlegm and blood stasis, which leads to the toxin, damages YangQi, and leads to the occurrence of cancer,it is also discussed that the method of help and suplement of Yang should be used in the whole process of tumor treatment.
