Objectives The ankle fracture with both deltoid ligament (DL) rupture and syndesmotic diastasis was treated by fixing syndesmosis after open reduction and plate fixation of the fibular fracture. The present study aimed to evaluate the effect of deltoid ligament augmentation (DLA) replacing syndesmosis fixation for the treatment of such a fracture. Methods This randomized controlled trial recruited acute ankle fracture patients with syndesmotic instability and DL rupture. Patients were randomized into DLA and trans-syndesmotic screw fixation (TSSF) groups. The primary outcome measure was the American Orthopedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot scale at 1 year post-surgery. The secondary outcome measures included Philips and Schwartz clinical scoring system of the ankle, the short-form (SF-36) questionnaire for quality of life, the range of ankle motion, the time to partial weight-bearing, the time to full weight-bearing, the time to return to previous work, the time to preinjury-level play, radiographic parameters, and complications. The analyses followed the intention-to-treat principle. Results A total of 60 patients were randomized, and 59 were included for analysis: 30 in the DLA group and 29 in the TSSF group. The AOFAS scores at 1 year after surgery showed no statistical significance between the two groups. However, clinical scores at 3 months after surgery, including AOFAS score, Philips and Schwartz score, and subscales of SF-36 (physical function, general health, and social function), were significantly better in the DLA group than the TSSF group. Time to partial-weight-bearing, full-weight-bearing, return to work, and return to preinjury-level play were significantly reduced in the DLA group. No major complications were observed in both groups, and no difference was detected in the radiographic parameters immediately after surgery and at 1-year follow-up. Conclusion DLA achieved satisfactory clinical and radiographic outcomes at 1-year follow-up with rapid recovery post-surgery. This treatment method should be considered as an alternative option in ankle fractures associated with deltoid ligament rupture and syndesmotic diastasis.
Abstract This review aimed to provide a comprehensive analysis of the etiology, epidemiology, pathology, and conventional treatment of heterotopic ossification (HO), especially emerging potential therapies. HO is the process of ectopic bone formation at non-skeletal sites. HO can be subdivided into two major forms, acquired and hereditary, with acquired HO predominating. Hereditary HO is a rare and life-threatening genetic disorder, but both acquired and hereditary form can cause severe complications, such as peripheral nerve entrapment, pressure ulcers, and disability if joint ankylosis develops, which heavily contributes to a reduced quality of life. Modalities have been proposed to treat HO, but none have emerged as the gold standard. Surgical excision remains the only effective modality; however, the optimal timing is controversial and may cause HO recurrence. Recently, potential therapeutic strategies have emerged that focus on the signaling pathways involved in HO, and small molecule inhibitors have been shown to be promising. Moreover, additional specific targets, such as small interfering RNAs (siRNAs) and non-coding RNAs, could be used to effectively block HO or develop combinatorial therapies for HO.
Objective:This study aimed to assess and compare the performance of ChatGPT and iFlytek Spark, two AI-powered large language models (LLMs), in generating clinical recommendations aligned with expert consensus on Enhanced Recovery After Surgery (ERAS)-guided ankle fracture treatment. This study aims to determine the applicability and reliability of AI in supporting ERAS protocols for optimized patient outcomes. Methods:A qualitative comparative analysis was conducted using 35 structured clinical questions derived from the Expert Consensus on Optimizing Ankle Fracture Treatment Protocols under ERAS Principles. Questions covered preoperative preparation, intraoperative management, postoperative pain control and rehabilitation, and complication management. Responses from ChatGPT and iFlytek Spark were independently evaluated by two experienced trauma orthopedic specialists based on clinical relevance, consistency with expert consensus, and depth of reasoning. Results:ChatGPT demonstrated higher alignment with expert consensus (29/35 questions, 82.9%), particularly in comprehensive perioperative recommendations, detailed medical rationales, and structured treatment plans. However, discrepancies were noted in intraoperative blood pressure management and preoperative antiemetic selection. iFlytek Spark aligned with expert consensus in 22/35 questions (62.9%), but responses were often more generalized, less clinically detailed, and occasionally inconsistent with best practices. Agreement between ChatGPT and iFlytek Spark was observed in 23/35 questions (65.7%), with ChatGPT generally exhibiting greater specificity, timeliness, and precision in its recommendations. Conclusion:AI-powered LLMs, particularly ChatGPT, show promise in supporting clinical decision-making for ERAS-guided ankle fracture management. While ChatGPT provided more accurate and contextually relevant responses, inconsistencies with expert consensus highlight the need for further refinement, validation, and clinical integration. iFlytek Spark's lower conformity suggests potential differences in training data and underlying algorithms, underscoring the variability in AI-generated medical advice. To optimize AI's role in orthopedic care, future research should focus on enhancing AI alignment with medical guidelines, improving model transparency, and integrating physician oversight to ensure safe and effective clinical applications.
目的 探讨Er:YAG激光不同模式、不同参数照射对种植体SA、SLA、Xpeed及RBM表面微形貌的影响.方法 选择波长为2940 nm的Er:YAG激光并设置为软组织模式及参数50 mJ、10 Hz,100 mJ、10 Hz,200 mJ、10 Hz;设置硬组织模式及参数100 mJ、10 Hz,200 mJ、10 Hz.均在水冷却环境下,采取非接触模式,工作尖距离种植体表面1 mm,对SA、SLA、Xpeed及RBM 4种种植体表面固定一点照射5 s.用扫描电子显微镜(SEM)观察照射前后表面微形貌的变化.结果 SEM观察可见,SA表面在软组织模式50 mJ、10 Hz及100 mJ、10 Hz照射下无变化,能量提高到200 mJ、10 Hz可见表面微形貌部分熔化;SLA表面在软组织模式50 mJ、10 Hz照射下无变化,能量提高到100 mJ、10Hz及200 mJ、10 Hz可见表面部分熔化及完全熔化;SA及SLA表面在硬组织模式100 mJ、10 Hz或200 mJ、10 Hz照射下,表面无变化.Xpeed和RBM表面在软组织模式50 mJ、10 Hz照射下分别表现为高峰塌陷和片状熔化;在能量提高到100 mJ、10 Hz及200 mJ、10 Hz均可见部分熔化及完全熔化;Xpeed及RBM表面在硬组织模式100 mJ、10 Hz照射下无变化,能量提高到200 mJ、10 Hz可见部分熔化;Xpeed表面未见涂层剥脱.结论 本研究显示,在用Er:YAG激光处理种植体表面时,为避免对种植体表面造成损伤,SA和SLA表面用软组织模式时能量参数需设置在50 mJ、10 Hz以下,硬组织模式时参数设置200 mJ、10 Hz以下.Xpeed及RBM表面不适合用软组织模式处理,可在硬组织模式100 mJ、10 Hz以下处理.
Objective:To elucidate the recent therapeutic efficacy of the intelligent fracture reduction robotic system in managing pelvic fractures.Methods:A retrospective evaluation of 49 pelvic fracture patients treated using the intelligent fracture reduction robotic system at Beijing Jishuitan Hospital's trauma orthopedics department between March 2021 and December 2022 was conducted. The cohort included 30 males and 19 females, with a mean age of 51.51±18.71 years (20-92 years range). Fractures were classified according to the Tile system: B1 type in 2 cases, B2 in 7, B3 in 3, C1 in 30, and C2 in 3. The median interval between injury and surgery was 6 days, with a range of 2-22 days. The robotic system assisted in pelvic fracture reduction and stabilization surgeries. Preoperative and postoperative evaluations involved pelvic CT scans, anteroposterior, inlet, and outlet radiographic images. Fracture displacement and reduction outcomes were assessed via X-ray imagery. Data captured included intraoperative blood loss, duration of surgery, fracture stabilization techniques, and postoperative monitoring period. The Majeed scoring system gauged functional outcomes.Results:Of the patients, 48 underwent minimally invasive interventions with robotic assistance, while one case necessitated open reduction and internal fixation due to an unsuccessful reduction. The duration between injury and operation ranged from 2 to 22 days. Average surgical time stood at 206.5±7.1 minutes (105-440 min range), and median intraoperative blood loss was 100ml (10-600 ml range). Using the Matta reduction criteria, 30 postoperative cases exhibited excellent and 9 good outcomes for posterior pelvic ring displacement, translating to a 93% (38/41) positive rate. For anterior pelvic ring shifts, 45 showed excellent and 3 good outcomes, culminating in a 100% (48/48) success rate. Follow-up for the 48 cases lasted 11.0 months (3-23 months range), with the Majeed functional score averaging 81.9±17.0 points (42-100 point range). 27 cases scored excellent, and 11 good, yielding a combined positive outcome rate of 79.2% (38/48).Conclusion:Employing the intelligent fracture reduction robotic system in pelvic fracture treatments facilitates minimally invasive interventions and yields favorable short-term clinical results.
Objective:To evaluate the emergency iliosacral screw fixation assisted by TiRobot for unstable posterior pelvic ring fracture.Methods:The 26 patients with unstable pelvic fracture were analyzed retrospectively who had undergone emergency iliosacral screw fixation at Department of Orthopedics & Traumatology, Beijing Jishuitan Hospital from June 2018 to December 2020. They were divided into 2 groups depending on whether orthopaedic TiRobot was used to assist screw insertion. In the observation group of 14 cases subjected to TiRobot-assisted insertion of iliosacral screws, there were 10 males and 4 females with an age of (45.9 ± 10.1) years; in the control group of 12 cases subjected to conventional manual insertion of iliosacral screws, there were 9 males and 3 females with an age of (49.2 ± 11.3) years. All the surgeries were conducted within 24 hours after injury. The 2 groups were compared in terms of screw insertion time, pin insertion, intraoperative blood loss, fluoroscopy time, postoperative screw position, fracture reduction and Harris hip score at the final follow-up.Results:The 2 groups were comparable because there was no significant difference between them in their preoperative general clinical data or follow-up time ( P>0.05). The screw insertion time [(16.1 ± 3.4) min] and fluoroscopy time [(8.1 ± 3.3) s] in the observation group were significantly shorter than those in the control group [(26.4 ± 5.4) min and (25.2 ± 7.4) s], and the pin insertions [1 (1, 2) times] and intraoperative blood loss [(10.5 ± 6.4) mL] in the former were significantly less than those in the latter [6 (3, 8) times and (24.8 ± 6.7) mL] (all P<0.05). Postoperatively, the sacroiliac screw position was excellent in 18 cases and good in 2 in the observation group while excellent in 14 cases, good in 2 and poor in 2 in the control group; the fracture reduction was excellent in 12 cases, good in one and fair in one in the observation group while excellent in 10 cases, good in one and fair in one in the control group, showing insignificant differences in the above comparisons ( P>0.05). There was no significant difference either in the Harris hip score at the final follow-up between the 2 groups ( P>0.05). Conclusion:Compared with conventional manual insertion of iliosacral screws, emergency iliosacral screw fixation assisted by TiRobot can effectively decrease surgical time and reduce operative invasion due to a higher accuracy rate of screw insertion.

OBJECTIVE:Currently, minimally invasive internal fixation is recommended for the surgical treatment of unstable pelvic fractures. The premise and difficulty of minimally invasive internal fixation are minimally invasive reduction of fractures. This review aimed to investigate the indications, surgical strategy and techniques, safety, and efficacy of intelligent robot-assisted fracture reduction (RAFR) system of pelvic ring injuries.METHODS:This retrospective study reviewed a case series from March 2021 to November 2021. A total of 22 patients with unstable pelvic fracture injuries underwent minimally invasive internal fixations. All pelvic ring fractures were reduced with our intelligent RAFR system. The robot system intelligently designs the optimal position and reduction path based on the patient's preoperative 3D CT. During the operation, the three-dimensional visualization of the fracture is realized through image registration, and the Robot completes the automatic reduction of the fracture. The global 3D point cloud error between the preoperative planning results and the actual postoperative reduction results was calculated. The postoperative reduction results of residual displacement were graded by the Matta Criteria.RESULTS:Minimally invasive closed reduction procedures were completed in all 22 cases with our RAFR system. The average global 3D point cloud reduction error between the preoperative planning results and the actual postoperative reduction results was 3.41mm±1.83mm. The mean residual displacement was 4.61mm±3.29mm. Given the Matta criteria, 16 cases were excellent, five were good, and one was fair, with an excellent and good rate of 95.5%.CONCLUSION:Our new pelvic fracture reduction robot system can complete intelligent and minimally invasive fracture reduction for most patients with unstable pelvic fractures. The system has intelligent reduction position and path planning and realizes stable pelvis control through a unique holding arm and a robotic arm. The operation process will not cause additional damage to the patient, which fully meets the clinical requirements. Our study demonstrated the safety and effectiveness of our robotic reduction system and its applicability and usability in clinical practice, thus paving the way towards Robot minimally invasive pelvic fracture surgeries.
Abstract Background Long-term fasting for elective surgery has been proven unnecessary based on established guidelines. Instead, preoperative carbohydrate loading 2 h before surgery and recommencing oral nutrition intake as soon as possible after surgery is recommended. This study was performed to analyze the compliance with and effect of abbreviated perioperative fasting management in patients undergoing surgical repair of fresh fractures based on current guidelines. Methods Patients with fresh fractures were retrospectively analyzed from the prospectively collected database about perioperative managements based on enhanced recovery of surgery (ERAS) from May 2019 to July 2019 at our hospital. A carbohydrate-enriched beverage was recommended up to 2 h before surgery for all surgical patients except those with contraindications. Postoperatively, oral clear liquids were allowed once the patients had regained full consciousness, and solid food was allowed 1 to 2 h later according to the patients’ willingness. The perioperative fasting time was recorded and the patients’ subjective comfort with respect to thirst and hunger was assessed using an interview-assisted questionnaire. Results In total, 306 patients were enrolled in this study. The compliance rate of preoperative carbohydrate loading was 71.6%, and 93.5% of patients began ingestion of oral liquids within 2 h after surgery. The median (interquartile range) preoperative fasting time for liquids and solids was 8 (5.2–12.9) and 19 (15.7–22) hours, respectively. The median postoperative fasting time for liquids and solids was 1 (0.5–1.9) and 2.8 (2.2–3.5) hours, respectively. A total of 70.3% and 74.2% of patients reported no thirst and hunger during the perioperative period, respectively. Logistic regression analysis showed that the preoperative fasting time for liquids was an independent risk factor for perioperative hunger. No risk factor was identified for perioperative thirst. No adverse events such as aspiration pneumonia or gastroesophageal reflux were observed. Conclusions In this study of a real clinical practice setting, abbreviated perioperative fasting management was carried out with high compliance in patients with fresh fractures. The preoperative fasting time should be further shortened to further improve patients’ subjective comfort.
Objective:To evaluate the effectiveness of perioperative fasting abbreviation in traumatic patients with orthopaedic trauma and diabetes mellitus undergoing selective surgery.Methods:The patients were selected for this prospective nonrandomized controlled study who had undergone selective surgery from June 2019 to June 2021 at Department of Orthopaedic Trauma, Beijing Jishuitan Hospital. They were divided into an intervention group and a control group according to the wards where they stayed. The intervention group was fasted for solids from 0 o'clock on the surgery day and received oral solution with 6.25% maltodextrin which had been prepared by the nutritional department 3 hours prior to surgery. The control group was fasted for either liquids or solids from the midnight before surgery. All patients were evaluated according to the wake-up score and defensive reflex score after surgery. Once they were awakened, they were allowed slag-free drinks. Normal food was allowed if there was no discomfort after 2 hours. The 2 groups were compared in terms of basic information, actual preoperative fasting time, total amount of preoperative drinking, and postoperative time for initial drinking and eating. The perioperative subjective feelings (anxiety, thirst, hunger, nausea, fatigue, dizziness, sweating, stomach discomfort, etc.), grip strength and blood glucose were observed and compared between the 2 groups. Adverse reactions in the 2 groups were also observed.Results:A total of 135 patients were included, including 52 in the intervention group and 83 in the control group. The intervention group consisted of 22 males and 30 females aged from 30 to 84 years; the control group consisted of 39 males and 44 females aged from 29 to 81 years. There was no significant difference in the basic information between the 2 groups, showing comparability ( P>0.05). The intervention group had significantly shorter preoperative fasting time [3.5 (2.5, 6.3) h versus 12.0 (9.0, 16.0) h], significantly higher water intake before surgery [300 (200, 300) mL versus 100 (100, 200) mL], significantly shorter postoperative fasting time [0.08 (0, 1.25) h versus 2.00(0, 6.00) h], and significantly reduced time to return to normal diet [2.0 (2.0, 2.3) h versus 3.0(2.0, 6.0) h] than the control group (all P<0.05). The symptoms of anxiety, fatigue, sweating, and stomach discomfort in the intervention group were significantly fewer than those in the control group throughout the evaluation period. The thirst in the intervention group was significantly alleviated than that in the control group immediately after returning to the ward after surgery, and the dizziness and hunger were significantly alleviated than those in the control group when the patients left the ward to the operation room before surgery and immediately after returning to the ward. The symptom of nausea after returning to normal diet in the intervention group was significantly relieved compared with the control group. All the comparisons above showed statistically significant differences ( P<0.05). The blood glucose in the intervention group 2 hours after taking slag-free drinks was significantly higher than that in the control group ( Z=-2.108, P=0.035). There was no significant difference in the blood glucose between the 2 groups during other measurement periods ( P>0.05). There were no serious adverse reactions in either of the 2 groups. Conclusion:The protocol of perioperative fasting abbreviation may be safe and feasible for the patients with orthopaedic trauma and diabetes mellitus undergoing selective surgery, because it shows benefits of improving the patients' subjective feelings and stabilizing the blood glucose perioperatively.
Objective:To investigate the current situation of perioperative fasting management in patients with orthopaedic trauma waiting for elective surgery (those combined with diabetes mellitus included) and the surgeons' awareness in China.Methods:From November 1st to December 31st, 2021, the questionnaire forms were distributed through WeChat in the exchange group of National Enhanced Recovery Surgery (ERAS) and the exchange group of national training orthopaedists in Beijing Jishuitan Hospital. The survey contents included: time for preoperative water deprivation and for postoperative recovery of drinking in patients without diabetes mellitus, the rationales for orthopaedists to choose perioperative dietary management, the orthopaedists'understanding of the existing guidelines, time for preoperative water deprivation and postoperative recovery of drinking and diabetes-related issues in patients with diabetes mellitus, and the time for postoperative recovery in all the patients (For the postoperative recovery of eating, there is no difference between patients with and without diabetes mellitus). The relationships were analyzed between some professional data of the orthopaedists and their rationales for choice of management strategies and their understanding of the guidelines.Results:A total of 565 valid questionnaires were collected. 12.92% (73/565) of orthopaedists required their patients without diabetes mellitus not to drink for at least 2 hours. In fact, the proportion of water prohibition from 0 o'clock on the day of operation was still the highest [24.07% (136/565)]. Respectively, 22.83% (129/565) and 42.12% (238/565) of the orthopaedists chose "water intake is allowed once awakened" and "water intake after at least 6 hours after operation" for their patients without diabetes mellitus. 33.98% (192/565) of the orthopaedists required all the patients fasted for at least 6 hours before surgery, and 44.25% (250/565) of the orthopaedists chose "eating can be resumed if there is no discomfort for 2 hours after water intake" .21.06% (119/565) of the orthopaedists demonstrated that they were quite familiar with the guidelines and carried out perioperative dietary management according to the guidelines. The management of water deprivation was more inconsistent for patients with diabetes mellitus, and more hospitals followed the traditional principles for water deprivation. The proportions of water deprivation starting at 0 o'clock on the day of operation, 8 hours before operation, 6 hours before operation and 4 hours before operation accounted respectively for 22.83% (129/565), 19.12% (108/565), 21.95% (124/565), and 18.94%% (107/565). The level of an orthopaedist's hospital and the professional rank of an orthopaedist were the factors related to the orthopaedist's understanding of the guidelines ( P<0.05). Conclusions:The current perioperative dietary management guidelines are not widely implemented or well known in Chinese faculties of orthopaedic trauma. The process of perioperative dietary management needs to be optimized for the patients combined with diabetes mellitus.
Objective:To compare the outcomes between hook thin plate compression technique and conventional screw-plate fixation in the treatment of adolescent tibial tubercle avulsion fractures.Methods:A retrospective analysis was performed of the 43 adolescent patients with tibial tubercle avulsion fracture who had been treated at Department of Orthopedics and Traumatology, Beijing Jishuitan Hospital from January 2018 to October 2020. There were 42 males and one female, aged from 13 to 17 years. According to their treatment methods, they were divided into an observation group (9 cases) treated with hook thin plate compression technique and a control group (34 cases) treated with fixation with cannulated screws alone or in addition with a 1/3 tube plate. The maximum range of knee flexion, B?stman score, and cases returning to school at one month after operation, as well as fracture union time, B?stman score and cases with complications at the last follow-up were compared between the 2 groups.Results:There were no significant differences in the preoperative general data between the 2 groups, showing comparability ( P>0.05). By the postoperative one month, the maximum range of knee flexion [90.0° (85.0°, 102.5°)], B?stman score [21.0 (18.5, 21.0)] and cases returning to school (7) in the observation group were significantly better than those in the control group [22.5° (15.0°, 30.0°), 11.0 (10.0, 13.0), and 0] ( P<0.001). The last follow-up revealed no significant differences between the 2 groups in frature union time [(2.7±0.5) months versus (2.8±0.5) months], B?stman score [30.0 (30.0, 30.0) versus 30.0 (30.0, 30.0), 30.0)] or cases with complications (1 versus 2) ( P>0.05). Conclusion:In the treatment of adolescent tibial tubercle avulsion fractures, compared with conventional fixation with cannulated screws alone or in addition with a 1/3 tube plate, hook thin plate compression technique may lead to better outcomes, because it allows early rehabilitation to shorten the postoperative immobilization time and promote early functional recovery.

Background The enhanced recovery after surgery (ERAS) program is aimed to shorten patients' recovery process and improve clinical outcomes. This study aimed to compare the outcomes between the ERAS program and the traditional pathway among patients with ankle fracture and distal radius fracture. Methods This is a multicenter prospective clinical controlled study consisting of 323 consecutive adults with ankle fracture from 12 centers and 323 consecutive adults with distal radial fracture from 13 centers scheduled for open reduction and internal fixation between January 2017 and December 2018. According to the perioperative protocol, patients were divided into two groups: the ERAS group and the traditional group. The primary outcome was the patients' satisfaction of the whole treatment on discharge and at 6 months postoperatively. The secondary outcomes include delapsed time between admission and surgery, length of hospital stay, postoperative complications, functional score, and the MOS item short form health survey-36. Results Data describing 772 patients with ankle fracture and 658 patients with distal radius fracture were collected, of which 323 patients with ankle fracture and 323 patients with distal radial fracture were included for analysis. The patients in the ERAS group showed higher satisfaction levels on discharge and at 6 months postoperatively than in the traditional group (P < 0.001). In the subgroup analysis, patients with distal radial fracture in the ERAS group were more satisfied with the treatment (P=0.001). Furthermore, patients with ankle fracture had less time in bed (P < 0.001) and shorter hospital stay (P < 0.001) and patients with distal radial fracture received surgery quickly after being admitted into the ward in the ERAS group than in the traditional group (P=0.001). Conclusions Perioperative protocol based on the ERAS program was associated with high satisfaction levels, less time in bed, and short hospital stay without increased complication rate and decreased functional outcomes.
Objective: To evaluate the clinical outcome of open reduction and internal fixation via digastric trochanteric flip osteotomy for acetabular posterior wall fracture with hip dislocation. Methods: From January 2014 to December 2016 a total of 39 patients who suffered posterior wall acetabular fracture and hip dislocation and treated in Beijing Jishuitan Hospital were included in this retrospective study. All the patients were divided into two groups according to surgery type. There were 22 cases in osteotomy group who underwent digastric trochanteric flip osteotomy (DTFO) via Kocher-Langenbeck (K-L) approach, while 17 cases in control group who only received surgery via (K-L) approach. The Matta scale was used to evaluate fracture reduction, while functional recovery scale (FRS) and Euro-Quality of 5 Dimension (EQ-5D) were applied to measure the clinical effect. Results: The baseline characteristics were similar for both groups and the mean follow-up period was (48±11) months. All the surgeries were performed successfully. The blood loss (t=1.52, P=0.12), bed days (t=1.22, P=0.25), complication rate (χ²=2.02, P=0.16) and operation time (t=1.31, P=0.23) showed no significant difference between the 2 groups (P>0.05). The fracture reduction (χ²=0.05, P=0.81) were similar between the 2 groups but the excellent rate were higher in osteotomy group. According to evaluation scales the FRS score and EQ-5D index favored osteotomy group(85±13 vs 80±15 and 0.86±0.12 vs 0.80±0.17, respectively, t=2.87, 3.47, both P<0.05). Conclusion: K-L approach with DTFO can provide clearier surgical field which is convenient for procedure, and compared to K-L approach the clinical effect is more satisfactory.
Objective:To analyze the factors which may influence postoperative early ambulation in patients with fresh fracture so as to further optimize the perioperative protocol based on the concept of enhanced recovery after surgery (ERAS).Methods:A retrospective analysis was conducted of the patients who had been treated for a single fresh fracture at the extremity, pelvis or acetabulum from May 2019 to July 2019. Collected were the data concerning basic features of patients, perioperative ERAS management and surgery, as well as early ambulation on the day of surgery or the first day after surgery. The patients were divided into an early ambulation group and a non-early ambulation group according to the time of ambulation. Statistical analyses were performed of the relationships between early ambulation and 20 potential factors concerning basic features of patients, perioperative ERAS management and surgery. Logistic correlation analysis was performed to identify risk factors for postoperative early ambulation.Results:A total of 306 patients were included, including 96 upper limb, 203 lower limb, 5 acetabular and 2 pelvic fractures. Of them, 150 ambulaed from bed on the day of surgery or the first day after surgery while 156 did not. Significant differences were observed between the 2 groups in fracture site, intake of carbohydrate liquids the night before surgery and the day of surgery, catheter indwelling, intraoperative liquid transfusion volume and postoperative complications ( P < 0.05). Logistic correlation analysis of the relationship between the above factors and postoperative early ambulation found that fracture site and intraoperative liquid transfusion volume were significantly correlated with postoperative early ambulation ( P < 0.05). Conclusions:About half of the patients with a single fresh fracture may ambulate early after surgery. Fracture site and intraoperative liquid transfusion volume may be significantly correlated with postoperative early ambulation.
Objective:To compare the perioperative data between the pararectus and ilioinguinal approaches in the internal fixation of acetabular fractures.Methods:A randomized controlled trial was conducted to enroll 74 patients with acetabular fracture who were admitted to Department of Orthopaedic Trauma, Beijing Jishuitan Hospital from June 2018 to January 2021. They were randomly divided into 2 groups. In group A of 37 cases, there were 28 males and 9 females with an age of (55.8±15.2) years, and 10 anterior column fractures, 7 anterior+posterior hemi-transverse fractures, one transverse fracture, 3 T-shaped fractures and 16 both column fractures according to Letournel-Judet classification. In group B of 37 cases, there were 28 males and 9 females with an age of (49.4±14.6) years, and 8 anterior column fractures, 6 anterior+ posterior hemi-transverse fractures, 2 transverse fractures, 2 T-shaped fractures and 19 both column fractures according to Letournel-Judet classification. Group A was subjected to open reduction and plate fixation via the pararectus approach while group B to open reduction and plate fixation via the ilioinguinal approach. The 2 groups were compared in terms of operation time, intraoperative blood loss, postoperative reduction and perioperative complications.Results:There were no significant differences in baseline data between the 2 groups, showing comparability between them ( P>0.05). The operation time in group A [150 (120, 180) min] was significantly shorter than that in group B [180 (150, 225) min] ( P<0.05). There were no significance differences between the 2 groups in intraoperative blood loss [800 (600, 1, 000) mL versus 1, 000(600, 1, 300) mL], rate of good to excellent reduction [91.9%(34/37) versus 78.4%(29/37)], or incidence of complications [24.3%(9/37) versus 45.9%(17/37)] ( P>0.05). Conclusion:Compared with the ilioinguinal approach, the pararectus approach can reduce operation time significantly in the internal fixation of acetabular fractures.
背景:重症坏死性软组织感染(NSTI)临床罕见,临床医师对其认识不足.目的:总结NSTI的临床特点、致病微生物及预后,为该病的临床诊治提供参考.方法:回顾性分析2015年1月至2020年1月收治的NSTI患者13例.记录患者基本信息、入院实验室检查结果、病原学数据、临床结果、NSTI预测评分[坏死性筋膜炎实验室风险指标(LRINEC)评分、SIARI评分、NAS评分]、患者危重程度评分[APACHEⅡ评分、序贯器官衰竭估计(SOFA)评分].结果:本组13例确诊为NSTI的患者死亡2例(15.4%).感染部位:下肢9例(69.2%),上肢、骶尾部、会阴区和腹部各1例.LRINEC评分、SIARI评分和NAS评分预测NSTI的假阴性率分别为7.7%(1/13)、38.5%(5/13)和23.1%(3/13).7例患者(53.8%)伤口分泌物粗大杆菌涂片阳性.Ⅰ型(混合微生物感染)6例(46.15%),Ⅱ型(单一微生物感染)6例(46.15%),无培养结果1例(7.7%).2例死亡患者均为化脓性链球菌感染.Ⅰ型与Ⅱ型NSTI患者实验室检查结果差异均无统计学意义(P均>0.05).从入院到初次手术的时间为9(4-15)h,重症监护治疗病房(ICU)住院时间为15(4-48)d,总住院时间为36(8-74)d.结论:早期诊断和治疗是改善NSTI患者预后最重要的手段.产气荚膜梭菌和化脓性链球菌感染是导致NSTI患者死亡的主要致病菌.目前LRINEC评分可能仍是对NSTI较有价值的筛查工具.
Pelvic fracture is the most complicated fracture in traumatic orthopedics. Tremendous muscle resistance is the main difficulty in carrying out pelvic reduction for the physician or robot-assisted reduction surgery in the future. During pelvic fracture surgery, the traction method is commonly utilized to reduce the reduction force against the sizeable muscle resistance. We proposed an elastic traction method to provide flexibility in the pelvic reduction and reduce the force needed to reset the pelvic. According to the experimental results in this paper, when adopting the elastic traction method in surgery, the reduction force can be reduced up to 56.2%. A musculoskeletal model of pelvic fracture reduction based on spring constraints was also established and verified by the experimental data. Through the simulation model, we investigated the influences of spring stiffness on the performance of elastic traction. The simulation results show that the minor spring stiffness is, the smaller the reduction force would be. In addition, we compared the maximum stress and reduction force applied on k-wire during a single K-wire pathway holding to complete reduction. We consider that the Kirschner pin 1 pathway is the optimal pathway in pelvic reduction.
在加速康复外科的研究中,围术期饮食管理是重要的一环.以往的加速康复外科研究通常将糖尿病视为相对禁忌证,但糖尿病患者更加需要科学的围术期饮食管理.本文通过文献综述,总结了糖尿病患者胃轻瘫的危险因素,胃排空的定性及定量测量方法,以及糖尿病患者围术期饮食研究方面的进展.目前认为合并糖尿病的骨折患者可以参照加速康复外科围术期禁食水流程处理,缩短术前禁食水时间,但对于高风险患者需警惕糖尿病性胃轻瘫.快速床旁B超检查是术前评估胃内容物的有效方式,可以通过定性及定量测量的方法评估患者的误吸风险.术前3h饮用总量小于400ml的碳水化合物饮品不会造成糖尿病患者术前及术中血糖升高,还可减轻胰岛素抵抗,提高患者主观舒适度.
