Background/Objectives: The therapeutic window for growth hormone (GH) therapy in late-pubertal males with advanced bone age (BA ≥ 15 years) is generally considered closed. Yet, open knee physes may indicate residual growth potential. This single-arm retrospective study evaluated the efficacy and safety of letrozole combined with GH in Chinese males with short stature, advanced BA (15 -< 18 years), and open knee physes. Methods: We included 139 male adolescents stratified by baseline BA: 15 ≤ BA < 16 (n = 24), 16 ≤ BA < 17 (n = 31), and 17 ≤ BA < 18 (n = 84). All received subcutaneous GH (0.05-0.07 mg/kg/day) and oral letrozole (2.5 mg/day). The primary outcome was the change in final adult height (FAH) from baseline predicted adult height (PAH). Longitudinal changes in height velocity and height SDS-BA were also assessed. Results: Among 75 patients who reached FAH, combination therapy was associated with a mean gain of 6.58 ± 3.46 cm over baseline PAH (p < 0.001). The gain was inversely correlated with baseline BA; the largest gain occurred in the 15 ≤ BA < 16 subgroup (10.04 ± 4.66 cm), significantly exceeding gains in the 16 ≤ BA < 17 (6.30 ± 3.01 cm) and 17 ≤ BA < 18 (5.70 ± 2.52 cm) subgroups (p < 0.001). In the youngest subgroup, FAH exceeded mid-parental height. The safety profile was acceptable; acne was the most common adverse event (46.76%), and no severe metabolic or endocrine disorders were observed. Conclusions: Letrozole plus GH therapy may be associated with improved FAH in late-pubertal males with advanced BA and open knee physes, with gains varying by baseline skeletal maturity and being numerically greatest in those with BA 15 -< 16 years. Knee epiphyseal status may be a useful consideration when individualizing therapy in this population.
OBJECTIVE:To evaluate the level of first morning voided (FMV) urinary luteinizing hormone (LH) in girls with breast development, and to determine the value of FMV urine LH in the evaluation of central precocious puberty (CPP). METHODS:From September 2018 to April 2021, among the patients who were admitted to the Department of Pediatrics of Peking University Third Hospital for "precocious puberty" and underwent gonadotropin-releasing hormone (GnRH) stimulation test, a total of 108 girls were enrolled. According to CPP diagnostic criteria, they were divided into CPP group (n=45) and non-CPP group (n=63). The clinical characteristics and hormone levels of the two groups were compared. Receiver operating characteristic (ROC) curve was used to analyze the cut-off value of FMV urinary LH in the diagnosis of CPP in girls. Further analyses were done to evaluate the value of FMV urinary LH in the diagnosis of CPP using correlation analysis between urinary LH level and common clinical cha-racteristics. RESULTS:ROC curve analysis showed that FMV urine LH level was significant for the diagnosis of CPP. The cut-off value of FMV urine LH was 0.69 IU/L (specificity 56.9%, sensitivity 85.0%, area under curve 0.804, P < 0.001). The basic clinical characteristics without GnRH stimulation test were analyzed by binary Logistic regression analysis, indicating that the level of FMV urine LH, uterine volume, ovarian volume and advanced T-bone age had predictive significance for CPP diagnosis in girls (OR values were 2.125, 1.961, 1.564 and 2.672, respectively). The prediction model was established and the area under the ROC curve was 0.904, P < 0.001. The level of FMV urine LH was positively correlated with the levels of serum LH, FSH and estrogen before GnRH stimulation test, the peak value of blood LH after GnRH stimulation test, T bone age and uterine volume, with r values of 0.462, 0.373, 0.242, 0.360, 0.373 and 0.263, respectively, and P values were < 0.001, < 0.001, 0.013, < 0.001, < 0.001 and 0.007, respectively. CONCLUSION:FMV urine LH can provide a good indication for the diagnosis of CPP. Combining with bone age advanced level and pelvic ultrasound measurement, the predictive value of FMV urine LH can be further improved for the diagnosis of CPP in girls.
Objective:To explore the therapeutic mechanism of Mongolian medicine Sendeng-4 decoction for rheumatoid arthritis by 99Tc m-hydrazinonicotinamide-(polyethylene glycol) 4-E[(polyethylene glycol) 4-c((Arg-Gly-Asp)fk)] 2 (3PRGD 2) imaging. Methods:A total of 200 female SD rats (age: 6-7 weeks) were divided into collagen-induced arthritis (CIA) group ( n=176) and blank control group ( n=24). Rats in the CIA group were divided into Sendeng-4 decoction treatment group ( n=24), etanercept treatment group ( n=24), and negative control group ( n=24) by simple random sampling method. 99Tc m-3PRGD 2 SPECT/CT imaging was performed before and after modeling and treatment. The differences of target/non-target (T/NT) ratio and serological, pathological, and immunohistochemical results among groups were compared by one-way analysis of variance or Kruskal-Wallis rank sum test. The correlation was analyzed by Pearson correlation or Spearman correlation analysis. Results:There were 95 (95/176) CIA models successfully established. The T/NT ratios of Sendeng-4 decoction treatment group and etanercept treatment group were lower than that of negative control group (0.260± 0.094, 0.238±0.099, 0.766±0.144 ; F=163.00, P<0.001), while there was no significant difference between the two drug treatment groups ( P>0.05). After drug treatment, serum levels of vascular endothelial growth factor (VEGF), tumor necrosis factor-α (TNF-α) and α vβ 3 were significantly lower than those of negative control group ( F values: 49.43-92.36, all P<0.001), pathological score was also lower than that of negative control group ( H=34.25, P<0.001), and levels of immunohistochemical makers (VEGF, TNF-α, α vβ 3, CD31, CD34) were also lower than those of negative control group ( H values: 13.51-26.84, all P<0.001), while there were no significant differences between the two drug treatment groups (all P>0.05). The T/NT ratios were positively correlated with above indictors in Sendeng-4 decoction treatment group ( r values: 0.56-0.59, rs values: 0.49-0.69), etanercept treatment group ( r values: 0.50-0.55, rs values: 0.46-0.70) and negative control group ( r values: 0.55-0.80, rs values: 0.58-0.86, P<0.001 or P<0.05). Conclusion:Verified by 99Tc m-3PRGD 2 SPECT/CT imaging and molecular pathology, Mongolian medicine Sendeng-4 decoction can inhibit neovascularization by down-regulating vascular factors such as VEGF, resulting in delaying the progression of the disease and improving clinical symptoms.
胸痛患者到急诊科就诊是常见现象,但心肌短暂缺血引起的胸痛症状持续时间较短,胸痛患者就诊时症状已经消失或缓解.在胸痛消失后,常规的心脏检查方法 一般难以准确诊断曾经发生过的心肌短暂缺血,这将会导致心肌缺血的漏诊.心肌短暂缺血后,心肌细胞血流减少,同时也会在细胞代谢、基因分子、受体等方面产生一定的变化,且这些变化的持续时间比心肌血流变化的持续时间较长,基于以上变化的分子影像学检查能够检测到症状消失后的心肌缺血,这种现象称心肌"缺血记忆".本文基于"缺血记忆"原理,在细胞代谢、基因分子、受体等方面阐述分子影像学诊断心肌缺血的新进展.
A girl, aged 15 years, was admitted due to sudden convulsion once and multiple pulmonary nodules on lung CT. Acrocyanosis or acropachy/toe deformity was not observed. Laboratory examinations showed an increase in hemoglobin (162 g/L) and a reduction in arterial partial pressure of oxygen (61.5 mm Hg). Lung CT showed irregular slightly high-density nodules in the middle lobe of the right lung, and contrast-enhanced CT scan showed obvious enhancement with thick vascular shadow locally. An investigation of medical history revealed that the girl's mother had a history of epistaxis and resection of pulmonary mass and the girl presented with tongue telangiectasia. The girl was diagnosed with hereditary hemorrhagic telangiectasia and pulmonary arteriovenous malformation. she was given interventional embolization therapy. Transcutaneous oxygen saturation reached 98% without oxygen inhalation on the day after surgery. Pulmonary angiography at 3 months after surgery showed the recurrence of pulmonary vascular malformation, and embolization of pulmonary arterial fistula was not performed since the guide wire could not enter the branch artery. There was still a need for long-term follow-up.
近年来,肺癌发病率逐年上升,也是恶性肿瘤中致死率排名第1位的疾病,晚期患者生存率极低.血管生成和淋巴结转移是恶性肿瘤的两个非常重要的特征,通常认为整合素αvβ3的表达与这两个特征均有关.放射性核素标记的精氨酸-甘氨酸-天冬氨酸肽能与整合素特异的结合,进而可以反映肿瘤血管的变化.达到对肺癌原发灶、转移灶及疗效的早期监测,本文对近年来国内外99mTc-3PRGD2 SPECT/CT用于肺部肿瘤诊断的研究进展进行综述,主要从99mTc-3PRGD2显像剂、9mTc-3PRGD2在肺癌原发灶显像方面、99mTc-3PRGD2 SPECT显像对非小细胞肺癌淋巴结转移监测、99mTc-3PRGD2 SPECT成像在肺癌患者远处转移的诊断、99mTc-3PRGD2 SPECT成像在肺癌患者化疗、靶向药物疗效评估等几个方面展开论述.
目的:探讨 18F-氟脱氧葡萄糖(FDG) PET/CT双时相显像及 18F-FDG联合 11C-胆碱(CHO) PET/CT多模态显像对原发性肝细胞肝癌(HCC)的诊断价值。 方法:回顾性分析2016年3月至2018年12月于内蒙古医科大学附属医院就诊的临床疑似原发肝占位性病变的73例患者的临床资料,其中男性41例、女性32例,年龄58~72岁;47例患者行 18F-FDG PET/CT双时相显像,26例患者行 18F-FDG联合 11C-CHO PET/CT多模态显像。分别测量并计算每个病变的最大标准化摄取值(SUV max)、肝本底SUV max、肿瘤SUV max/肝本底SUV max比值(T/L)。以病变良恶性为状态变量,分别以 18F-FDG、 18F-FDG延迟、 18F-FDG双时相的SUV max和 18F-FDG、 11C-CHO、 18F-FDG联合 11C-CHO的SUV max为检验变量绘制受试者工作特征(ROC)曲线并进行两两比较,以组织病理学检查或临床随访结果为"金标准",对比分析各种显像方法的诊断效能。计量资料的比较采用配对 t检验;采用 Z秩和检验比较各组间ROC曲线的差异。 结果:(1) 18F-FDG PET/CT双时相显像:47例患者共检出49个病变(其中32个为高代谢病变),经组织病理学检查或临床随访结果证实,40个为HCC(高分化8个、中低分化32个),9个为良性病变。中低分化HCC的病变SUV max、肝本底SUV max和T/L的差异均有统计学意义( t=4.51、-2.53、4.80,均 P<0.05);高分化HCC的病变SUV max和T/L的差异均有统计学意义( t=2.76、2.62,均 P<0.05);良性病变SUV max的差异无统计学意义( t=0.00, P>0.05 )。 18F-FDG PET/CT显像分别与其延迟显像、双时相显像的SUV max的ROC曲线间的差异均有统计学意义( Z=2.315、2.376,均 P<0.05);而延迟显像与双时相显像的SUV max的ROC曲线间的差异无统计学意义( Z=0.252, P>0.05)。(2) 18F-FDG联合 11C-CHO PET/CT显像:26例患者共检出26个病变(其中18个为高代谢病变),经组织病理学检查或临床随访证实,22个为HCC(高分化9个、中低分化13个),4个为良性病变。高分化HCC的病变SUV max、肝本底SUV max间的差异均有统计学意义( t=9.49、6.57,均 P<0.001),而T/L的差异无统计学意义( t=2.01, P>0.05);中低分化HCC的病变SUV max的差异无统计学意义( t=-1.68, P>0.05),肝本底SUV max、T/L间的差异均有统计学意义( t=8.41、-5.43,均 P<0.001);良性病变的SUV max的差异无统计学意义( t=1.51, P>0.05)。 18F-FDG与其联合 11C-CHO PET/CT的SUV max的ROC曲线间的差异有统计学意义( Z=2.037, P<0.05)。 结论:18F-FDG PET/CT双时相显像及 18F-FDG联合 11C-CHO PET/CT多模态显像可分别提高中低及高分化原发性HCC的检出率,对肝癌患者治疗方案的决策及临床预后判定有重要的指导价值。
Objective:To compare the quantitative parameters of myocardial blood flow based on SPECT imaging and those determined by PET imaging in coronary microvascular disease (CMVD) animal models, in order to clarify the accuracy and feasibility of SPECT quantitative analysis in CMVD.Methods:Seven Saanen milk goats (either male or female; (20±5) kg), were selected for establishing CMVD animal models by microsphere embolization. Dynamic myocardial perfusion imaging (DMPI) with one-day method of resting + ATP stress 99Tc m-methoxyisobutylisonitrile (MIBI) SPECT was performed before and after the modeling, respectively. One-day method of resting + ATP stress 13N-ammonia PET DMPI was performed after the modeling. The quantitative parameters determined by SPECT and PET after the modeling, including stress myocardial blood flow (SMBF), resting myocardial blood flow (RMBF) and myocardial flow reserve (MFR), were compared by paired t test. Parameters based on SPECT after modeling were compared with those of baseline levels. Bland-Altman analysis was applied to access the agreement between SPECT and PET. Results:Four of the seven experimental goats were fully imaged. The RMBF(ml·g -1·min -1; 1.52±0.27 vs 1.29±0.20), SMBF(ml·g -1·min -1; 0.74±0.19 vs 0.99±0.26), and MFR (0.53±0.16 vs 0.76±0.10) of the left ventricle (global) obtained by SPECT and PET in CMVD models were not significantly different ( t values: 3.121, 1.195, 1.930, all P>0.05). Among left anterior descending branch (LAD), left circumflex (LCX) and right coronary artery (RCA), the RMBF, SMBF and MFR values quantified by SPECT and PET were neither statistically significant ( t values: 0.182-2.734, all P>0.05). Bland-Altman analysis showed the quantitative parameters measured by SPECT and PET DMPI in left ventricle, LAD, LCX, RCA had a good consistency. The difference between the two methods for determining RMBF was up to 0.63 ml·g -1·min -1, and that of SMBF was up to 0.66 ml·g -1·min -1. All points are within the 95% confidence limit; MFR differs at most by 0.56, and 14/16 points were within 95% confidence limit. The RMBF (ml·g -1·min -1) of left ventricle measured by SPECT after modeling was not significantly different from that before modeling (1.52±0.27 vs 1.57±0.36; t=0.166, P>0.05); the SMBF (ml·g -1·min -1) and MFR after modeling were significantly lower than those before modeling (0.74±0.19 vs 2.34±0.89, 0.53±0.16 vs 1.39±0.31, t values: 3.836, 6.309, both P<0.05). Similar results were found when comparing the parameters of LAD/LCX/RCA after modeling with those before modeling (RMBF t values: 0.191, 0.235, 0.195, all P>0.05; SMBF/MFR t values: 0.411-19.911, all P<0.05). Conclusion:The blood flow quantitative parameters measured by SPECT imaging have a good consistency with those based on PET imaging, and the myocardial blood flow quantitative analysis of SPECT can evaluate the blood flow perfusion of CMVD.
Myhre syndrome is a rare disorder caused by a heterozygous mutation in the SMAD4 gene. Affected patients may exhibit dysmorphic facial features, intrauterine growth retardation, short stature, obesity, muscle hypertrophy, thickened skin, limited joint movement, hearing impairment, and varying degrees of psychomotor developmental disorder. Serious complications of the cardiovascular and respiratory system may be seen later in life. We report the case of a Chinese boy with Myhre syndrome presenting with a novel symptom of giant testicles where treatment with growth hormone combined with letrozole successfully improved his short stature. This case shows that letrozole combined with growth hormone can improve height in children with Myhre syndrome without adverse effects.
Hepatocellular carcinoma (HCC) is one of the leading causes of cancer mortality worldwide. Various imaging modalities provide important information about HCC for its clinical management. With the continuous progress of PET/CT research in oncology, it has played crucial roles in detecting, distinguishing, accurately staging, and evaluating local, residual and recurrent HCC. Dual tracer technology has become an important technology to assist HCC diagnosis, and the advent of new radiotracers has improved the ability to detect lesions. This paper reviews the current status of PET/CT for HCC.
目的 探讨99mTc-3PRGD2?SPECT/CT显像对孤立肺结节(SPN)的诊断价值.方法 收集2015年1月至2019年12月SPN患者79例,男51例,女28例;年龄43~81岁,平均年龄(62.39±8.99)岁.所有患者均行99mTc-3PRGD2?SPECT/CT平面及融合断层显像,测量SPN感兴趣区(ROI)计数值为T,在SPN对侧正常肺组织内画出与病灶同样大小的ROI,测量其计数值为NT,算出T/NT比值.与穿刺病理检查结果比较,分析99mTc-3PRGD2?SPECT/CT平面及融合断层显像T/NT对SPN的诊断价值.结果 与病理检查结果比较,99mTc-3PRGD2?SPECT/CT平面显像良性组和恶性组T/NT分别为2.3±1.4和3.7±2.5,二者比较差异有统计学意义(P<0.05).99mTc-3PRGD2?SPECT/CT平面显像T/NT比值ROC曲线下面积(AUC)0.734(P<0.05),提示该检查对判断SPN良恶性具有诊断意义.99mTc-3PRGD2?SPECT/CT平面显像?T/NT?比值最佳临界值为2.825.其诊断SPN良恶性特异度、灵敏度、准确率、漏诊率、误诊率、阴性预测值(PV-)阳性预测值(PV+)、分别为49.1%、95.5%、62.0%、4.5%、50.9%、96.6%、42%.99mTc-3PRGD2?SPECT/CT平面显像与融合断层显像比较,融合断层ROC?曲线下面积为0.919(P<0.05),平面显像ROC曲线下面积为0.866(P<0.05),提示断层显像对SPN良恶性诊断的准确度更高.结论 99mTc-3PRGD2?SPECT/CT平面及融合断层显像均可对SPN良恶性进行诊断,但融合断层诊断效果更好.
Objective:To explore the value of 99mTc-3PRGD2 SPECT/CT imaging combined with 3D printing template in the diagnosis of solitary pulmonary nodules.Methods:All of 79 patients with SPN were enrolled in the respiratory department of the Affiliated Hospital of Inner Mongolia Medical University in the January 2015 to December 2019, they were 51 males and 28 females, the average age of 62.39±8.99 years old. To analyze the value of 99mTc-3PRGD2 SPECT/CT imaging in the diagnosis of SPN and the related factors of complications after percutaneous pulmonary puncture with 3D printing template.Results:All 79 patients successfully completed SPECT/CT imaging and biopsy. SPECT是核医学显像中的一种重要成像仪器.常规的平行孔准直器-SPECT设备通过旋转双探头以实现断层成像功能,由于其灵敏度低、空间分辨率低以及采集时间长,致使图像质量大幅度降低.近年来,IQ-SPECT以及采用固态碲-锌-镉(CZT)晶体作为探测器的新型心脏专用SPECT已应用于临床,其灵敏度、特异性和准确性均优于平行孔准直器-SPECT,但CZT-SPECT成本太高.新型多针孔准直器(MPH)不仅可以搭载在普通SPECT机上,还能够在达到心脏专用CZT-SPECT性能的同时明显降低成本.本文将对不同准直器的SPECT设备在心肌成像中的应用及进展进行综述.
The traditional SPECT has been considered unable to quantitatively analyze due to the limitations of device performance and reconstructing algorithm. Because of the development of SPECT devices, the emergence of SPECT/CT, and the improvement of reconstructive algorithms, SPECT can be used for quantitative analysis like PET. The current quantitative SPECT researches focus on the usefulness of quantitative SPECT technology in clinical diagnosis and the guidance and evaluation of radionuclide therapy. This review summarizes the development of SPECT quantification, the current status of quantitative SPECT researches in clinical diagnosis and radionuclide therapy, as well as the limitations and advantages of SPECT quantification.
目前肺癌的各种治疗方法大多集中于肿瘤细胞本身,对于肿瘤生存的微环境及肿瘤能量代谢关注较少.肿瘤能量代谢对肺癌的发生、发展、侵袭及转移具有极其重要的作用,而乏氧微环境可能是驱动能量代谢的关键因素,但作用机制尚未完全阐明.针对肿瘤微环境与能量代谢的关系及机制进行探讨,能够指导临床从能量代谢的角度寻求肿瘤治疗策略,为肿瘤治疗提供有效靶点.而利用正电子发射计算机断层显像/CT、磁共振成像等分子影像学检查方法对肿瘤能量代谢进行活体水平评估,能为临床提供一种无创的检测手段.
18F-FDG PET/CT心肌代谢显像是目前被公认的监测患者存活心肌的"金标准",在冠心病的临床诊疗、血管再通术的预后评价等方面起指导作用。保证合格的图像质量是发挥其作用的关键。然而,18F-FDG PET/CT心肌代谢显像图像质量受到许多因素影响,其中血糖水平、胰岛素功能是主要的影响因素。通过调整显像前饮食结构、禁食时间、长期服用药物、检查日的血糖水平、调节血糖的方法以及注射显像剂的时间等可提高图像质量,并提升显像的成功率。为此,笔者对18F-FDG PET/CT心肌代谢显像图像质量影响因素的研究现状及最新进展进行综述。
Objective:To investigate the feasibility of 99Tc m-hydrazinonicotinamide-(poly-(ethylene glycol)) 4-E[(poly-(ethylene glycol))4-c((Arg-Gly-Asp)fK)] 2 (3PRGD 2) in the early diagnosis of rheumatoid arthritis (RA). Methods:Sixty female Wistar rats were divided into control group ( n=10; injected with saline of 0.3 ml/piece) and collagen-induced arthritis (CIA) group ( n=50; injected with type Ⅱ collagen emulsion of 0.3 ml/piece). Rats in 2 groups were subjected to 99Tc m-3PRGD 2 planar imaging before modeling, 25 and 45 d after modeling. The changes of the target/non-target ratio (T/NT) of the lesion joint and mediastinum before and after modeling were measured and analyzed in CIA rats, and compared with rats in control group. Pathological examination was conducted. Repeated measures analysis of variance and independent-sample t test were used to analyze the data. Results:Thirty-two rats in CIA group were successfully established, and obvious synovitis and synovial thickening, neovascularization were observed in the images. The T/NT of diseased joints in CIA group before modeling, 25 and 45 d after modeling were 0.158±0.023, 0.402±0.144, and 0.705±0.163 ( F=286.924, P<0.01). The T/NT of diseased joints at 25 and 45 d after modeling were significantly different from those of control group (0.160±0.028 and 0.158±0.032; t values: -10.484 and -20.917, both P<0.01). Immunohistochemistry results showed positive expressions of vascular endothelial growth factor, α vβ 3 and tumor necrosis factor-α in the synovial tissue in of diseased joints in rats of CIA group. Conclusion:99Tc m-3PRGD 2 has high sensitivity for joint synovial neovascularization in rat rheumatoid arthritis models and is expected to be used for early diagnosis of RA.

正电子发射计算机断层成像/磁共振(PET/M R)正从科研逐渐走向临床,但 PET/M R临床应用仍面临挑战,甚至缺乏标准化 PET/M R采集程序和后处理重建方案.目前,国内外关于一体化肝胆系统 PET/M R 标准化检查的探讨仍较少.为此,复旦大学附属中山医院联合10家联影一体化PET/MR的早期装机医院,在制定 PET/MR全身显像工作流程及协议规划[1 ]的基础上,围绕肝胆系统疾病的一体化PET/M R应用进行了大量的临床探索及验证,并结合国内外指南、共识和文献,经多次讨论形成了专家共识,以期与国内同道进一步探讨、修订和完善一体化肝胆系统 PET/M R 检查规范.
