Реактивация вируса гепатита В (HBV) у пациентов с лимфопролиферативными заболеваниями, получающих химиотерапию, может быть причиной развития фульминантной печеночной недостаточности и даже смерти при позднем распознавании и лечении HBV инфекции. Скрининг маркеров вирусов гепатита в группе больных онко-гематологическими заболеваниями является обязательным для клиницистов, практикующих лечение злокачественных опухолей. Обсуждаются способы профилактики и лечения реактивации HBV инфекции у больных с лимфомами.
Wilson-Konovalov's disease is a rare genetic pathology of copper metabolism that in the first place affects liver and CNS. Due to autosomal-recessive inheritance of this condition, it most frequently occurs in sibs. We report a case of Wilson-Konovalov's disease in two sisters differing in its clinical course: severe abdominal variant in the younger sister and largely neurologic form in the elder one. This observation demonstrates clinical variability of Wilson-Konovalov's disease, the possibility of its late clinical manifestation (at the age 45 years), the necessity of examination of all sibs of a proband regardless of age, and the possibility of radical improvement of prognosis even when the disease is diagnosed at the stage of decompensated liver cirrhosis.
Primary sclerosing cholangitis (PSC) is autoimmune disease characterized by inflammation and sclerosis of the bile duct, which leads to the development of secondary biliary cirrhosis. Many women with primary sclerosing cholangitis are at reproductive age (mean age of onset is 40 years old, but the cases of the disease in other age groups were described). There is a risk of negative impact of primary sclerosing cholangitis in the mother (exacerbation PSC) and the fetus (intrauterine fetal hypoxia, premature birth), but overall prognosis for the mother and child is favorable (if pregnant does not have cirrhosis of the liver complications). Pregnant women with varicose veins of the esophagus have a high maternal mortality. Ursodeoxycholic acid is the drug of choice for the treatment of pruritus in pregnant women with primary sclerosing cholangitis
В 2009 г. (21 год) больной назначены урсодезоксихо-левая кислота по 750 мг/сут и курсами — S-аденозил-L-метионин.В 2010 г. появились ксантелазмы. Выполнена повтор-ная биопсия печени, выявившая морфологическую кар-тину билиарного цирроза.В течение 2010—2011 гг. — неоднократные госпита-лизации в связи с приступами боли в правом подребе-рье, нарастанием желтухи и кожного зуда.Госпитализирована в клинику им. Е. М. Тареева в сентябре 2011 г. в состоянии средней тяжести с жало-бами на кожный зуд, желтушность кожных покровов, общую слабость, ощущение тяжести в правом подребе-рье. При поступлении обращали на себя внимание вы-раженная желтушность кожных покровов, ксантелазмы, «печеночные ладони», следы от расчесов на коже, осо-бенности строения лицевого скелета (широкий выступа-ющий лоб, глубоко посаженные глаза, длинный прямой нос, выступающий подбородок, оттопыренные ушные раковины (рис. 2, см. вклейку). Определялся пансисто-лический шум в точке Боткина—Эрба, во втором меж-реберье справа, над брюшным отделом аорты. Размер печени по Курлову 12/2—10—9. Primary sclerosing cholangitis (PSC) is a chronic slowly developing cholestatic diseases of liver characterized by non-purulent destructive inflammation and fibrosis of bile ducts leading to secondary biliary cirrhosis. PSC etiology remains unknown and the disease is regarded as an autoimmune pathology. In 60-80% of the patients PSC combines with other autoimmune diseases (over 40 nosological forms). This paper deals with the diversity and prevalence of systemic manifestations of PSC in a group of 93 patients admitted to Tareev Nephrology Clinic. Autoimmune diseases were diagnosed in 79.6% of the patients. PSC was most frequently associated with ulcerative colitis (49.5%), Crohn's disease (17.2%), hemorrhagic vasculitis (8.6%) and autoimmune thyroiditis (7.59%). Nervous system, kidneys, lungs and eyes were affected less frequently. 20.3% of the patients had more than one (2-5) manifestations of PSC in different combinations. It is concluded that the knowledge of extrahepatic PSC symptoms will promote its early diagnosis in patients with cholestasis of uncertain origin.
A case of a rare hereditary disease, Alagille-Watson syndrome, is reported. It most frequently affects liver and cardiovascular system which leads to biliary cirrhosis and cardiovascular catastrophe in young patients. Pathogenesis and clinical variants of the disease are described.

Наблюдения, как правило, касаются перекреста лабо-раторных и серологических маркеров или гистологиче-ских признаков, свойственных АИГ и ПБЦ. Описания вариантных форм аутоиммунных заболеваний печени и системных заболеваний соединительной ткани встре-чаются редко.Мы наблюдали двух больных, длительное время страдавших системным заболеванием соединитель-ной ткани — БШ — с развитием вариантной формы АИГ — ПБЦ.Приводим клинические наблюдения.1. Больная 68 лет, инженер, инвалид II группы.В 1964 г. (в возрасте 22 года) появились рецидивиру-ющий конъюнктивит, сухость и ощущение песка в гла-зах, уменьшение количества слезной жидкости, сухость во рту. С 1970 г. присоединились рецидивы паротита, по-явился распространенный кариес с разрушением эмали зубов, эпизодически — субфебрилитет, сердцебиение, тремор кистей рук, увеличение СОЭ до 50 мм/ч.С 1974 г. больная находилась под наблюдением в НИИ ревматологии в связи с поздней стадией БШ с генерализованным сухим синдромом, постоянно полу-чала преднизолон (5—10 мг/сут). С середины 1970-х годов появились немотивированная нарастающая мы-шечная слабость вплоть до полной обездвиженности, выпадение волос, ломкость ногтей, упорный запор. Ди-агностирован аутоиммунный тиреоидит с явлениями гипотиреоза и гипотиреоидной миопатии, в связи с чем Известно, что основные аутоиммунные заболевания печени — аутоиммунный гепатит (АИГ), первичный билиарный цирроз печени (ПБЦ) и первичный скле-розирующий холангит (ПСХ) — нередко проявляют-ся рядом внепеченочных поражений, обусловленных также аутоиммунными реакциями [1—7]. При этом манифестация заболевания артралгиями, лихорадкой, миалгией или миопатией, полиневропатией, кожным геморрагическим васкулитом, сухим синдромом, по-ражением щитовидной железы, сердца, почек встре-чается у 20—40% пациентов [3, 6, 8]. У ряда больных отмечается сочетание аутоиммунного заболевания пе-чени (АИГ, ПБЦ, ПСХ) с системными заболеваниями соединительной ткани: системной красной волчан-кой (СКВ), ревматоидным артритом (РА), болезнью Шегрена (БШ), системной склеродермией, а также с CREST-синдромом или воспалительными заболевани-ями кишечника — язвенным колитом и болезнью Кро-на. В последние годы обращают внимание на наличие перекрестных или вариантных форм аутоиммунных заболеваний печени: АИГ—ПБЦ, АИГ—ПСХ [6—12]. Перекрестные синдромы между различными аутоим-мунными заболеваниями печени с одновременным вы-явлением клинических признаков, биохимических и серологических маркеров и гистологической картины в печени, свойственных двум заболеваниям, встречает-ся часто (18%) и труден для диагностики в связи с от-сутствием четких критериев и разграничений. Two clinical observations of the variant form of hepatic lesion: autoimmune hepatitis--primary biliary cirrhosis with systemic manifestations are presented in patients with long-standing Sjogren's syndrome, one at the stage of lever cirrhosis, the other at stage F2 of fibrosis. Difficulties encountered in diagnostics and the necessity of changing the entire spectrum of autoimmune markers characteristic of Sjogren's syndrome, autoimmune hepatitis and primary biliary cirrhosis are discussed. The possibility of different forms of hepatic lesions in autoimmune hepatitis--primary biliary cirrhosis is emphasized.
Primary sclerosing cholangitis (PSC) is a chronic slowly progressive cholestatic liver disease characterized by non-purulent destructive inflammation and sclerosis of the extra- and intrahepatic bile ducts, leading to secondary biliary cirrhosis. The etiology of the disease is unknown; however, bacterial and genetic factors are presumed to be implicated. In 66-84% of cases, PSC is accompanied by inflammatory bowel diseases, such as ulcerative colitis and Crohn's disease. The diagnosis of PSC is based on the clinical presentation of the disease and on the data of magnetic resonance imaging or endoscopic retrograde cholangiography. The treatment of PSC has not been developed; there is evidence that ursodeoxycholic acid reduces cholestasis. Orthotopic liver transplantation is indicated at the stage of decompensated liver cirrhosis.
The patients with established autoimmune hepatitis having alkaline phosphatase or alkaline phosphatase/AST correlation increase more than 1,5 and whose conditions didn’t improve after immunosuppressor treatment should undergo ERCP or MRT cholangiography to identify the primary sclerosing cholangitis or AMA and to verify the possible primary biliary cirrhosis. On the other side, a range of patients with PBC and PSC had a high activity of hepatic processes that can be caused by combined autoimmune hepatitis. A combined therapy of immunosuppressor and ursodeoxycholic acid can impact the disease prognosis.

The aim of our study was to identify clinical and morphological signs of liver damadge in patients with pulmonary sarcoidosis. Patients and methods: 65 patients with morphologically proved pulmonary sarcoidosis were examined. Pulmonary function testing, high-resolution CT (HRCT), liver tests were evaluated. Biopsy of lung (65), liver (52) were performed. Biopsy of liver were performed if the signs of cytolisis and/or cholestasis were present. Results: 65 patients had markers of active pulmonary sarcoidosis: respiratory failure which correlated with HRCT sings of activity such as ground - glass opaque (p=0,0053) and morphology of specific granulomatous, 52 patients also presented slight hepatomegaly, sings of cytolisis (AST 1,2-2,5 norms, ALT 1,3 – 2,7 norms) and/or cholestasis (GGT 1,6-3,4 norms) which correlated with morphological signs of liver granulematosis: the non-caseating granulomas were in all biopsies, three types of histologic change were found: inflammatory (78%) cholestatic (51%), and vascular (19%).Inflammatory changes included inflammation suggestive of chronic active hepatitis. Among those with cholestasis, 9 had a pattern of periductal fibrosis. Vascular changes consisted of sinusoidal dilatation (3 cases) and nodular regenerative hyperplasia (7 cases). No cirrhosis were found. Conclusion: Liver involvement can be considered as a marker of extra-pulmonary activity in pulmonary sarcoidosis.
